| 摘 要： | 目的探讨血清降钙素原（PCT）在重症肺炎中的临床价值。方法以56例进入本研究的重症肺炎患者作为病例组,另随机抽取同期住院的56例轻症肺炎患者作为对照组。2组均于诊断24 h内测定血清PCT、血常规、CRP、体温,同时将病例组按PCT水平分为〈0.5、≥0.5～2.0、≥2.0～10.0、≥10.0μg.L-14个等级,分析各组的病死率、需要使用呼吸机例数以及抗生素使用时间和住院时间。结果病例组血清PCT、CRP高于对照组（均P〈0.05）,而2组WBC计数、中性粒细胞（N）百分比及体温差异均无统计学意义（均P〉0.05）;随着PCT水平的升高,重症肺炎患者需要呼吸机辅助呼吸的比例逐渐增高,病死率亦呈增高趋势,抗生素使用时间以及需住院时间也延长（均P〈0.05）。结论血清PCT对重症肺炎的诊断及判断病情、评估预后有指导意义。

| Abstract: | Objective To explore the clinical value of procalcitonin（PCT） quantification in serious pneumonia.Methods Serum PCT levels,white blood cell count,the percentage of neutrophils,C reactive protein（CRP） concentrations and body temperature were determined within 24 hours after diagnosis in 56 patients with serious pneumonia（case group） and 56 patients with mild pneumonia（control group）.Moreover,patients with serious pneumonia were divided into four groups depending on different levels of serum PCT：0.5 μg·L-1group,≥0.5～2.0 μg·L-1group,≥2.0～10.0 μg·L-1group and ≥10.0 μg·L-1group.The mortality,the number of patients required mechanical ventilation,the length of using antibiotics and hospital stay were analyzed.Results The levels of serum PCT and CRP in case group were obviously higher than those in control group（P0.05）,but no significant differences in white blood cell count,the percentage of neutrophils and body temperature were found between the two groups（P0.05）.With increasing PCT levels,the number of patients required mechanical ventilation,the morbidity,the length of using antibiotics and hospital stay increased（P0.05）.Conclusion Serum PCT has clinical value in diagnosis,judgment and prognosis of serious pneumonia. |
