Examined whether recall is disproportionately disrupted by amnesia compared with recognition, using 7 amnesics (mean age 51 yrs) without a history of alcoholism, 9 amnesic alcoholic Korsakoff syndrome patients (mean age 66 yrs), and 9 controls (mean age 53.2 yrs). It was postulated that if amnesia affects memory uniformly across different direct memory measures, recall of normal controls should not differ from the recall of amnesics when recognition scores of these 2 groups are equated. On the other hand, if recall is disproportionately disrupted, normal recall should be superior to amnesic recall even when recognition is equated. In the present study, amnesic recognition was equated with that of controls by providing amnesics with 8 sec of study time and normal Ss with 0.5 sec. Normal recall was superior to amnesic recall even when no differences were found in recognition. The results further specify the selective nature of amnesia. A core feature of dissociative identity disorder (DID) is the amnesia that exists between personalities. This study investigated autobiographical memory in a patient, HS, prior to and after her diagnosis with DID. This diagnosis was associated with increased recall of traumatic memories that were reported by a child personality. The child personality was able to recognize only half of the memories reported by the host personality. HS's responses were dissimilar to responses of control and nonexperiment participants. These findings suggest that DID is associated with alterations in autobiographical memory and that memories differ across personalities. The authors examined whether perception of emotional stimuli is normal in amnesia and whether emotional arousal has the same enhancing effect on memory in amnesic patients as it has in healthy controls. Forty standardized color pictures were presented while participants rated each picture according to emotional intensity (arousal) and pleasantness (valence). An immediate free-recall test was given for the pictures, followed by a yes-no recognition test. Arousal and valence ratings were highly similar among the amnesic patients and controls. Emotional arousal (regardless of valence) enhanced both recall and recognition of the pictures, and this enhancement was proportional for amnesic patients and controls. To examine the relationship between recall and recognition memory in amnesia, the authors conducted 2 experiments in which recognition memory was equated between patients with amnesia and control participants. It was then determined whether recall was also similar across groups. In Experiment 1, recognition was equated by providing amnesic patients with additional study exposures; in Experiment 2, recognition was equated by testing controls following a longer delay. These different methods of equating recognition across groups led to divergent results because amnesic patients' recall performance was lower than controls' recall performance in Experiment 1 but not in Experiment 2. These findings are accounted for by considering the differential contribution of recollection and familiarity to the performance of amnesic patients and controls in the 2 experiments. The neuropsychological performance of five patients with an anterior communicating artery (ACoA) syndrome (amnesia, confabulation and personality changes) was studied. Neuroimaging techniques revealed a basal forebrain and frontal lobe pathology in all patients. The limbic system appeared intact. There was no evidence for an intellectual deterioration relative to the estimated premorbid IQ in four patients. Regarding attention, all patients showed significant deficits. Visuospatial disabilities could not be observed. On tests of executive functioning, all patients did exhibit severe problems. Every patient displayed a profound amnesic syndrome. A retrograde amnesia could be documented and was characterised by a temporal gradient. Short-term memory appeared normal. Concerning long-term memory, all patients scored out of the normal range on total immediate recall tasks. Four patients showed a normal recognition performance but produced a large number of false alarms. Despite a normal recognition performance, they were impaired in delayed recall tasks. However, one patient showed a full-blown amnesic syndrome, because his delayed recall and recognition of learned items were both depressed. Our results with regard to long-term memory functioning support the hypotheses which assume (1) the existence of recognition superiority and pathological false recognition, and (2) basal forebrain amnesia in ACoA patients.  相似文献

