To date little research has examined safety climate in relation to macroergonomics and how the two distinct sub-disciplines can be integrated to affect safety outcomes. The purpose of macroergonomics is to design a fully “harmonized” work system that improves numerous aspects of organizational performance and effectiveness, and this is accomplished by incorporating the foundational theoretical framework of sociotechnical systems theory (STS). Two broad subsystems within such a system are the personnel subsystem, the ways individuals perform tasks, and the technological subsystem, the tasks to be performed. Management is an important aspect of the personnel subsystem, and there is a growing body of research regarding supervisors’ influence over employee safety. One such area of research is safety climate, which is based on the perception of workers regarding safety and organizational practices. Two major factors of safety climate are management commitment to safety and communication pertaining to safety as a true priority from both top management and direct supervisors. This article describes the conceptual overlaps of macroergonomics and safety climate in order to present a conceptual model that integrates these domains using the framework of mesoergonomics. In conclusion, we discuss how this model can serve as a framework to guide the analysis and design of work systems and subsequent organizational interventions.  相似文献

There is a need to develop a comprehensive framework for the safety assessment of Intelligent Transport Systems (ITS). This framework should: (1) cover all three dimensions of road safety—exposure, crash risk and consequence, (2) cover, in addition to the engineering effect, also the effects due to behavioural adaptation and (3) be compatible with the other aspects of state of the art road safety theories. A framework based on nine ITS safety mechanisms is proposed and discussed with regard to the requirements set to the framework. In order to illustrate the application of the framework in practice, the paper presents a method based on the framework and the results from applying that method for twelve intelligent vehicle systems in Europe. The framework is also compared to two recent frameworks applied in the safety assessment of intelligent vehicle safety systems.  相似文献

The relationship between investment in employee health and non-health outcomes has received little research attention. Drawing from social exchange and climate theory, the current study uses a multilevel approach to examine the implications of worksite health investment for worksite safety and health climate and employee safety compliance and commitment to the worksite. Data were collected from 1932 personnel working on 31 offshore installations operating in UK waters. Installation medics provided corporate workforce health investment details for 20 of these installations. The findings provide support for a strong link between health investment practices and worksite safety and health climate. The results also found a relationship between health investment practices and organizational commitment among employees. These results suggest that health investment practices are associated with committed workforces and climates that reflect a priority on health and safety.  相似文献

Safety assessment based on conventional tools (e.g. probability risk assessment (PRA)) may not be well suited for dealing with systems having a high level of uncertainty, particularly in the feasibility and concept design stages of a maritime or offshore system. By contrast, a safety model using fuzzy logic approach employing fuzzy IF–THEN rules can model the qualitative aspects of human knowledge and reasoning processes without employing precise quantitative analyses. A fuzzy-logic-based approach may be more appropriately used to carry out risk analysis in the initial design stages. This provides a tool for working directly with the linguistic terms commonly used in carrying out safety assessment. This research focuses on the development and representation of linguistic variables to model risk levels subjectively. These variables are then quantified using fuzzy sets. In this paper, the development of a safety model using fuzzy logic approach for modelling various design variables for maritime and offshore safety based decision making in the concept design stage is presented. The objective of this paper is to discuss the following questions: Do concurrent organizational changes have a direct impact on aviation maintenance and safety, if so, how can this be measured? These questions were part of the investigation carried out by the Accident Investigation Board, Norway (AIBN). The AIBN investigated whether Norwegian aviation safety had been affected due to major organizational changes between 2000 and 2004. The main concern was the reduction in safety margins and its consequences.This paper presents a summary of the techniques used and explains how they were applied in three airlines and by two offshore helicopter operators. The paper also discusses the development of safety related indicators in the aviation industry. In addition, there is a summary of the lessons learned and safety recommendations. The Norwegian Ministry of Transport has required all players in the aviation industry to follow up the findings and recommendations of the AIBN study.  相似文献

This paper discusses the evolution of the term ‘safety culture’ and the perceived relationship between safety culture and safety of operations in nuclear power generation and other hazardous technologies. There is a widespread belief that safety culture is an important contributor to safety of operations. Empirical evidence that safety culture and other management and organizational factors influence operational safety is more readily available for the chemical process industry than for nuclear power plant operations. The commonly accepted attributes of safety culture include good organizational communications, good organizational learning, and senior management commitment to safety. Safety culture may be particularly important in reducing latent errors in complex, well-defended systems. The role of regulatory bodies in fostering strong safety cultures remains unclear, and additional work is required to define the essential attributes of safety culture and to identify reliable performance indicators.  相似文献

