Obesity and low levels of physical and metabolic fitness are risk factors for cardiovascular disease and diabetes. The purpose of this investigation was to attenuate obesity and improve physical and metabolic fitness in elementary school children. Schools have the opportunity, mechanisms, and personnel in place to deliver nutrition education, fitness activities, and a school food service that is nutritious and healthy. Cohorts from grades 3 to 5 in two school districts in rural Nebraska (Intervention/Control) participated in a 2-year study of physical activity and modified school lunch program. Data collection for aerobic capacity, body composition, blood chemistry, nutrition knowledge, energy intake, and physical activity was at the beginning and end of each year. Int received enhanced physical activity, grade specific nutrition education, and a lower fat and sodium school lunch program. Con continued with a regular school lunch and team sports activity program. At year 2, Int lunches had significantly less energy (9%), fat (25%), sodium (21%), and more fiber (17%). However, measures of 24-hour energy intake for Int and Con showed significant differences for sodium only. Physical activity in the classroom was 6% greater for Int compared to Con (p < 0.05) but physical activity outside of school was approximately 16% less for Int compared to Con (p < 0.05). Body weight and body fat were not different between schools for normal weight or obese children. No differences were found for cholesterol, insulin, and glucose; however, HDL cholesterol was significantly greater and cholesterol/HDL was significantly less for Int compared to Con (p < 0.05). It appears that compensation in both energy intake and physical activity outside of school may be responsible for the lack of differences between Int and Con.  相似文献

BACKGROUND: Although the literature on increased physical fitness and psychological outcomes has grown large, a number of methodological limitations remain unaddressed. The present study was designed to address a number of these limitations while examining the short- and long-term psychological effects following completion of a 12-week aerobic fitness program using bicycle ergometry (and confirmed increases in fitness). METHOD: Following completion of a 12-week aerobic fitness program (and through 12 months of follow-up), 82 adult participants completed the Beck Depression Inventory, Profile of Mood States, State-Trait Anxiety Inventory, and the Tennessee Self-Concept Scale. Physiological measures used to assess changes in aerobic fitness were maximal work load, submaximal heart rate at a standard work load, predicted maximum oxygen uptake, and resting heart rate. RESULTS: Exercise participants experienced a positive fitness change and psychological improvement over the initial 12-week program compared to a control group. At 1 year follow-up, physiological and psychological benefits remained significantly improved from baseline. CONCLUSIONS: Overall, results indicate that exercise-induced increases in aerobic fitness have beneficial short-term and long-term effects on psychological outcomes. We postulate that participants in the exercise group did not increase the amount of weekly exercise they performed over the 12-month follow-up period and thus the maintenance of the psychological improvements occurred concurrent with equal or lesser amounts of exercise.  相似文献

BACKGROUND: Wolff-Parkinson-White syndrome is thought to be a congenital disease, however, its exact prevalence is not known. This may be because of the intermittent activity of accessory pathways in some cases and fluctuations in autonomic tone. AIMS: To investigate the prevalence of ventricular preexcitation by electrocardiography and reported symptoms in each school age child in Yamanashi prefecture. METHODS: From 1994 to 1996, answers to a questionnaire, results of physical examination, and electrocardiography were obtained from all schoolchildren in Yamanashi prefecture (n = 92,161; total population 880,000) on admission to elementary school (age 6 to 7 years, n = 28,395), junior high school (age 12 to 13 years, n = 31,206), and high school (age 14 to 15 years, n = 32,837). RESULTS: Elementary and junior high school students had a significantly lower prevalence of preexcitation than high school students (0.073% and 0.070% v 0.174%, p < 0.001). The prevalence of left free wall pathway was highest in high school students (n = 27) compared with elementary (n = 6) and junior high school students (n = 5) (p < 0.005). The only symptom noted in the answers to the questionnaire was palpitations. The symptomatic cases were more frequent in high school (n = 13) than in elementary (n = 1) and junior high school (n = 2) children, but not significantly. No student with preexcitation had associated heart disease or family history of Wolff-Parkinson-White syndrome or sudden death. CONCLUSIONS: The prevalence of preexcitation in younger schoolchildren was less frequent than previously reported. The prevalence of preexcitation and left free wall pathways increased with age. The symptoms were few and there was no significant morbidity.  相似文献

