Although thoracoabdominal injuries are uncommon in the athlete, they can be catastrophic if unrecognized or if diagnosis and treatment are delayed. This article reviews thoracic, intrathoracic, abdominal, and groin injuries in the athlete, and how they can be diagnosed and managed.  相似文献

The aim of this study was to determine whether low-field magnetic resonance (MR) imaging can safely and accurately depict inflammatory changes in patients with anal dynamic graciloplasty, in whom high-field MR imaging is contraindicated and ultrasonography and computed tomography are inadequate. A 0.2-T field-strength MR examination was performed in six patients with anal dynamic graciloplasty malfunction in whom reoperation was contemplated. The following sequences were applied:T 2-weighted turbo spinecho with fat saturation,T 1-weighted conventional spin-echo, and contrastenhancedT 1-weighted conventional spin-echo with fat saturation. Results indicated that none of the patients experienced relevant discomfort, pacemaker malfunction, or electrode dislocation with low-field MR imaging. Inflammatory pelvic changes were visualized in four patients and atrophy of the transposed gracilis muscle in another. Surgery was thus avoided in the four, who underwent conservative treatment for their pelvic inflammation. It was concluded that these prelininary results demonstrate the feasibility of MR imaging with a low field strength in patients with anal dynamic graciloplasty. In such patients, in whom diagnostic imaging had been problematic, the potential for safe and accurate visualization will be a boon to treatment planning.  相似文献

Robbins (1988) reported data that he viewed as inconsistent with Miller and Schachtman's (1985a) comparator hypothesis of conditioned response generation. Here we explain why we do not find his experiments a compelling test of the comparator hypothesis. We also briefly review other studies that tested the same predictions of the comparator hypothesis that Robbins examined. We conclude that there is considerable evidence that following excitatory or inhibitory conditioning with a target conditioned stimulus (CS) and unconditioned stimulus (US), extinction of other cues that were present during CS training ordinarily increases excitatory responding and decreases inhibitory responding to the CS. However, consistent with Robbins's conclusion, there is scant evidence that after CS–US training, enhancing the associative value of other cues that were present during CS training influences excitatory or inhibitory responding to the CS. The implications of these conclusions for the comparator hypothesis as an explanation of differences in acquired behavior and as a heuristic tool are considered. Using conditioned suppression of barpressing to investigate the stability of a conditioned stimulus–unconditioned stimulus (CS–UCS) association, the present authors gave 151 water-deprived rats either a few pairings of the CS with a strong footshock UCS or many pairings with a weak footshock UCS so that barpress suppression in response to CS was equated. Exp I established training parameters that yielded this equivalence. Specifically, rapid acquisition to a preasymptotic level of responding with strong shock produced suppression comparable to the asymptotic level reached more slowly with weak shock. Exp II showed that although equivalent performance was obtained from extensive conditioning with a weak shock or limited conditioning with a strong shock, only extensive conditioning with weak shock resulted in retarded acquisition of an association between that same CS and a footshock level perceived as midway between the 2 initial training shock intensities as implied by asymptotic performance in Exp I. Exp III demonstrated that the observed retardation in Ss given many conditioning trials with weak shock was CS-specific. It is concluded that the malleability of learned behavior is not simply a function of initial associative strength but is dependent on the path during initial acquisition. With the use of parameters intended to maximize the potential to observe hyperalgesia, the possibility was examined that hyperalgesia might be the immediate response to aversive stimulation, whereas analgesia is delayed (L. D. Matzel and R. R. Miller; see record 1987-33731-001). Consistent with the later prediction, analgesia in rats, as assessed by latency to paw lick in response to thermal stimulation, increased as a function of the delay between a tailshock (Exp 1) or footshock (Exp 3) and the test of pain sensitivity. However, in neither case was a hyperalgesic response observed at shock offset. In Exp 2, the strength of the analgesic response was found to increase as a direct function of both the time since the tailshock and tailshock intensity over the limited ranges examined, but no hyperalgesia was observed immediately after either low- or high-intensity shock. In Exp 4, the opiate antagonist naloxone was found to attenuate both a weak immediate and stronger delayed analgesia, suggesting a common underlying mechanism. In behaving Hermissenda, a preparatory conditioned response developed across repeated pairings of light (conditioned stimulus; CS) and rotation (unconditioned stimulus; US) with intertrial intervals (ITIs) of 60 and 120 s, but not 30 s. Likewise, contiguous in vitro stimulation of the visual and vestibular receptors, an analog of behavioral conditioning, resulted in an increase in the input resistance (i.e., excitability, a correlate of conditioning) of the B photoreceptors of the Hermissenda's eye, but only with ITIs greater than 60 s. Calcium signaling in the B cell, critical to the induction of this neuronal plasticity, was attenuated with shorter ITIs owing to (a) a reduction of the light-induced generator potential and hence voltage-dependent Ca2+ influx during the light CS, (b) a depression of the Ca2+ current that persisted throughout shorter ITIs, and (c) a steady-state inactivation of the Ca2+ current as a result of a sustained depolarization persisting from the previous trial. These results are consistent with a 2-process theory of associative learning in which a primary process (Ca2+ influx) may be opposed by a secondary process (depression of the Ca2+ current) during short ITIs. Conducted 2 experiments with 120 naive Sprague-Dawley rats to examine factors that contribute to retarded emergence of conditioned responding to a conditioned stimulus/stimuli (CS) trained in a context in which unsignaled unconditioned stimulus/stimuli (UCS) had previously been administered. In both experiments, water-deprived Ss were used in a conditioned lick suppression task to measure the conditioned response (CR) elicitation potential of the CS and the training context. From Exp I, it was determined that nonreinforced exposure to the excitatory context after UCS preexposure and prior to CS–UCS pairings in that context eliminated the CR deficit observed on a subsequent test of the CS. From Exp II, it was determined that the recovery induced by contextual deflation after CS training was specific to deflation of the context in which the CS was trained as opposed to another excitatory context. At two sites, microplots under winter wheat were given 140 kg N ha–1 as labelled ammonium nitrate split in 80 kg N ha–1 at tillering and 60 kg N ha–1 at shooting. Soil and plant samples were analyzed at shooting, after anthesis and at grain harvest and a15N balance was established. The average recovery rate of 95% indicates that there were no marked N losses due to leaching and denitrification, which is attributed to the low rainfall in the two months after fertilizer application. Between 19 and 23% of the fertilizer N remained in the 0–30 cm soil layer as organically bound soil N. Up to 64% was taken up by the above-ground crop. On the loamy sand, 4% of the fertilizer N at harvest remained in the roots in the 0–30 cm layer and only 3% was found as inorganic N in the 0–90 cm soil layer. The fertilizer N applied diminished plant uptake of soil N in the period between fertilizer application and harvest. As compared with the control, the fertilized plants extracted 25 and 28% less soil N from loamy sand and loess soil, respectively. The results show that application of mineral N fertilizer helps to maintain the mineralizable N content of the soil, which has been accumulated in the course of long-term intensive crop production, by adding N to the soil organic pool and simultaneously reducing the supply of soil N to the plants.  相似文献

