OBJECTIVE: Marginal leakage of amalgam restorations may lead to secondary caries and pulpal damage. The purpose of this study was to determine the effect various cavity liners might have on microleakage. METHOD AND MATERIALS: Mesio-occlusodistal amalgam restorations with margins on enamel and dentin were treated with different liner materials (an adhesive system, a topical fluoride gel, a cavity varnish, and a glass-ionomer cement) in vitro. Following restoration, the teeth were submitted to thermocycling in a stained solution and sectioned to allow assessment of microleakage. RESULTS: On enamel, the control group (no liner) and the glass-ionomer-lined group had equivalent leakage scores and were superior to every other group. On dentin, only the glass-ionomer specimens had superior performance. The cavity varnish and fluoride-lined specimens exhibited the highest leakage scores. CONCLUSION: The use of liners does not reduce microleakage on amalgam restorations when the cavity margins remain on enamel. On dentin margins, a glass-ionomer liner can reduce microleakage.  相似文献

OBJECTIVES: The pulpal reaction and the marginal sealing of in vivo restored samples using resin composite and Scotchbond Multi-Purpose adhesive system (3M Dental Products) were analyzed in this study. METHODS: Twelve Class I non-exposed cavity preparations were placed on premolars to be removed for orthodontic reasons. They were restored and observed at 7 d and 28 d. RESULTS: The examination of the resin-dentin interface under the scanning electron microscope (SEM) revealed: 1) a gap-free attachment between adhesive resin and dentinal surface in 80% of the areas studied, 2) penetration of resin tags into the dentinal tubules, and 3) formation of a 3-5 micrometer thick acid-resistant hybrid layer. Microfissures measuring about 10 micrometers were observed in only 20a% of the areas studied; these were located along the walls of the cavities, especially near the enamel in zones where there was a lower concentration of dentinal tubules. The histological analysis, carried out 7 d after preparation of the restoration, did not show any alteration of the pulp. After 4 wk, reparative dentin was produced in the pulpal areas corresponding to the restored cavities. SIGNIFICANCE: The quantity of newly formed dentin is correlated with the distance from the cavity to the pulp. The results indicate that acid-etching of vital dentin using 10% maleic acid does not impair pulpal healing in deep Class I cavities and that the Scotchbond Multi-Purpose adhesive system is able to preserve the morphological and biological integrity of the pulpo-dentinal complex.  相似文献

This in-vitro study evaluated the microleakage of Class V restorations prepared using 10 per cent maleic acid and a composite resin. Thirty human premolar teeth were evenly distributed and randomly assigned to three groups. Conventional retentive preparations, etched with 10 per cent maleic acid for either 15, 30, or 60 seconds, were cut in the enamel on the facial surface of each tooth to a 1.5 mm depth (dentin). All teeth were restored with Z-100, a small particle composite resin. The teeth were then stored in deionized water for seven days, thermocycled, stained with methylene blue dye, invested, and sectioned vertically through the centre of the restoration. Leakage was established along each wall of the sectioned restoration. Analysis of variance (ANOVA) tests indicate that the restored teeth in Group 1 (15-second etch) had significantly greater microleakage (p < 0.05) on the enamel wall than the restored teeth in Group 2 (30-second etch) or Group 3 (60-second etch). In addition, Group 1 restorations had significantly greater overall microleakage (p < 0.05) than Group 2 or Group 3 restorations. Although the results were not statistically significant, it would appear that etching with 10 per cent maleic acid for 30 seconds could be clinically significant. Since a 30-second etch time was found to produce the least amount of microleakage (not statistically significant), it can be assumed that this etch time would also be optimal for etching enamel and dentin. Similarly, since Group 1 revealed the most overall microleakage, it can be assumed that a 15-second etch would be inadequate for etching enamel and dentin. Restorations in Groups 2 and 3 displayed statistically significant lower overall microleakage results.  相似文献

