彩色多普勒超声对四肢血管栓塞疾病的诊断效果评价

目的:探讨四肢血管栓塞疾病患者应用彩色多普勒超声诊断的临床价值。方法:选取我院2015年10月-2018年8月期间就诊的136例疑似四肢血管栓塞患者为研究对象,均给予彩色多普勒超声检查,观察患者病变血栓位置、大小、阻塞程度及血流变化情况,以血管造影诊断为金标准,分析彩色多普勒超声诊断准确性。结果:136例患者经血管造影诊断均确诊为四肢血管栓塞疾病,上肢血管栓塞8例,下肢血管栓塞128例(其中静脉血管栓塞82例,动脉血管栓塞46例),彩色多普勒超声检查完全符合,诊断准确性为100.0%;血管栓塞阻塞程度分型:不完全性阻塞24例,完全性阻塞58例;位置不同分型:第Ⅰ型(腹股沟韧带远段型)血管栓塞62例,第Ⅱ型(腹股沟韧带近段型)血管栓塞16例,第Ⅲ型(混合型)血管栓塞4例。结论:在四肢血管栓塞疾病诊断中应用彩色多普勒超声,可实时、准确判定病情,具有操作简便、无创伤、重复性好等优势,值得临床应用与推荐。

Evaluation of Color Doppler Ultrasound in the Diagnosis of Vascular Embolism in Extremities

Objective: To evaluate the clinical value of color Doppler ultrasonography in the diagnosis of vascular embolism in extremities. Methods: 136 patients with suspected vascular embolism in extremities from October 2015 to August 2018 in our hospital were selected as study subjects. All patients were examined by color Doppler ultrasound to observe the location and size of thrombus. The degree of occlusion and the changes of blood flow were analyzed by using angiography as the gold standard and the diagnostic accuracy of color Doppler ultrasound was analyzed. Results: All the 136 patients were diagnosed as extremity vascular embolism by angiography, including 8 upper limb and 128 lower extremity vascular embolism(including 82 venous embolism, 46 Arterial embolism). The diagnostic accuracy of color Doppler ultrasonography was 100.0%. The degree of arterial embolization was classified as incomplete occlusion in 24 cases and complete occlusion in 58 cases. There were 62 cases of type Ⅰ(distal inguinal ligament), 16 cases of type Ⅱ(proximal segment of inguinal ligament) and 4 cases of type Ⅲ(mixed type). Conclusion: The application of color Doppler ultrasound in the diagnosis of extremities vascular embolism disease can be used in real time and accurately. It has the advantages of simple operation, no trauma and good reproducibility. It is worthy of clinical application and recommendation.
