The three-dimensional obturation of the root canal system is widely accepted as a key factor for successful endodontic therapy. The purpose of this study was to evaluate the obturation of lateral canals and the main canal using cold lateral condensation versus the gutta-percha coated rigid carrier. Thirty epoxy blocks with five lateral canals placed at varying angles from the main canal were used. Each experimental group was obturated by a board certified endodontist with clinical experience in the respective obturation technique. The length of gutta-percha and sealer in the lateral canals was measured under a microscope (x30, Unitron) to the nearest 0.5 mm. The blocks were sectioned with an Isomet Plus precision saw (Buehler, Lake Bluff, IL) and copious water irrigation perpendicular to the main canal at the apex, the height of contour, and at 0.8, 1.6 and 2.4 mm from the canal apex. A microscope (x100, Leitz, Switzerland) was used to determine voids. There was significantly (p < .001) more gutta-percha in the lateral canals with the gutta-percha coated rigid carrier technique. In contrast, the cold lateral condensation technique had significantly (p < .001) more sealer in the lateral canals. However, there was no significant (p < .05) difference, in gutta-percha-plus-sealer filling of the lateral canals, between the two techniques. In the apical 1 mm of the main canal there were significantly (p < .011) fewer voids with the gutta-percha coated rigid carrier technique compared to the cold lateral condensation. In the model chosen, the gutta-percha coated rigid carrier technique and the cold lateral condensation technique were equally effective in filling lateral canals. In filling the main canal, however, the coated rigid carrier technique was more effective.  相似文献

The purpose of this prospective study was to determine the overall incidence and distribution of lumbo-sacral degenerative changes (i.e. disc protrusion or extrusion, facet degeneration, disc degeneration, nerve root canal stenosis and spinal stenosis) in patients with and without a lumbo-sacral transitional vertebra (LSTV). The study population consisted of 350 sequential patients with low back pain and/or sciatica, referred for medical imaging. In all cases CT scans of the lumbo-sacral region were obtained. In 53 subjects (15%) and LSTV was found. There was no difference in overall incidence of degenerative spine changes between the two groups. We did find, however, a different distribution pattern of degenerative changes between patients with and those without an LSTV. Disc protrusion and/or extrusion occurred more often at the level suprajacent to the LSTV than at the same level in patients without LSTV (45.3% vs 30.3%). This was also the case for disc degeneration (52.8% vs 28%), facet degeneration (60.4% vs 42.6%) and nerve root canal stenosis (52.8% vs 27.9%). For spinal canal stenosis there was no statistically significant difference between the two categories. In conclusion, our findings indicate that an LSTV does not in itself constitute a risk factor for degenerative spine changes, but when degeneration occurs, it is more likely to be found at the disc level above the LSTV.  相似文献

The root canal anatomy and pulp chamber morphology of 216 maxillary permanent first molar teeth of known age was examined using a radiographic technique after infusion of the root canal system with a radiopaque sodium iothalomate gel. This technique proved to be a rapid and effective method for the examination of root canal morphology and is recommended when root canal anatomy needs to be examined before further investigations are carried out. Over 95% of palatal and disto-buccal roots contained a single root canal. The mesio-buccal root was more complex. All types of configurations were seen. Only 26% of mesio-buccal roots showed a single canal. The pulp canal in all roots appeared to narrow at an early age. In the mesio-buccal root, a definite two-directional calcification pattern was apparent in most teeth by the age of 10. The rate of progress of root formation was very variable. There was no apparent relationship between the type of canal system in the mesio-buccal root and the type of canal orifice present. The transverse cross-sectional shape of the pulp chamber was trapezoidal in 81% of teeth.  相似文献

This case demonstrates failure of root canal treatment of a maxillary central incisor following incomplete mechanical removal of a Ca(OH)2 intracanal dressing and subsequent resorption of the material from the apical portion of the root canal. Retreatment some 4 years later involved the removal of the contents of the root canal and permanent obturation by lateral condensation of cold gutta-percha with a sealer. Follow-up showed complete periradicular bone healing with a lamina dura evident. An explanation of the processes involved is offered, based upon histopathological and microbiological evidence in the literature.  相似文献