Three groups of 40 college students were each given different treatments in an experiment designed to investigate the effect of systematic physical fitness training combined with counseling on measured self-concept. One experimental group received a fitness program consisting of flexibility training and systematic distance running; a 2nd group received the same physical training plus 1 hr/wk of group counseling designed to reinforce progress made in the fitness program; and a 3rd group (control) received no physical fitness training or counseling. After a 10-wk program, the group receiving fitness training and running made a significant gain in self-concept as measured by the Tennessee Self-Concept Scale. The Ss were divided into low and high self-concept groups for statistical analysis. It was found that the students who received fitness training and counseling and had low self-concepts on the pretest measure made significant positive changes in self-concept. Preflight, inflight, and postflight exercise response tests were conducted on the astronauts of the second Skylab mission (Skylab 3) as part of an evaluation of physiological adaptation to long-term weightlessness. The flight phase of this mission was 59 d in duration. An exercise protocol was designed around a bicycle ergometer which was used to apply work loads approximating 25, 50, and 75% of each crewman's measured maximum aerobic capacity (VO2 max). Respiratory gas exchange (VO2, VCO2, and VE), heart rate, and blood pressure were measured during all tests; cardiac output was measured at selected times during preflight and postflight tests. Data obtained both at rest and during exercise in flight showed no consistent changes which would indicate a degraded physical work capacity. In fact, heart rate during exercise actually decreased for all crewmen in flight. This response indicated improved physical fitness in flight relative to preflight. The improved physical condition of this crew relative to that of the first Skylab crew is attributed to frequent performance of high levels of aerobic exercise in flight. The postflight period of readaptation of 1 G was characterized by a marked tachycardia, during which time stroke volume was decreased. OBJECTIVES: This study examined cross sectionally the physical activity patterns associated with low, moderate, and high levels of cardiorespiratory fitness. METHODS: Physical activity was assessed by questionnaire in a clinic population of 13,444 men and 3972 women 20 to 87 years of age. Estimated energy expenditure (kcal.wk-1) and volume (min.wk-1) of reported activities were calculated among individuals at low, moderate, and high fitness levels (assessed by maximal exercise tests). RESULTS: Average leisure time energy expenditures of 525 to 1650 kcal.wk-1 for men and 420 to 1260 kcal.wk-1 for women were associated with moderate to high levels of fitness. These levels of energy expenditure can be achieved with a brisk walk of approximately 30 minutes on most days of the week. In fact, men in the moderate and high fitness categories walked between 130 and 138 min.wk-1, and women in these categories walked between 148 and 167 min.wk-1. CONCLUSIONS: Most individuals should be able to achieve these physical activity goals and thus attain a cardiorespiratory fitness level sufficient to result in substantial health benefits.  相似文献