Based on elements of social exchange theory and other conceptualizations of trust, a model was developed situating organizational trust as a central component to the relationship that safety climate has with organizational outcomes. Specifically, the model specified that two facets of safety climate - upward safety communication and management attitudes toward safety - would be positively related to organizational trust. Increased levels of trust would then predict increased motivation to engage in safe job-related behaviors, increased job satisfaction, and decreased turnover intentions. Another hypothesis investigated whether job safety relevance would moderate the relationship between safety climate and trust. Online survey research was conducted with 599 employees from 97 workgroups across a New England grocery store chain. Hierarchical linear modeling indicated support for trust mediating the relationship between safety climate and organizational outcomes; further, the relationship between safety climate and trust was stronger within workgroups where safety was more relevant.  相似文献

The aim of this study was to explore whether work characteristics (job demands, job control, social support) comprising Karasek and Theorell's (1990) Job Demands–Control–Support framework predict employee safety performance (safety compliance and safety participation; Neal and Griffin, 2006). We used cross-sectional data of self-reported work characteristics and employee safety performance from 280 healthcare staff (doctors, nurses, and administrative staff) from Emergency Departments of seven hospitals in the United Kingdom. We analyzed these data using a structural equation model that simultaneously regressed safety compliance and safety participation on the main effects of each of the aforementioned work characteristics, their two-way interactions, and the three-way interaction among them, while controlling for demographic, occupational, and organizational characteristics. Social support was positively related to safety compliance, and both job control and the two-way interaction between job control and social support were positively related to safety participation. How work design is related to employee safety performance remains an important area for research and provides insight into how organizations can improve workplace safety. The current findings emphasize the importance of the co-worker in promoting both safety compliance and safety participation.  相似文献

The acknowledgement of industrial installations as complex systems in the early 1980s outstands as a milestone in the path to operational safety. Process plants are social–technical complex systems of a dynamic nature, whose properties depend not only on their components, but also on the inter-relations among them. A comprehensive assessment of operational safety requires a systemic approach, i.e. an integrated framework that includes all the relevant factors influencing safety. Risk analysis methodologies and safety management systems head the list of methods that point in this direction, but they normally require important plant resources. As a consequence, their use is frequently restricted to especially dangerous processes often driven by compliance with legal requirements. In this work a new safety index for the chemical industry, termed the ‘Proceso’ Index (standing for the Spanish terms for PROCedure for the Evaluation of Operational Safety), has been developed. PROCESO is based on the principles of systems theory, has a tree-like structure and considers 25 areas to guide the review of plant safety. The method uses indicators whose respective weight values have been obtained via an expert judgement technique. This paper describes the steps followed to develop this new Operational Safety Index, explains its structure and illustrates its application to process plants.  相似文献

Management of safety is always based on underlying models or theories of organization, human behavior and system safety. The aim of the article is to review and describe a set of potential biases in these models and theories. We will outline human and organizational biases that have an effect on the management of safety in four thematic areas: beliefs about human behavior, beliefs about organizations, beliefs about information and safety models. At worst, biases in these areas can lead to an approach where people are treated as isolated and independent actors who make (bad) decisions in a social vacuum and who pose a threat to safety. Such an approach aims at building barriers and constraints to human behavior and neglects the measures aiming at providing prerequisites and organizational conditions for people to work effectively. This reductionist view of safety management can also lead to too drastic a strong separation of so-called human factors from technical issues, undermining the holistic view of system safety. Human behavior needs to be understood in the context of people attempting (together) to make sense of themselves and their environment, and act based on perpetually incomplete information while relying on social conventions, affordances provided by the environment and the available cognitive heuristics. In addition, a move toward a positive view of the human contribution to safety is needed. Systemic safety management requires an increased understanding of various normal organizational phenomena - in this paper discussed from the point of view of biases - coupled with a systemic safety culture that encourages and endorses a holistic view of the workings and challenges of the socio-technical system in question.  相似文献

