Hypodontia, congenitally missing teeth is more common in permanent than primary dentition. The present investigation reports the prevalence and pattern of hypodontia in the primary and permanent dentitions, excluding third molars in a sample of Saudi children. The sample consists of 1,300 children, aged 5 to 10 years of age. Clinical and radiographic examinations were performed. The prevalence of children with hypodontia was found to be 2.6 percent. The mandibular second premolar was the tooth most frequently absent and account for 45 percent of the total missing teeth. In primary dentition, the maxillary lateral incisor was the tooth most frequently absent (9%). A peg-shaped permanent maxillary lateral incisor was present in 0.7 percent of the sample. Congenitally missing teeth were almost equally distributed between maxillary (52%) and mandibular (48%) arches.  相似文献

This study examined risk factors associated with incisor injury in 3396 third and fourth grade school children in Alachua County, Florida. One of six orthodontists completed a standardized examination form for each child to assess severity of incisor injury, gender, age, race, skeletal relationships, morphologic malocclusion, incisor exposure, interlabial gap, TMJ sounds, chin trauma, and history of lower facial trauma. One in five (19.2%) exhibited some degree of incisor injury. This was limited to a single tooth in 73.1% of those with injury, while enamel injury predominated (89.4%). The majority of the injuries (75.4%) were localized in the maxillary arch, with central incisors the most frequently traumatized. Chi-square tests of association indicated that gender, race, school, orthodontist, history of lower facial trauma, chin trauma, profile, and maxillary and mandibular horizontal positions were associated with incisor injury (P < 0.05). Wilcoxon rank sum tests identified differences in age, overjet, time of screening, and interlabial gap between those with and without injury (P < 0.05). Results of logistic regression analyses indicated risk of incisor injury was greater for children who had a prognathic maxilla, a history of trauma, were older, were male, and had greater overjet and mandibular anterior spacing.  相似文献

Prenatal exposure to anticonvulsant medication has been shown to cause craniofacial dysmorphology, prenatal growth retardation, hypoplastic nails and phalanges, and visceral abnormalities. In this study we examined maxillary and mandibular stone dental casts (45) and panoramic radiographs (39) from 45 individuals with ages 4.5 to 22.0 years for changes in mesiodistal crown size of deciduous and permanent teeth, and the presence of dental anomalies. These individuals had been exposed prenatally to antiepileptic drugs (AEDs). Mesiodistal crown diameters were measured from the dental casts and converted into standard scores (Z), using published normative data from the University of Michigan Longitudinal Craniofacial Growth Series. A significant increase in mesiodistal crown dimensions of the posterior maxillary teeth was observed, specifically in primary molars and their permanent premolar successors, as well as permanent molars. Changes in tooth size were more common in females than in males. Dental maturity, assessed using the panoramic radiographs, was equal to chronologic age. An increased frequency of hypodontia was the only notable dental anomaly.  相似文献

PURPOSE OF STUDY: To examine factors which may predispose to tooth fracture, and to assess the compressive strengths of foods and sweets which are associated with tooth fracture in vivo. POPULATION STUDIED: Consecutive dental patients presenting with one or more fractured posterior teeth at the surgeries of three general dental practitioners, over a four-month period. METHOD: A pro forma was designed to elicit information on the factors associated with tooth fracture and the nature and extent of such fractures. Three general dental practitioners were requested to complete a pro forma for each patient presenting with a fractured posterior tooth over a four-month period. Foods and sweets considered to be associated with tooth fracture were identified and their compressive strengths tested. FINDINGS: A total of 129 cases of fractured posterior teeth were recorded, of which 48% occurred in the mandibular arch and 52% in the maxillary arch. In the mandible, 75% of tooth fractures occurred in molars while in the maxillary arch 50% occurred in molars. In 57% of cases assessed, no identifiable causative item was noted. Forty-five per cent of fractures were in teeth which had been restored on three or more surfaces. Compressive forces of 0.16KN to 2.2KN were obtained for food items implicated in tooth fractures. CONCLUSION: As mesio-occlusodistal restorations were identified as a major predisposing factor to tooth fracture in this study, with mandibular first molar teeth particularly affected, placement of cuspal coverage restorations may be considered to be a justifiable preventative measure in teeth identified as being at risk.  相似文献