The hippocampus and frontal lobes both contribute to episodic memory performance. In the present study, the authors evaluated the relative contributions of hippocampus, frontal lobes, anterior temporal cortex, and posterior cortex to memory performance in neurodegenerative patients and normal older controls. Subjects (n = 42) were studied with structural MRI and a memory paradigm that measured delayed recall, semantic clustering during recall, recognition discriminability, and recognition response bias. Data were analyzed with multiple regression. Consistent with the authors' hypotheses, hippocampal volumes were the best predictor of delayed recall and recognition discriminability, whereas frontal volumes were the best predictor of semantic clustering and response bias. Smaller frontal volumes were associated with less semantic clustering during recall and a more liberal response bias. Results indicate that hippocampal and frontal contributions to episodic memory can be dissociated, with the hippocampus more important for memory accuracy, and frontal structures more important for strategic processing and decision making. The relationship between recall and recognition has been a central topic for the study of memory. A test of alternative views about recall and recognition was arranged by studying amnesic patients. In amnesia, damage has occurred to a brain system important for declarative (conscious) memory, but skill learning, priming, and other forms of nonconscious memory are intact. Recall and recognition were found to be proportionately impaired in amnesic patients, and confidence ratings for the recognition judgments were commensurate with the level of impaired performance. The results are contrary to views that either recognition memory or associated confidence judgments are ordinarily supported significantly by nonconscious memory. The results favor the view that recall and recognition are related functions of declarative memory and equivalently dependent on the brain system damaged in amnesia. Controversy surrounding dissociative identity disorder (DID) has focused on conflicting findings regarding the validity and nature of interidentity amnesia, illustrating the need for objective methods of examining amnesia that can discriminate between explicit and implicit memory transfer. In the present study, the authors used a cross-modal manipulation designed to mitigate implicit memory effects. Explicit memory transfer between identities was examined in 7 DID participants and 34 matched control participants. After words were presented to one identity auditorily, the authors tested another identity for memory of those words in the visual modality using an exclusion paradigm. Despite self-reported interidentity amnesia, memory for experimental stimuli transferred between identities. DID patients showed no superior ability to compartmentalize information, as would be expected with interidentity amnesia. The cross-modal nature of the test makes it unlikely that memory transfer was implicit. These findings demonstrate that subjective reports of interidentity amnesia are not necessarily corroborated by objective tests of explicit memory transfer. This study examined the effects of repetition and spacing of repetitions on amnesia patients' recognition and recall of a list of words. Like controls, amnesia patients recognized items better when repetitions were spaced compared with when they were massed. This finding was attributed to the additional rehearsal that distributed presentations typically encourage. Amnesia patients also showed normal spacing effects in a recall task, suggesting that they were able to benefit from the variable encoding that spaced repetitions allow to establish additional retrieval cues. However, even though instructions to encode repeated items in a variable manner enhanced massed presentations to the point where spacing no longer produced an advantage for the normal controls, it did not have a similar effect for the amnesia patients. This led to the conclusion that amnesia patients cannot take advantage of strategically provided opportunities to enhance their variable encoding of interitem associations. Instead, it is suggested that the automatic activation of different aspects of items and interitem associations is responsible for the spacing effect in their recall. Whether individual differences in demographic, psychometric, and biological domains can predict episodic memory in dementia was investigated. Mildly to moderately demented very old persons performed episodic memory tasks (free recall and recognition of slowly and rapidly presented random words, free and cued recall of organizable words, and recognition of dated and contemporary famous faces). A factor analysis of the memory measures yielded 2 factors, 1 indexing recall and 1 recognition. Controlling for severity of dementia, only 2 predictors contributed to performance: (a) Block Design (a marker of fluid intelligence) was positively related to recall, and (b) age was negatively related to recognition. Although these results are similar to data reported on predictors of episodic memory in normal aging, (a) the number of predictive variables appears to be reduced in dementia, and (b) age seems to affect recall and recognition differentially in normal aging and dementia. Within the framework of the distinction between episodic and semantic memory, it has been argued that these two memory systems are organised in a hierarchical way. The hierarchical hypothesis assumes that episodic memory is a specific subsystem of semantic memory and therefore implies that episodic memory cannot exist without semantic memory. If this hypothesis is correct, it should be expected that patients with impaired semantic memory also have impaired episodic memory. In the