The fickle fates of push and pull in the dissemination of mental health programs for children.

As the demand for evidence-based programs has increased, considerable research has concentrated on the development and testing of innovative and promising prevention and treatment programs. However, a knowledge gap remains between research in children's mental health and the educational and health services providers who could use the programs produced. In contrast to the elaborate national system set up for monitoring and disseminating medications, the infrastructures to support the evaluation and dissemination of prevention and treatment programs targeting children's mental health have only begun to be developed. Based on my experiences in a community-based research project that has worked for the last decade to develop and evaluate the WITS (Walk away, Ignore, Talk it out, and Seek help) programs for the prevention of peer victimization, I illuminate obstacles to the dissemination of evidence-based prevention programs for children and solutions that are beginning to address these obstacles. In particular, I argue that enduring, integrated community, provincial and federal infrastructures are needed to improve program dissemination and to monitor their effects on children's well-being. Influences of gender and weight gain on short-term relapse to smoking in a cessation trial.

Few researchers have studied whether weight gain has an impact on short-term relapse to smoking. The authors of this study investigated predictors of relapse among 989 participants (60% women) in a randomized, double-blind, 10-week multicenter trial to determine the effect of fluoxetine (30 or 60 mg) versus placebo in combination with behavioral counseling for smoking cessation. Medication compliance and smoking status were biochemically verified. At Visit 2, participants were asked to set a quit date within the subsequent 2 visits. A proportional hazards regression model was used to predict risk of relapse within the first 3 months of quitting. Weight gain predicted relapse, but for men only. Female gender also predicted relapse. The results led the authors to question whether postcessation weight gain interventions should be restricted to women smokers. Psychology interns as patient suicide survivors: Incidence, impact, and recovery.

The authors investigated incidence, impact, and methods of coping with patient suicide during the training years of psychology graduate students. All 54 interns in clinical psychology at the Boston VA in 1983–1988 were surveyed. One in 6 Ss had experienced a patient's suicide at some time during their training. The group who experienced a patient's suicide and a group who experienced a patient's suicide attempt both reported high levels of stress on the Impact of Event Scale. Trainees with patient suicides reported stress levels equivalent to that found in patient samples with bereavement and higher than that found with professional clinicians who had patient suicides. Trainees most frequently turned to supervisors for both support and formulation of the suicide. Preparatory efforts at suicide education were found to be minimal and inadequate.
