血管紧张素转换酶和血管紧张素原基因多态性与冠心病相关性研究

摘    要:目的探讨血管紧张素转换酶和血管紧张素原基因多态性与冠心病的关系。方法选取270例江西籍汉族住院病人,根据造影结果分成冠心病组(至少有1支冠状动脉狭窄≥70%)161例,对照组(冠状动脉造影正常)109例。酚/氯仿/无水乙醇法提取基因组DNA。聚合酶链式反应(PCR)和基因突变聚合酶链反应(MS-PCR)技术及琼脂电泳检测ACEI/D、AGTM235T基因多态性。结果冠心病组ACEI/DDD基因型频率和D等位基因频率、AGTM235T TT基因型频率和T等位基因频率显著高于对照组(P〈0.05或P〈0.01)。Logistic回归分析冠心病的独立危险因素结果表明:ACE/DD和AGT/TT基因型是冠心病和心肌梗死的危险因子(P〈0.05)。含有TT和至少一个D等位基因的基因型组合较其他几种基因型组合,冠心病和心肌梗死发病的危险性更高(P〈0.05或P〈0.01)。结论 ACEI/DDD型和D等位基因、AGTM235T TT型和T等位基因是冠心病发病的危险因素。含有TT和至少一个D等位基因的基因型组合较其他几种基因型组合,冠心病和心肌梗死发病的危险性更高。

Abstract:Objective To explore the relationship between angiotensin-converting enzyme(ACE)and angiotensinogen(AGT)gene polymorphism and coronary artery disease(CAD).Methods A total of 270 Han Chinese patients from Jiangxi province who underwent coronary angiography(CAG)were divided into control group(normal CAG,109 patients)and CAD group(at least stenosis greater than or equal to 70% in one coronary artery,161 patients).DNA was extracted by phenol/chloroform/absolute alcohol methods.Polymorphysim of ACE I/D and AGT M235Twere examined by PCR,MS-PCR and Agar gel electrophoresis.Results DD genotype and D allele of ACE I/D,and TT genotype and T allele of AGT M235T in CAD group were more frequent than those in control group(P〈0.05or P〈0.01).Logistic regression analysis revealed that the DD and TT genotype were independent risk factors for CAD and myocardial infarction(MI)(P〈0.05).Compared with the control,the genotype combination including AGT 235 TT and at least one D increased in CAD group(P〈0.05 or P〈0.01).Conclusion DD genotype and D allele of ACE,and TT genotype and T allele of AGT may be the risk factors for CAD and MI.The combination of AGT 235TT and at least one D allele is more apt to develop CAD and MI than others.
