Value of repeat angiography in patients with spontaneous subcortical hemorrhage

BACKGROUND and PURPOSE: Neuroradiological investigations do not disclose a source of bleeding in some patients with spontaneous subcortical hemorrhage. These patients may harbor undetected vascular malformations and may be at risk of rebleeding in the future. We investigated patients with subcortical hemorrhage with use of repeat angiography and MRI to determine the incidence of occult vascular malformations and the risk of bleeding during follow-up. METHODS: We reviewed a consecutive series of 137 patients with subcortical hemorrhage during a 10-year period (June 1987 through June 1997). If the patient was <65 years old and the first angiogram and/or MRI did not show a source of bleeding, repeat angiography was recommended. All angiographic and MRI studies were reviewed. The relationship between the identified bleeding source and clinical variables such as patient age, sex, and history of hypertension and the size and location of the hematoma were examined. RESULTS: One hundred seven patients (78%) underwent angiography on admission, 10 (7%) had immediate surgery for hematoma without angiography, and 20 (15%) had neither angiography nor surgery. Overall, an etiology for the hemorrhage was found in 55 cases (40%). Vascular malformations were common in young patients without preexisting hypertension. A second angiogram was obtained in 22 patients, and 4 arteriovenous malformations were demonstrated. Rebleeding at the site of the initial hemorrhage was not observed after a mean follow-up of 68 months. CONCLUSIONS: Angiography performed acutely after hemorrhage may not demonstrate vascular malformations. Consideration should be given to repeat angiography in patients who do not have a specific cause for hemorrhage.     

Real-time monitoring system of tool breakage using Kalman filtering

In this paper, a real-time monitoring system of tool breakage, which measures the cutting torque from the spindle motor current, is described. The system uses an auto-regressive (AR)-model which is adapted for the measured signal using the Kalman filtering technique. The tool breakage is detected by monitoring the residual, that is, the deviation of the observed signal from the estimated value obtained by the AR-model. Additional processing of the residual is introduced to discriminate the signature of tool breakage from the effect caused by the marked change in the cutting condition. To reduce the computation time, the fast calculation algorithm is adopted for estimating the coefficient vector of the AR-model. The monitoring experiment has proven the effectiveness of the system in a milling operation.     

Assessment of insulin resistance in acromegaly associated with diabetes mellitus before and after transsphenoidal adenomectomy

With a euglycemic hyperinsulinemic clamp method, whole-body insulin resistance was assessed in 6 cases with acromegaly associated with diabetes mellitus before and after transsphenoidal adenomectomy. The glucose infusion rate (GIR) correlated well with the plasma IGF-I level but poorly with that of GH. Further improvement in insulin sensitivity occurred 3-4 months after operation without substantial changes in plasma levels of both GH and IGF-I or glycemic control. These results indicate that GH excess can induce insulin resistance in association with plasma IGF-I and also through undefined secondary effect.     

Broiler-house environment monitoring system using sensor network and mail delivery system

In this paper, we propose the system that combines the sensor network with the mail delivery system to construct the system that observes an environmental change of the broiler-house. As a result of hearing of the producer, the environment system needs to able to be observed some broiler-houses, to inspect the summary data from the cellular phone, and to transmit the warning mail in a rapid temperature change. A basic part of the system is a sensor network by the sensor module that we developed. Only the sensor modules are put in the each broiler-house, and the network by wireless LAN communication is constructed, because the system needs to watch of two or more broiler-houses, and it is difficult to setup a large-scale system at the broiler-house. The always-connected high-speed Internet is preferable to accumulate, to process data, and to offer it to the user in a comprehensible form. But, it is difficult to build always-connected high-speed Internet at the chicken farm which is used by experiment. The server is set up in the remote place, and we propose the system that delivers data from the chicken farm with mail. The verification of the effectiveness of the proposed system and the problem are examined by actually setting up the system that proposes it in the broiler-house, and operating it. Galanin modulation of seizures and seizure modulation of hippocampal galanin in animal models of status epilepticus