Examined relationships between a variety of exercise and fitness-related variables and heart rate recovery from a mental stressor. 108 women (mean age 26.4 yrs) participated in 2 laboratory sessions. In the 1st session, exercise and fitness measures were taken. In the 2nd session, Ss completed health and stress-related questionnaires, and heart rate recovery from a serial subtraction task was measured. Faster heart rate recovery was significantly correlated with several fitness indices including {v}{o}?Max, self-reported fitness level, and level of activity in sports. However, faster heart rate recovery was not associated with improved health or mood, or lower general life stress; benefits in these areas were related to higher levels of fitness and exercise. Symptom-limited, graded exercise treadmill testing was performed by 4,968 white and black adults, ages 18-30 yr, during the baseline examination for the Coronary Artery Risk Development in Young Adults (CARDIA) study. Compared with nonsmokers, the mean exercise test duration of smokers was 29-64 s shorter depending on race/gender group (all P < 0.001), but mean duration to heart rate 130 (beats.min-1) ranged from 20-50 s longer (P < 0.05). In each race/gender group, test duration to heart rates up to 150 was 15-35 s longer (P < 0.05) in smokers than in nonsmokers after adjustment for age, sum of skinfolds, hemoglobin, and physical activity score. The mean maximum heart rate was lower in smokers than in nonsmokers (difference ranging from 6.7 beats.min-1 in white men to 11.2 beats.min-1 lower in black women, P < 0.001), although maximum rating of perceived exertion was nearly identical in smokers and nonsmokers. Chronic smoking appears to blunt the heart rate response to exercise, so that exercise duration to submaximal heart rates is increased even though maximal performance is impaired. This may result from downloading of beta-receptors caused by smoking. Smoking status should be considered in the evaluation of physical fitness data utilizing submaximal test protocols, or else the fitness of smokers relative to nonsmokers is likely to be overestimated.  相似文献

Evaluated the physical and psychological benefits of an 8-wk individualized exercise program for 83 adults (aged 22–75 yrs). The 49 Ss who completed the program differed from the 34 dropouts on preexercise data only on persistence (i.e., the pretest time spent in exercise), which indicated a possible motivational difference. Those who completed the program showed statistically significant improvements in terms of persistence, fitness, and physical self-concept and also showed reduced psychological tension (as measured by the Profile of Mood States). Persistence correlated significantly with vigor, and fitness correlated significantly with both vigor and physical self-concept. Analysis of Ss categorized by initial levels of fitness and stress showed that changes in stress following exercise occurred only for those who were below the mean of the sample both physically and psychologically prior to beginning the exercise program. Strategies to motivate those clients who may benefit most from therapeutic exercise programs as an adjunct to stress management are discussed. Exercise induced plasma catecholamine levels, of post myocardial infarct patients, were compared before and after a 27 week training program in two groups. Heavy Intensity Exercise (H.I.E.) and Light Intensity Exercise (L.I.E.). The men (age 38-54 years) were exercised on a bicycle ergometer at constant submaximal work loads (test load) and to a symptom limited maximum work load, both before and after the training program. Forearm venous samples were obtained after 30 minutes rest and within 3 minutes of completion of the test load. After training a significant increase in both maximum work load and heart rate (P less than 0.05) was observed in the H.I.E. group. At the test load these individuals had a small (6%) decrease in heart rate. No significant changes were observed in the L.I.E. group at submaximum or maximum exercise. Prior to training, exercise resulted in a significant increase in norepinephrine (NE) with no change in epinephrine (E). Training resulted in a significant reduction (P less than 0.05) in the NE response at the test load of the H.I.E. group while no change was found in the L.I.E. group. Endurance jogging reduced the sympathetic response to moderate exercise.  相似文献

OBJECTIVE: To develop and test the intra-rater reliability of an interview-administered questionnaire that assesses lifetime patterns of total physical activity including occupational, household, and exercise/sports activities. METHODS: The questionnaire was developed and pretested using cognitive interviewing techniques on a sample of women with and without previous breast cancer diagnoses. A pilot study was conducted with 115 women who were interviewed twice, 6 to 8 wk apart by interviewers trained in cognitive interviewing methods. Respondents used recall calendars to record their education, occupations, life events, and physical activity patterns before the interviews. Interviewers helped respondents recall their lifetime exposures, including their occupational, household, and exercise/sports activities, using these calendars and memory-probing strategies. Activity levels were estimated as the average number of hours of activity per week over different time periods. Means and correlation coefficients were estimated and compared for the two time periods. RESULTS: The questionnaire was found to be highly reliable. The test-retest correlations for hours per week spent in total lifetime physical activity was 0.74, for lifetime occupational activity was 0.87, for household activity was 0.77, and for exercise/sports activities was 0.72. CONCLUSIONS: This is the first questionnaire to measure lifetime physical activity by collecting data on each type of physical activity separately over lifetime and by measuring frequency, intensity, and duration of each activity. It is also the first physical activity questionnaire to be developed, refined, and administered using cognitive-based methods employed in survey research. Respondents were able to reliably recall their lifetime physical activity patterns. This instrument can be used for any disease outcome for which physical activity may be a risk factor.  相似文献

