Burnout is an increasingly important problem in modern work organizations. Few studies, however, have explicitly applied an adequate theoretical understanding of the performance of modern organizations. This article aims to initiate both a discussion of this phenomenon and higher-quality research into the emergence of burnout based on an understanding of the economic-technological rationalization and control (management control) of production and service processes. In applying production control, both technical and bureaucratic, group and attitudinal control systems are increasingly integrated. This so-called systematic control strategy is one of the major causes of burnout. The cumulative effect of an increasing workload combined with reduced resources due to economic considerations and technocratic implementation of production control is assumed to be relevant to the development of burnout. The authors' propositions apply to service workers in human service organizations and to key workers in enterprises using flexible specialization combined with self-directed work groups.  相似文献

In home care nursing work is redesigned by differentiated practice (= skill mix) and specialization. The aim of this redesign is efficient use of staff, and maintenance or improvement of quality of care. In this article the effects of differentiated practice and specialization are studied in respect of quality of care. The perspective of the client was taken as the starting point. Quality of care was defined as the discrepancy between clients' expectations and perceptions of several aspects of care. The effects of work redesign were determined on the basis of a comparison between an experimental (n = 103) and a control group (n = 108). Data-analysis showed hardly any differences. Possible explanations are the premature status of the projects and the fact that clients are usually (very) satisfied. The margin in which differences between the experimental and the control group should occur is therefore very small. However, by means of this approach a detailed picture can be obtained of the subjective standard clients use to judge the quality of care.  相似文献

Low back pain is a benign situation which affects 50 to 80% of all people in the course of their lives. Usually it heals within some weeks. The few who do not get better within some months are in serious danger of ending up in a human, social and medical impasse. In this respect, ordinary clinical examination methods are of limited value. Functional examination is a very important tool both clinically and from the aspect of health insurance. Work physiological function measurements give possibilities of comparing the information on patients with normative data. They also establish a basis for quality control after treatment. The purpose of this article is to describe measurement methods to assess the level of physical function of persons with chronic back pain. The criteria for choosing the methods of examination are also discussed.  相似文献

This article is devoted to one important issue in the development of rare and precious metals industry— analytical control (AC). The current state, importance, problems, and prospects of the development of AC as an integral part of the production of rare and precious metals and quality assurance of products are considered. Modern methods of AC—atomic-spectral, mass-spectral, X-ray fluorescent, combined, and rational fields of their application—are characterized. It is shown scientific-and-technical progress is inextricably associated with a cardinal increase in a nomenclature of materials based on rare and precious metals and an increase in requirements for their quality. This will require the development of new methods of AC and improving existing ones, standardizing them, and metrological support. To carry out this work, it is necessary to attract research organizations of the Russian Academy of Sciences, universities, branch institutes with research laboratories that have survived the dissolution of the Soviet Union, and activate factory science. It is necessary to effectively use the achievements of advanced analytical laboratories abroad and participate in international comparative trials. At the same time, special attention is paid to unsolved problems—a scientifically justified formulation of requirements for new types of products based on rare and precious metals; the development and metrological assessment of sampling techniques; the development of high-quality metrological support for the AC of production of rare and precious metals; improving analytical methods; standardizing analytical methods; the accreditation of analytical laboratories; and the education and training of highly qualified analytical chemists.  相似文献

The damage control concept represents an extension of modern trauma resuscitation into the operating room. This surgical concept has found its most versatile and important use in severe abdominal trauma. The two critical concerns during damage control laparotomy are achieving hemostasis and preventing uncontrolled spillage of intestinal contents or urine. All else is secondary, and contrary to the traditional sequence of abdominal surgery, it is also deliberately disregarded. The common denominator of the many techniques presented in this article for a bail-out laparotomy is the need for a rapid decision, creative improvisation, and awareness that the outcome is determined by the patient's physiologic envelope and not by anatomic integrity, which can be achieved at reoperation.  相似文献
