A sizable part (~2%) of the human genome encodes for proteases. They are involved in many physiological processes, such as development, reproduction and inflammation, but also play a role in pathology. Mast cells (MC) contain a variety of MC specific proteases, the expression of which may differ between various MC subtypes. Amongst these proteases, chymase represents up to 25% of the total proteins in the MC and is released from cytoplasmic granules upon activation. Once secreted, it cleaves the targets in the local tissue environment, but may also act in lymph nodes infiltrated by MC, or systemically, when reaching the circulation during an inflammatory response. MC have been recognized as important components in the development of kidney disease. Based on this observation, MC chymase has gained interest following the discovery that it contributes to the angiotensin-converting enzyme’s independent generation of angiotensin II, an important inflammatory mediator in the development of kidney disease. Hence, progress regarding its role has been made based on studies using inhibitors but also on mice deficient in MC protease 4 (mMCP-4), the functional murine counterpart of human chymase. In this review, we discuss the role and actions of chymase in kidney disease. While initially believed to contribute to pathogenesis, the accumulated data favor a more subtle view, indicating that chymase may also have beneficial actions.  相似文献

Spasmodic Dysphonia is a voice disorder caused due to spasm of involuntary muscles in the voice box. These spasms can leads to breathy, soundless voice breaks, strangled voice by interrupting the opening of the vocal folds. There is no specific test for the diagnosis of spasmodic dysphonia. The cause of occurrence is unknown, there is no cure for the disorder, but treatments can improve the quality of voice. The main aim and objectives of the study are (i) to diagnose the dysphonia and to have comparative analysis on both continuous speech signal and sustained phonation /a/ by extracting the acoustic features. (ii) to extract the acoustic features by means of semi automated method using PRAAT software and automated method using FFT algorithm (ii) to classify the normal and spasmodic dysphonic patients using different classifiers such as Levenberg Marquardt Back propagation algorithm, K-Nearest Neighbor (KNN) and Support Vector Machine (SVM) based on sensitivity and accuracy. Thirty normal and thirty abnormal patients were considered in the proposed study. The performance of three different classifiers was studied and it was observed that SVM and KNN were 100% accurate, whereas Levinberg BPN network produced an accuracy of about 96.7%. The voice sample of dysphonia patients showed variations from the normal speech samples. Automated analysis method was able to detect dysphonia and provides better results compared to semi automated method.  相似文献

Infective spondylodiscitis (ISD) is a rare but potentially devastating condition in hemodialysis (HD) patients. Reports are limited especially in patients receiving high‐flux HD and hemodiafiltration (HDF). In a retrospective analysis, 13 patients on our maintenance high‐flux HD/HDF program were identified as having has infective spondylodiscitis over a 10‐year period (1997–2006), an incidence of approximately 1 episode every 215 patient‐years. The incidence was around 3 times higher in patients dialyzing with tunnelled central venous catheters (TCVC) than in those with arteriovenous fistulae. Affected patients were elderly (mean age 70 years) and had multiple comorbidities. Access problems, particularly TCVC infection, were common in the months preceding it's onset. Tunnelled central venous catheter removal during these episodes did not necessarily prevent it. Diagnosis was based on a history of back pain, raised C‐reactive protein, positive blood cultures, and characteristic magnetic resonance findings. Many patients were apyrexial and had normal white cell counts. In our patients on high‐flux HD/hemodiafiltration, its incidence appears comparable to that in conventional HD settings. No patients had infection with waterborne organisms. Blood cultures were positive in 77%. Gram‐positive organisms predominated, particularly Staphylococcus aureus. The major route of infection was hematogenous, with the most likely source the venous access. All received antibiotics for 6 to 12 weeks or until death. Only 2 patients underwent surgical drainage. Mortality was high (46%) and predicted by the development of complications, and by pre‐existing cardiovascular comorbidity. Prevention, using strategies to reduce the prevalence of bacteremia, including limiting the use of TCVC, should be an overriding aim.  相似文献