The objective of our follow-up study was to evaluate the success of two different types of partially prefabricated post-and-core systems following crown placement. Two hundred and thirty-seven Permador posts and 49 Radix posts were evaluated clinically. Radiological follow-up was performed for 230 Permador posts and 47 Radix posts. The restorations had been in situ for an average of 3.9 years (Permador posts) and 2.3 years (Radix posts), respectively. Root canal fillings were evaluated as to their length, their homogeneity and their flushness with the walls of the root canal. The post-and-cores were evaluated for their orientation in the root canal and for the length of the inserted post relative to the length of the root. The position of the post was classified as centric in 78.0% of the restorations followed, and eccentric in 19.5%, while 2.5% showed a root perforation. The minimal requirement that the length of the post-and-core should at least be equal to the length of the clinical crown was met in 85% of the cases. Eighteen (6.3%) of the 286 teeth examined had to be extracted. There were significant correlations between insertion periods, horizontal bone loss, length of the root canal filling, and position of the post on one hand and tooth loss on the other.  相似文献

The purpose of this study was to evaluate the importance of root canal therapy in the healing process of severe intrabony defects. Four beagle dogs were used and 32 interproximal intrabony defects, up to the apical third, were created. Wire ligatures were placed into these defects for plaque accumulation. Three weeks later, the ligatures were removed and 4 different treatment modalities were employed: group 1) scaling and root planing (SRP); group 2) modified Widman flap (MWF); group 3) modified Widman flap and root canal therapy performed at the same time (RCT/MWF); and group 4) modified Widman flap and root canal therapy performed 3 weeks after the surgical procedure (MWF + RCT). Postoperative oral hygiene was obtained by spraying a 0.12% chlorhexidine solution 3 times a week. The animals were sacrificed 7 weeks after treatment. Blocks were obtained and processed for routine histology. Results were expressed as a percentage of the total defect length (TDL). No differences were observed when SRP was compared to MWF. New bone formation (BF) presented better results for SRP (43.4%) and MWF (53.4%) when compared to RCT/MWF (15.5%). New cementum formation (CF) presented better results for SRP (59.8%) and MWF (64.6%) when compared to RCT/MWF (19.3%) and MWF + RCT (31.5%). Connective tissue repair (CTR) presented better results for SRP (72.4%) and MWF (74.2%) when compared to RCT/MWF (47.5%) and MWF + RCT (44.4%). Results were statistically significant at the level of 0.05. Within the limits of this study, it was concluded that root canal therapy performed simultaneously or 3 weeks after surgery modified the healing of intrabony defects, impairing new bone formation, new cementum formation and new attachment.  相似文献

Using a computer-aided three-dimensional (3-D) reconstruction method, measurements were made at eight representative sites of diameters and the cross-sectional area of the facial canal, facial nerve, and the space between the canal and nerve. Materials used were serial histology sections of seven normal human temporal bones obtained from individuals of different ages. Two areas of decreased cross-sectional area of the facial canal were found: the proximal part of the labyrinthine portion and the middle part of the tympanic portion. These narrowings in the canal appeared to be correlated with decreased superior-inferior diameter of the facial canal in those portions, especially in adult temporal bones, and also with decreased cross-sectional area, both of the nerve and of the space between the canal and the nerve, in these portions. The developmental etiology of these narrowings is speculated on, and their possible relationship to Bell's palsy is discussed.  相似文献

