Using a standardized schedule of questions, this study examined (a) the prevalence of self-report of violent thoughts by patients hospitalized for mental disorders compared with nonpatients, (b) the persistence of violent thoughts after discharge, and (c) the relation between patients' violent thoughts while hospitalized and violent acts within 20 weeks after hospital discharge. About 1/3 of the patients reported thoughts of violence while hospitalized, more than twice the proportion found among nonpatients. Reporting violent thoughts in hospital was significantly related to engaging in violent acts within 20 weeks after discharge for non-White patients, patients without major mental disorder but with substance abuse diagnoses, patients with high symptom severity, and patients whose reports of violent thoughts persisted after discharge. Comments on the article by A. E. Shealy (see record 2005-09632-004) describing the teaching approach in forensic psychology. The current author notes that if, as Shealy proposes, advantages and positive rewards are available to psychologists participating in the judicial system, then it is also important to evaluate the negative aspects. Furthermore, a confrontation of the inequities with the advocate and adversarial approach needs more exploration in Shealy's system. The author describes a graduate program in forensic psychology at the University of Washington, in which students in law, psychiatry, and psychology, psychology interns, and other health science professionals will be able to participate. In a recent article Goldenberg (1978) indicated that Shealy (1977), in his presentation of a format for teaching mental health workers basic forensic procedures and issues, failed to specify the potential rewards and costs of courtroom participation. Having participated as clinical psychology interns in a forensic psychology seminar led by and modeled after Shealy's (1977) format, the authors felt they could offer a meaningful rejoinder based on their personal experiences with both the rewards and potential pitfalls of such judicial participation, and on our understanding of the basic adversary nature of the legal system. Being threatened, harassed, attacked, or confronted by a patient with a weapon is becoming more common and is likely to occur at some point in a mental health professional's career. Effective violence management programs can reduce the incidence of violence. Yet, few resources have been provided to assist psychologists and other mental health professionals to deal with aggressive patients. The authors offer strategies for the management of aggressive behavior that can be implemented to empower practitioners to take precautions when necessary in a quick and efficient manner when dealing with violent and potentially violent patients. The key to human intelligent behavior lies in the capacity for thinking, reasoning, and problem solving and an inability to communicate through language. These abilities are often seriously impaired in people with mental disorders, and their language may reveal delusions and scattered, incoherent thoughts as well as "word" salad and idiosyncratic words. Depressed and anxious individuals often reveal self defeating thoughts and inappropriate reasoning, such as regarding minor slip-ups as catastrophes and dwellling on the negative aspects of experiences that are generally positive. Cognitive research seeks to understand how thinking, reasoning, and using language develop and function nomally, how they are represented in the brain, and how they can break down in aging, brain injury, and various mental disorders. Aspects of this research inform the work of therapists as they challenge irrational beliefs and dysfunctional reasoning, encourage clients to re-evaluate their problems, and aid people in overcoming the aftereffects of brain injury and disease. Reviews the book, Canadian Mental Health Law and Policy by John E. Gray, Margaret A. Shone, and Peter F. Liddle (2000). The authors of this book have produced a multidisciplinary text that succinctly summarizes the main points of Canadian mental health laws and the variability across provinces and territories in these laws and in their typical interpretation and application. This book offers a concise guide to the laws dealing with key issues such as involuntary hospital admission, authorization of psychiatric treatment, and treatment refusal. Although not always an easy read for those unfamiliar with legal terms and writing, the book is geared to a wide audience that includes legal and policy specialists, mental health professionals, and advocates for the mentally ill. These authors are to be congratulated for their efforts to disseminate and explain information on Canadian mental health law and to promote a more humane and scientifically grounded set of legal standards and interpretations that are pertinent to the disposition and treatment of Canadian citizens with severe mental disorders. People's concerns with how others perceive and evaluate them can lead to behaviors that increase the risk of illness and injury. This article reviews evidence that self-presentational motives play a role in several health problems, including HIV infection; skin cancer; malnutrition and eating disorders; alcohol, tobacco, and drug use; injuries and accidental death; failure to exercise; and acne. The implications of a self-presentational perspective for research in health psychology, the promotion of healthful behaviors, and health care delivery are discussed. Examines assessment issues related to deaf clients, focusing on cultural and linguistic bias. Reliability and validity problems encountered when administering intellectual and personality assessments to hearing-impaired Ss are discussed. Factors influencing psychological assessment are addressed, including inaccurate beliefs and problems with rapport, miscommunication, and using interpreters. It is recommended that research be conducted to refine the psychometric properties of assessment instruments used, that psychologists become more sophisticated about deaf culture, and that psychologists interested in working with deaf clients need to become more skilled in the use of sign language. Law enforcement personnel routinely face various critical incidents in the course of their workday including confrontations with irate, violent, and/or mentally challenged individuals. While less frequent, law enforcement personnel also are called in response to hostage incidents, barricaded subject incidents, and attempted suicide/suicide by cop incidents. Over the past 30 years, response strategies utilized by law enforcement personnel have been evolving and have been influenced by mental health professionals. This article briefly reviews the histories of Critical Incident Teams (CITs) and Crisis Negotiation Teams (CNTs), discusses the roles