A cross-sectional dental examination of 1,560 randomly selected unrelated children of Gulbarga, Karnataka has shown about 30% of the children with deciduous teeth suffering from one or more carious teeth. The proportion of male children with df teeth was larger than female ones; the difference between the two being statistically significant (chi 12 = 8.0387). Its incidence was higher in children of low income group. Muslims (36.2%) were found to be affected more than Hindus (29.6%) and Christians (27.8%). An increase in the frequency of carious children was observed with the increasing age excepting the children aged 12 years and above who have shown a decreasing trend. In both the sexes mandibular teeth were found more susceptible than maxillary ones. No side difference was found in either jaw. Lower second molars were affected more, followed by lower first and upper second molars. Toothwise prevalence of caries by age showed no consistent trend either in maxilla or mandible.  相似文献

Four hundred and fifty-five teeth that were judged clinically to have a questionable prognosis were observed over a 40-year span in 166 patients in a private practice. Therapy initially was debridement with oral hygiene instruction, followed by flap curettage procedures and preventive maintenance recall therapy. An assessment of compliance based on oral hygiene and recall attendance was also performed. A total of 55 (12%) teeth were lost with an average survival rate of 8.8 years prior to extraction. No teeth were lost during the first 2 years of observation. The majority of extracted teeth were maxillary second molars (38.2%), followed by maxillary first molars (25.5%), and mandibular second molars (16.4%). Tooth loss patterns appeared to be bilaterally symmetrical, with 51% of teeth lost in the right quadrants and 49% lost in the left quadrants. Indications for extraction were primarily periodontal abscesses. Teeth that were considered to have a very questionable prognosis have been retained for many years after therapy, supportive treatment, and patient compliance with recall attendance.  相似文献

This study describes mesial and distal enamel thickness of the permanent posterior mandibular dentition. The sample comprised 98 Caucasian adults (59 males, 39 females) 20 to 35 years old. Bitewing radiographs of the right permanent mandibular premolars and first and second molars were illuminated and transferred to a computer at a fixed magnification via a video camera. Enamel and dentin thicknesses were identified and digitized on the plane representing the maximum mesiodistal diameter of each tooth. The results showed that there were no significant sex differences in either mesial or distal enamel thickness. Enamel on the second molars was significantly thicker (0.3 to 0.4 mm) than enamel on the premolars. Distal enamel was significantly thicker than mesial enamel. There was approximately 10 mm of total enamel on the four teeth combined. Assuming 50% enamel reduction, the premolars and molars should provide 9.8 mm of additional space for realignment of mandibular teeth.  相似文献

The prevalence, distribution and location of dental caries were studied in the deciduous dentition of 185 skeletons aged between 6 months and 13 years from a large, mostly mediaeval, cemetery, where successive phases of use could be distinguished. The main phases dated from 1240 to 1440 AD. During this period, no consistent trend or pattern of caries was identified, but there was a significantly higher caries prevalence in the age band 6-12.9 years than in the age band 0.1-5.9 years. The caries prevalence in the juveniles from the earlier Northumbrian, or Anglo-Saxon, phase of the cemetery was higher than in any mediaeval phase group, with mostly approximal surfaces affected. In the mediaeval population, first deciduous molars generally showed lower caries prevalence than second deciduous molars, significantly lower for the older age band, and maxillary molars consistently showed lower levels than mandibular molars. The highest caries rate, shown by mandibular second molars in the older age band, was 7.9%. In deciduous molars, the occlusal surface was most frequently affected by caries, whereas in the permanent molars of the same population the approximal surfaces had shown the highest level of attack. The overall caries prevalence in deciduous teeth in the mediaeval population of Whithorn was 2.3% of the teeth present, lower than the levels found in previous Scottish and English population groups, including a small assemblage of children from the Scottish sites of Elcho, Southwick and Kirkhill.  相似文献

