This study examined how a previous episode of depression is related to daily pain and reactions to pain among individuals with fibromyalgia, a chronic pain syndrome. Seventy-one women with fibromyalgia (including 30 who were previously depressed) rated their pain and mood 3 times daily for 30 days. Each night, participants rated the extent to which they responded to pain by catastrophizing, how much control they had over that day's pain, their ways of coping with pain that day, and the effectiveness of their coping efforts. Multivariate multilevel regression models revealed that after controlling for neuroticism and current depressive symptoms, formerly depressed and never-depressed individuals differed in how they coped with increased pain and in how they appraised the efficacy of their coping efforts. Formerly depressed participants who also reported more current depressive symptoms showed a greater decline in pleasant mood on more painful days than did formerly depressed participants who were experiencing fewer current depressive symptoms. We developed a multidimensional coping inventory to assess the different ways in which people respond to stress. Five scales (of four items each) measure conceptually distinct aspects of problem-focused coping (active coping, planning, suppression of competing activities, restraint coping, seeking of instrumental social support); five scales measure aspects of what might be viewed as emotion-focused coping (seeking of emotional social support, positive reinterpretation, acceptance, denial, turning to religion); and three scales measuring coping responses that arguably are less useful (focus on and venting of emotions, behavioral disengagement, mental disengagement). Study 1 reports the development of scale items. Study 2 reports correlations between the various coping scales and several theoretically relevant personality measures in an effort to provide preliminary information about the inventory's convergent and discriminant validity. Study 3 uses the inventory to assess coping responses among a group of undergraduates who were attempting to cope with a specific stressful episode. On the basis of previous work, freshmen should evidence improved health after writing about their thoughts and feelings associated with entering college. One hundred thirty subjects were assigned to write either about coming to college or about superficial topics for 20 min on 3 days. One fourth of the subjects in each group wrote during the 1st, 5th, 9th, or 14th week of classes. Physician visits for illness in the months after writing were lower for the experimental than for the control subjects. Self-reports of homesickness and anxiety were higher in the experimental group 2–3 months after writing. By year's end, experimental subjects were either superior or similar to control subjects in grade average and in positive moods. No effects emerged as a function of when people wrote, suggesting that the coping process can be accelerated. Implications for comparing insight treatments with catharsis and for distinguishing between objective and self-report indicators of distress are discussed. Conducted a series of meta-analyses, involving 43 studies, to ascertain whether there were systematic patterns in the empirical literature that described when attention strategies were more or less effective than avoidant strategies. Results of an overall analysis of studies providing tests of attention vs avoidance indicated little evidence for one strategy's superiority. However, supplementary analyses suggested there were boundary conditions that defined the relative efficacy of a specific strategy. Overall, avoidance was associated with more positive adaptation in the short-run. However, attention was superior to avoidance if the former involved a focus on sensory schemata rather than emotional processing. If attention involved an emotional interpretational set or no explicit set, then it was associated with more negative outcomes than avoidance. In terms of long-term outcomes, avoidance indicated better outcomes initially, but with time, attention was associated with more positive outcomes. A final set of analyses found that both attention and avoidance facilitated adaptation as compared with no instruction controls. The meta-analyses suggested the important role of interpretational set and whether one looks at the immediate or at the long-term effects of coping. Individual coping strategies and dyadic coping independently predict partner well-being and relationship functioning; however, it is unclear whether the coping processes are inter-related and whether they uniquely contribute to romantic relationship functioning. One hundred heterosexual dating couples rated the individual coping strategy of negative mood regulation as well as positive and negative dyadic coping. Relationship functioning was assessed via partners' reports of relationship satisfaction and observers' ratings of negative interaction in conflict. Actor-Partner Interdependence Models (APIMs; Cook & Kenny, 2005; Kashy & Kenny, 2000) revealed associations between individual coping and dyadic coping in the predicted directions. APIMs also indicated the unique contributions of positive and negative dyadic coping to relationship functioning, above and beyond contributions of individual coping strategies. The present study investigated the relation of pain coping strategies to pain, health status, and psychological distress in a group of osteoarthritis patients with chronic knee pain. Fifty-one patients completed the Coping Strategies Questionnaire (CSQ), the McGill Pain Questionnaire, the Arthritis Impact Measurement Scale (AIMS), and the Symptom Checklist-90 Revised (SCL-90R). Medical status variables included roentgenograph (x-ray) findings, obesity measures, disability status, and chronicity of pain. Factor analysis of the CSQ revealed two factors (Coping Attempts, Pain Control and Rational Thinking) that accounted for 60% of the variance in CSQ responses. Regression analyses controlling for demographic and medical variables identified the Pain Control and Rational Thinking factor as a significant predictor of the outcome measures. Reviews the literature on strategies children use for coping with pain before professional intervention. The development of direct methods of assessing children's behavioral and cognitive coping strategies is traced; and evidence for developmental differences in the types of strategies children employ is presented. Initial evidence regarding the effectiveness of spontaneous coping strategies is presented. Future