present study, two experiments investigated the influence of semantic encoding on recognition memory performance in a population of 20 patients with Alzheimer's disease and 18 normal controls. Both experiments assessed recognition memory for semantically-related items. In Experiment 2, but not in Experiment 1, subjects were explicitly instructed to make a semantic association between the items. Alzheimer's disease patients were impaired, compared to the normal controls, on the recognition memory performance of both experiments. The ability to make a semantic association between two items was significantly and positively correlated with the subjects' performance on the recognition tasks. A further analysis showed that patients who were impaired on the semantic association task did significantly worse on the recognition task of Experiment 2 than normal controls and patients who were unimpaired on the semantic association task. These findings are discussed in the context of memory deficits in Alzheimer's disease, and are interpreted as supporting the view that episodic memory for an item is affected by the level of semantic awareness of that same item.  相似文献

This study investigated episodic memory and metamemory for verbs and nouns in patients who have cognitive impairments associated with Parkinson’s disease (PD). PD patients and healthy control participants were asked to recall word pairs and provide feeling-of-knowing (FOK) judgments for the items they were unable to recall. This was followed by a 4-alternative recognition test. PD patients were impaired in both recall and recognition, compared with controls. In terms of metamemory, PD patients were less confident in their ability to recognize the unrecalled items in a future recognition test. Most important, accuracy of PD patients’ FOK judgments was not above chance and was lower than that of control participants. The PD group correctly recognized fewer verbs than nouns, but type of material (verb vs. noun) had no impact on recall or FOK judgments. A large sample of chronic postconcussive patients with and without overt malingering signs was compared with objectively brain-injured patients on common episodic memory and malingered amnesia measures. Probable malingerers and traumatically brain-injured Ss were not differentiated on popular episodic recall tests. In contrast, probable malingerers performed poorly on the Rey 15-item, Rey Word Recognition List, Reliable Digit Span, Portland Digit Recognition Test, and Rey Auditory Verbal Learning Test recognition trial. These findings validated both commonly cited malingering measures and newly introduced methods of classifying malingering in real-world clinical samples. Studies have shown lower false recognition of semantically related lure words in patients with global amnesia than in matched controls. This pattern has been interpreted as suggesting that medial temporal and diencephalic structures that are damaged in amnesia and that contribute to veridical memory also contribute to false recognition. It has been argued that whereas controls form and retain a well-organized representation of the semantic gist of studied items, patients with amnesia can retain only a degraded gist representation. However, these studies are subject to an alternative interpretation involving greater source confusions in controls. The authors used a categorized-pictures paradigm to test recognition under conditions in which source confusions were unlikely to occur. Relative to controls, patients with amnesia showed reduced false recognition of categorically related pictorial lures, thereby supporting the notion of degraded gist representations in amnesia. Performance of patients with Korsakoff's syndrome and herpes encephalitis was compared on a retrograde amnesia (RA) test, asking subjects to recall and recognize the definitions of words that had come into the language at different time periods. Performance was also compared on a related test in which participants were asked to produce the words to definitions they were given in free recall and cued recall versions. It was hypothesized that, if the temporal gradient in remote memory results from a shift of information from episodic to semantic memory, then there should be a temporal gradient on these tasks, possibly steeper (i.e., greater relative sparing of early memories) in the patients in the Korsakoff group than in the herpes encephalitis group, who have widespread temporal lobe damage. Furthermore, in comparing semantic and episodic remote memory tests, consolidation theory would predict uniform temporal gradients across such tasks, whereas multiple trace theory would predict a differential pattern. The results showed that patients with Korsakoff's syndrome and patients with herpes encephalitis were significantly impaired across all time periods on the vocabulary tests, with only minimal evidence of temporal gradients, relative to healthy participants, and there was no evidence of a differential pattern of impairment between the two patient groups. Comparison with performance on measures of episodic retrograde amnesia, in which there was a differential pattern of temporal gradient, suggests that the