The aim of this study was to test the hypothesis that a biocompatible membrane, when placed between the gingiva and cortical bone in teeth with periodontal defects that occurred following mechanical endodontic perforation, would facilitate greater regeneration than in control sites not treated with guided tissue regeneration. One beagle dog with a healthy periodontium was used in the study. The maxillary right first and second molars and the mandibular left first and second molars acted as the experimental group in which furcation perforations were treated by guided tissue regeneration. The maxillary left and mandibular right first and second molars served as the controls in which furcation perforation lesions were only treated by open flap debridement. Clinical, histological, and standardized radiographic evaluation showed significant differences between the test and control groups. In addition, digital subtraction radiography revealed a gain in alveolar bone height and increased density at all experimental sites, and a loss at all control sites. Histological evaluation showed extensive regeneration of both alveolar bone and connective tissue at experimental sites, but none at control sites. The results of this study suggest that the use of guided tissue regeneration in furcation lesions produced by endodontic perforations will result in significant new bone and connective tissue attachment.  相似文献

Plaster models of the teeth of 3-year-old Japanese children (96 males, 98 females) were used to record the crown length, crown width and crown thickness of 5 maxillary and 5 mandibular deciduous teeth (30 measurement values). These measurements were used to devise a number of sex determination formulae. A sex-determination formula using all 30 values was calculated. Furthermore, a number of practical formulae were derived from only the crown width and crown thickness values because the deciduous teeth wear in 4 years and older children rapidly progress, making the crown length measurement unreliable. These formulae were calculated for the maxillary teeth alone and mandibular teeth alone. The formulae based on only the crown width or thickness were also calculated for both maxillary and the mandibular teeth. A step-wise discriminant analysis was then used to ascertain the most reliable measurements and a practical formula subsequently devised. The results obtained were as follows: 1. The mean value for each measurement was greater in males than in females. 2. Significant differences in the values recorded were seen in 28 out of the 30 measurements taken. The measurement items not exhibiting these significant differences were the crown width of the maxillary lateral incisor and the crown thickness of the mandibular second molar. 3. The accuracy rates for the sex-determination analysis and the step-wise sex determination analysis calculated using all 30 values were 78.6% and 75.7%, respectively. 4. The accuracy ranges for the modified sex-determination formulae and the associated step-wise sex determination analyses were 70.6-78.4% and 67.0-76.8%, respectively.  相似文献

A six-year-old boy presented with extraoral eruption of a mandibular permanent canine and a loosely attached lateral incisor in the labial vestibule, 3 months following trauma to the chin. The trauma not only caused displacement of the permanent tooth buds, but also resulted in transposition of the lateral incisor and canine with subsequent ectopic eruption. The case is presented to demonstrate one of the infrequent complications of trauma involving the unerupted tooth buds of permanent teeth and its unusual clinical presentation.  相似文献

The chronological age, skeletal age, and morphological age at the time of mineralization of 14 stages of the permanent teeth of 121 boys and 111 girls of the serial experimental sample of the Burlington Growth Centre were contrasted according to whether or not the children had 4- or 5-cusped mandibular permanent first molars, and whether or not they had agenesis of one or more third molars. In the boys with 4-cusped mandibular first molars, the mineralization of the teeth occurred at significantly earlier chronological and skeletal ages when they were significantly shorter in stature. Tooth mineralization was not consistently earlier in the girls with 4-cusped molars, but these girls tended to be taller and heavier than those with 5-cusped molars. In contrast, in both sexes with agenesis of third molars, the mineralization of the teeth was significantly delayed according to chronological, skeletal and morphological evaluation. The change in timing of mineralization was greatest for the second premolars and second molars, and least for the first molars.  相似文献

Maxillary and mandibular molar and incisor vertical dimensions were evaluated in subjects who had excessive, normal, and short lower anterior face height in relation to upper face height. Sexual dimorphism was also investigated. The dentoalveolar heights were compared between Class I and Class II, dental and skeletal malocclusions. The sample was drawn from the Burlington Growth Centre sample and consisted of 188 male and 156 female subjects at age 12 years, for whom lateral head films were available. This sample was classified into excessive, normal, and short lower anterior face height, using the ratio upper anterior face height/lower anterior face height (UAFH/LAFH). The results showed that the dentoalveolar heights are significantly different between faces with excessive, normal, and short lower anterior face heights, except for the lower posterior dental height, which showed no difference between short and normal lower anterior face height subjects. All dentoalveolar heights are larger for male subjects except for the upper posterior dental height. Dentoalveolar heights are similar between Class I and Class II dental and skeletal malocclusions. The upper teeth present a higher correlation to the UAFH/LAFH ratio than the lower teeth. Stepwise regression analysis shows that 22% of the variation in the ratio is explained by the maxillary and mandibular molars and 41% is explained by the maxillary and mandibular incisors.  相似文献

