The rate of forgetting words, pictures, and designs was assessed in a subsample of individuals with moderate to severe traumatic brain injury (TBI) who showed disproportionately severe persistent memory impairment. Relative to control Ss, the patients had an abnormally fast rate of forgetting pictures, but not words or designs. TBI patients with persistent memory impairment may process some forms of information inefficiently but do not appear to have a fundamental deficit in memory consolidation. The role of the thalamus in memory is potentially very complex. However, most studies on the memory impairments of thalamic lesions have focused on retrospective memory, rarely on prospective memory. To explore the effect of thalamic lesions on event-based prospective memory (EBPM) and time-based prospective memory (TBPM), respectively, and to verify the hypothesis that the thalamus is involved in the prospective memory, EBPM and TBPM tasks were administered to 18 thalamic stroke patients and 18 age- and education-matched healthy controls. In the EBPM task, subjects were asked to perform an action whenever particular words were presented. In the TBPM task, subjects were asked to perform an action at certain times. Compared with the performance of healthy controls on EBPM and TBPM tasks (EBPM, 4.3 ± 1.5; TBPM, 5.4 ± 1.0), there was a significant difference in the performance of thalamic stroke patients in the TBPM tasks (2.4 ± 1.6), but no difference was found in EBPM tasks (3.7 ± 1.1). A battery of tests that uses computer and laser disk technology to simulate learning and memory tasks of everyday life, such as recalling names after introduction and remembering items on a grocery list, is described. The performance of 110 persons suffering Age-Associated Memory Impairment was factor analyzed and yielded everyday memory dimensions of verbal memory, visual memory, psychomotor speed, attention, and vigilance. These computerized performance dimensions are associated with relevant standard clinical measures of memory performance, such as the Benton Visual Retention Test and the Wechsler Memory Scale Paired-Associated Learning and Logical Memory subtests. The clinical and research utility of the computerized measures and the new diagnostic category of Age-Associated Memory Impairment are discussed. One third of patients with aneurysmal subarachnoid haemorrhage (ASAH) present with headache only. A prompt diagnosis is crucial, but these patients must be distinguished from patients with non-haemorrhagic benign thunderclap headache (BTH). The headache characteristics and associated features at onset in subarachnoid haemorrhage and benign thunderclap headache were studied to delineate the range of early features in these conditions. In this prospective study, one of two observers interviewed 102 patients with acute severe headache by means of a standard questionnaire. The patients were alert on admission and had no focal deficits. ASAH was subsequently diagnosed in 42 patients, non-aneurysmal perimesencephalic haemorrhage (PMH) in 23 patients, and BTH in 37 patients. Headache developed almost instantaneously in 50% of patients with ASAH, 35% of patients with PMH, and 68% of patients with BTH and within 1 to 5 minutes in 19%, 35%, and 19%, respectively. Loss of consciousness was reported in 26% of patients with ASAH, 4% of patients with PMH and 16% of patients with BTH, and transient focal symptoms in 33%, 9%, and 22% respectively. Seizures and double vision had occurred only in ASAH. Vomiting and physical exertion preceding the onset of headache were more frequent in patients with ASAH (69% and 50%) and those with PMH (83% and 39%) than in those with BTH (43% and 22%). Headache developed almost instantaneously in only half the patients with aneurysmal rupture and in two thirds of patients with benign thunderclap headache. In patients with acute severe headache, female sex, the presence of seizures, a history of loss of consciousness or focal symptoms, vomiting, or exertion increases the probability of ASAH, but these characteristics are of limited value in distinguishing ASAH from BTH. Aneurysmal rupture should be considered even if focal signs are absent and the headache starts within minutes.  相似文献

