Tobacco use poses one of the greatest health problems at school. Its prevention through health education should be assumed up by all members of the school community and health personnel (especially family and respiratory physicians). We designed an anti-tobacco program lasting three years aimed at all the students of the 6th, 7th and 8th grades at school as well as those in the pre-university years that was implemented in a rural area. This study reports the results of the attitudes of the 610 students (256 boys and 354 girls, between 11 and 20 years old) collected at the end of the program. 59.4% of the boys and 44.1% of the girls reported having smoked on some occasion. The mean age at the time of the first contact with tobacco was 11.8 +/- 2.4 years, this being significantly different between the boys and the girls. The main reasons for starting to smoke tobacco were quoted as peer pressure (57.1%) and curiosity (55.5%). That their parents smoked was only adduced as a reason by 29.5% of the students. Knowledge of the harmful effects of tobacco was limited and only 57% related tobacco smoking to lung cancer and only 41% to coronary pathology. At the end of the program 13.6% of the students involved smoked. 93.9% of those who quit smoking related this event to the program. Of those still smoking, 63.6% said they would like to quit. This justifies the need to reinforce the knowledge that will allow them to switch attitudes and stop smoking. Intervention programs in school populations have proved to be useful in the struggle to decrease smoking among school children. If an adolescent can avoid smoking it is likely that s/he will not smoke in adulthood. It is necessary to further develop this type of anti-tobacco program as reflected in this paper.  相似文献

PURPOSE: The prevalence and correlates of early smoking were investigated among schoolchildren in grades 4-6 living in multiethnic, low-income neighborhoods in Montreal. METHODS: As part of the evaluation of a school-based heart health promotion program, baseline data on the prevalence of early smoking were collected from 2285 students aged 9-13 years in 24 inner-city elementary schools during May-June 1993. RESULTS: Overall, 28.7% of boys and 20.3% of girls had smoked. Girls began trying later than boys, but by age 13 the prevalence of experimental or regular smoking by girls overtook that of boys. Univariately, ever smoking varied considerably by family origin, from 2.1% among Vietnamese girls, to 35.8% among Portuguese boys. In multivariate analysis, age, perceived smoking habits of friends, and smoking by family members, were strong correlates of smoking. Being of Asian family origin was negatively associated with smoking. None of the indicators of social class were significantly associated with ever smoking. CONCLUSIONS: Smoking prevention should begin with children even younger than age 9 in multiethnic, low-income, inner-city neighborhoods. With the exception of a lower prevalence of smoking among Asian children, ethnicity and social class were not strong influences on early smoking behavior in this population.  相似文献

It has been suggested that concern about body image and weight gain is of particular relevance to women taking up smoking. In this article we present findings from a cohort study among 646 adolescents between the ages of 15 and 18 years. The results indicate that boys start smoking later than girls, and that concern about weight gain, body image and dieting plays a more important role in girls' than in boys' smoking habits at the age of 15. These factors are unrelated to boys' smoking habits at the age of 18, whereas they contribute significantly in predicting girls' smoking habits three years later. Furthermore, the results show that among girls, those who smoke are more concerned about gaining weight. Our findings emphasize the importance of implementing smoking prevention programmes at different age levels, using different motivating factors for boys and girls.  相似文献