Forty freshly single rooted teeth were selected for this study. The coronal part of teeth were decapitated and the root canals were prepared by modified step-back technique with K-flex files to size 35# at the apex, five ml of 5.25% NaOCl was used as irrigating solution. According to the dentin thickness at the mesio-distal direction of the prepared roots, roots were divided into two groups: Group (I), dentin and cementum thickness from 1.5-2 mm, and Group (II), dentin and cementum thickness 1 mm. According to the filling technique used the teeth were subdivided into four subgroups each of five teeth; Subgroup (A) single gutta-percha cone and Ketac-endo root canal sealer, Subgroup (B) lateral condensation technique with gutta-percha and Ketac-endo sealer, Subgroup (C) laterally condensed gutta-percha with Roth 801 cement and Subgroup (D) teeth left unfilled as negative controls. The prepared roots were placed into acrylic resin so that 10 mm of the root was exposed. A slowly increasing force was applied vertically using a blunt punch with 1 mm tip thickness attached to Instron testing machine. The results showed that group (I) with 1.5-2mm dentin thickness required greater force to fracture in all experimental groups, than that of group (II) with 1 mm dentin thickness. So the amount of remaining dentin thickness significantly affected the resistance to fracture of the prepared roots.  相似文献

PURPOSE: To evaluate a dental adhesive system that uses a single conditioning/primer agent. MATERIALS AND METHODS: Twenty-five flat enamel and dentin bonding sites were prepared to 600 grit on human molar teeth. The Clearfil Liner Bond 2 adhesive system was used to bond Clearfil AP-X composite to both enamel and dentin. After 24 hours of water storage, shear bond strengths were determined using an Instron testing machine. Fifty V-shaped cavity preparations were prepared in human molar teeth with an enamel and cementum margin. Composite restorations were placed using the new adhesive system. The teeth were stored for 24 hours, thermocycled, stained with AgNO3 , sectioned and examined for microleakage. SEM examinations were also completed to evaluate the effects of the treatment steps on enamel and dentin surfaces. RESULTS: Mean shear bond strengths for the experimental adhesive to enamel and dentin were 28.2 +/- 4.9 and 19.4 +/- 3.1 MPa. A t-test revealed that the enamel bond strength was significantly greater (P<0.05) than the dentin strength. No marginal leakage was observed from the enamel margins of the restorations. Three restorations showed minimal leakage from the cementum margins. SEM examinations showed resin penetration into both the conditioned enamel and dentin surfaces. The adhesive system produced high bond strengths to both enamel and dentin, exhibited very minimal microleakage and was easy to use.  相似文献

PURPOSE: To evaluate the shear bond strength to the dentin of permanent teeth and failure site of Dentastic hydrophilic dentin bonding agent. MATERIALS AND METHODS: Forty permanent noncarious molar teeth stored in distilled water were obtained. The teeth were cleaned with pumice and a rubber cup. The mesio-buccal surface of the teeth was ground flat with hand pressure with a series of SiC paper ending with the 600 grit to provide a uniform surface on dentin to which the resin composite could be applied. After preparing the tooth surface, the teeth were stored in distilled water for 48 hours. They were then divided at random into four groups of 10 specimens each: Group 1: Dentastic, five coats of primer; Group 2: Dentastic, three coats of primer; Group 3: Dentastic, five coats of primer, light-cured adhesive before resin bonding; Group 4: Dentastic, three coats of primer, light-cured adhesive before resin bonding. All specimens were thermocycled (500x) and sheared in a testing machine. After shear testing, the debonded sites of all samples were examined with a stereomicroscope and a scanning electron microscope. RESULTS: The results in MPa were: Group 1: 22.63 +/- 6.24; Group 2: 23.35 +/- 5.14; Group 3: 23.58 +/- 5.66; Group 4: 27.26 +/- 8.22. ANOVA and Student-Newman-Keuls showed no statistically significant difference between the groups. In all groups, all specimens failed at the dentin (dentin cohesive failure = dentin fracture) or at the resin (resin cohesive failure = resin fracture). This means that the bond strength of the product is stronger than the cohesive strengths of the dentin and the resin.  相似文献