relative preservation of early episodic remote memories in patients with Korsakoff's syndrome does not result from an episodic-to-semantic shift in the quality with which memories are stored. OBJECTIVES: To assess explicit memory and two components of implicit memory--that is, perceptual-verbal skill learning and lexical-semantic priming effects--as well as resting cerebral blood flow (CBF) and oxygen metabolism (CMRO2) during the acute phase of transient global amnesia. METHODS: In a 59 year old woman, whose amnestic episode fulfilled all current criteria for transient global amnesia, a neuropsychological protocol was administered, including word learning, story recall, categorical fluency, mirror reading, and word stem completion tasks. PET was performed using the (15)O steady state inhalation method, while the patient still exhibited severe anterograde amnesia and was interleaved with the cognitive tests. RESULTS: There was a clear cut dissociation between impaired long term episodic memory and preserved implicit memory for its two components. Categorical fluency was significantly altered, suggesting word retrieval strategy--rather than semantic memory--impairment. The PET study disclosed a reduced CMRO2 with relatively or fully preserved CBF in the left prefrontotemporal cortex and lentiform nucleus, and the reverse pattern over the left occipital cortex. CONCLUSIONS: The PET alterations with patchy CBF-CMRO2 uncoupling would be compatible with a migraine-like phenomenon and indicate that the isolated assessment of perfusion in transient global amnesia may be misleading. The pattern of metabolic depression, with sparing of the hippocampal area, is one among the distinct patterns of brain dysfunction that underlie the (apparently) uniform clinical presentation of transient global amnesia. The finding of a left prefrontal hypometabolism in the face of impaired episodic memory and altered verbal fluency would fit present day concepts from PET activation studies about the role of this area in episodic and semantic memory encoding/retrieval. Likewise, the changes affecting the lenticular nucleus but sparing the caudate would be consistent with the normal performance in perceptual-verbal skill learning. Finally, unaltered lexical-semantic priming effects, despite left temporal cortex hypometabolism, suggest that these processes are subserved by a more distributed neocortical network.  相似文献

Confirmatory factor analysis was used to test competing models of declarative memory. Data from middle-aged participants provided support for a model comprised of 2 2nd-order (episodic and semantic memory) and 4 1st-order (recall, recognition, fluency, and knowledge) factors. Extending this model across young-old and old-old participants established support for age invariance. Tests of group differences showed an age deficit in episodic memory that was more pronounced for recall than for recognition. For semantic memory, there was an increase in knowledge from middle to young-old age and thereafter a decrease. Overall, the results support the view that episodic memory is more age sensitive than semantic memory, but they also indicate that aging has differential effects within these 2 forms of memory. We investigated the ability of amnesic patients to learn new facts (e.g., Angel Falls is located in Venezuela) and also to remember where and when the facts were learned (i.e., source memory). To assess the susceptibility of fact and source memory to retrograde amnesia, patients prescribed electroconvulsive therapy were presented facts prior to the first treatment and were tested after their second treatment. All amnesic patients exhibited marked fact memory impairment. In addition, some amnesic patients exhibited source amnesia (i.e., they recalled a few facts but then could not remember where or when those facts had been learned). Source amnesia was unrelated to the severity of the memory deficit itself, because patients who exhibited source amnesia recalled as many facts as the patients who did not. These results show that the deficit in amnesia includes an impairment in acquiring and retaining new facts. Source amnesia can also occur, but it is dissociable from impaired recall and recognition and appears to reflect difficulty in remembering the specific context in which information is acquired. To investigate amnesia between identities in dissociative identity disorder (DID), the authors assessed explicit and implicit memory performance on a directed-forgetting task in 12 DID patients who switched from one state to an "amnesic" state between presentation and memory testing. DID patients were instructed either to remember or to forget neutral and emotional words. Besides an overall decrease in explicit memory, patients demonstrated selective forgetting of to-be-forgotten, but not of to-be-remembered words in the amnesic state. Patients did not exhibit any directed forgetting within the same state. Implicit memory was fully preserved across states. Independent of state, patients recalled more emotional than neutral information. These results may extend the conceptualization of memory processes in DID, suggesting an important role for retrieval inhibition.