Using Hermissenda as subjects, massed-trials training deficits were examined. Associative pairings of light and rotation induced a progressively greater conditioned foot contraction in response to light as the intertrial interval (ITI) was extended (up to 8 min). In contrast, a short ITI (30 s) produced no evidence of learning. In a corresponding in vitro conditioning experiment that mimicked training of the intact animal, facilitation of neuronal excitability in the animal's B photoreceptors paralleled the results obtained in vivo. Imaging of intracellular Ca2+ using Fura-2 indicated that Ca2+ levels remained elevated during short ITIs. This Ca2+ accumulation appears to induce activation of protein phosphatases because normal facilitation of the B photoreceptors was induced with a short ITI if training occurred in the presence of a phosphatase inhibitor. These results suggest that intracellular Ca2+ and protein phosphatases contribute interactively to the kinetics of memory formation and provide evidence that an accumulation of intracellular Ca2+ across training trials may impede memory formation. It has previously been reported that general learning ability (GLA) correlates positively with exploratory tendencies in individual outbred mice. This finding suggests the possibility that variations in stress reactivity modulate GLA and thus its relationship to exploratory tendencies. Here, the authors investigated the potential role of stress reactivity in regulating this relationship by assessing the effects of the anxiolytic chlorodiazepoxide (CDP; 10 mg/kg) on subjects' performance in a battery of diverse learning tasks as well as exploratory behaviors and stress reactivity. CDP-treated mice exhibited reductions in stress-induced corticosterone levels and behavioral reactivity to mild stressors and a corresponding increase in exploration. However, CDP-treated mice did not exhibit facilitated acquisition of any of the learning tasks and expressed GLA comparable to controls. Results indicate that although reduced stress reactivity promotes exploration, this does not translate into an up-regulation of GLA, suggesting that the relationship between GLA and exploration is not mediated by stress reactivity. The authors propose that variations in GLA reflect individuals' propensity for novelty seeking, whereas exploration reflects both stress reactivity and novelty seeking, the latter of which may underlie the relationship between exploration and GLA.