We examined the role of hippocampal galanin in an animal model of status epilepticus (SE). Control rats showed abundant galanin-immunoreactive (Gal-IR) fibers in the dentate hilus, whereas no Gal-IR neurons were observed. Three hours after the onset of self-sustaining SE (SSSE), induced either by intermittent stimulation of the perforant path for 30 min (PPS) or by injection of lithium and pilocarpine, Gal-IR fibers disappeared in the hilus and remained absent for up to 1 week afterward. Twelve hours after the induction of SE by PPS or 3 hr after pilocarpine administration, Gal-IR neurons appeared in the hilus; these neurons increased in number after 1 d and gradually declined 3 and 7 d later. Galanin concentration in the hippocampus, measured by ELISA, significantly decreased on the plateau of SSSE and increased 24 hr after PPS. Galanin (0.05 nmol) injected into the hilus prevented the induction of SSSE, and 0.5 nmol of galanin stopped established SSSE. These effects were attenuated by galanin receptor antagonists (M35 > M40 >/= M15). 2-Ala-galanin (5 nmol), a putative agonist of galanin type 2 receptors, prevented but was unable to stop SSSE. M35 facilitated the development of SSSE when given before PPS. We suggest that hippocampal galanin acts as an endogenous anticonvulsant via galanin receptors. SE-induced galanin depletion in the hippocampus may contribute to the maintenance of seizure activity, whereas the increase of galanin concentration and the appearance of galanin-immunoreactive neurons may favor the cessation of SSSE. The seizure-protecting action of galanin SSSE opens new perspectives in the treatment of SE.     

Nanobiotechnology as an emerging research domain from nanotechnology: A bibliometric approach

Nanotechnology has been intensively investigated by bibliometric methods due to its technological importance and expected impacts on economic activity. However, there is less focus on nanobiotechnology, which is an emerging research domain in nanotechnology. In this paper, we study the current status of the former, with our primary focus being to reveal the structure and research domains in nanobiotechnology. We also examine country and institutional performance in nanobiotechnology. It emerged that nanostructures, drug delivery and biomedical applications, bio-imaging, and carbon nanotubes and biosensors are the major research domains, while the USA is the leading country, and China has also made substantial contribution. Most institutions having a major impact in the area of nanobiotechnology are located in the USA.     

Cytologic diagnosis of signet-ring cell carcinoma of the breast

OBJECTIVE: To examine the cytologic features of signet-ring cell carcinoma (SRCC), defined as carcinoma dominated by signet-ring cells, of the breast and to discuss problems that occur in cytodiagnosis. STUDY DESIGN: Five cases of SRCC of the breast were examined cytopathologically. Signet-ring cells were subclassified into intracytoplasmic lumina (ICL) type and non-ICL type. ICL type had large ICL containing mucin. Non-ICL-type cells had wide, amorphous cytoplasm diffusely dispersed with mucin. RESULTS: In cases 1 and 2, fine needle aspiration biopsy (FNAB) revealed many signet-ring cells (non-ICL type), suggesting SRCC. Histologic diagnoses were ductal SRCC containing many signet-ring cells (non-ICL type). In cases 3 and 4, signet-ring cells (ICL type) were found sporadically among carcinoma cells without signet-ring features. Signet-ring cells were not regarded as the major component of the cells; thus, the cytologic diagnoses were lobular carcinoma, not otherwise specified. Pathologic diagnoses were lobular SRCC. Signet-ring cells were mostly ICL type. In case 5, most carcinoma cells on the smears showed signet-ring features (non-ICL type), suggesting SRCC. The histologic diagnosis was lobular SRCC, and signet-ring cells were mostly non-ICL type. CONCLUSION: Ductal SRCC yielded more cellular smears as compared with lobular SRCC; therefore, cytologic diagnosis was easier in the former.     

Far infrared laser frequencies of CH3OD and N2H4

The frequencies of 26 laser lines with wavelengths between 57 and 534 μm have been measured in the optically pumped laser gases CH₃OD and N₂H₄. A pair of stabilized cw 12_{CO 2} lasers was used as a frequency standard for the heterodyne frequency measurements. Seven of the 26 lines are new.