Neurones of the trigeminal mesencephalic nucleus (Mes V) and ganglion innervating the periodontium of incisor, canine and molar teeth in 10 monkeys and 10 baboons were counted and mapped using the horseradish peroxidase (Hrp), retrograde axonal transport method. Periodontal afferent neurones of all these teeth were well represented in the Mes V, although the incisors had a significantly higher number of labelled neurones than the canines or molars. The primary cell bodies of the periodontal afferents were located mainly in the caudal part of the ipsilateral Mes V from the level of the inferior colliculus to the floor of the fourth ventricle in the pons. The caudal periodontal Mes V neurones may be favourably located to make collateral connections with the trigeminal motor nucleus for jaw reflexes. Incisors and canines had a large and predominantly ipsilateral representation of Hrp-labelled neurones in the ganglion. In contrast, molar representation in the ganglion was sparse and all labelled neurones supplied ipsilateral teeth. The maxillary and mandibular teeth had a somatotopic distribution within the respective maxillary (middle) and mandibular (posterolateral) compartments of the trigeminal ganglion. It is suggested that the anterior teeth with greater connections to the Mes V and the ganglion may impart greater sensory perception and be involved in jaw reflexes to ensure a good occlusal relation during mastication, while the afferent connections of the molars may initiate complex jaw reflexes during the occlusal phase of mastication.  相似文献

470 injured teeth of 370 patients who consulted the Dental Clinic of Ege University, Izmir, Turkey for examination or treatment between 1981-1993 were evaluated. Information concerning sex, age of patients at the time of injury, cause of trauma, number of injured teeth, type of tooth and type of trauma were recorded. More boys suffered traumatic injuries (64.8%) than girls (35.2%). Patients aged 11-15 years old exhibited the highest number of injuries (34.4%) followed by the 6-10 years old group (24.5%). Most injuries involved one tooth (60%) and maxillary central incisors were the most often affected teeth (66.2%). The leading cause of injury was undefined falls (45.1%). At the initial examination, cases seen after a long posttraumatic period showed more complications than those presented within a short time period. Educational programs about the importance of dental trauma, the benefits of immediate attendance and conservation of avulsed and fractured teeth would be very helpful for patients. Additionally improving the knowledge of the dental practitioner about trauma would be another important point in solving the problem.  相似文献

This study examined furcation dimensions and morphology in first and second mandibular molar teeth. One hundred thirty-four extracted human mandibular molars with divergent roots were selected. Teeth were viewed at 7X magnification on a dissecting microscope interfaced with a computer equipped with a state-of-the-art histomorphometry software program. Various aspects of furcation anatomy were measured and recorded. Data were examined by using analysis of variance for all paired comparisons. For nonparametric data, the Kruskal Wallis test was used. Results indicated that 61.94% of buccal and 50.75% of lingual molar surfaces presented with cervical enamel projections (CEPs), with the highest frequency noted in second molars. CEPs ranged from 0.98 mm to 1.33 mm, whereas root trunk heights varied between 2.23 mm and 2.93 mm. Generally, lingual molar surfaces had longer root trunks when compared to buccal surfaces. Root separation increased by approximately 0.5 mm at each 1-mm increment apical to the furcal roof. This study provides new information regarding the furcal anatomy of mandibular molar teeth and supplements previous reports that suggest the CEP is a common problem which must be addressed by clinicians when treating molar teeth.  相似文献

