This is a report of successful management for a far advanced, chemorefractory testicular cancer patient. A 29-year-old male was referred to our hospital for the treatment of progressive lung metastases with elevated hCG level, which had recurred after complete remission following 3 courses of BEP chemotherapy and progressed after transient partial regression following 2 courses of intensified EP chemotherapy. In addition, a 3 cm in diameter, solitary brain metastasis was detected on CT. First, we performed wedge resection of bilateral pulmonary lower lobe for chemorefractory pulmonary metastases. Histological examination revealed viable embryonal carcinoma identical to the primary one. Thereafter, whole brain irradiation in combination with VIP chemotherapy (etoposide 100 mg/m2, cisplatin 20 mg/m2 and ifosfamide 1200 mg/m2 daily for 5 consecutive days) was carried out to treat brain metastasis. By 2 cycles of VIP therapy and irradiation (36 Gy), partial tumor regression and normalization of hCG level were achieved, leading to salvage surgery of the brain metastasis which histologically proved to be necrosis. Following an additional cycle of VIP therapy, the patient has been free of recurrence 24 months after completion of the treatment.  相似文献

We have developed a blood cardioplegia delivery system for children. Essential points of a delivery system in pediatric cardiac surgery are (1) a small amount of priming volume of a delivery system, and (2) slow, steady infusion of a cardioplegic solution. We changed a heat exchanger to a smaller one for reduction of priming volume, and changed a roller pump tube to a smaller one for slow, steady infusion. Thus, priming volume of a delivery system has reduced from 180 to 100 ml, and we can infuse a cardioplegic solution at a steady rate less than 10 ml/min. Our clinical experience with this system suggests that this blood cardioplegia delivery system is useful for pediatric cardiac surgery.  相似文献

Starch characteristics such as gelatinization properties determine the quality of various products of rice. The eating, cooking and processing qualities of rice are a function of the gelatinization of rice starch. The gelatinization temperature (GT) of rice is controlled by the starch synthase IIa (SSIIa) gene. In the present study, the amount of the starch‐associated SSIIa protein and its relationship with amylopectin structure and GT were investigated. The results indicated that the starch associated SSIIa protein content was positively correlated with GT and level of amylopectin chains with degree of polymerization 12‐24 (DP12‐24), and negatively correlated with the fraction of chains with DP6‐11 and short chain ratio (SCR), i.e. DP6‐11/DP12‐24 (P<0.001). GT was also negatively correlated with DP6‐11 SCR, and positively correlated with DP12‐24. The SSIIa protein content and amylopectin structure showed significant difference between the two SSIIa single nucleotide polymorphisms (SNPs, i.e. guanine‐cytosine/thymine‐thymine (GC/TT)) groups. Rice with GC and TT SNPs had average SSIIa protein contents of 1.076 and 0.681, respectively. Rice with the GC SNPs displayed a lower amount of chains with DP6‐11 and more chains with DP 12‐24 than those with TT SNPs. A mechanism is postulated to explain how SSIIa could determine the distribution of amylopectin side‐chains, which affects the gelatinization temperature of rice starch.  相似文献

Methane, ethane, propane, and n-butane were polymerized in a plasma created by a radio-frequency glow discharge. It was found that the kinetics of polymer deposition were similar for all monomers but methane. It is suggested that the deviation from “normal” behavior of methane may be due to the difference in mechanism of formation of reaction intermediates. The “characteristic map” for the plasma polymerization of ethane was determined. It was shown that, in addition to transparent films, powders may be formed at low pressures and low monomer flow rates and unstable discharges at high pressures and low flow rates. With increasing power input, the unstable regions are decreased while the powdery regions are increased. The rates of polymer deposition were found to depend on pressure, flow rate, and power. An empirical equation is proposed that relates the rates of deposition for ethane, propane, and n-butane to these parameters.  相似文献

We propose a simple thin-film glucose biosensor based on a plasma-polymerized film. The film is deposited directly onto the substrate under dry conditions. The resulting films are extreme thin, adhere well onto the substrate (electrode), and have a highly cross-linked network structure and functional groups, such as amino groups, which enable a large amount of enzyme to be immobilized. Since this design allows fabrication through a dry process, with the exception of the enzyme immobilization, which is the last stage of the process, the chip fabrication can be designed as a full-wafer process to achieve mass production compatibility. The resulting sensors produced using this film are more reproducible, exhibit lower noise, and reduce the effect of interference to a greater degree than sensors made using conventional immobilization methods, e.g., via 3-(aminopropyl)triethoxysilane. The obtained film is a good interfacial design between enzyme and electrode; enzyme two-dimensionally locates very close to the electrode in a manner that is quite reproducible. Therefore, a wide dynamic range (up to 60 mM) and rapid response time (11.5+/-0.8 s) were obtained. Because of its highly cross-linking network structure, the amperometric response due to interferences such as ascorbic acid and acetaminophen was reduced by size discrimination of plasma-polymerized films.  相似文献

The goal of this study was to assess the occurrence of anniversary reactions in Gulf War veterans 2 years after the conclusion of Operation Desert Storm. Subjects were administered questionnaires and asked to identify specific months of best and worst functioning, and months of least or most symptoms of posttraumatic stress disorder (PTSD). Negatively experienced months were compared to documented dates of exposure to traumatic events during the war. Anniversary reactions occurred with a frequency greater than chance and were seen most in individuals exposed to a greater number, and to more severe types, of traumatic events. This suggests that anniversary reactions are etiologically linked to traumatic events and may be a part of the syndrome of PTSD.  相似文献

Water droplets placed on hydrophilic surface decrease the friction coefficient in the process of sliding over the hydrophobic surface. The droplets placed between these surfaces sustain loads of up to 170 mN and the friction coefficient for these surfaces is equal to 1.1 · 10−3. The low value of the friction coefficient is explained by the losses of energy for the adhesion hysteresis of water on the hydrophobic surface. In micromachines, the procedure of lubrication with water droplets efficiently decreases the friction coefficient due to the repulsion of water from the hydrophobic surface. The existence and distribution of T lymphocyte subpopulations in the mammary parenchymal tissue of cows were immunohistochemically detailed during early lactation. CD2+, CD4+, and CD8+ T lymphocytes were localized primarily in the mammary parenchymal tissue. CD8+ T lymphocytes were predominant over CD4+ T lymphocytes and occurred in close contact with the alveolar epithelium and between epithelial cells in the central area of the upper mammary gland. CD4+ T lymphocytes were present in equal numbers in the epithelial and connective tissue area. Occasionally, both CD4+ and CD8+ T lymphocytes formed cell clusters in the interalveolar connective tissue. The ratio of CD4+ T lymphocytes to CD8+ T lymphocytes was less than 1.0 and was lower in the epithelial area than in the connective tissue. The distribution of CD4+ and CD8+ T lymphocytes was similar in the parenchymal tissue of the gland cistern. The observation that there is a preferential presence of CD8+ T lymphocytes in the epithelial area of the bovine mammary gland during early lactation might indicate that these cells participate in the maintenance of the integrity of the epithelium.  相似文献
