目的研究肝癌门静脉支架植入术对门静脉压力的影响。方法12例肝癌门静脉主干癌栓患者依次接受食管胃底曲张静脉栓塞和门静脉支架植入治疗,分别测量曲张静脉栓塞前(P1)、栓塞后(P2)和支架植入后(P3)门静脉主干(PV)、未累及的门静脉分支(PVB)、脾静脉(SV)、肠系膜上静脉(SMV)压力并进行统计学分析。结果PV组P1为(45.17±2.25)cm H 2 O,P2为(48.33±2.20)cm H 2 O(P<0.01),P3为(39.33±2.44)cm H 2 O,与P2比较差异有显著性(P<0.01)。PVB组P1为(38.08±2.97)cm H 2 O,P2为(38.83±2.94)cm H 2 O(P>0.05),P3为(37.41±2.37)cm H 2 O,与P2比较差异无显著性(P>0.05)。SV组P1为(44.67±2.13)cm H 2 O,P2为(48.17±2.20)cm H 2 O,(P<0.01),P3为(41.67±2.20)cm H 2 O,与P2比较差异有显著性(P<0.01)。SMV组P1为(45.25±2.21)cm H 2 O,P2为(48.42±2.19)cm H 2 O(P<0.01),P3为(41.25±2.24)cm H 2 O,与P2比较差异有显著性(P<0.01)。结论门静脉主干癌栓患者食管胃底曲张静脉栓塞可使门静脉压力升高,门静脉支架植入可降低门静脉压力。 相似文献

应用颗粒堆积效率最大值原理阐述了优选钻井液中暂堵剂颗粒尺寸的"理想充填"理论和D90规则,提出了一种特别有利于对中、高渗储层实施暂堵和保护的新方法.依据该方法研制、开发的配套软件能够根据相关地层参数,快速、准确地对具有不同粒径的暂堵剂产品按一定比例进行合理的复配组合.试验结果表明,优选出的复配暂堵方案能有效地形成致密泥饼,与传统方法相比,岩心的渗透率恢复值明显提高,从而能有效阻止钻井液中固相颗粒和滤液侵入油气层.现场试验证实,应用该项技术可取得十分理想的保护储层的效果. 相似文献

We examined the effects of different cytokine combinations and culture conditions on the expansion and modulation of cell surface antigens of CD34+ derived dendritic cells (DCs), the most efficient antigen-presenting cells capable of stimulating resting T cells in the primary immune response. Cells with a dendritic morphology and expressing HLA-DR, CD1a, S100 and CD83 were maximally expanded under serum-free conditions with the addition of SCF, GM-CSF, TNF-alpha, TGF-beta and Flt-3 ligand (fold increase of CD1a+ cells = 102 +/- 32 after 2 weeks of culture). CD34+ cells were also grown under continuous flow conditions in an artificial capillary system: after 14d of culture, the expansion in the total cell number was lower than that of the static cultures (3.3 +/- 2 v 18.9 +/- 4) but the percentage of CD1a+/CD83+/ CD80+ cells was considerably higher, whereas the CD14+ cells were significantly reduced (8.9 +/- 2 v 26 +/- 13). In continuous perfusion cultures, low levels of DC precursors and of LTC-IC were still present up to day 14. The DCs generated under flow conditions stimulated the mixed leucocyte reaction (MLR) more than the cells grown in static cultures. By electron microscopy, cells grown in the continuous flow system showed an increased number of large cells with numerous dendritic processes and abundant multilamellar complexes. The cells expanded under these conditions were sorted on the basis of their light-scatter properties into two fractions: one containing a predominance of CD1a+/S100+/ CD8 3+/CD80+/CD14- 'large cells' with great internal complexity (mature DCs); the second including 'small cells' either CD33+/CD14+, CD33+/CD15+ or CD33+/CD13-/CD14. Effect of routine screening for Down's syndrome on the significance of isolated fetal hydronephrosis
OBJECTIVE: To determine the risk of Down's syndrome in fetuses with isolated hydronephrosis at 18-23 weeks in an unselected general population after routine screening for Down's syndrome, using first trimester nuchal translucency measurement and second trimester maternal serum biochemistry. POPULATION: All pregnant women undergoing a routine 18-23 week ultrasound scan, from a population who had been offered screening for Down's syndrome. SETTING: A district general hospital serving a low risk obstetric population. METHODS: Prospective study of all routine 18-23 weeks ultrasound scans. The prevalence of isolated hydronephrosis and Down's syndrome was determined and the relative risk for Down's syndrome was calculated for different ultrasound findings. RESULTS: 10,971 women were scanned at 18-23 weeks during the study period. Down's syndrome was diagnosed in 14 of 20 cases before this stage using first trimester nuchal translucency measurement and second trimester maternal serum biochemistry. Isolated fetal hydronephrosis was diagnosed in 423 pregnancies (3.9%); none of these pregnancies were affected by Down's syndrome. The relative risk for Down's syndrome was 0.18 (95% CI 0.06-0.53) for women with a normal scan (n = 9983). When multiple ultrasound markers were found (n = 565), the relative risk for Down's syndrome was 2.00 (95% CI 0.18-22.10) and 9.00 (95% CI 1.14-71.30) for all other aneuploidies. CONCLUSION: The finding of isolated fetal hydronephrosis does not significantly increase the age-related risk for Down's syndrome. The presence of multiple ultrasound markers is associated with an increased risk of aneuploidies other than Down's syndiome. These findings are explained by the reduced prevalence of Down's syndrome as a result of prior screening and diagnosis of this condition. 相似文献
