Technology offers the health services potentials for improving the quality and scope of medical care, for extending the capacities of health workers, and for gaining control of costs. However, there are obstacles to achieving these potentials, some inherent in the nature and tradition of the medical care process, and some in the structure of management incentives in hospitals and industries that might contribute useful products and services. Spontaneously, professionals from the biomedical and physical sciences have formed coalitions to pursue a variety of technological developments. Most such projects seek an appropriate institutional setting, organizational relationship, and source of support. They seek an optimal balance of institutional, industrial, and government participation. This paper presents a basis for dividing the costs of development between public and private investment and proposes the staging of technological development through trials in government, private, and educational health services. An estimate of justifiable development investments in health technology is an order of magnitude higher than current levels, suggesting that development funds should be viewed as a tax on the revenue stream of the health services rather than a source of competition for research funds.  相似文献

Communication systems have become a fundamental utility for many facets of private and public life. Increasing system capabilities and advances in technology stimulate expectations of the public for new telecommunication services. Quite a variety of new services have been proposed which will have to be analyzed as to their utility, customer acceptance, and feasibility of implementation. In this paper an analytical approach is selected to assess these services by defining their communication structure and arranging them into classes corresponding to information flow, mode of information exchange, and communication pattern. Services requiring considerable technical expenditures and having a significant impact on future network configurations are pointed out. These services reqmre subscriber-individual and broad-band communication channels. For selected classes of services, a conceptual implementation in three types of networks is presented. It is recommended that several new services be implemented by using facilities of the existing public telecommunication network.  相似文献

The provision of efficient public safety services continues to be one of the high priority efforts to improve the quality of life in our nation. These efforts are too often equipment oriented without realizing the system input of providing better services in terms of benefit to the public as opposed to providers of the services. These services are multielement containing many items which can be quantified and qualitative aspects which are difficult to assess. The methodology presented herein is designed to specifically evaluate such multi-element systems in a logical manner. Its use expanded to other than emergency medical service systems will permit weak areas to be identified, develop comparisions and rank system elements as to their individual contributions to "bottom line" results, and to help establish priorities for allocation of funding resources.  相似文献

The advantages of using integrated services digital networks (SDNs) as an infrastructure for telemedicine applications are discussed. The applications, developed in a Norwegian Telecom Research project, are grouped according to their use in the health services: telediagnosis, distance learning, medical information, and administrative health information. Hospitals are convenient settings for deployment of ubiquitous computing technology. Not only are they technology-rich environments, but their workers experience a high level of mobility resulting in information infrastructures with artifacts distributed throughout the premises. Hospital information systems (HISs) that provide access to electronic patient records are a step in the direction of providing accurate and timely information to hospital staff in support of adequate decision-making. This has motivated the introduction of mobile computing technology in hospitals based on designs which respond to their particular conditions and demands. Among those conditions is the fact that worker mobility does not exclude the need for having shared information artifacts particular locations. In this paper, we extend a handheld-based mobile HIS with ubiquitous computing technology and describe how public displays are integrated with handheld and the services offered by these devices. Public displays become aware of the presence of physicians and nurses in their vicinity and adapt to provide users with personalized, relevant information. An agent-based architecture allows the integration of proactive components that offer information relevant to the case at hand, either from medical guidelines or previous similar cases.  相似文献

Telehealth is the use of electronic information and communication technology to deliver health and medical information and services over large and small distances. Broadband wireless services available today, along with more powerful and convenient handheld devices, will enable a transformational change in health management and healthcare with the introduction of real-time monitoring and timely responses to a wide array of patient needs. Further, a network of low-cost sensors and wireless systems help in creating constantly vigilant and pervasive monitoring capability at home and at work. This paper addresses recent efforts in this growing field, including standards, system architectures, propagation models, and lower layer protocols for body area networks. The paper also suggests the use of cooperative transmission-based strategies for such wireless topologies.  相似文献

The proliferation of technology in health care, spurred by environmental factors encouraging the adoption of computerized patient records (CPRs), has led to a widely held perception of fully computerized patient information systems as the industry norm. To test the validity of this assumption, using data from a national survey of certified health information managers, we examined the CPR technology adoption rates reported by health information managers, assessing variation across practice settings, regions, and organizational types. Results show that significant nonadoption, and regional variation, exists in the implementation of CPRs. Overall, nonuniform diffusion of computerized health information technology was found, despite national mandates that promote and at times require uniform adoption. A significantly greater number of hospital-based patient records were computerized, compared to clinics and other practice settings. Managers were frequently found to maintain duplicate CPRs and paper-based patient record systems, even after the initial implementation period. Nonuniform regional CPR adoption and redundant paper-based systems were found to be a common practice in medical systems, due in part to cultural factors, mistrust of computerized data, and lack of technology training and knowledge.  相似文献