The objective of this study was twofold: to evaluate the microleakage behavior of three dentin bonding systems and to determine if bovine teeth are comparable substrates to human teeth when studying the microleakage of various materials. The materials evaluated were Scotchbond Multi-Purpose Adhesive, Prisma Universal Bond 3, and All-Bond 2. All three bonding systems were used in combination with Prisma APH hybrid composite for comparison of microleakage behavior. Sixty class 5 preparations were cut at the cementoenamel junction for groups containing 30 human and 30 bovine teeth. A 1 mm 45 degree bevel was placed at the enamel margin. Teeth were grouped according to the dentin bonding system used and then restored according to the manufacturer's directions. After restoration, the teeth from each group were stored in distilled water at 37 degrees C for 3 days. The teeth were then thermocycled between 4 degrees C and 58 degrees C for 100 cycles and returned to distilled water at 37 degrees C for an additional 4 days. The teeth were then sealed with nail polish up to 1 mm from the margins of the restoration and placed in 45Ca isotope for 2 hours. The teeth were then sectioned and placed on x-ray film to produce autoradiographs. Microleakage was evaluated for the enamel and dentin margins separately using the following scale: 0 = no leakage, 1 = penetration of isotope to less than 1/2 the distance to the axial wall, 2 = penetration of isotope greater than 1/2 of the distance to the axial wall but short of the axial wall, and 3 = penetration of isotope to the axial wall or beyond. The materials were compared to each other using the Mann-Whitney U and Kruskal-Wallis tests. The gingival margins were compared to the incisal margins for all materials. No statistically significant differences in microleakage were revealed between the incisal and gingival location for human substrates, but there was statistically significant greater gingival microleakage for bovine substrates. All-Bond 2 leaked significantly more than Scotchbond Multi-Purpose for human substrates at the incisal margin. All-Bond 2 had significantly more microleakage than Prisma Universal Bond 3 at both dentin and enamel margins for the bovine substrate. There were no statistically significant differences in microleakage among the bonding systems for the human substrate. No statistically significant differences between the microleakage behavior of human and bovine substrates were found. These results support the use of bovine teeth for in vitro microleakage studies.  相似文献

Estrogen receptors have been demonstrated in many osteogenic cell lines. Recently, we showed that estrogen deficiency induced by ovariectomy caused enhanced dentin formation in adult rats, suggesting that estrogen receptors may be present in dental tissues. Nothing is known about estrogen receptors in human teeth. We used immunohistochemical staining and immuno-blotting to demonstrate the presence of estrogen receptors in human pulp and/or the pulpo-dentinal border. Unerupted human wisdom teeth were surgically removed, frozen in liquid nitrogen, and prepared for immunological studies. Western blot analysis with monoclonal antibodies specific for human estrogen-receptor-related antigens demonstrated an approximately 29-kDa clear double band in the material scraped from the predentin-odontoblast border and in the fluid that emerged into the pulpal chamber, evidently from the odontoblasts. A weaker double band was also present in pulpal tissue samples. By immunohistochemical staining, estrogen-receptor-related antigens were visualized in the predentinal-odontoblast region and in the pulpal blood vessels. Our results suggest the presence of estrogen receptors in human teeth, and thus the previously reported enhancement of the dentin formation in rats after ovariectomy may be mediated via these receptors.  相似文献