Assessed the short-term memory capacities of 4 chronic, schizophrenic and 4 nonschizophrenic psychiatric patients who served as controls. The information to be remembered was presented both visually and verbally and was later probed for after a variable interval by either visual or verbal cues. Schizophrenics and controls did not differ with respect to which type of cue retrieved more of the information, suggesting that the modality in which the information was stored was the same for both groups. However, schizophrenics were markedly inferior to controls regarding both the initial acquisition of information and the maintenance of it in storage. Older adults are assumed to have poor destination memory—knowing to whom they tell particular information—and anecdotes about them repeating stories to the same people are cited as informal evidence for this claim. Experiment 1 assessed young and older adults' destination memory by having participants tell facts (e.g., “A dime has 118 ridges around its edge”) to pictures of famous people (e.g., Oprah Winfrey). Surprise recognition memory tests, which also assessed confidence, revealed that older adults, compared to young adults, were disproportionately impaired on destination memory relative to spared memory for the individual components (i.e., facts, faces) of the episode. Older adults also were more confident that they had not told a fact to a particular person when they actually had (i.e., a miss); this presumably causes them to repeat information more often than young adults. When the direction of information transfer was reversed in Experiment 2, such that the famous people shared information with the participants (i.e., a source memory experiment), age-related memory differences disappeared. In contrast to the destination memory experiment, older adults in the source memory experiment were more confident than young adults that someone had shared a fact with them when a different person actually had shared the fact (i.e., a false alarm). Overall, accuracy and confidence jointly influence age-related changes to destination memory, a fundamental component of successful communication. Everyday memory was tested in a group of adults manifesting Age-Associated Memory Impairment; a computerized battery of tests was constructed to simulate memory tasks of daily life. Confirmatory and other structural equation models were estimated for the entire sample of 273 Ss and for 3 age groups. A 4-factor model was found to fit the data well and was invariant across age and gender. After education had been controlled, only the General Recall factor was found to be consistently related to age in both men and women; the other 3 factors—Narrative Memory, Digit Recall, and Visual Memory—were related to age only in men. Confirmatory factor analyses of the everyday memory tests combined with several psychometric memory tests suggested that some of the latter (the Benton Visual Retention Test and Wechsler Memory Scale Hard Paired Associates) load on more than 1 factor of everyday memory, suggesting complex relationships between the 2 types of tests. The earliest neuroanatomical changes in amnestic mild cognitive impairment (aMCI) involve the hippocampus and entorhinal cortex, structures implicated in the integration and learning of associative information. The authors hypothesized that individuals with aMCI would have impairments in associative memory above and beyond the known impairments in item memory. A group of 29 individuals with aMCI and 30 matched control participants were administered standardized tests of object-location recall and symbol-symbol recall, from which both item and associative recall scores were derived. As expected, item recall was impaired in the aMCI group relative to controls. Associative recall in the aMCI group was even more impaired than was item recall. The best group discriminators were measures of associative recall, with which the sensitivity and specificity for detecting aMCI were 76% and 90% for symbol-symbol recall and were 86% and 97% for object-location recall. Associative recall may be particularly sensitive to early cognitive change in aMCI, because this ability relies heavily on the medial temporal lobe structures that are affected earliest in aMCI. Incorporating measures of associative recall into clinical evaluations of individuals with memory change may be useful for detecting aMCI. Objective: The present study aimed to investigate prospective memory (PM) in persons with mild cognitive impairment (MCI). Method: Twenty individuals with MCI (10 with an amnestic profile and 10 with a dysexecutive profile of cognitive impairment) and 20 control subjects (CS) were recruited. In the PM tasks, subjects had to execute three actions after 20 min had elapsed (time-based condition) or after a timer rang (event-based condition). Separate scores were computed for correct recall of the intention to perform the actions (prospective component) and for correct execution of the actions (retrospective component). Results: Although individuals with MCI were less accurate than CS in both prospective (Cohen's d ranged from 1.04 to 2.23) and retrospective (Cohen's d ranged from 0.81 to 1.06) components of the experimental task, they were significantly more impaired in the former than the latter component (Cohen's d = 0.42). Moreover, the deficit in the prospective component of the time-based task was particularly evident in MCI participants presenting with a dysexecutive impairment in respect to amnestic MCI individuals (Cohen's d = 0.99). Conclusions: Results of the present