高效液相色谱法检测ICP脐血胆汁酸谱及妊娠结局

目的：探讨不同程度妊娠期肝内胆汁淤积症脐血胆汁酸谱及其妊娠结局。方法：高效液相色谱法检测妊娠期肝内胆汁淤积症脐血8种胆汁酸。结果：①对照组和轻度ICP组比较,脐血TCA差异有统计学意义（P〈0.05）;对照组和重度ICP组比较,脐血TCA、GCA和TCDCA差异均有统计学意义（P〈0.05）：轻度组和重度组间比较TCA、GCA、TCDCA差异有统计学意义（P〈0.05）。②对照组和轻度ICP组比较,ALT、AST差异均有统计学意义（P〈0.05）,而TBIL、DBIL没有差异;轻度ICP组和重度ICP组比较,ALT、AST、TBIL、DBIL差异均有统计学意义（P〈0.05）。③重度ICP组早产儿例数显著高于对照组,差异均有统计学意义（P〈0.05）。结论：不同程度的ICP母血肝功能指标和脐血胆汁酸谱都有明显的差异特征。

Bile acid profile of umbilical cord of intrahepatic cholestasis of pregnancy by high performance liquid chromatography and pregnancy outcome.

Objective：To investigate the bile acid profile of umbilical cord of intrahepatic cholestasis of pregnancy（ICP） and the pregnancy outcome.Methods：8 kinds of bile acids of umbilical cord of ICP were analyzed by high performance liquid chromatography.Results：（1）The taurocholic acid（TCA） concentration of the umbilical cord of the mild ICP group was significantly higher than the control group（P〈0.05）.The TCA,glycocholic acid（GCA） and taurochenodeoxycholic acid（TCDCA） concentration of the umbilical cord of the severe ICP group were significantly higher than the control group（P〈0.05）;the TCA,GCA and TCDCA concentration of the umbilical cord of the severe ICP group were significantly higher than the mild ICP group（P〈0.05）.（2） The alanine aminotransferase（ALT） and glutamic acid aminotransferase（AST） levels of the mild ICP group were significantly higher than the control group（P〈0.05）,while the total bilirubin（TBIL） and the direct bilirubin（DBIL） levels were not different between the two groups.The ALT,AST,TBIL and DBIL levels of the severe ICP group were significantly higher than the mild ICP group（P〈0.05）.（3） The cases of the preterm in the severe ICP group were significantly higher than the control group（P〈0.05）.Conclusion：The characteristics of the maternal liver function parameters and the newborn＇s bile acid profiles in the mild ICP group and the severe ICP were different.