PURPOSES: To evaluate (1) the shear bond strength to the dentin of primary teeth and failure site of hydrophilic dentin bonding agents, (2) the interfacial micromorphology of these adhesives on primary teeth. MATERIALS AND METHODS: Seventy-six primary noncarious molars stored in distilled water were obtained. The teeth were cleaned with pumice and a rubber cup. The mesio-buccal surface of the teeth was ground flat with hand pressure with a series of SiC paper ending with the 600 grit to provide a uniform surface on superficial dentin to which the adhesives and resin composite could be applied. After preparing the dentin surface, the teeth were stored in distilled water for 48 hours. They were then rinsed and dried with compressed air and divided at random into four groups of 16 specimens each: Group 1: Dentastic; Group 2: One-Step; Group 3: Prime & Bond 2.0; Group 4: Compoglass SCA. Z100 resin was used in all groups. All specimens were thermocycled (500x) and sheared in an Instron machine. After shear testing, the debonding sites of all samples were examined with a stereomicroscope and selected samples were also examined with the scanning electron microscope. Three additional samples per group were used to evaluate the resin adaptation to dentin. RESULTS: The results in MPa were: Dentastic 19.62 (4.67); One-Step 11.24 (3.67), Prime & Bond 22.38 (6.47), Compoglass SCA 18.88 (4.04). ANOVA (P < 0.0001) revealed that there was a significant difference between the groups. The Student-Newman-Keuls test (P < 0.05) showed no statistically significant difference between Dentastic, Prime & Bond and Compoglass SCA. However, these three groups were statistically significantly higher than One Step. In the Dentastic group, 14 of 16 samples revealed resin cohesive failure (resin fracture) while two of 16 displayed dentin cohesive failure (dentin fracture). In the One Step group, 15 samples failed at the resin and one sample showed dentin cohesive failure. In the Prime & Bond group, 12 specimens revealed resin cohesive failure while four displayed dentin cohesive failure. In the Compoglass SCA group, 13 samples had resin cohesive failures while three had dentin cohesive failures. All samples revealed an intimate adaptation to the dentin displaying resin tag formation.  相似文献

The effects of pulsed Nd:YAG laser irradiation with different laser initiators on the permeability and ultrastructure of the root canal wall dentin were investigated in vitro. Forty extracted human single-rooted teeth were randomly assigned to four groups. Group 1 teeth were not lased as a control. Group 2 specimens received four 10-s duration laser exposures for a total exposure of 40 s/canal. In group 3 specimens, the root canals were painted with black ink and then lased by the same method as group 2 teeth. In group 4 specimens, root canals were treated with 38% Ag(NH3)2F and then lased by the same method as group 2 teeth. Laser parameters were set at 2 W, 20 pps. After being placed in 0.6% rhodamine B solution for 48 h, the teeth were sectioned for study by stereoscope and scanning electron microscopy. Statistical analysis showed there were significant differences (p < 0.05) in dentin permeability in the apical areas between groups 3 and 1, 4 and 1, and 4 and 2. Scanning electron microscopic examination showed that laser treatment alone had no obvious effects on the root canal wall. The root canal surfaces prepared for by laser irradiation with black ink or 38% Ag(NH3)2F revealed melting, smear layer evaporation, and open dentinal tubules. Black ink was more effective than 38% Ag(NH3)2F as a Nd:YAG laser initiator.  相似文献

Amalgam remains unchallenged as a posterior restorative material. But its inability to bond to the teeth leads to some amount of microleakage at the restoration-tooth interface with associated problems such as post operative sensitivity, pulpal complications etc. Also a broken amalgam restoration requires replacement which will further weaken the tooth structure. Recently, 4-META has been introduced which can graft amalgam and composite to enamel, dentin and old amalgam restorations. In this study, the bonding and marginal sealing abilities of 4-META was assessed both at the tooth-amalgam interface and old amalgam fresh amalgam interface.  相似文献