of mental health professionals in the development and continuing evolution of CITs and CNTs, presents some of the obstacles to mental health involvement with law enforcement, and highlights areas in need of more empirical research. A literature review on the attitudes of police officers, lawyers, and mental health professionals toward the disabled indicates that police officers' attitudes are related to the extent of their formal education and the amount of information they have about relating to persons with various types of disabilities. Lawyers who have disabled clients tend to be more knowledgeable and to have more positive attitudes than lawyers without such clients, but it is not easy to turn lawyers into advocates for disabled clients. Mental health professionals, who may be called on to provide advice to police officers or lawyers or to testify in court cases, have some negative aspects that may impair their helpfulness, but their attitudes are more positive than those of less educated persons. Data also indicate that the attitudes of psychologists are less negative than those of psychiatrists. It is concluded that rehabilitation psychologists need to undertake studies of the attitudes of these groups and to set up programs designed to change the attitudes of members of these professions. To better understand the relationship dynamics of couples experiencing husband-to-wife violence, the interaction patterns of 29 domestically violent (DV), 15 distressed but nonviolent (DNV), and 13 happily married (HM) couples were examined using the Marital Interaction Coding System and lag sequential analyses. DV spouses were generally more often aversive and less often facilitative than nonviolent couples. DV couples were significantly more likely to engage in negative reciprocity than their DNV or their HM counterparts. DV wives were as likely as their husbands to reciprocate negative behavior. Furthermore, no support was found for a negative reinforcement hypothesis that husbands' aversiveness was shaped or maintained by wives' capitulation. Objective: To examine the relationship between psychiatric disorders and sexual behaviors among adolescents receiving mental health treatment. Adolescents in mental health treatment have been found to have higher rates of HIV risk behavior than their peers, but data concerning the relationship between psychopathology and risk are inconsistent and limited. Method: Eight hundred and forty adolescents (56% female, 58% African American, mean age = 14.9 years) and their parents completed computerized assessments of psychiatric symptoms via the Computerized Diagnostic Interview Schedule for Children (Shaffer, 2000a, 2000b). Adolescents also reported on sexual risk behaviors (vaginal/anal sex, condom use at last sex) and completed urine screens for a sexually transmitted infection (STI). Results: Adolescents meeting criteria for mania, externalizing disorders (oppositional defiant, conduct, and attention-deficit/hyperactivity disorders), or comorbid for externalizing and internalizing disorders (major depressive, generalized anxiety, and posttraumatic stress disorders) were significantly more likely to report a lifetime history of vaginal or anal sex than those who did not meet criteria for any psychiatric disorder (odds ratio [OR] = 2.0, 2.3, and 1.9, respectively). Adolescents meeting criteria for mania were significantly more likely to have 2 or more partners in the past 90 days (OR = 3.2) and to test positive for a STI (OR = 4.3) relative to adolescents who did not meet criteria for a psychiatric disorder. Conclusions: The presence of internalizing and externalizing disorders, especially mania, suggests the need for careful screening and targeting of adolescent sexual behavior during psychiatric treatment. Contends that psychology can be used to prevent heart disease, cancer, accidents and violence, and respiratory disease through programs such as those designed to eliminate smoking, control obesity, reduce stress, and intervene when multiple risk factors are present. Psychological techniques—as useful and cost-saving companions to traditional medical treatments—can increase checkup visits and participation in health screening, improve adherence to medication schedules, aid recovery from surgery, and facilitate outpatient medical service delivery. Since 40–60% of physician visits are for nonmedical reasons that are psychological and behavioral in nature, psychologists can reduce medical care overutilization and unnecessary medical expenditures by offering less costly and more appropriate alternatives. Psychological interventions for health disorders include treatment for pain, asthma attacks not responsive to medical treatment, heart rate variability, gastrointestinal problems, skin disorders, and self-destructive behaviors. The purpose of this qualitative study was to explore how mental health professionals and African American pastors and their churches could collaborate to overcome minority disaster mental health disparities. Forty-one African American pastors of churches located in south Mississippi, in counties directly affected by Hurricane Katrina, participated in semi-structured interviews approximately 1 year after the storm. The majority of participants reported being interested in collaborating with mental health professionals to: (a) develop educational and outreach opportunities, (b) lead assessment procedures, (c) offer consultation activities, (d) provide clinically focused services, and (e) utilize spiritual resources and support. Participants provided further insight into how these collaborative activities could be modified to meet post-disaster needs and offered novel applications. Following from these discussions, the article provides a number of recommendations that can be used to aid in the development of disaster collaborative activities between African American pastors and churches and mental health professionals to serve minority communities while also decreasing disparities. In addition to labeling people as mentally ill, the law often labels them as incompetent to exercise certain rights or to play particular roles. Indeed, under the broad dicta of Zinermon v. Burch, incompetency labeling may increase dramatically. This article uses principles of social and cognitive psychology to examine the effects of incompetency labeling. Such labeling is shown to produce potentially serious adverse effects. It often alters the way others view and react to the labeled individual and affects his or her self-esteem and self-concept in ways that may inhibit performance, diminish motivation, and depress mood. After analyzing those negative side effects of incompetency labeling, the article examines the implications of these findings for mental health law and makes a number of proposals for changing the law to avoid or minimize these adverse effects.