The aim of the present study was to explore the possibility of identifying general safety climate concepts in health care and petroleum sectors, as well as develop and test the possibility of a common cross-industrial structural model. Self-completion questionnaire surveys were administered in two organisations and sectors: (1) a large regional hospital in Norway that offers a wide range of hospital services, and (2) a large petroleum company that produces oil and gas worldwide. In total, 1919 and 1806 questionnaires were returned from the hospital and petroleum organisation, with response rates of 55 percent and 52 percent, respectively. Using a split sample procedure principal factor analysis and confirmatory factor analysis revealed six identical cross-industrial measurement concepts in independent samples—five measures of safety climate and one of safety behaviour. The factors’ psychometric properties were explored with satisfactory internal consistency and concept validity. Thus, a common cross-industrial structural model was developed and tested using structural equation modelling (SEM). SEM revealed that a cross-industrial structural model could be identified among health care workers and offshore workers in the North Sea. The most significant contributing variables in the model testing stemmed from organisational management support for safety and supervisor/manager expectations and actions promoting safety. These variables indirectly enhanced safety behaviour (stop working in dangerous situations) through transitions and teamwork across units, and teamwork within units as well as learning, feedback, and improvement. Two new safety climate instruments were validated as part of the study: (1) Short Safety Climate Survey (SSCS) and (2) Hospital Survey on Patient Safety Culture-short (HSOPSC-short). Based on development of measurements and structural model assessment, this study supports the possibility of a common safety climate structural model across health care and the offshore petroleum industry.  相似文献

This study attempts to identify idiosyncrasies of organizational factors on safety and their influence mechanisms in Taiwan and Japan. Data were collected from employees of Taiwanese and Japanese oil refinery plants. Results show that organizational factors on safety differ in the two countries. Organizational characteristics in Taiwanese plants are highlighted as: higher level of management commitment to safety, harmonious interpersonal relationship, more emphasis on safety activities, higher devotion to supervision, and higher safety self-efficacy, as well as high quality of safety performance. Organizational characteristics in Japanese plants are highlighted as: higher level of employee empowerment and attitude towards continuous improvement, more emphasis on systematic safety management approach, efficient reporting system and teamwork, and high quality of safety performance. The casual relationships between organizational factors and workers' safety performance were investigated using structural equation modeling (SEM). Results indicate that the influence mechanisms of organizational factors in Taiwan and Japan are different. These findings provide insights into areas of safety improvement in emerging countries and developed countries respectively.  相似文献

This paper develops a step-by-step methodology for the application of Full Bayes (FB) approach for before-and-after analysis of road safety countermeasures. As part of this methodology, it studies the posterior prediction capability of Bayesian approaches and their use in crash reduction factor (CRF) estimation. A collection of candidate models are developed to investigate the impacts of different countermeasures on road safety when limited data are available. The candidate models include traditional, random effects, non-hierarchical and hierarchical Poisson-Gamma and Poisson-Lognormal (P-LN) distributions. The use of random effects and hierarchical model structures allows treatment of the data in a time-series cross-section panel, and deal with the spatial and temporal effects in the data. Next, the proposed FB estimation methodology is applied to urban roads in New Jersey to investigate the impacts of different treatment measures on the safety of “urban collectors and arterial roads” with speed limits less than 45 mph. The treatment types include (1) increase in lane width, (2) installation of median barriers, (3) vertical and horizontal improvements in the road alignment; and (4) installation of guide rails. The safety performance functions developed via different model structures show that random effects hierarchical P-LN models with informative hyper-priors perform better compared with other model structures for each treatment type. The individual CRF values are also found to be consistent across the road sections, with all showing a decrease in crash rates after the specific treatment except guide rail installation treatment. The highest decrease in the crash rate is observed after the improvement in vertical and horizontal alignment followed by increase in lane width and installation of median barriers. Overall statistical analyses of the results obtained from different candidate models show that when limited data are available, P-LN model structure combined with higher levels of hierarchy and informative priors may reduce the biases in model parameters resulting in more robust estimates.  相似文献

