The posterior maxilla has traditionally been one of the most difficult areas to successfully place dental implants due to poor bone quality and close approximation to the maxillary sinus. Sinus augmentation procedures have become a viable means of assuring adequate bone for the placement of dental implants in this area. However, with the techniques currently employed, a considerable variation in the quality of bone attained with the sinus augmentation procedure exists. The purpose of this in vivo study was to evaluate the healing response and bone formation stimulated by 3 doses of recombinant human osteogenic protein-1 (rhOP-1), 0.25, 0.6, and 2.5 mg OP-1 per gram of collagen matrix; natural bone mineral; or collagen matrix alone (control) placed in the maxillary sinus of adult chimpanzees. Results were assessed using clinical, histologic, and radiographic techniques. Radiographic analysis of the computed tomography scans taken at 1 week, and 2.5, 4.5, and 6.5 months revealed a more rapid mineralization with the 2.5 mg OP-1/g collagen matrix and natural bone mineral treatment groups. The incremental bone mineral density (BMD) increase for these 2 treatments from 1 week to 2.5 months was over 2.5 times the increase found with the collagen matrix alone; these 2 treatments also had a higher BMD at the most superior slices evaluated when compared to the other 3 groups. Biopsy specimens were taken at 3.5, 5.5, and 7.5 months and for all 5 treatment groups bone formation was observed at all time points in the majority of the specimens. At 7.5 months the 2.5 and 0.6 mg OP-1/g collagen matrix treatment groups had an increase in the percent bone area when compared to the matrix alone control. In conclusion, these results demonstrate that sinus augmentation with natural bone mineral or 2.5 mg OP-1/g collagen matrix induce comparable radiographic and histologic evidence of bone formation and that both of these treatments performed superior to the control group of collagen matrix alone based upon all methods of evaluation.  相似文献

Exposure to child physical abuse and parents' domestic violence can subject youth to pervasive traumatic stress and can lead to posttraumatic stress disorder (PTSD). This article presents evolving conceptualizations in the burgeoning field of trauma related to family violence exposure and describes how the often repeating and ongoing nature of family violence exposure can complicate a PTSD diagnosis. In addition, recent literature indicates that children exposed to family violence may experience problems in multiple domains of functioning and may meet criteria for multiple disorders in addition to PTSD. Considerations salient to the recognition of traumatic stress in this population and that inform assessment and treatment planning are presented. The capability of high-quality mammography to discover early, preclinical breast cancer has demanded the development of a new relationship between radiologists, surgeons, and pathologists. This paper presents the results of a study of the effect of matrix yield strength, at a constant Widmanstätten α microstructure, on the nucleation and growth of voids in an α-β titanium alloy, CORONA-5, Ti-5Al-4.5Mo-l.5Cr. Four microstructures with a retained β matrix and involving coarse or fine Widmanstätten α particles in coarse or fine β grains were used in different heat treatment conditions resulting in yield strengths from 765 to 1018 MPa. Void nucleation occurred atα/α boundaries, grain boundary α/matrix interfaces, α twin/matrix interfaces, and α twin/untwin boundaries. The void nucleation strain varied from 0 to 0.26 and was a function of both microstructure and yield strength. Void growth rates increased with yield strength for all microstructures except for those containing coarse α which decreased with yield strength. Intense shear was observed in colonies of fine α structures and was considered to be the cause of the observed rapid void growth rates in the fine β+ fine α microstructures. Surface cracking occurred in several aged conditions during straining. This cracking was attributed to strain concentration in α.  相似文献

A series of single-crystal Al2O3-fiber-reinforced Ni3Al-based intermetallic matrix composites were fabricated by pressure casting. The matrices employed were binary Ni3Al, Ni3Al-0.5 at. pct Cr, and Ni3Al-0.34 at. pct Zr. The development of microstructure upon oxidation in air at either 1100 °C or 1200 °C was investigated by optical, scanning, and transmission electron microscopy. In air-oxidized binary Ni3Al, some of the fibers were fully or partially covered with a layer of oxide. A weak fiber/matrix bond in this system, which led to fiber debonding during composite processing, is believed to be responsible for the ingress of O into the composite and oxidation of the matrix in the debonded regions at the fiber/matrix interface. Addition of Cr to Ni3Al resulted in an almost threefold increase in fiber/matrix bond strength. No oxidation of the interface was observed. A thick layer of oxide was formed around all the fibers when the composite was thermally cycled prior to isothermal annealing. Addition of Zr to Ni3Al resulted in the formation of a layer of ZrO2 on the surface of the fibers during composite processing. The ZrO2 layer provided a fast path for the diffusion of O, which led to the formation of a rootlike oxide structure around the fibers. The rootlike structure consisted of a network of Al2O3-covered ZrO2.  相似文献

In response to a need to match drug users to the most appropriate and cost-effective level of care, it was hypothesized that socially anxious methadone-maintained patients would attain greater benefit from coping skills training provided in the context of a low-intensity enhanced standard methadone maintenance intervention (E-STD) than in the context of a high-intensity, socially demanding day treatment program (DTP). Social anxiety was assessed in 307 methadone-maintained patients using the Social Anxiety and Distress Scale prior to randomization to either E-STD or DTP. The hypothesis was supported: Socially anxious patients were drug free longer during treatment, were more likely to be abstinent at treatment completion, and had greater reductions in HIV risk behaviors if assigned to the lower intensity intervention, which was provided at ? the cost of the DTP. The study examines whether the link between the marital relationship and sibling interaction is direct or mediated by the mother-child relationship. Seventy-three same-sex siblings pairs aged 3 years 6 months to 8 years 6 months were observed during free play. Mothers completed questionnaires assessing marital functioning and their relationship with their 2 children. Results indicated that older siblings' negative behavior is linked with negative dimensions of the marital and the mother-child relationship, whereas younger siblings' negative behavior is linked with the mother-child and the differential mother-child relationship. Siblings; positive behavior, although linked with spacing, is not linked with positive dimensions of family interaction. Most important, the linkage between negative marital relations and older siblings; negative behavior was found to be mediated by maternal power assertion, thereby supporting the indirect model of negative family interaction.  相似文献
