Pulmonary hypertension (PH), defined by a mean pulmonary arterial pressure (mPAP) greater than 20 mmHg, is characterized by increased pulmonary vascular resistance and decreased pulmonary arterial compliance. There are few measurable biomarkers of PH progression, but a conclusive diagnosis of the disease requires invasive right heart catheterization (RHC). Patient-specific cardiovascular systems-level computational models provide a potential non-invasive tool for determining additional indicators of disease severity. Using computational modelling, this study quantifies physiological parameters indicative of disease severity in nine PH patients. The model includes all four heart chambers, the pulmonary and systemic circulations. We consider two sets of calibration data: static (systolic and diastolic values) RHC data and a combination of static and continuous, time-series waveform data. We determine a subset of identifiable parameters for model calibration using sensitivity analyses and multi-start inference and perform posterior uncertainty quantification. Results show that additional waveform data enables accurate calibration of the right atrial reservoir and pump function across the PH cohort. Model outcomes, including stroke work and pulmonary resistance-compliance relations, reflect typical right heart dynamics in PH phenotypes. Lastly, we show that estimated parameters agree with previous, non-modelling studies, supporting this type of analysis in translational PH research.  相似文献

Most preventable deaths among hungry people take place outside of emergency contexts. In countries not involved in conflicts or natural disasters malnutrition is directly implicated in the deaths of millions of children and mothers each year. Thus, WFP's great efforts focused on saving lives in emergencies should be mirrored by efforts aimed at tackling malnutrition, and hence saving lives, beyond emergencies as well. While food sufficiency is not the same as good nutrition, food is nevertheless an important part of the nutrition equation. New scientific evidence confirms that it is possible to have positive nutritional impacts with food aid. Consistent with Strategic Priority No. 3, WFP seeks to use food resources to achieve nutritional impacts in three complementary ways: a) enhancing the effectiveness and impact of targeted mother and child health and nutrition interventions (MCHN) that combine food and appropriate nonfood inputs; b) enhancing the nutritional value of WFP food (for instance, through micronutrient fortification); and c) enhancing the nutritional impact of other WFP (non-MCHN) interventions. These approaches represent a mainstreaming of nutrition across WFP's activities. Adoption of evidence-based programming, joint interventions with partners, and new project designs offer the promise of greater WFP effectiveness and impact in the coming years.  相似文献

An ambitious program of High-Speed Railway (HSR) is under construction in China and already soon the network will overtake its counterparts in Europe. Reflecting on experiences in Europe, the authors explore the conditions for place making qualities of six new HSR station areas in China. The superb inter-city connectivity of the new infrastructure nodes raises a lot of expectations about the economic and social potential of the station areas, but the experiences in Europe thus far are very differentiated. With regards to the attraction of economic activities usually the existing differences between different economic regions are reproduced rather than shaping completely new conditions. Also the expectations of developing completely new integrated urban centers around stations in the urban periphery are rather ambitious. The paper questions the planning responses to HSR development in Chinese cities and investigates the institutional conditions that frame these responses. The authors explore how the plans for Chinese HSR station development account for their objectives and reflect on the potential of the station area planning by investigating the institutional parameters of urban development.  相似文献