Short-term tradeoffs between productivity and safety often exist in the operation of critical facilities such as nuclear power plants, offshore oil platforms, or simply individual cars. For example, interruption of operations for maintenance on demand can decrease short-term productivity but may be needed to ensure safety. Operations are interrupted for several reasons: scheduled maintenance, maintenance on demand, response to warnings, subsystem failure, or a catastrophic accident. The choice of operational procedures (e.g. timing and extent of scheduled maintenance) generally affects the probabilities of both production interruptions and catastrophic failures. In this paper, we present and illustrate a dynamic probabilistic model designed to describe the long-term evolution of such a system through the different phases of operation, shutdown, and possibly accident. The model's parameters represent explicitly the effects of different components' performance on the system's safety and reliability through an engineering probabilistic risk assessment (PRA). In addition to PRA, a Markov model is used to track the evolution of the system and its components through different performance phases. The model parameters are then linked to different operations strategies, to allow computation of the effects of each management strategy on the system's long-term productivity and safety. Decision analysis is then used to support the management of the short-term trade-offs between productivity and safety in order to maximize long-term performance. The value function is that of plant managers, within the constraints set by local utility commissions and national (e.g. energy) agencies. This model is illustrated by the case of outages (planned and unplanned) in nuclear power plants to show how it can be used to guide policy decisions regarding outage frequency and plant lifetime, and more specifically, the choice of a reactor tripping policy as a function of the state of the emergency core cooling subsystem.  相似文献

Organizational risk indicators are proposed as a tool for risk control during operation of offshore installations, as a complement to QRA-based indicators. An organizational factor framework is developed based on a review of existing organizational factor frameworks, research on safety performance indicators, and previous work on QRA-based indicators. The results comprise a qualitative organizational model, proposed organizational risk indicators, and a quantification methodology for assessing the impact of the organization on risk. The risk indicators, when validated, will aid in a frequent control of the risk in the periods between the updating of the quantitative risk assessments.  相似文献

One of the biggest challenges for organizations in today's competitive business environment is to create and preserve a self-sustaining safety culture. Typically, the key drivers of safety culture in many organizations are regulation, audits, safety training, various types of employee exhortations to comply with safety norms, etc. However, less evident factors like networking relationships and social trust amongst employees, as also extended networking relationships and social trust of organizations with external stakeholders like government, suppliers, regulators, etc., which constitute the safety social capital in the Organization—seem to also influence the sustenance of organizational safety culture. Can erosion in safety social capital cause deterioration in safety culture and contribute to accidents? If so, how does it contribute? As existing accident analysis models do not provide answers to these questions, CAMSoC (Curtailing Accidents by Managing Social Capital), an accident analysis model, is proposed. As an illustration, five accidents: Bhopal (India), Hyatt Regency (USA), Tenerife (Canary Islands), Westray (Canada) and Exxon Valdez (USA) have been analyzed using CAMSoC. This limited cross-industry analysis provides two key socio-management insights: the biggest source of motivation that causes deviant behavior leading to accidents is ‘Faulty Value Systems’. The second biggest source is ‘Enforceable Trust’. From a management control perspective, deterioration in safety culture and resultant accidents is more due to the ‘action controls’ rather than explicit ‘cultural controls’. Future research directions to enhance the model's utility through layering are addressed briefly.  相似文献

Communication between employees and supervisors about safety-related issues is an important component of a safe workplace. When supervisors receive information from employees about safety issues, they may gain otherwise-missed opportunities to correct these issues and/or prevent negative safety outcomes. A series of three studies were conducted to identify various safety silence motives, which describe the reasons that employees do not speak up to supervisors about safety-related issues witnessed in the workplace, and to develop a tool to assess these motives. Results suggest that employees stay silent about safety issues based on perceptions of altering relationships with others (relationship-based), perceptions of the organizational climate (climate-based), the assessment of the safety issue (issue-based), or characteristics of the job (job-based). We developed a 17-item measure to assess these four motives, and initial evidence was found for the construct and incremental validity of the safety silence motives measure in a sample of nurses.  相似文献

In the Norwegian offshore oil and gas industry risk analyses have been used to provide decision support for more than 20 years. The focus has traditionally been on the planning phase, but during the last years a need for better risk analysis methods for the operational phase has been identified. Such methods should take human and organizational factors into consideration in a more explicit way than the traditional risk analysis methods do. Recently, a framework, called hybrid causal logic (HCL), has been developed based on traditional risk analysis tools combined with Bayesian belief networks (BBNs), using the aviation industry as a case. This paper reviews this framework and discusses its applicability for the offshore industry, and the relationship to existing research projects, such as the barrier and operational risk analysis project (BORA). The paper also addresses specific features of the framework and suggests a new approach for the probability assignment process. This approach simplifies the assignment process considerably without loosing the flexibility that is needed to properly reflect the phenomena being studied.  相似文献
