From hospital to municipal cogeneration systems: an Italian case study

A mixed integer linear programming model combined with a more traditional design by scenarios is proposed to optimize facilities size and operation mode of a municipal energy system involving significant civil centres and a hospital. Moving from the need of a new heat and power station for the local hospital due to the construction of new pavilions, the opportunity of involving other centres in the neighbourhood in a distributed cogeneration system is analysed, increasing system complexity step by step. Smaller cogeneration units tailored to hospital needs are rewarding ventures with relatively low risks but, in a country whose traditional power generation systems heavily rely on fossil fuels and where energy policy and market conditions can make it profitable to sell surplus power, district heating systems foster the installation of larger cogenerators and lead thereby to higher profits and to better performance as for primary energy savings and greenhouse gases emission reduction. A socio-ecological analysis of hospital resilience to extreme weather events

Hospitals play a critical role in helping communities respond effectively to extreme weather events (EWEs). Despite predictions of more EWEs, little is known about the process by which hospital infrastructure resilience to such events can be built. Using Gunderson and Holling’s Adaptive Cycle, a new theoretical perspective based on socio-ecological resilience theory is provided to understand this process. Data were collected using semi-structured interviews, observations of disaster drills and disaster planning meetings, as well as additional documentary analysis of past incident reports. The research findings were then refined and validated in a focus group meeting with respondents. The findings indicate that there are significant organizational barriers which prevent facilities managers improving the resilience of hospital facilities to future EWEs. It was found that the disaster planning process is ad hoc and non-inclusive, focused on man-made disasters and compliance driven, top-down approach, under-resourced and is driven by a general ignorance of the importance of resilient-built facilities to health care delivery during an EWE. It is concluded that to produce more resilient hospital infrastructure, there needs to be a more well-resourced, integrated and collaborative approach to disaster management planning which enables health facilities managers to play a more central role in disaster planning decisions. There also need to be better systems, technologies and training implemented to manage information about health infrastructure performance before, during and after EWEs.

Replies to a comment made by Paul Hauck (1959). The author uses Hauck's comment as an opportunity to examine some assumptions and assertions of various psychologists in and out of state hospital service. The author's main concern is that if psychologists confine themselves to tests and become too closely identified with them, they may suddenly find themselves without function in working with emotionally disturbed humans if it is finally decided that personality testing is invalid or irrelevant or both. 对我国“医药合谋”的内在机理进行了数理分析，结果表明：医疗机构可以从药品更高的回扣率中荻利，但是回扣率的提高会减少药商的利润，两者在回扣率上不存在共同利益。因此医疗机构不去要求药商提供更大的回扣率，而是与药商达成多开药、开高价药的“双赢”合谋模式。并在此基础上提出了进一步深化改革的建议。

Behavioral rehabilitation of the "treatment-refractory" schizophrenia patient: Conceptual foundations, interventions, and outcome data.

This article describes an intensive, inpatient behavioral rehabilitation program for patients with schizophrenia who have been considered "treatment-refractory" at state hospitals. The program is a public-private partnership involving state and private hospitals and community residence providers. The essential elements of this program are described, along with the conceptual and philosophical bases of its treatment and examples of staff behaviors critical to treatment success. Outcome data are then discussed to emphasize the point that when evidence-based psychological treatment is implemented with this population, outcomes can be positive in most cases, and therefore, the number of treatment-refractory patients is actually far less than is estimated on the basis of response to medication alone.