research directions to be taken may include extension to pediatric chronic pain populations, outcome studies, and investigation of parents' roles in children's development and application of coping strategies. This study investigated the relationship between two intrapersonal variables-- stressful life change events and reported coping strategies-- and high school completion status among early adolescents. The sample was comprised of 92 students (44 females and 48 males) who had completed questionnaires regarding the number and types of life change events they had experienced and the coping strategies they reported using during ninth grade. A 3-year follow-up study of these students was completed to determine whether they finished high school or reported dropping out. Discriminant analysis was used to build a prediction model and indicated that the coping factors of Social Activities and Seeking Professional Support significantly predicted high school dropout status, whereas Family Involvement was negatively related to this outcome. These findings are discussed in light of current research, and suggestions for future research are provided. Interventions that focus on the contextual variables related to family and peers are indicated, as is the need for school psychologists and other school mental health professionals to offer coping skills training. A study was conducted to examine the role of pain episodes and the role of active and passive pain coping strategies in depicting depression in 287 patients with rheumatoid arthritis (RA). The independent effects of pain and pain coping strategies, as well as the interaction effects between pain and pain coping strategies on depression, were evaluated cross-sectionally and prospectively over a 6-month interval. The cross-sectional findings revealed that pain, passive coping, and the interaction between pain and passive coping contributed independent variance, all accounting for higher depression. Of principal interest was the finding that the frequent use of passive pain coping strategies in the face of high pain contributed to the most severe level of depression over time. These results were obtained after controlling for the potentially confounding effects of prior depression, functional disability, and medication status. These data imply that there may be a potential benefit of developing techniques to reduce the use of passive pain coping strategies to deal with chronic arthritis pain in cognitive–behavioral pain management programs. This study examines the relationship among global coping self-efficacy, coping, and coping effectiveness within athletes. We hypothesized that there would be a positive association between coping self-efficacy and coping effectiveness, which would be mediated by coping. It was also predicted that coping strategies within the task-oriented coping dimension would be positively associated with coping effectiveness, whereas strategies from the disengagement- and distraction-oriented coping dimensions would be negatively associated with coping effectiveness. Participants were 353 athletes between the ages of 18 and 29 years, who completed a measure of coping self-efficacy the night before they competed, in addition to a measure of the athletes' use of coping strategies and their perceived coping effectiveness, which was completed immediately after the competitive event. The field of business ethics is entrenched in a cognitive approach that portrays the ethical decision-making process as a completely deliberate and reasoned exercise. In light of growing concerns about the veracity of this approach, I build upon current knowledge of how the brain functions to present a neurocognitive model of ethical decision making. The model suggests that ethical decision making involves 2 interrelated yet functionally distinct cycles, a reflexive pattern matching cycle and a higher order conscious reasoning cycle, and thereby describes not only reasoned analysis, but also the intuitive and retrospective aspects of ethical decision making. The model sparks research in new areas, holds significant implications for the study of ethical decision making, and provides suggestions for improving ethical behavior in organizations. Examined characteristics that might relate to quality of life in postmastectomy patients with operable breast cancer. 113 women who had been treated for breast cancer during 3.5 yrs preceding the study were surveyed. 53 Ss had received mastectomies alone. The remaining 60 Ss, who had shown some spread of disease to the lymph nodes only, received long-term, prophylactic chemotherapy as an adjunct to mastectomy. Analyses indicated that level of cancer-specific denial was the variable most strongly associated with postmastectomy distress. This study examined personal and contextual predictors of active and avoidance coping strategies in a community sample of over 400 adults and in a sample of over 400 persons entering psychiatric treatment for unipolar depression. Sociodemographic factors of education and income (except for active-cognitive coping), personality dispositions of self-confidence and an easy-going manner, and contextual factors of negative life events and family support each made a significant incremental contribution to predicting active and avoidance coping. Among both healthy adults and patients, active and avoidance coping were positively associated with negative life events. Individuals who had more personal and environmental resources were more likely to rely on active coping and less likely to use avoidance coping. Moreover, for both groups, most of the predictors continued to show significant relations with active and avoidance coping strategies even after the stable component in coping was controlled in a longitudinal design. A comprehensive framework to understand the determinants of coping can be of practical value in suggesting points for therapeutic interventions aimed at fostering more adaptive coping efforts. The mathematical basis of a generalization of the redundancy index proposed by D. Stewart and W. Love (see record 1968-18070-001) for canonical analysis is developed and extended to forms of canonical analysis besides canonical correlations. It is shown that the redundancy index measures the extent to which one matrix may be reconstructed from another, thereby meeting the objections of W. A. Nicewander and D. A. 19–23 yr-old college students completed anxiety and coping diaries during 10 periods that began 7 days before an academic stressor and continued through the evening after the stressor. Profile