Historically, the healthcare system has not made effective use of information technology. On the face of things, it would seem to provide a natural and richly varied domain in which to target benefit from IT solutions. But history shows that it is one of the most difficult domains in which to bring them to fruition. This paper provides an overview of the changing context and information requirements of healthcare that help to explain these characteristics. First and foremost, the disciplines and professions that healthcare encompasses have immense complexity and diversity to deal with, in structuring knowledge about what medicine and healthcare are, how they function, and what differentiates good practice and good performance. The need to maintain macro-economic stability of the health service, faced with this and many other uncertainties, means that management bottom lines predominate over choices and decisions that have to be made within everyday individual patient services. Individual practice and care, the bedrock of healthcare, is, for this and other reasons, more and more subject to professional and managerial control and regulation. One characteristic of organisations shown to be good at making effective use of IT is their capacity to devolve decisions within the organisation to where they can be best made, for the purpose of meeting their customers’ needs. IT should, in this context, contribute as an enabler and not as an enforcer of good information services. The information infrastructure must work effectively, both top down and bottom up, to accommodate these countervailing pressures. This issue is explored in the context of infrastructure to support electronic health records. Because of the diverse and changing requirements of the huge healthcare sector, and the need to sustain health records over many decades, standardised systems must concentrate on doing the easier things well and as simply as possible, while accommodating immense diversity of requirements and practice. The manner in which the healthcare information infrastructure can be formulated and implemented to meet useful practical goals is explored, in the context of two case studies of research in CHIME at UCL and their user communities. Healthcare has severe problems both as a provider of information and as a purchaser of information systems. This has an impact on both its customer and its supplier relationships. Healthcare needs to become a better purchaser, more aware and realistic about what technology can and cannot do and where research is needed. Industry needs a greater awareness of the complexity of the healthcare domain, and the subtle ways in which information is part of the basic contract between healthcare professionals and patients, and the trust and understanding that must exist between them. It is an ideal domain for deeper collaboration between academic institutions and industry.  相似文献

Target localization is an attractive subject for modern systems that utilize different types of distributed sensors for location based services such as navigation, public transport, retail services and so on. Target localization could be performed in both centralized and decentralized manner. Due to drawbacks of centralized systems such as security and reliability issues, decentralized systems are become more desirable. In this paper, we introduce a new decentralized and cooperative target localization algorithm for wireless sensor networks. In cooperative consensus based localization, each sensor knows its own location and estimates the targets position using the ranging techniques such as received signal strength. Then, all nodes cooperate with their neighbours and share their information to reach a consensus on targets location. In our proposed algorithm, we weight the received information of neighbour nodes according to their estimated distance toward the target node. Simulation results confirm that our proposed algorithm is faster, less sensitive to targets location and improves the localization accuracy by 85% in comparison with distributed Gauss–Newton algorithm.  相似文献

High altitude platform station (HAPS) is a new airborne technology suited in the stratosphere layer, which is expected to remain quasi-stationary while delivering a host of wireless services for the next generation of mobile and fixed devices. The theoretical basis behind the spectrum planning for this new emerging technology is classified under the worst case (WC) prediction model, which insures protection for existing services, but in contrast it deprives achieving spectrum efficiency. However, a new spectrum allocation for HAPS in the reliable band 5850–7075 MHz is on the table of the International Telecommunications Union (ITU) under the agenda item 1.20 of the World Radio Conference-2012 (WRC-12). In this article, a comprehensive intersystem interference transformation technique for the WC prediction model to the minimum coupling loss (MCL) approach is proposed to define the technical compatibility between HAPS and Fixed Satellite Service (FSS) gateways. In advance; the interference probability has been performed to investigate the impact on sharing constrains between systems; by using Monte-Carlo statistical simulation. The paper presents an accomplished statistical simulation model that has proved the ability to convey the interference probability into an unwanted range of parameters; which achieves practical, actual and optimum spectrum sharing.  相似文献

As the population ages and the number of people with chronic diseases increases, society is changing from the existing diagnostic- and treatment-oriented medical services to preventive- and management-oriented medical services. This paradigm is named Ubiquitous Smart Healthcare. Various multimedia-based healthcare services are becoming especially key to health management in the smart age. In other words, technology, research, and integration of multimedia healthcare services, such as video data, voice data, IoT networks, and the like, are proceeding. Due to well-being trends and lifestyles of the aging society, medical services are rapidly becoming ubiquitous, in which the individual and the medical professional cooperate closely in hospital-centered treatment services to provide health management and promotion, and prevention of diseases. To realize this, various bio-signal measurement techniques based on multimedia are being studied and applied. Therefore, in this paper, we propose a remote voice signal-measurement system that combines two Impulse Radio Ultra Wide Band (IR-UWB) systems to provide a capable IR real-time location service, and we design and implement them. Also, we can reduce the complexity of existing voice signal–measurement algorithms, and we optimize the system by proposing a filter suitable for voice signal–measurement algorithms. As a result, we confirm that the voice signal can be measured by extending the measurement distance (based on wireless communications) by only about 3 m to 10 m. By applying the measurement and analysis method for voice-signal data in the wireless communication system environment, a healthcare service with improved reliability can be applied. In particular, it is possible to provide a multimedia healthcare computing environment in which new diagnostic data can be found and utilized in the field of multimedia-based healthcare service technology.