The purpose of this study was to determine the anesthetic efficacy of a supplemental intraosseous injection of 2% lidocaine with 1:100,000 epinephrine in teeth diagnosed with irreversible pulpitis. Fifty-one patients with symptomatic, vital maxillary, and mandibular posterior teeth diagnosed with irreversible pulpitis received conventional infiltrations or inferior alveolar nerve blocks. Pulp testing was used to determine pulpal anesthesia after "clinically successful" injections. Patients who were positive to the pulp tests, or were negative to the pulp tests but felt pain during endodontic access, received an intraosseous injection using 1.8 ml of 2% lidocaine with 1:100,000 epinephrine. The results demonstrated that 42% of the patients who tested negative to the pulp tests reported pain during treatment and required supplemental anesthesia. Eighty-one percent of the mandibular teeth and 12% of maxillary teeth required an intraosseous injection due to failure to gain pulpal anesthesia. Overall, the Stabident intraosseous injection was found to be 88% successful in gaining total pulpal anesthesia for endodontic therapy. We concluded that, for posterior teeth diagnosed with irreversible pulpitis, the supplemental intraosseous injection of 2% lidocaine (1:100,000 epinephrine) was successful when conventional techniques failed.  相似文献

PURPOSE: To determine the quantitative contribution of dentin hybridization to bonded assembly strength and demonstrate the micromorphology of the interface with and without collagen present. MATERIALS AND METHODS: Four groups of 10 molar teeth were finished to a 320 grit dentin smear layer. Two groups served as controls and two experimental groups were subjected to collagenase digestion of the collagen exposed by acid conditioning. All-Bond 2 and Amalgambond were used to bond Bisfil and Epic resin composite, respectively. Stored in water at 37 degrees C for 24 hours the assemblies were tested in a shear mode at a crosshead speed of 5 mm/minute. Means and standard deviations were subjected to analysis for statistical significance. Twenty four teeth in four groups were examined by scanning (SEM) and transmission electron microscopy (TEM) for the relationship between resin and conditioned dentin with and without the collagen network. RESULTS: All-Bond 2 and Amalgambond controls were 28.41 +/- 3.9 and 19.04 +/- 5.96 MPa, collagenase-treated groups scored 26.43 +/- 2.90 and 19.70 +/- 4.25 MPa respectively. No significant difference existed between the control and experimental groups. SEM showed an intertubular collagen network with patent tubules and a pronounced porous, irregular dentin topography following collagen digestion. A distinct hybrid zone and tubular penetration was observed but the collagenase-treated specimens showed only resin in the tubules and their lateral extensions. TEM confirmed the absence of a distinct hybrid zone in the collagenase groups with a tight, gap-free junction between the resin and the undemineralized dentin. An electron dense zone (< 50 nm) at the leading edge of conditioning was observed for All-Bond 2 and Amalgambond groups. It was concluded that the resin-reinforced or hybridized, collagenous network does not detract from, nor contribute any significant quantitative value per se to dentin bonding with the systems tested.  相似文献

As information on amelocemental junction of deciduous teeth is limited, this topographical area was investigated by scanning electron microscopy (SEM) to verify differences from that of permanent teeth. Twenty-six carious and non-carious human maxillary and mandibular primary teeth were placed in a fixative immediately after extraction. Pulpal tissue was removed from the pulpal chambers and root. The primary teeth blocked onto stubs and all specimens were platinum coated and examined by SEM. In these specimens an overlapping of cementum onto to enamel and an edge to edge relationship was dominant. No gaps between enamel and cementum were observed. The amelocemental junction of deciduous teeth seem to differ to that described for permanent teeth.  相似文献

PURPOSE: Clinical evaluation of repairing old amalgam restorations using composite inlays cemented with two different bonding agents. MATERIALS AND METHODS: Occluso-proximal Class II inlay cavities were prepared in 50 old defective amalgam restorations. Extra oral system (EOS) composite inlays were cemented with two different bonding agents (All-Bond 2 or Geristore). After final finishing and polishing of each restoration, evaluation was carried out at baseline, 6 months, 12 months, and 24 months using US Public Health Service criteria. The data were collected and statistically analyzed using Chi-square test. RESULTS: No significant difference was found between the two bonding materials at any time interval.  相似文献