analysis was used to examine the anxiety and coping processes in relation to 2 trait anxiety grouping variables: debilitating and facilitating test anxiety (D-TA and F-TA). Anxiety and coping changed over time, and high and low levels of D-TA and F-TA were associated with different daily patterns of anxiety and coping. Participants with a debilitative, as opposed to facilitative, trait anxiety style had lower examination scores, higher anxiety, and less problem-solving coping. Covarying F-TA, high D-TA was associated with a pattern of higher levels of tension, worry, distraction, and avoidant coping, as well as lower levels of proactive coping. Covarying D-TA, high F-TA was associated with higher levels of tension (but not worry or distraction), support seeking, proactive and problem-solving coping. Analyzes the role of personal stress and coping processes from the perspective of a cognitively oriented theory developed by R. S. Lazarus and colleagues, beginning in 1951. Believing that an event is controllable does not always lead to a reduction in stress or to a positive outcome, and believing that an event is uncontrollable does not always lead to an increase in stress or to a negative outcome. The present author shows how 2 forms of control, generalized beliefs about control and situational appraisals of control, fit into the overall model of Lazarus and colleagues. Situational appraisals of control are explored, especially as they relate to health matters. The theoretical formulation of stress and coping is drawn on to examine 3 important issues: (a) how believing one has control in a stressful transaction can heighten threat, (b) the relationship between control and coping, and (c) pathways through which control can affect the adaptational outcomes of stressful encounters. Increasingly, patients are expecting to be more involved than they traditionally have been in medical and surgical decision making. The unilateral process of informed consent is evolving into one of informed collaborative choice. Hysterectomy is a procedure that is frequently performed when reasonable surgical and nonsurgical alternatives remain. When professional consensus as to the clear recommendation for hysterectomy is not present, patient choice is particularly important. Because more than 80% of health-care decisions, including those in which one of the choices is hysterectomy, are elective, gynecologists and other health care providers increasingly will need to develop more efficient and collaborative methods to integrate patient autonomy and choice into the decision-making process. There is mounting evidence that both clinical and nonclinical outcomes (satisfaction and cost) may be improved when properly informed consumers collaborate in making medical and surgical decisions. Legal liability for adverse outcomes may be decreased by increased patient participation in medical and surgical decision making. The era of managed care has created an agency problem stemming from the fact that consumers (patients) are concerned that necessary procedures and other treatments may be withheld because of cost considerations. Health plans and medical groups likely will be required to provide objective information about the options that consumers (patients) have when faced with choices, including decision making and hysterectomy. By incorporating patient expectations and preferences as part of the process of decision making, an ethically acceptable and effective method of "rationing by patient choice" may be feasible. Figure 3 is a graphic depiction of such a process of informed collaborative choice progressing from effective choices through efficient choices and then to the one providing the best value for an individual patient.  相似文献

Examined (a) whether Type A (coronary-prone) individuals respond with more psychophysiological arousal to threat to self-esteem than to threat of shock, (b) whether differences between Type A and Type B individuals in psychophysiological responses are greater under high than low stress, and (c) whether Type A and Type B individuals differ in how they cognitively cope with stress. 84 male college students participated. Results indicate that Type A Ss manifested higher pulse rates across all conditions and greater systolic and diastolic blood pressures in response to threat to self-esteem than did Type B Ss. Type A Ss also employed more suppression in response to both threat to self-esteem and threat of shock and employed more denial in response to threat to self-esteem than did Type B Ss. Examined children's and parents' reactions to postoperative pain, including stress appraisal and cognitive-behavioral coping processes from a developmental perspective. 30 younger children (aged 7–9 yrs) and 30 older children (aged 10–16 yrs) and their parents provided interview and psychometric data about pain and coping on the day following surgery. Observational data also was provided by nurses. All children described a variety of self-control strategies found to be helpful in managing postoperative pain. Older children were more likely to report using cognitive coping strategies, yet they reported lower overall self-efficacy. Coping strategy use, perceived self-efficacy, and frequency of catastrophizing thoughts were significantly predictive of children's pain, affective distress, and physical recovery. Parental anxiety was positively related to child anxiety, and inversely related to child self-efficacy and frequency of cognitive coping. Whereas career development theory has proposed that the crystallization of career choices occurs as individuals resolve relevant career development tasks, the supporting empirical evidence has been inconclusive. To develop a clearer understanding of the nature of the career decision-making process, this study assessed the degree to which career choice crystallization is associated with vocationally mature attitudes, behavior, and knowledge. Measures of vocational maturity and career choice crystallization (which was defined by measures of career decidedness and commitment to career choice) were administered to 158 community college students (mean age?=?21.35). A canonical analysis was employed to identify the relationships between age, gender, career choice crystallization, and vocational maturity. The analysis yielded one significant canonical root, which indicated that most of the shared variance between these two sets of variables was contributed by the relationship between the career planning component of vocational maturity and the two career choice crystallization scales.
