Presents the case study of a 27-yr-old female admitted to a national asthma center for a complex of respiratory problems that had defied medical intervention. Hypnotic techniques and insight-oriented psychotherapy resulted in hospital release with only the asthma problem remaining after 4 mo. Psychotherapy continued for 5 more months. Hypnotic technique allowed S to recover control and reverse the chain of intractable events in the course of her illness. Demonstrates a positive relation between the formal recognition of psychologists for outpatient mental health services under the Civilian Health and Medical Program of the Uniformed Service (CHAMPUS), Hawaii Medical Service Association, and Medicaid programs and the extent of services that they provide. Utilization data for 1983 outpatient mental health services by Hawaii residents show that the imposition of a medical referral condition under the Hawaii Medicaid program curtailed the use of psychological services relative to psychiatric services and restrained patients' freedom of choice. Reports changes in the behavior of the 55 siblings of 27 treated predelinquents. Siblings were 3 yrs old or older. The parents of the referred predelinquents had been trained in social learning techniques of child management. Prior analyses of home observation data showed significantly reduced rates of deviant behaviors for the identified problem children. These reductions were maintained over a 12-mo follow-up. The child management procedures taught to the parents were presumably applied to siblings as well as to the identified problem child. Analyses were conducted for the data from the siblings. The baseline data show no significant differences between siblings and identified problem children. At termination of treatment, there were significant reductions in rates of deviant behavior for the siblings. Follow-up results show the effects were maintained over 6 mo. 31 patients who had received a brief evaluation at the mental health department of a health maintenance organization either were placed on a 4-mo waiting list or received insight-oriented, time-limited psychotherapy. An 18-mo follow-up telephone interview was used to examine whether Ss showed insight into the link between their illness behavior and psychological factors. Examination of medical utilization before and after the mental health intervention showed that the acquisition of insight was unrelated to changes in utilization. The Canadian Council on Hospital Accreditation (1985) has insisted that a hospital-wide quality assurance program is essential for accreditation, and hospital departments are in various stages of quality assurance program development. The present authors review the rationale and issues (e.g., professional autonomy, malpractice, health care costs, consumerism) that forced the development of quality assurance and some generally accepted definitions of key concepts. The current status of the Children's Hospital of Eastern Ontario Psychology Department's program of quality assurance is described, with special reference to structure, process, and outcome criteria. Implications that quality assurance programs have for the professional practice of psychology and functioning within the public hospital setting are discussed. A survey of 60 leading Universities was made to determine present practices and policies regarding the use of college students as subjects in psychological research. The methodological implications arising from differences in procedures used for recruiting subjects and arranging their experimental appointments were noted. The study also examined procedures employed by investigators to protect the physical and psychological safety of the participating students. Current procedures were found to vary considerably and it was recommended that they be reexamined from both the methodological and ethical aspects. The negative economic impact from managed health care has led some clients to request and, consequently, their psychologists to explore unique financial arrangements, such as bartering. Bartering is not precluded by ethics or law, but in this article, the author opposes bartering by exploring the ethical and legal considerations involved and the dual relationships that occur. Clinical considerations and the potential for exploitation in the relationship are examined, and caveats about bartering are offered. For the psychologist who accepts bartering, guidelines for keeping a bartering arrangement are provided. Even if carefully monitored for clinical contradictions or exploitation, bartering imposes a high risk of allegations of misconduct (e.g., impropriety or a conflict of interest) and should be avoided. Analyzes social functions of psychological services and raises fundamental policy issues based on the following conclusions: (a) The manifest functions of human services—to identify, correct, and prevent failures of psychological adaptation—are largely unrelated to, and often incompatible with, their latent functions—the provision of caring and social control. (b) Both types of functions emerged in reaction to forces in the technological society served. (c) Though psychological services are useful to society, they cannot, in principle, effectively implement their manifest functions. (d) Providers of psychological services may find new, more constructive directions in supporting decentralized social, economic, and technological innovations that promote alternatives to a centralized, megatechnological society. Three cases illuminate quality assurance problems and pertinent principles and policies for managing them in the areas of clinical, industrial/organizational, and school psychology. The 1st involves a psychologist who routinely required clients to complete the Minnesota Multiphasic Personality Inventory (MMPI) at home and had the tests scored and interpreted by a computerized service. In the 2nd, psychologists decided to discontinue the use of an unvalidated computer interpretation of the MMPI in screening for a business. In the 3rd, a school psychologist violated ethical and professional principles by selling his own unvalidated computerized test scoring and interpretation service. In each case, applicable policies, specialty guidelines, and ethical principles and their interpretation are discussed, together with the educative ramifications. The authors' experience with 45 papillosphincterotomies, performed for diseases of bile passages and chronic pancreatitis, is reported. The details of the operative technic, employed by the authors, are described. The technic of papillosphincterotomy on a conus-shape sound by means of a "laser scalpel" proved to be the most perfect one. Careful observation of the surgical technic made it possible to gain favourable results--only 1 of 45 operated patients died. Papillosphincterotomy is largely advocated in stenoses and concretions in the large duodenal papilla, strictures of choledocho-duodenoanastomoses, care should be taken not to overwiden indications to the operation concerned.  相似文献

Comments on the G. J. Meyer et al (see record 2001-00159-003) summary of evidence and issues associated with psychological assessment. H. N. Garb et al conclude that Meyer et al did not establish that psychological test validity is comparable to medical test validity. By focusing on meta-analyses, they ignored results for most medical tests and psychological test scores. Furthermore, although it may appear that they routinely summarized results from meta-analyses, in some cases they actually calculated their own effect sizes. For at least some studies, their effect sizes appear to be misleading, making their review of the literature difficult to interpret. Presents 4 cases, focusing on quality assurance problems that came to the attention of the Committee on Professional Standards of the American Psychological Association directly or through other governance bodies. The clinical case involves access to information about a court worker, a patient of the practitioner's partner, who was thwarting the gains made by the practitioner's patient. The counseling case involves a request for a gay or lesbian counselor. The industrial/organizational case involves evaluation of a candidate for a position described by a personnel director, but not independently analyzed, and a refusal to provide a copy of the subsequent report to the candidate. The school case involves a request made of a psychologist to be an ombudsperson for a school administration, while ethically being required to serve the student.