Growth factors involved in normal wound healing may promote tissue repair when applied as a direct pulp capping medication. A minimal pulp exposure was made in rat molars, a pulp capping medication was placed, and the cavity was sealed. Epidermal growth factor, basic fibroblast growth factor, insulin-like growth factor II, platelet-derived growth factor-BB, and transforming growth factor-beta 1 (TGF-beta 1)--each absorbed onto a sterile collagen membrane (BioMend; Calcitek, Carlsbad, CA)--were used separately as pulpal medicaments. Dycal, unimpregnated collagen membrane, and no medication were used as controls. Eight samples from each treatment regimen were collected 2 and 3 weeks after surgery. Pulpal soft and hard tissue responses were graded. Data were analyzed by one-way ANOVA and Tukey-Kramer tests. No significant differences were detected after 2 wk. Pulp treated with TGF-beta 1 showed significantly improved soft and hard tissue healing at week 3, compared with the procedure control. We conclude that TGF-beta 1 as a pulp-capping medication enhances reparative dentin formation in rat molars.  相似文献

The purpose of this study was to investigate the effect of dentinal disinfection with a 2% chlorhexidine or a 0.11% I2-KI/CuSO4 solution on the shear bond strength of three resin-modified glass-ionomer cements: Fuji II LC, Photac-Fil, and Vitremer. The occlusal surfaces of extracted human teeth were flattened to dentin. Specimens were randomly assigned to one of nine treatment groups (n = 12). For each glass-ionomer material, there was a control group and two treatment groups in which the dentin was treated with either a 2% chlorhexidine or a 0.11% I2-KI/CuSO4 solution before the dentin was treated with the recommended dentin conditioner prior to glass-ionomer bonding. Specimens were stored for 1 day in water, thermocycled, and tested in shear until failure. The chlorhexidine solution did not significantly affect the shear bond strengths of any of the cements, but the I2-KI/CuSO4 solution significantly lowered the bond strengths of Vitremer and Fuji II LC compared to the controls.  相似文献

This study evaluated the histologic responses of two different adhesive systems on the teeth of seven healthy adult monkeys. Class V cavities in 128 teeth were observed at 7,21 to 27, and 90 to 97 days. Statistical data showed no significant differences in pulp inflammation between the Optibond (Kerr), XR-Bond (Kerr), and IRM (Caulk) groups in pulps at the three time intervals. Some odontoblastic disruption and inflammation was seen in pulps restored with XR-Primer at the three time intervals. Pulp healing was similar in teeth treated with both direct-capped Optibond and calcium hydroxide, and there was no significant increase in pulpal inflammation with time. No abscesses or necrotic pulps were seen in any teeth. Optibond and XR-Bond adhesive systems are biologically acceptable in nonhuman primate pulp tissues.  相似文献

Monkey pulpal responses to four adhesive resin systems used as direct pulp capping agents were histopathologically evaluated at 3, 7, 14, 30, and 60 days after operation. No serious inflammatory reaction of the pulp, such as necrosis or abscess formation, was observed. Slight inflammatory cell infiltration was the main inflammatory reaction of the exposed pulp, and the exposed area became occluded with dentin bridging as the observation period increased. Healing of exposed dental pulp beneath adhesive resin capping slightly differed depending on the materials used.  相似文献

Histological assessment of the dental pulps of 55 carious primary teeth was carried out 3 to 58 months after treatment by the 'atraumatic' technique involving application of 40 per cent silver fluoride to residual caries followed by restoration with glass ionomer cement. Fifty of the 55 teeth examined showed a favourable pulpal response, inducing presence of abundant reparative dentine and a wide odontoblast layer. Histological comparisons were made between these teeth and others not treated with silver fluoride but restored with glass ionomer cement, amalgam or zinc oxide and eugenol. Possible mechanisms of the action of silver fluoride in arresting residual caries are discussed. The question of whether or not treatment of carious dentine with silver fluoride represents a biologically acceptable clinical procedure cannot be answered on the basis of pulpal histology alone. The very high concentration of fluoride in commercial preparations of silver fluoride raises several questions concerning its clinical safety.  相似文献