study show that the ability of subjects with MCI to comply effectively with a planned delayed intention is impaired and suggest that dysexecutive disorders are likely responsible for this deficit. The authors attempt to provide a better understanding of the differences between the normal memory decline characteristic of age-associated memory impairment (AAMI) and the pathological decline typical of mild Alzheimer's disease (AD). Batteries of traditional memory tests and computer-simulated everyday-memory tests discriminated between the 2 groups, which were matched on age, gender, and education, with reasonable degrees of accuracy (87.5% and 88.4%, respectively). False positives were the most frequent classification errors when using either battery. These results indicate that it is possible to use ecologically valid memory assessment paradigms without sacrificing discriminant validity. We document the dissociation of preserved calculation skills in a patient with impaired auditory short-term memory. The patient (MRF) had a memory span of three digits. Furthermore, he showed rapid decrement in performance of single digits and letters with both auditory and visual presentation in the Brown-Peterson forgetting task. Analysis of his calculation skills revealed a normal ability to solve auditorily presented multidigit addition and subtraction problems such as 173 + 68 and to execute the Paced Auditory Serial Addition Task (Sampson, 1956, 1958; Gronwall, 1977). In addition, his performance on other tests, including arithmetic manipulation of natural numbers, decimals and fractions, approximation, magnitude, ratio, and percentage, appeared to be normal (Hitch, 1978b). It is argued that these findings require a revision of Baddeley and Hitch's (1974) concept of the function of working memory.  相似文献

Everyday memory performance was examined longitudinally in 2 groups of Ss meeting the diagnostic criteria for age-associated memory impairment (AAMI). One group of 157 participants in a drug trial for reversing memory loss in AAMI was tested over multiple sessions. The other group of 75 persons did not participate in a drug trial and thus was tested only twice. Both groups were retested for longitudinal follow-up about 4 yrs after initial session. Follow-up test performance remained fairly stable relative to initial performance in both groups. The drug study group showed large practice effects during the course of the drug studies, but these effects subsided after the drug studies' end. This study investigated prospective memory and its relationship to executive and memory functions in persons with Parkinson's disease (PD). Twenty-three individuals with PD and 25 healthy comparison participants participated in the study. In the prospective memory tasks, participants were asked to execute 3 actions after 20 min (time-based condition) or after a timer ring (event-based condition). A score was computed for the correct recall of the intention to perform the actions (prospective component) and for the correct execution of the actions (retrospective component). Participants with PD also received an extensive neuropsychological test battery. PD participants were less accurate than comparison participants in the prospective component of the time-based but not the event-based task. Individuals with PD were also impaired on the retrospective component of both tasks. In the PD group, a general trend toward significant correlations was found between performance level on the prospective memory component of the time-based task and scores on executive and working memory measures. These results document that prospective memory is impaired in PD possibly in relation to a dysregulation of cognitive functions associated with frontal systems. The claim that a person's memory for an event may be altered by information encountered after the event has been based on studies showing that Ss who are given false or misleading information about a previously witnessed event perform more poorly on tests of memory for the event than Ss who are not misled. It is argued that the available evidence does not imply that misleading postevent information impairs memory for the original event because the procedure used in previous studies is inappropriate for assessing effects of misleading information on memory. A more appropriate procedure is presented that involves a change in the recognition test phase. This procedure was used in 6 experiments with 792 undergraduates. Findings indicate that misleading postevent information had no effect on memory for the original event. Several recent studies that seem to contradict this conclusion are reviewed, and it is shown that the studies do not pose problems for the present authors' position. Findings are also considered in relation to the more general claim that information about an event from various sources are integrated into a single representation