This purpose of this article is to explore differences by gender and school grade in patterns of association among social influences and tobacco use. Data from the 1999 (N = 15,038) and 2000 (N 35,828) National Youth Tobacco Survey (American Legacy Foundation, 1999, 2000), a nationally representative, repeated cross-sectional survey, were used in the analysis. The authors compared effects on adolescent smoking. Direct paths from social environment to current smoking increased from middle school to high school. Indirect paths with social image mediating this relationship revealed a smaller increase. The pattern was constant across subsamples. Social image of smokers mediated the influence of social environment on adolescent smoking. Social image had a greater effect on smoking among middle school boys and high school girls. This study examined how parenting factors were associated with adolescent problem behaviors among urban minority youth and to what extent these relationships were moderated by family structure and gender. 228 6th grade students reported how often they use alcohol, smoke cigarettes, or engage in aggressive or delinquent behaviors; a parent or guardian reported their monitoring and other parenting practices. Findings indicated that boys and those from single-parent families engaged in the highest rates of problem behavior. More parental monitoring was associated with less delinquency overall, as well as less drinking in boys only. Eating family dinners together was associated with less aggression overall, as well as less delinquency in youth from single-parent families and in girls. Unsupervised time at home alone was associated with more smoking for girls only. Implications for prevention interventions are discussed. PURPOSE: To determine if ethnic and gender differences in smoking (lifetime prevalence and 30-day prevalence) exist among a cohort of Asian, black, Hispanic, and white inner-city adolescents during the 3-year middle school period. METHODS: Students in 22 urban schools completed self-report questionnaires and provided carbon monoxide breath samples at three annual assessments. Chi-square analyses were conducted to test for associations between ethnic group (Asian, black, Hispanic, and white) and smoking and to test for gender differences in smoking within each ethnic group. Additional analyses examined differences in smoking between two Hispanic subgroups (Puerto Rican and Dominican). RESULTS: Ethnicity was associated with lifetime smoking prevalence at all three assessment points but was only associated with 30-day smoking prevalence at the 2-year follow-up. However, there were no differences in smoking between Puerto Rican and Dominican youth. Black girls reported higher lifetime smoking prevalence than black boys at all three assessments. At the 2-year follow-up, Asian boys reported higher lifetime smoking prevalence than Asian girls; Hispanic girls reported higher 30-day prevalence than Hispanic boys. CONCLUSIONS: White and Hispanic adolescents were at higher risk for smoking relative to Asian and black adolescents. With the exception of white youth, gender differences were found within each ethnic group.  相似文献

This study was an examination of the possible mechanisms of risk among adolescents (n?=?55) exposed to the stress associated with the diagnosis of cancer in a parent. Girls whose mothers had cancer reported significantly more anxious-depressed symptoms than girls whose fathers were ill or boys whose mothers or fathers had cancer. Increased family responsibilities and the use of ruminative coping were examined as possible mechanisms leading to increased distress in girls with ill mothers. Although girls reported the use of more ruminative coping, rumination did not account for the impact of maternal cancer on girls' distress. Girls whose mothers were ill reported more stressful events reflecting family responsibilities. Furthermore, family responsibility stress fully accounted for the interaction of gender of the ill parent and gender of the adolescent in predicting anxious-depressed symptoms. In a cohort study of 1080 pupils who were followed for 5 years from when they left compulsory school (from age 16 to age 21 years), smoking habits were found to correlate with unemployment among both boys and girls. Pupils who were smokers in school had a higher risk of becoming unemployed than non-smokers. Irrespective of early smoking, smoking habits developed more unfavourably among unemployed young people than among those with no unemployment during the period studied. The odds ratio of being a smoker at the age of 21 years when unemployed more than 20 weeks during the observation period, compared with those without or with short unemployment, was 2.44 for men and 3.45 for women. When adjusted for the influence of socio-economic background, education, economy and smoking habits at the start of the period, the odds ratio was 1.7 (95% CI 1.01-2.86) for men and 2.0 (1.13-3.53) for women. The adjusted odds ratio for increasing or starting smoking during the period was 1.5 (95% CI 0.89-2.56) for men and 2.0 (1.18-3.35) for women. No significant correlation was found between snuffing and unemployment. Thus, it seems that unemployment is a risk factor for development of tobacco smoking in young people, especially among women.  相似文献

PURPOSE: The purpose of this study was to identify factors protective against the adverse health correlates of sexual abuse in reservation-based American Indian and Alaskan Native adolescents. METHODS: Data were taken from the National American Indian Adolescent Health Survey administered in 1988-1990 to 13,923 youths. Included in this analysis were 991 females and 166 males who reported a history of sexual abuse. Chi-square analysis was used to identify significant protective factors in sexually abused youths who did not report suicidality or hopelessness. Discriminant function analysis was used to determine which factors distinguished this group from those who experienced adverse health correlates. RESULTS: Separate multivariate analyses for boys and girls demonstrated that for girls, family attention, positive feelings toward school, parental expectations, and caring exhibited by family, adults, and tribal leaders were associated with absence of suicidality and hopelessness. For suicidality in boys, significant protective factors were enjoyment of school, involvement in traditional activities, strong academic performance, and caring exhibited by family, adults, school people, and tribal leaders. No significant protective factors against hopelessness were identified for boys. CONCLUSIONS: To minimize hopelessness and suicidal involvement among youth who have been sexually abused, strategies should be planned, implemented, and evaluated that support family caring and connectedness, strengthen school attachment and performance, and improve tribal connectedness.  相似文献