We propose an extension of the classical CSMA/CD protocol that eliminates its three main drawbacks. The new protocol, called dual‐mode CSMA/CD (CSMA/DM), operates in two modes, light mode and heavy mode. The light mode of CSMA/DM is almost the same as the original CSMA/CD protocol and is primarily used when the LAN load is light. The heavy mode is a collision free mode and is applied when the LAN load is heavy. The proposed modification to CSMA/CD is minimal while performance gain is significant. CSMA/DM automatically switches between its two modes based on the observed LAN load. Under heavy mode, the monitor station in a CSMA/DM LAN allocates bandwidth to those busy stations without collisions. The newly added priority scheme in the heavy mode allows stations to reserve bandwidth for their high priority frames. Compared with the current collision‐free fast Ethernet and Gigabit Ethernet, CSMA/DM has the advantage of not needing any extra switches or hubs while maintaining comparable performance. Its priority scheme provides more flexibility on bandwidth distribution than in fast Ethernet and Gigabit Ethernet. It can also be easily adapted for high‐speed wireless LANs. More importantly, CSMA/DM should be a good alternative of the widely used CSMA/CA (collision avoidance) in mobile ad hoc networks (MANET) and sensor networks. The validity of existing thermal comfort models is examined for upper primary school children in classroom settings. This is of importance to enhance productivity in the learning environment and to improve the control of artificial heating and cooling, including the potential for energy savings. To examine the thermal perceptions of children aged 10–12 years in non-air-conditioned classrooms, three sets of field experiments were conducted in boys’ and girls’ primary schools in Shiraz, Iran. These were undertaken during regular class sessions covering cool and warm conditions of the school year, polling responses from 1605 students. This paper illustrates the overall methods and reports the results of the warm season field survey (N?=?811). This investigation suggests that predicted mean vote-predicted percentage of dissatisfied (PMV/PPD) underestimates children's actual thermal sensation and percentage dissatisfied in the investigated classrooms. The analysis shows that sampled children may be slightly less sensitive to indoor temperature change than adults. The upper acceptable temperature derived from children's responses corresponding to mean thermal sensations of +0.85 is 26.5°C, which is about 1°C lower than the ASHRAE upper 80% acceptability limit. This implies that sampled children feel comfortable at lower temperatures than predicted by the ASHRAE Adaptive model during the warm season.  相似文献

Glaucoma is a multifactorial ocular disease. Ophthalmologists mostly use fundus or optical coherence tomography (OCT) for diagnosis of glaucoma. In this study, a hybrid computer-aided-diagnosis (H-CAD) system has been proposed that integrates both fundus and OCT imaging technologies for reliable diagnosis of glaucoma. Fundus module inspects the outer layer of eye's posterior part. It considers a variety of structural and textural features and makes a decision using support vector machine (SVM). In OCT module, the cup to disc ratio (CDR) has been computed by examining the internal layers of the retina. The cup contour has been extracted from inner-limiting-membrane (ILM) layer using a set of novel techniques for the calculation of cup diameter. Similarly, in the disc diameter calculation the retinal-pigment-epithelium (RPE) layer termination points have been identified by a number of innovative strategies to locate disc margin. Furthermore, a new criterion based on the mean value of RPE-layer end points has been proposed for the determination of cup edges. A local-dataset annotated by four ophthalmologists has been used for evaluation of proposed H-CAD system. The evaluations and results have shown that the final result of H_CAD system is more trustable than its contemporary automated models.  相似文献

ABSTRACT

Recent studies in primary schools highlight a need to develop the adaptive comfort model for schoolchildren in classrooms. This study investigates the application of the principal methods underlying the adaptive comfort theory for children relating thermal comfort indoors to the prevailing mean outdoor temperature. Children’s sensitivity to indoor temperature change are examined using data from a field study conducted in Iranian schools. This sensitivity is used to estimate the comfort temperatures in classroom situations with a minimum level of adaptation. Different metrics for the outdoor climate are employed to understand an expression of the climate which best predicts children’s comfort temperature. A sensitivity analysis is performed to derive the relation between indoor comfort and the climate that gives rise to the strongest correlation coefficient. Although the basic adaptive comfort relationships are applicable for children, the exponential method to calculate the prevailing mean with lower decay values leads to higher correlation with children’s comfort temperature. The slope of children’s comfort equation in relation to outdoor temperature is shown to be shallower than those of adults. Results indicate that children are more sensitive to temperature change within a single school day than across the overall survey period of several days.  相似文献

In this work, the problem of online secure state estimation and attack reconstruction in the face of offensives that corrupt the sensor measurements and modify the actuator commands of cyber–physical systems is investigated for designing a resilient controller for the system. The states of cyber–physical system and its actuator attacks are estimated/reconstructed online using a novel adaptive line-by-line super-twisting observer, whereas sparse stealth attacks on unprotected sensors are reconstructed using a sparse recovery algorithm. The estimated attacks are used for attack compensation by a resilient controller. The efficacy of the proposed technique is illustrated via simulation on a real electric power system under deception actuator attack and stealth sensor attack.  相似文献