INTRODUCTION: The permanence of necrotic tissues and/or bacteria may be responsible for persistent root canal infection. Root canal irrigation plays an important role in the success of endodontic treatment given that, on the one hand, it encourages the progressive elimination of the smear layer and, on the other, it neutralises microbic flora within the canal. The aim of this study was to test root canal irrigants from a microbiological point of view: sodium hypochlorite 5% (Niclor Ogna, Milan) and a new generation irrigant based on chlorhexidine gluconate and cetrimide (Cetrexidine VEBAS S. Giuliano Milanese-MI). METHODS: The experiments were performed on a series of obligatory anaerobic bacteria which are most frequently found in the endodontium, all belonging to the international collection "American Type Culture Collection" (ATCC) and supplied by Dasit SpA, Cornaredo (MI). The following bacterial strains were used in the experiment: Actinimyces odontolyticus ATCC 17929, Fusobacterium nucleatum ATCC 25586, Prevotella melanginogenica ATCC 25845, Porphyromonas gingivalis ATCC 33277. The working concentration (CFU/ml) was defined as 0.5 McFarland which corresponds to a concentration of microorganisms equivalent to approx. 1.5 x 10(3) bacteria. Each irrigant was kept in contact with the bacterial species used for this experiment for 10', 20', 30'. RESULTS: The results obtained confirm the bactericidal efficacy of both irrigants used, even after a short contact time. CONCLUSIONS: This does not mean that all irrigants are the same and/or promise the same results. This was a microbiological study, but in practice it is important to bear in mind other variables, such as contact time. Moreover, in order to increase the probability of endodontic treatment, root canal irrigants are also required to satisfy other criteria: biocompatibility, scarce toxicity, high proteolytic power.  相似文献

The present case describes the root canal treatment of a rare morphological pattern, namely a mandibular permanent canine with three root canals. The effects of pulsed Nd:YAG laser irradiation with different laser initiators on the permeability and ultrastructure of the root canal wall dentin were investigated in vitro. Forty extracted human single-rooted teeth were randomly assigned to four groups. Group 1 teeth were not lased as a control. Group 2 specimens received four 10-s duration laser exposures for a total exposure of 40 s/canal. In group 3 specimens, the root canals were painted with black ink and then lased by the same method as group 2 teeth. In group 4 specimens, root canals were treated with 38% Ag(NH3)2F and then lased by the same method as group 2 teeth. Laser parameters were set at 2 W, 20 pps. After being placed in 0.6% rhodamine B solution for 48 h, the teeth were sectioned for study by stereoscope and scanning electron microscopy. Statistical analysis showed there were significant differences (p < 0.05) in dentin permeability in the apical areas between groups 3 and 1, 4 and 1, and 4 and 2. Scanning electron microscopic examination showed that laser treatment alone had no obvious effects on the root canal wall. The root canal surfaces prepared for by laser irradiation with black ink or 38% Ag(NH3)2F revealed melting, smear layer evaporation, and open dentinal tubules. Black ink was more effective than 38% Ag(NH3)2F as a Nd:YAG laser initiator.  相似文献

Various methods have been used to study the root canal morphology of human teeth. The purpose of this study was to develop a technique for the formation of a resin replica of the root canal morphology of maxillary first premolars. 30 recently extracted maxillary first premolars were used for the study. An occlusal access cavity was prepared and the teeth placed in 5% sodium hypochlorite solution to dissolve the pulp remnants. The teeth were then rinsed in water. A two-part resin system was then introduced into the root canal through the access cavity opening. Subsequent to curing the resin using a specific curing cycle the specimens were put in 20% hydrochloric acid. This facilitated the dissolution of the tooth and the retrieval of the resin replica of the root canal. The different types of root canal morphology observed are discussed. The relevance and utility of the resin models and further possibilities in this field are discussed.  相似文献

Root canal instruments can be subdivided into instruments made of different alloys (stainless steel, nickel-titanium and nickel-aluminium) and instruments with different geometric forms (e.g., instruments with short cutting segments). Several types of stainless steel and titanium-based instruments are presented and assessed by a review of the current literature. In summary, flexible stainless steel instruments with noncutting tips seem to be a decisive improvement in the development of an ideal root canal instrument. They are superior to titanium-based instruments in both cutting efficiency and instrumentation of curved root canals.  相似文献