Evaluated 120 healthy, sedentary, middle-aged adults randomly assigned to either a 6-mo home-based aerobic exercise training program or to an assessment-only control condition. Adherence across the 6-mo period by the exercise group was demonstrated by self-report and heart rate microprocessor methods to exceed 75%. A 14-item Likert rating scale was used to measure a variety of psychological variables. Significant between-groups differences favoring the exercise group were found on scale items closely associated with actual physical changes that occurred with exercise (satisfaction with shape/appearance, perceived fitness). Previous studies concerning psychological benefits of exercise among the elderly has focused predominantly on the effects of aerobic exercise. In the present study, psychological and behavioral adaptations in response to 12-weeks of strength training were examined in medically healthy but sedentary 42 older adults (mean age = 68 years). The purpose of this study was to evaluate the effects of high and low intensity resistance training intensity on a) muscular fitness, b) psychological affect, and c) neurocognitive functioning. Subjects were randomly assigned to high intensity/low volume (EXH: 2 sets of 8 to 10 repetitions for 75 to 85% of 1 RM), low intensity/high volume (EXL: 2 sets of 14 to 16 repetitions for 55 to 65% of 1 RM), or no exercise control programs. Prior to and following the 12-week program, subjects underwent comprehensive physiological and psychological evaluations. Physiological assessment included measurements of blood pressure, heart rate, arm and leg muscle strength, body composition, and oxygen consumption (VO2max). Psychological measures included evaluations of mood, anxiety, and physical self-efficacy as well as cognitive functioning. The results of this study indicated that both high and low intensity strength programs were associated with marked improvements in physiological fitness and psychological functioning. Specifically, subjects in the strength training programs increased overall muscle strength by 38.6% and reduced percent body fat by 3.0%. Favorable psychological changes in the strength-trained subjects included improvements in positive and negative mood, trait anxiety, and perceived confidence for physical capability. The treatment effects of neurocognitive functioning were not significant. In summary, this study demonstrated that participation in 12-weeks of high or low intensity strength training can improve overall physical fitness, mood, and physical self-efficacy in older adults while cognitive functioning remains constant.  相似文献

Apparently healthy middle-aged men (n = 175) were recruited from a population sample and completed questionnaires about habitual physical activity, smoking, beverage consumption and sleep habits. Body mass index (BMI), heart rate and blood pressure were measured at rest and during submaximal exercise; frequency of ventricular premature beats (VPB) on an ECG rhythm strip; hand grip strength; and serum cholesterol. These characteristics were correlated with duration of treadmill exercise by the Bruce protocol. Univariate analysis indicated that treadmill performance was significantly and positively correlated with leisure-time physical activity and personal reports of sweating and/or dyspnea occurring regularly during such physical activity. Performance was negatively correlated with age, BMI, resting heart rate, cigarette smoking, and consumption of caffeine-containing beverages, but was insignificantly related to job physical activity, hand grip strength, alcohol consumption, sleep habits, blood pressure, cigar smoking, serum cholesterol, and the frequency of VPB. A 0.75 multiple correlation coefficient was found between treadmill performance and 11 of the above variables and the r is increased to 0.81 by adding heart rate during submaximal exercise. It is concluded that substantial prediction of work capacity and physical fitness of population is achieved by questionnaires and easily obtained, noninvasive physical measures.  相似文献