The purpose of this investigation was to examine the relationship between the root surface structure of human teeth and the age of the individual and also to evaluate whatever contribution this relationship might give to multiple regression methods for age estimation. The material, consisting of 1000 permanent teeth, excluding molars, was examined by means of a new scoring system (Surface Roughness Scores, SRS) for surface roughness, in addition to the scoring systems of Gustafson (RG) and Johanson (RJ) for root resorption. Statistical analyses using the SPSS package indicated a symmetric left/right distribution of root surface structure. The Pearson correlation between age and RJ varied from -0.02 for maxillary central incisors to 0.54 for mandibular central incisors and was approximately the same for RG. These two scoring systems seem to be of little value in methods for age estimation. Correlations between age and the new scoring systems SRS were significant for all teeth and varied from 0.44 for maxillary second premolars to 0.68 for mandibular first premolars. There was no detectable influence of gender or reason for extraction. However, the SRS could not be assessed with sufficient reproducibility, and the estimates were therefore too subjective to be used as the sole criterion for age estimation. This scoring system could, however contribute positively to a multiple regression method.  相似文献

Some adult cleft palate patients show severe maxillary transverse contraction and posterior crossbite. This case report demonstrates successful surgical-orthodontic treatment of such a patient. Surgically assisted rapid maxillary expansion (SA-RME) was completed prior to comprehensive orthodontic treatment. The osteotomy was performed on both the buccal and lingual aspects of the posterior maxillary alveolus. A Hyrax-type maxillary-expansion appliance was used, and the screw (0.2 mm, one quarter turn) was turned two or three times per day. Comprehensive orthodontic treatment was initiated after extraction of the mandibular first premolars and four third molars. The maxillary lateral incisors were also extracted after active orthodontic treatment. The amount of expansion achieved using SA-RME was greater at the posterior than at the anterior maxilla. Midpalatal suture opening occurred. After orthodontic treatment, occlusal stability was satisfactory. This case demonstrates the effectiveness of SA-RME in adult cleft palate patients with severe posterior crossbite.  相似文献

This study investigated the effect of different etching times on the retention of fissure sealants in second primary and first permanent molars. Eighty-four children with a total of 144 second primary molars and 264 first molars were included in the study. Etching times of 15, 30, 45 and 60 seconds were used. The fissure sealants were evaluated at 6 and 12 months. The results showed that the overall retention rate of fissure sealants in second primary molars was 73.0% at 6 months and 64.7% at 12 months, whereas in first permanent molars the retention rates were 60.7% at 6 months and 44.1% at 12 months respectively. There was no significant difference in the retention of fissure sealants either on second primary molars or on first permanent molars at a 6- and 12-month follow-up with the different etching times. It was concluded that the different etching times did not appear to affect the retention of fissure sealants on the first permanent molars or second primary molars. It might therefore be prudent to etch the teeth for a much shorter period than conventionally recommended.  相似文献

A retrospective cross-sectional cephalometric investigation was undertaken to examine the facial form of a group of Finnish children with juvenile chronic arthritis (JCA). Following digitization, the radiographs were divided into three age groups, and according to whether or not 'bird-face' deformity was present. From a total of 67 cases (39 females and 28 males) 19 per cent were judged to be 'affected'. Analyses were carried out and the groups compared using t-tests. The mandible was found to be smaller both in ramal height and body length in the affected sample, with reduction in posterior face height being only partly compensated by increase in bony apposition at the angle producing antegonial notching. There was posterior rotation of the mandible with a reduction in angles S-N-B and S-N-Pog, and an increase in the gonial angle, the angle between the mandibular plane and S-N, maxillary, and occlusal planes. The changes in the maxilla were less marked. Although S-N-A was reduced in all three age groups, it was not significantly so. Maxillary length (ANS-PNS) was significantly smaller in the two younger age groups. In the vertical plane maxillary dimensions were reduced in the two younger age groups. A highly significant increase in the occlusal to maxillary planes angle was observed in all groups. There was, however, no difference in S-N to maxillary planes angle, indicating a more steeply inclined occlusal plane due to subnormally erupted maxillary molars. Although the inter-incisal angle was reduced there was no significant difference in the incisor inclinations in relation to the jaws and despite the posterior rotation of the mandible there was no significant increase in size of overjet or in the frequency of anterior open bite.  相似文献
