Prophylactic treatment of migraine with gamma-linolenic and alpha-linolenic acids

Polyunsaturated fatty acids (PUFA) were administered to 168 patients over a period of 6 months in an open-label uncontrolled study. In 129 patients available for study, 86% experienced reduction in severity, frequency and duration of migraine attacks, 22% became free of migraine and more than 90% had reduced nausea and vomiting. Self-medication changed to simple analgesics in the majority except in 14% of patients without improvement.     

Immunostimulatory DNA: sequence-dependent production of potentially harmful or useful cytokines

Certain bacterial immunostimulatory (i.s.) DNA sequences containing unmethylated CpG motifs stimulate antigen-presenting cells (APC) to express a full complement of costimulatory molecules and to produce cytokines including interleukin (IL)-12 and tumor necrosis factor (TNF)-alpha. While IL-12 is key to their T helper cell (Th)1-promoting adjuvant activity, secretion of toxic levels of TNF-alpha is harmful in that it promotes toxic shock. Given the beneficial as well as harmful consequences of i.s. DNA, we investigated the possibility of identifying DNA sequences, i.e. CpG oligodeoxynucleotides (ODN) which differentially activate IL-12 versus TNF-alpha cytokine production in APC. Here, we describe an i.s. DNA sequence with these characteristics. While its potential to induce IL-12 is preserved, its ability to trigger TNF-alpha release is strongly curtailed both in vitro and in vivo. I.s. DNA could be segregated into lethal and non-lethal in a mouse toxic shock model. The non-toxic i.s. DNA was useful as an adjuvant, thus allowing cytotoxic T cell responses to the soluble protein ovalbumin and conferring a resistant Th 1 phenotype to BALB/c mice lethally infected with Leishmania major. This i.s. CpG motif may thus be prototypic for a useful immunostimulating DNA sequence that lacks harmful side effects.     

Trusting the Electronic Edition

This paper discusses the contested role of images in electronic editions, and summarizes some of the chief arguments for their inclusion. I then argue that, to truly determine the importance of images to the function of electronic editions, we must understand the contribution the image makes to the form of textual knowledge provided by the edition. I suggest a distinction between editions which are primarily pedagogical in their aims, those which aim above all at scholarly authority, and those which attempt to provide textual information as high-quality data which can be analysed and processed. I conclude that the latter represents the most significant future trend in electronic editing. This revised version was published online in July 2006 with corrections to the Cover Date.     

A specific antidote for the treatment of digitalis poisoning in uremic patients

Two chronic haemodialyzed patients with digitalis intoxication are reported. One of them took digoxin 0.25 mg three times daily for an unknown period and the other took digitoxin 0.1 mg twice daily for two weeks. The symptoms of intoxication were mainly concealed by uremic syndrome. The diagnosis was established by noticed sinus bradycardia, first- and second-degree atrioventricular block in ECG and the determination of sera levels of glycosides (serum digoxin concentration was 7.36 ng/ml, serum digitoxin concentration was 46.5 ng/ml) in both cases. Considering the probable long elimination period of digitalis and the potentially life-threatening situation the patients were given digoxin-specific antibody (Fab) fragments with potassium replacement therapy. The symptoms disappeared within a few hours after therapy, side effects and rebound toxicity did not develop. In connection with these cases the aim of this report is to publish a method which can reverse the life-threatening digitalis intoxication in patients suffering from renal failure as well. As to the above method, the authors have not found any similar case reports in the Hungarian medical literature.     

Path integral Monte Carlo simulations of the melting of molecular hydrogen surfaces

We have used Path Integral Monte Carlo to study the surface melting of molecular hydrogen. Density profiles perpendicular and parallel to the bare H₂ surface are computed showing the formation of a liquid adlayer at 6 K, less than half the bulk melting temperature of para-hydrogen, 13.8 K. To estimate the onset temperature and depth of H₂ surface melting we determine the static structure factor within the individual H₂-layers for wave vectors in the plane and find no crystalline order down to 3 K in a partially filled H₂ adlayer at the free surface. We find quantum effects amplify the melting point depression at the free H₂ surface compared to bulk by a factor of five over classical Lennard-Jones solids and find that the zero-point fluctuations of molecules at the surface are much enhanced over their bulk values. We see vacancy formation in the solid before melting.     

Families of ergodic and exact one-dimensional maps

We give a parametrized family of rational interval maps of degree two, each ergodic, exact and preserving a measure equivalent to a Lebesgue measure. The family includes the unique quadratic Chebyshev polynomial as its only polynomial map. We extend the family to other settings on the circle and real line. We also give numerical approximations to the entropy of the equivalent invariant measure and the Hausdorff dimension of the singular measure of maximal entropy.     

Roles for Psychologists in End-of-Life Care: Emerging Models of Practice.

Professional psychologists are increasingly likely to encounter opportunities to work with patients and families facing end-of-life issues. Psychologists can provide psychological assessment, intervention for patients and families, consultation with and support of health care team members, grief therapy, and program development and evaluation. Psychological services are useful for healthy individuals who wish to make thoughtful plans about their own future care, patients with life-limiting illnesses, families stressed by providing end-of-life care, bereaved individuals, and health care providers who face issues of burnout and strain. Several challenges for psychologists working in end-of-life care are noted, including training, development of clear roles, and reimbursement. Exciton-polariton transition induced by elastic exciton-exciton collisions in ultrahigh quality AIGaAs alloys

The stationary and time-resolved polariton radiation in ultrahigh quality AIGaAs layers have been studied. It has been found that elastic exciton-exciton collisions lead to the appearance of a low-energy line of polariton radiation. We show that the rate of exciton-to-polariton transitions caused by elastic exciton-exciton collisions is determined not only by the density of the excitonic gas, but also by its temperature; this is in accordance with existing theoretical predictions. Allogeneic sibling umbilical-cord-blood transplantation in children with malignant and non-malignant disease

Allogeneic bone marrow transplantation is limited by the availability of suitable marrow donors and risk of graft-versus-host disease (GVHD) and opportunistic infection. In an attempt to ameliorate these limitations, umbilical cord blood has been postulated as an alternative source of allogeneic haemopoietic stem cells for transplantation. From September, 1994, umbilical cord blood from sibling donors has been used to reconstitute haemapoiesis in 44 children with acquired or congenital lympho-haemapoietic disorders, neuroblastoma, or metabolic diseases. Patients who had HLA-identical and HLA-1 antigen disparate grafts, had a probability of engraftment at 50 days after transplantation of 85%. No patient had late graft failure. The probability of grade II-IV GVHD at 100 days was 3% and the probability of chronic GVHD at one year was 6%. With a median follow-up of 1.6 years, the probability of survival for recipients of HLA-identical or HLA-1 antigen disparate grafts is 72%. We conclude that umbilical cord blood is a sufficient source of transplantable haemopoietic stem cells for children with HLA-identical or HLA-1 antigen disparate sibling donors with very low risk of acute or extensive chronic GVHD. The feasibility of umbilical-cord-blood transplantation with HLA-2 and HLA-3 antigen disparate sibling donors remains to be determined.
