Numerous hypospadias correction techniques were described in the literature. The technique varies according to the position of the meatus and the importance of the chordee. These new techniques and the care taken in the manipulation of the tissues tend to decrease complications like stenoses and fistulas, frequent in the long urethroplasty. The correction of hypospadias should conform to aesthetic and plastic surgery. More than 300 corrections were carried out in our service. Surgical technique varied in the long run. Currently, we choose as often as possible a correction in one time: release of the chordae and urethroplasty. Straightening of the penis is obtained on one hand by release of the cutaneous chordae and wide dissection of the hypoplastic urethral plate and one or more dorsal plications according to Nesbit are carried out if it proves necessary. In the distal, glandular and coronal forms, the correction is of type M.A.G.P.I. (meatal advancement, glanduloplasty). The complication rate is extremely low. The aesthetic and functional result is very satisfactory. In the proximal forms, Duckett technique is used only in the obligatory cases given the large number of complications. It is generally replaced by Onlay technique. The urethral plate is left in continuity and serves as support to the pedicled and vascularized flap. In the intermediate situations with a middle shaft hypospadias, Mathieu technique is again of application. The aesthetic and functional result of this type of surgery requires good knowledge and careful application of the technique adapted to each case.  相似文献

提出了一种基于服务请求的角色映射与策略集成机制来解决服务组合中的访问控制问题.为了准确有效地建立域间角色映射,提出了唯一可映射集(unique mapping set,UMS)的概念,通过求取和搜索对应局部域角色层次的UMS,可找到满足外部服务请求的最合适可映射角色集.为了解决由于角色映射造成的域间策略冲突,将角色映射关系划分成3种不同的模式,通过建立或转换不同的角色映射模式可有效避免循环继承、SoD约束违背等常见冲突.与其他方法相比,这种冲突消解方法不仅可以保证服务请求的可满足性,而且能较好地保护各参与域的自治性和安全性.  相似文献

Portland cement concrete is presented as the most widely used, yet most ignored—by the materials science and engineering community —advanced industrial material. The complexity and the time, temperature, and humidity dependence of its structure and properties are described. Concrete can be produced anywhere in the world at reasonable strengths, but concrete structures are failing, not because of lack of intrinsic strength, but largely because of their exposure to increasingly hostile environments. Low-flow (1 litre min-1) sevoflurane anaesthesia was used in 16 patients undergoing laparoscopic cholecystectomy (group LSC, n = 8) or tympanoplasty (group TP, n = 8), and concentrations of sevoflurane degradation products were measured. Degradation products in the circuit were measured hourly, and end-tidal carbon dioxide concentration, inspired and end-tidal sevoflurane concentrations, and carbon dioxide elimination were monitored. The only degradation product detected was CF2=C(CF3)-O-CH2F (compound A). The mean maximum concentrations of compound A were 21.6 (SEM 1.6) ppm and 19.6 (0.8) ppm in the LSC and TP groups, respectively (ns). The maximum temperatures of soda lime were 46.4 (0.5) degrees C and 44.8 (0.5) degrees C, respectively (P < 0.05). Hourly end-tidal sevoflurane concentrations and concentrations of sevoflurane degradation products were the same for both groups. Carbon dioxide elimination was the same for both groups 1 h after the start of anaesthesia, but was higher in group LSC after 2 h (P < 0.05). Intraperitoneal carbon dioxide insufflation associated with laparoscopic cholecystectomy had no effect on the concentration of sevoflurane degradation products.  相似文献