Prevalence of smoking was studied in two secondary schools in Zagreb. Data were collected from 637 pupils by questionnaires. Depending on school, between 31 to 45% pupils smoke. Different trends were observed in boys and girls. In boys from the families of a higher parental education prevalence is 28%; in those from lower 42% (p < 0.02). Smoking in girls still increases: in both populations girls surpassed boys (p < 0.001): observed frequencies are 47% in lower and 40% in a group with higher parental education. Higher frequency of smoking was observed in smoking families (p < 0.001). A majority of smokers begin smoking in the last grade of primary school, mostly because of peer group pressure and curiosity. Approximately 50% of regular smokers smoke more than half a pack daily. They comprise 12% of the surveyed population. Smokers do not assign low risk to smoking any more, but knowledge about diseases connected with smoking is insufficient. To reduce smoking among adolescents it is necessary to create negative attitude toward smoking and social-resistance skills during primary school education.  相似文献

Used a longitudinal design to test the main and stress-buffering effects of instrumentality on measures of trait anxiety, depression, and self-esteem. Ss were early (junior high school) and middle (senior high school) adolescent boys and girls. The hypothesis that instrumentality would be negatively related to psychological distress was supported by the cross-sectional and longitudinal regression analyses of every subgroup of Ss. The hypothesized stress-buffering effect of instrumentality was found in the cross-sectional and longitudinal analyses of senior high girls, specifically in the context of interpersonal negative events (relationship stress). The analyses for junior high girls revealed that instrumentality was beneficial only at a low level of relationship stress. There were no significant interaction effects in the regression analyses of boys. The findings suggest that, when experiencing relationship stress, instrumentality is helpful for middle adolescent girls but not for early adolescent girls. Therefore, future research on adolescents' life-stress adjustment should consider the roles of gender, age, personality, and type of life stress. The objectives are to describe smoking habits among 11-15-year olds in Denmark in the period 1984-1994 and to describe the association between smoking and social background factors. The article reports data about children at the age of 11, 13 and 15 years from four cross-sectional studies in 1984-1985, 1988, 1991 and 1994 (n = 678, 1667, 1860 and 4046) based on stratified random samples of schools. Data were collected by standardized questionnaires. Median age for first smoking experience changed from approx 12 years in 1984-1985 to approx. 14 years in 1994. The proportion of smokers among 15 year-olds is almost unchanged from 1984 to 1994. In 1994, the proportion of smokers is respectively 2% and 4% among 11 year-old girls and boys, 16% and 11% among 13 year-old girls and boys, and 33% and 25% among 15 year-old girls and boys. The proportion of girls and boys in the three age groups respectively who smoke every day is 0%, 1%, 3%, 4%, 17%, and 11%. In conclusion, smoking habits among children changed considerably from the 1950's to 1980's. The smoking habits have not changed much since the 1980's apart from a slight delay in first smoking-experience.  相似文献

Sex differences in predictors of smoking cessation were investigated among 337 male and 490 female participants in the RAND adolescent panel study. Participants reported smoking at least 11–20 times during the past year at Grade 10, with cessation defined as not smoking during the past year at Grade 12. Controlling for demographics, sex-specific analyses indicated that girls who quit smoking within 2 years had friends who smoked less frequently, perceived less parental approval of their smoking, had weaker intentions to continue smoking, used marijuana less frequently, attended fewer different schools, were more likely to have an intact nuclear family, experienced greater peer support, and rated themselves as healthier. Similar analyses for boys yielded results that were generally weaker and nonsignificant, with smoking quantity accounting for several associations in the sex-specific models. Despite these differences, interaction tests revealed significant sex differences for only three predictors. This study examined the relationship between smoking and participation in unhealthy behaviors among Mexican-American adolescents through a secondary analysis of national data. Mexican-American adolescents (N = 580), ages 10 through 18 years who were interviewed as part of the 1993 Teenage Attitudes and Practices Survey (TAPS II), were selected for analysis. Data collected included smoking status of the adolescent and participation in certain unhealthy behaviors. Among girls in the study, smokers were more likely to not wear a seat belt, be involved in physical fighting, not be involved in organized sports, perform poorly in school, say they like to do risky things, and ride in a car with a drunk or high driver. For boys, smoking was significantly associated with liking to do risky things, fighting, not attending church, and poor academic performance. These results suggest that Mexican-American adolescents who smoke may be at higher risk for engaging in behaviors that could compromise their health and safety, and for not being involved in activities that may exert a protective influence.  相似文献