OBJECTIVE: In this study, the effects of pulsed Nd:YAG laser irradiation during root canal treatment of infected teeth were investigated histopathologically in dogs. SUMMARY BACKGROUND DATA: Effects of Nd:YAG laser on infected root canal treatment have not been reported in vivo. METHODS: One hundred thirty-five teeth with a single root, including incisors and premolars, in 15 healthy adult beagle-strain dogs were used in this study. After inducing infection in the teeth, each root canal was shaped with at least a # 40 K-file, then the canal was irradiated using the following parameters: 1 W, 30 pps for 1 and 2 sec; 2 W, 30 pps for 1 and 2 sec. Efficacy of debris removal and evaporation on the root canal walls at 2 weeks, and the degree of inflammation of the periapical region at 2, 4, and 8 weeks after laser irradiation were examined histopathologically by light microscopy. RESULTS: Effective debris removal was observed in all cases of the laser-treated groups. No evaporation was observed except at the conditions of 2 W for 2 sec. Inflammation of periapical region in the laser-treated groups was similar in the control group at 2 weeks, but was significantly less than that in the control group at 8 weeks (p < 0.05). CONCLUSION: These results suggest that pulsed Nd:YAG laser is useful for one-visit root canal treatment of infected teeth in dogs, if appropriate parameters are selected, and this is a potential therapy for human apical lesions of teeth.  相似文献

The objective of this in vitro study was to compare and assess two endodontic handpieces during the preparation of simulated root canals. One hundred and sixty simulated root canals in clear resin blocks, of two angles and positions of curvature, were prepared using either Shaper files activated by the MM1500 Sonic Air handpiece (Micro-Mega, Besan?on, France) or Excalibur files activated by the W & H Excalibur 969 handpiece (W & H, Bürmoos, Austria). Files of 21-mm or 25-mm length were used. When preparing the canals, the files were used either in line with or perpendicular to the canal curvature. Preparation of the canals was carried out using a technique described in the manufacturers' literature. A variety of parameters were used to compare the handpieces, including an assessment of the canal preparation time, the deformation or fracture of instruments, loss of canal length, weight loss from the resin blocks and the overall postpreparation canal shape. Both handpieces provided an efficient and easy method of preparing and shaping the root canal with little operator fatigue. The MM1500 Sonic Air handpiece took significantly more time (P < 0.001) to prepare the canals and was associated with both more loss of working length (P < 0.05) and more loss of weight (P < 0.001). Both handpieces created a high percentage of aberrations in the shapes of prepared canals. The MM1500 Sonic Air handpiece created significantly more aberrations than the Excalibur handpiece (P < 0.05 for zip and elbow, P < 0.05 for danger zone and coronal narrow). The effects of the variables, canal curvature, file length and the plane of use of the file, on the performance of the handpieces, were also assessed.  相似文献

We have previously demonstrated that detectable levels of prostaglandin E2 exist in periapical exudates (PE-PGE2) from root canals of periapical periodontitis. Our data from a cross-sectional study also suggested the association of PE-PGE2 concentrations with the active phase of periapical lesions. The objective of this study was to investigate the longitudinal changes of PE-PGE2 levels during root canal treatment. Periapical exudate specimens were quantitatively sampled from root canals of 20 nonvital teeth at consecutive treatment visits to measure PGE2 concentrations. The mean PE-PGE2 levels significantly decreased following the endodontic therapy (158.0 +/- 54.1 pg/microliter in the first samples versus 48.4 +/- 37.9 pg/microliter in the second samples). A significant correlation was also found between the changes in PE-PGE2 levels and the clinical disease expression of periapical periodontitis (p < 0.01). Decreased PGE2 production is considered to reflect the remission of disease activity. These results suggest the central roles of PGE2 in regulatory mechanisms for the expression of periapical disease.  相似文献

