The authors describe the use of the fiber bronchoscopic method in the respiratory distress syndrome of adults (RDSA). The method permits visual assessment of bronchial status and allows obtaining cell elements and mediators which are regarded not merely as a result of bronchoalveolar involvement, but as markers of the degree of this involvement. Special attention is paid to cellular and chemical components of bronchial secretion and to detection of correlation with the identical values of arterial and venous blood. The authors classify the available data on bronchological methods of examination, define their role in the diagnosis and treatment of RDSA, and analyze correlation between these data and clinical stages in the course of the syndrome.  相似文献

Many bacterial, protozoal and viral infections trigger a cell-mediated immune response. Of special importance for the clinical outcome of disease, however, is the relative predominance of T helper (Th) cell populations (Th1 and Th2) secreting different patterns of lymphokines. Preferential development of one Th subset occurs apparent at the early stages of an infection, suggesting that the mechanisms driving the immune response in one direction or the other operate soon after exposure to the antigen. Cytokines are among the most important factors regulating T cell differentiation and expansion of the different T cell subtypes. As in experimental candidiasis, listeriosis, yersiniosis and murine retrovirus induced immunodeficiency syndrome (MAIDS), interleukin-4 (IL-4) is of central importance also for the clinical course of murine cutaneous leishmaniasis. It has been demonstrated that the presence of IL-4 is essential for the development of disease promoting Th2 cells whereas neutralization of IL-4 in vivo led to establishment of protective immunity against leishmania. A naturally occurring antagonist of IL-4 is the soluble IL-4 receptor (sIL-4R), which retains its ligand binding properties and binds IL-4 with high affinity. We therefore examined the immunomodulatory and therapeutic capacity of recombinant sIL-4R in murine cutaneous leishmaniasis. BALB/c mice were treated with recombinant sIL-4+ during the onset of the immune response. This treatment rendered BALB/c mice clinically resistant to Leishmania major (L. major), led to reduced parasite load, shifted the pattern of cytokines towards Th1 type and provided durable resistance against reinfection with L. major.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Extracellular Tat can act as a viral toxin on uninfected cells of different tissues, including the CNS and the immune system, thus in order to immunize humans against Tat we have prepared a biologically inactivated but immunogenic Tat (Tat Toxoid). Tat Toxoid is not toxic in mice even at high doses. It triggers high levels of specific Tat Abs in the mouse and rabbit. Furthermore, in humans Tat Toxoid immunization was safe and induced in seronegatives persistent high levels of Tat Abs and in immunodeficient patients a significant rise of these specific Abs. Facing acute HIV-1 infection, the presence of high level of circulating Tat Abs promoted by Tat Toxoid vaccine should prevent Tat-induced immunosuppression and allow anti-HIV-1 cellular response to develop. As a consequence, early release of beta-chemokines could enhance host resistance towards HIV-1, and, in infected people, inhibit viral replication and evolution towards AIDS.  相似文献

Immunological examination of women occupied in production of penicillin revealed a decrease in the phagocytic activity of the blood neutrophiles and the bactericidal properties of the skin, an increase in the quantitative composition of the autoflora of the skin and changes in its biochemical properties. Correlation between the changes in the values of the natural non-specific immunity as dependent on the level of the contact with the antibiotic was shown.  相似文献

An experimental product incorporating 500,000 IU of procaine penicillin G and 600 mg of sodium novobiocin in 2% aluminum monostearate-peanut oil gel (10-ml dose) was infused after the final milk-out at end of lactation into all 4 mammary quarters of 56 cows that were infected in at least 1 quarter. The therapeutic and prophylactic efficacies were published in the companion report. Infusion of the product in all quarters of 5 lactating cows resulted in only slight irritation. Penicillin was eliminated by the 11th milking and novobiocin by the 5th. After infusion in the dry udder, the antibiotics were no longer detectable in serous secretion after 14 days and failed to appear in urine at the earliest (7-day) sampling after administration. Neither antibiotic was detectable in the 1st postpartum milking after nonlactating periods as short as 3 weeks.  相似文献

Studied in the course of one-stage epidemiologic survey of 1191 individuals were the threshold of gustatory sensitivity to table salt and nycterine natriuresis, relation of these indices to dietary habits, some risk factors for cardiovascular disorders and clinical parameters. Differing relations were established between gustatory sensitivity to table salt and natriuresis and age and gender of the examinees, the level of blood pressure, aggravated heredity in respect of arterial hypertension, smoking and alcohol abuse, this enabling one to embark upon a differential approach to dietetic correction of salt consumption.  相似文献

目的 了解流动女工的心理健康状况,促进流动女工心理健康的改善.方法 采用心理健康测试量表(SCL-90)整群随机抽取某电子企业2560名流动女工进行现状调查.结果 流动女工存在不同程度的心理问题,与年龄和受教育程度有一定的相关性,常见的心理问题为强迫症状、敌对、抑郁和人际关系敏感等,检出率为15.08%.结论 该企业流动女工的心理健康问题与年龄偏小、文化程度偏低等有关,建议适当丰富业余文化生活,并加强女工的再教育等预措施以减轻心理健康问题.  相似文献

OBJECTIVE: To determine the efficacy of a murine anti-tumor necrosis factor (TNF) monoclonal antibody in the treatment of Escherichia coli peritonitis and sepsis in the rabbit. DESIGN: Prospective, paired, randomized, blinded, controlled animal trial. SETTING: Animal research laboratory. SUBJECTS: Male New Zealand white rabbits. INTERVENTIONS: Anesthetized rabbits were cannulated with indwelling femoral arterial and venous catheters. Peritonitis and sepsis were induced by intraperitoneal challenge using live E. coli O18ac bacteria. All animals were treated with gentamicin and ceftriaxone 1 hr after challenge. One group (prophylaxis experiment) consisting of ten rabbit pairs (the prophylaxis group), was treated with either murine anti-TNF monoclonal antibody or an equivalent volume of 5% albumin 3 hrs before E. coli challenge. A second group (therapeutic experiment) of 17 rabbit pairs, the treatment group, was also treated with murine anti-TNF monoclonal antibody or albumin control 1 hr after E. coli challenge. MEASUREMENTS AND MAIN RESULTS: All animals were bacteremic 1 hr after challenge. Physiologic measures of sepsis (heart rate, mean arterial pressure, serum bicarbonate, and arterial pH) did not differ between control, prophylaxis, and treatment groups. Peak serum TNF concentration was significantly (p < .01) lower in animals receiving anti-TNF monoclonal antibody, in both the prophylaxis and treatment groups, than in control animals. The survival rate was not improved significantly in either the prophylaxis or treatment group. CONCLUSIONS: Prophylactic and therapeutic use of anti-TNF monoclonal antibody in a rabbit model of E. coli peritonitis and sepsis significantly lowers TNF concentrations but does not ameliorate the physiologic effects of sepsis and does not significantly improve survival.  相似文献