of the event. Extended previous research (J. A. Cooper et al, 1991) and examined whether the impairment in verbal and nonverbal short-term memory (STM) documented in chronically treated patients with Parkinson's disease (PD) is attributable to the pathology of the disease or to antiparkinsonian drug therapy. Verbal recall by a group of 58 never-treated PD patients was virtually perfect when the retention intervals were distractor free but was impaired, as compared with a matched group of 34 healthy control Ss, following distractor-filled intervals. Nonverbal recall by the PD group showed significant forgetting after distractor-free but not distractor-filled intervals. Recall was not related to depressive symptoms, motor disability, or distractor task performance. Thus, STM impairment in PD occurs for both verbal and nonverbal material and arises directly from the disease pathology rather than from antiparkinsonian medication. There is evidence that nonverbal memory problems in obsessive compulsive disorder (OCD) are mediated by impaired strategic processing. Although many studies have found verbal memory to be normal in OCD, these studies did not use tests designed to stress organizational strategies. This study examined verbal and nonverbal memory performance in 33 OCD patients and 30 normal control participants with the Rey-Osterrieth Complex Figure Test and the California Verbal Learning Test. OCD patients were impaired on verbal and nonverbal measures of organizational strategy and free recall. Multiple regression modeling indicated that free recall problems in OCD were mediated by impaired organizational strategies used during learning trials. Therefore, verbal and nonverbal episodic memory deficits in OCD are affected by impaired strategic processing. Contrary findings notwithstanding, the prevailing notion is that recognition memory is little affected by Parkinson's disease (PD). Both a power analysis and a meta-analysis were conducted to help clarify the degree of recognition memory deficit associated with PD. The power analysis confirmed that, in general, memory studies of PD participants have been underpowered. This analysis indicated the need to pool study results in a subsequent meta-analysis, the main finding of which was that recognition memory deficits do occur with PD. The largest deficit occurs in PD participants with dementia. Nevertheless, deficits also occur in PD participants without dementia on medication, but nondopaminergic central nervous system abnormalities are more likely to underlie this deficit than PD medication itself. Future development of a theory of cognitive dysfunction in PD should take into account these recognition memory deficits, which may increase with disease progression. Forty-six preclinical Alzheimer's disease (AD) participants and 188 nondemented control persons from the Kungsholmen Project (L. B?ckman et al., 2004) were compared on prospective memory (ProM) and retrospective memory (RetM) tasks 3 years before dementia diagnosis. The preclinical AD participants showed deficits in both ProM and RetM. Most interestingly, logistic regression analyses revealed that ProM made an independent contribution to the prediction of AD over and above that of RetM. This finding suggests that ProM and RetM tap partly different cognitive operations. Furthermore, within the ProM task, both the retrospective and prospective components were similarly impaired in preclinical AD. Within RetM, the preclinical AD participants were impaired on indices of encoding, storage (forgetting), and retrieval of information. Hence, the findings indicate a rather global episodic memory impairment in preclinical AD that cuts across type of memory assessed (ProM and RetM) as well as across different components of both the ProM and RetM tasks. A large body of work has revealed that people remember emotionally arousing information better than neutral information. However, previous research has revealed contradictory effects of emotional events on memory for neutral events that precede or follow them: In some studies, emotionally arousing items have impaired memory for immediately preceding or following items, and in others arousing items enhanced memory for preceding items. By demonstrating both emotion-induced enhancement and impairment, Experiments 1 and 2 clarified the conditions under which these effects are likely to occur. The results suggest that emotion-induced enhancement is most likely to occur for neutral items that (a) precede (and so are poised to predict the onset of) emotionally arousing items, (b) have high attentional weights at encoding, and (c) are tested after a delay period of a week rather than within the same experimental session. In contrast, emotion-induced impairment is most likely to occur for neutral items near the onset of emotional arousal that are overshadowed by highly activated competing items during encoding.