PURPOSE: We sought to evaluate the effectiveness of continued training with deep water running (DWR) on the healing process from a lower leg fracture in maintaining aerobic fitness. CASE SUMMARY: An elite male middle distance runner presented with a Jones fracture of the right foot. The patient opted for nonsurgical treatment. Non-weight-bearing immobilization in a cast proceeded for 14 weeks. The patient continued training by simulating land running and team land workouts with DWR. DWR was the principal mode of exercise through week 24. Physiological and race performance assessments noted a progressive and complete return to preinjury fitness and performance levels. DISCUSSION: DWR can be used by competitive runners during a rehabilitative period to maintain fitness and does not impede the healing process. In developing a DWR regimen, it should be noted that the exercise heart rate when working above the ventilatory threshold will be approximately 12 bpm lower in water. RELEVANCE: DWR may be feasible for maintenance of competitive edge during lower trunk injury rehabilitation.  相似文献

We investigated the influence of nutrition and exercise interventions within cognitive/behavioral and public health formats on weight and blood lipid profiles in obese children. Compliance was also examined as well as the relationship of the compliance measures with clinical outcome variables. Three conditions were compared over 16 sessions: nutrition and eating-habit change followed by exercise (NE), exercise followed by nutrition and eating-habit change (EN), and an information control (INFO). NE and EN were presented in a cognitive/ behavioral framework which focused on the development of self-regulation whereas the INFO condition received the same material in a public health/educational model. NE and EN participants evidenced modest, yet significant, reductions in weight and blood lipids, and the impact of these two interventions endured at a five-year follow-up. In contrast, INFO participants displayed stable weight and blood lipids during the course of the program, and most remained morbidly obese at follow-up. Improved nutrition, increased physical activity and fitness were significantly correlated with weight and lipid reductions.  相似文献

Exercise has been found to be effective for prevention of weight gain and maintenance of a stable weight in adults. The objective of this study was to evaluate the effect of a school-based aerobic exercise program on the obesity indexes of preschool children. Subjects were 292 second-year elementary school pupils from 2 kindergartens in Hat Yai municipality, Songkhla province, southern Thailand. A specially designed exercise program, including a 15-min walk before beginning the morning class and a 20-min aerobic dance session after the afternoon nap, 3 times a week, was conducted for 29.6 wk. Weight, height, and triceps skinfold thickness were measured 4 times. At the end of the study, the prevalence of obesity, using 95th percentile National Center for Health Statistics triceps-skinfold-thickness cutoffs, of both the exercise and control groups decreased. That of the exercise group decreased from 12.2% at baseline to 8.8% (Wilcoxon signed-rank test, P = 0.058), whereas that of the control group decreased from 11.7% to 9.7% (Wilcoxon signed-rank test, P = 0.179). A sex difference in the response of body mass index (BMI) to exercise was observed. Girls in the exercise group had a lower likelihood of having an increasing BMI slope than the control girls did (odds ratio: 0.32; 95% CI: 0.18, 0.56). In conclusion, our study suggests that a 29.6-wk school-based exercise program can prevent BMI gain in girls and may induce a remission of obesity in preschool-age children.  相似文献

1. Exercise testing with stepwise increasing workload was performed in 257 healthy children (134 boys, 123 girls) on a bicycle ergometer, which was independent of variations in pedal speed within a certain range. The procedure of exercise was adapted to the children as far as possible. 2. Nomograms and standard values for heart rate during exercise and for physical working capacity at a heart rate of 170/min (W170) were established. The increase of heart rate (from 1.0 to 2.5 Watt/kg) was well correlated to the age, length, body weight and body surface. If heart rate, respectively W170 are related to workload per kg body weight, results are dependent on sex, but not on age. 3. By means of the new standard values a rough evaluation of cardiorespiratory condition is possible, if the heart rate is measured at moderate exercise during steady state. A more precise assessment of the cardio-respiratory fitness is granted by the W170. By this method physical working capacity can be determined in children with heart diseases in a relative simple manner. 4. Normally in stepwise increasing loads the exercise takes 6 minutes per workload. If this time is reduced, an underestimation of the expected heart rate, respectively an overestimation of W170 is observed, which depends on the extent of the shortening of exercise.  相似文献