The moderating influences of family structure and parent–adolescent distress on the relationship between peer variables and drug use were examined in a predominantly African American sample of 630 10th graders at 9 urban high schools. Both peer pressure and peer drug use were significantly related to the reported frequency of drug use. The relationship between peer pressure and drug use was stronger among girls than boys, and also among adolescents in families without fathers or stepfathers. The association between peer pressure and drug use also increased as a function of the level of mother–adolescent distress among adolescents who were not living with fathers or stepfathers. Neither gender nor family structure moderated the relationship between peer drug models and drug use. However, the association between peer drug models and drug use increased as a function of the level of mother–adolescent distress. As adolescence is a critical period of development, and as boys are less inclined than girls to approach the school facility for adolescent counselling, segregated consulting hours were introduced for boys to attract those with problems. The frequency of consultations by boys increased by 25 per cent, and 70 per cent of the boys reported a preference for the segregated consulting hours; 75 per cent appreciated the absence of girls from the waiting room; and of the 42 per cent with special preferences regarding the gender of the staff encountered, half reported preferring a man. Most of the boys presented with defined problems, though many revealed other problems, often relating to sexuality, in the course of consultation. The availability of segregated consulting hours for boys with adolescent problems is important, and often the only way to reach young boys who need help.  相似文献

Community and gender differences in body image were examined in a randomly selected sample of 284 young adolescent boys and girls from 2 middle-class communities. Data gathered from interviews and questionnaires supported hypothesized Community?×?Sex interactions and sex differences. Specifically, girls' body image and weight satisfaction were higher in 1 community and lower in the other. Demonstrating no community differences, boys reported more positive body image and satisfaction with weight and perceived themselves to be more average in weight than did the girls. Community differences were found in girls' participation in school-related activities, satisfaction with athletic strength, and degree of perceived school cliquishness. When these variables were covaried in several analyses of covariance, community and sex differences in body and weight satisfaction were eliminated or reduced considerably. This study investigated differences in metamotivational dominance among adolescents who engaged in different amounts of substance use, specifically, whether students who labeled themselves as 'heavy' alcohol consumers or cigarette smokers would be more Paratelic and Negativism Dominant than those who labeled themselves 'nondrinkers' or 'nonsmokers'. 93 secondary school students (Years 9 and 10) participated in the study (68 boys, 25 girls). One-way analyses of variance indicated self-labeled heavy drinkers scored significantly more Paratelic Dominant than nondrinkers, while heavy smokers scored significantly more Paratelic and Negativism Dominant than nonsmokers. Paratelic Dominant adolescents who reported engaging in heavy smoking may have done so because smoking is defined by society as a risky behaviour and so provides arousal. Education and preventive programs that focus on health hazards of smoking may, in fact, increase the likelihood of this behaviour by both individuals who score Paratelic and Negativism Dominant.  相似文献

The purpose of this research was to examine the role that young children's same-sex peer interactions play in influencing early school competence. The authors also examined the degree to which effortful control (EC) moderated these relations. The same-sex play preferences of 98 young children (50 boys and 48 girls; mean age = 54.77 months) were observed during the fall semester. At the end of the fall semester, one set of teachers reported on children's EC, and at the end of the following spring semester, another set reported on children's school competence (social, academic, and perceptual-motor). Results revealed that EC moderated the relations of children's same-sex play to their school competence. These patterns differed for boys and girls such that same-sex play was positively related to school outcomes for boys high in EC and for girls low in EC.