Low apical leakage along root fillings following an application of calcium hydroxide was reported in a few methylene blue dye penetration studies. It has been found recently that methylene blue is decolored by calcium hydroxide, indicating that the short penetration of methylene blue may not be due to a tight seal only. Of the 80 roots of human maxillary central incisors used in this study, 40 roots (group 1) received calcium hydroxide root canal dressing whereas another 40 roots (group 2) did not. All the roots were then obturated with gutta-percha and Tubli-Seal sealer. Leakage along 20 filled roots in each group was measured using a modified fluid transport model at 48 h, 2, 4, 8 and 16 weeks after obturation; whereas leakage of another 20 filled roots in each group was measured using dye penetration with 1% methylene blue. Using the fluid transport model, no significant difference was found between the two groups at any time interval (P = 0.4847, 0.3875, 0.9490, 0.4786, 0.9148 respectively after 48 h, 2, 4, 8 and 16 weeks); using the methylene blue penetration method, leakage in group 1 (with root canal dressing) was significantly less than that in group 2 (without root canal dressing) (P = 0.0374). The contradiction in results from the different models indicated that problems existed with the models.  相似文献

The accuracy of tympanometric estimates of ear canal volume was evaluated by testing the following two assumptions on which the procedure is based: (a) ear canal volume does not change when ear canal pressure is varied, and (b) an ear canal pressure of 200 daPa drives the impedance of the middle ear transmission system to infinity so the immittance measured at 200 daPa can be attributed to the ear canal volume alone. The first assumption was tested by measuring the changes in ear canal volume in eight normal subjects for ear canal pressures between +/- 400 daPa using a manometric procedure based on Boyle's gas law. The data did not support the first assumption. Ear canal volume changed by a mean of .113 ml over the +/- 400 daPa pressure range with slightly larger volume changes occurring for negative ear canal pressures than for positive ear canal pressures. Most of the volume change was attributed to movement of the probe and to movement of the cartilaginous walls of the ear canal. The second assumption was tested by comparing estimates of ear canal volume from susceptance tympanograms with a direct measurement of ear canal volume adjusted for changes in volume due to changes in ear canal pressure between +/- 400 daPa. These data failed to support the second assumption. All tympanometric estimates of ear canal volume were larger than the measured volumes. The largest error (39%) occurred for an ear canal pressure of 200 daPa at 220 Hz, whereas the smallest error (10%) occurred for an ear canal pressure of -400 daPa at 660 Hz. This latter susceptance value (-400 daPa at 660 Hz) divided by three is suggested to correct the 220-Hz tympanogram to the plane of the tympanic membrane. Finally, the effects of errors in estimating ear canal volume on static immittance and on tympanometry are discussed.  相似文献

This study evaluated a technique that included retrofill to repair molar root canal perforations and guided tissue regeneration to restore the periodontium that was removed from the furcation area for access to the sites. Six dogs had root canal therapy on mandibular fourth premolars and first molars. The distal root of each tooth was perforated on the furcation aspect halfway between the furcation and the apex. Replaced flap surgery was performed for access to prepare and fill the perforation site with intermediate Restorative Material. A bone xenograft was placed in the access ostectomy site and covered with GoreTex Augmentation Material (GTAM). Controls included unfilled perforations and not using bone grafts and/or GTAM. Dogs were killed at 6, 12, and 24 wk postsurgery. Complications were more common when root perforations were left unfilled. Histomorphometry revealed a statistically significant decrease in inflammation and more bone fill when root perforations were filled and GTAM was used, respectively.  相似文献

It was demonstrated in vitro that it is possible to achieve obturation of the root canal system with a system utilizing a reduced pressure of 15 hPa (15 mbar). The purpose of this study was to develop a procedure to reach a vacuum of at least 15 hPa within root canals in vivo. It was also determined if root canals could be obturated in daily practice with this vacuum technique. After hand instrumentation of the root canals a tooth was prepared for obturation of the canals with the new technique by obtaining a tightly fitting attachment to a vacuum pump. The result showed that it was possible in vivo to reach reduced absolute pressures of 10 hPa in roots and to fill the root canal system with the new system.  相似文献
