Abdominoplasty assessed by survey, with emphasis on complications

Some 958 surgeons replied to a survey designed to determine the complications of abdominoplasties. The results appear in this paper. We believe that the patient contemplating an abdominoplasty and the surgeon planning it should be aware of the unpleasant reality of these complications, which range from the annoying to the lethal. It is of some solace, however, that despite the inevitability of readily visible scars, nearly all of these patients are quite satisfied with their results.     

Postoperative complications after coronary bypass operations in patients with pulmonary impairment]

Two women with atrial escape-capture bigeminy were reported in whom sinus P waves and escape P waves alternated with each other. Their electrocardiographic findings showed that, during the bigeminal rhythm, 2:1 exit block of sinus impulses associated with entrance block of escape impulses occurred in the sinoatrial junction. Three phenomena are suggested to explain the observations. (1) Seeming bradycardia-dependent sinoatrial exit block may have occurred, probably as a result of phasic changes in vagal tone due to respiration, with decreased vagal tone depressing conductivity in the sinoatrial junction to a greater degree than automaticity in the sinus node. (2) In case 1, type II second-degree entrance block in the sinoatrial junction may have occurred, reflecting the presence of sinus parasystole with incomplete entrance block. (3) Sinus escape also may have occurred, especially in case 2. These phenomena have never been reported before.     

Metabolic complications secondary to obstruction of a Kock pouch afferent limb

Compositae dermatitis is rare in childhood, with few cases documented in the literature. We report a 7-year-old boy who presented with a dermatitis mainly affecting his dominant hand. Patch testing revealed contact sensitivity to sesquiterpene lactone (SL) mix, and subsequent testing with Compositae oleoresins showed sensitivity to daisy, dandelion and chrysanthemum. The dermatitis improved on avoidance of handling plants. We have not documented any other cases of Compositae sensitivity in 187 other children, aged under 16 years, who have undergone patch testing with SL mix between 1992 and 1996 in our department. This case illustrates that Compositae sensitivity may present with a localized dermatitis and that, although uncommon, sensitization may occur in early childhood.     

Experience with the Mainz modification of ureterosigmoidostomy

BACKGROUND: The purpose of this study was to report experience with the revived surgical concept of ureterosigmoidostomy in its low pressure modification and to discuss its value within the current spectrum of urinary diversion. METHODS: Between February 1992 and September 1997 modified ureterosigmoidostomy (rectosigmoid pouch; Mainz pouch II) was performed in 34 patients aged 1.9-76.9 (mean 55.8) years as a primary urinary diversion after radical cystectomy for bladder cancer (n = 30) and benign conditions (bladder exstrophy, three patients; intractable urinary incontinence, one). All patients were followed prospectively according to a standard protocol including assessment of continence, renal function and acid-base balance. RESULTS: There were no perioperative deaths. In one patient dislocation of a ureteral stent in the early postoperative course required insertion of a percutaneous nephrostomy. All patients were continent during the day. One patient experienced night-time incontinence but rejected a conversion procedure. In one case ureterosigmoidostomy was replaced by an ileal conduit after several episodes of septicaemia. One nephrectomy was performed for ureterointestinal obstruction. Mild hyperchloraemic acidosis was seen in two patients. CONCLUSION: Bowel frequency and urge incontinence, the major weaknesses of classical ureterosigmoidostomy, can be overcome by detubularization of the rectum. As the modified procedure is quick, safe and easy to perform with highly satisfactory results, the rectosigmoid pouch has potential in reconstructive urology.     

Postoperative healing complications associated with Gore-Tex Periodontal Material. Part II. Effect of complications on regeneration

The gain in open probing attachment level as a result of a guided tissue regeneration procedure with Gore-Tex Periodontal Material was measured in 62 sites. The incidence of complications was evaluated. When sites with a complication were compared to those without the complication, there was no statistically different gain or loss of newly regenerated tissue. However, the occurrence of sloughing of the gingival flap decreased the amount of gain in open probing attachment level. Analysis of the data also revealed that the combined use of allografts and a delay in the removal of the Gore-Tex Periodontal Material had a synergistic positive effect on the gain in open probing attachment level. No association between these treatment variables and the incidence of the complications was found.

In the testicle of Graphosoma italicum and Eurydema ventralis the histochemical localization of 3-beta-HSD, SDH, MDH and LDH was followed up. The enzymic activity was detected in various stages of the sexual cells; the strongest activity was found in the evoluted spermatids and spermatozoa. As far as the nutritive cells of the testicle are concerned, the reaction of these different enzymes appeared very weak or absent.     

Cracking the crock: how to deal with factitious eye disease

The methanol extract of wax gourd (Japanese name "Tougan"), the fruits of Benincasa hispida COGN. (Cucurbitaceae), was found to show inhibitory activity on the histamine release from rat exudate cells induced by antigen-antibody reaction. Through bioassay-guided separation, four known triterpenes and two known sterols were isolated as active components together with a flavonoid C-glycoside, an acylated glucose, and a benzyl glycoside. Among the active triterpenes and sterols, two triterpenes, alnusenol and multiflorenol, were found to potently inhibit the histamine release.

Fluorescence line narrowing applied to the study of proteins

Fluorescence line narrowing is a high resolution spectroscopic technique that uses low temperature and laser excitation to optically select specific subpopulations from the inhomogeneously broadened absorption band of the sample. When applied to the study of fluorescent groups in proteins one can obtain vibronically resolved spectra, which can be analyzed to give information on spectral line shapes, vibrational energies of both the ground and excited state molecule, and the inhomogeneous distribution function of the electronic transitions. These parameters reveal information about the chromophoric prosthetic group and the protein matrix and are functions of geometric strains and local electric fields imposed by the protein. Examples of the use of fluorescence line narrowing are discussed in investigations of heme proteins, photosynthetic systems and tryptophan-containing proteins.

Postoperative allergic complications: the role of the anesthetist in conducting the immuno-allergic investigation

When peri-anaesthesia anaphylactic and/or anaphylactoid reactions occur, anaesthetist is the first investigator: the quality of immuno-allergological investigations depends on these initial investigational procedures. We have used sample kits for several years in order to make easier the immediate investigation. From retrospective analysis of the allergic complications which happened in 1997, the importance of these sample kits as well as the anaesthetist's part in the immuno-allergological management are examined. Nine observations were itemized (0.047%): 3 generalized erythema observations (grade I), in which atracurium was incriminated twice, and propacetamol once; 2 observations of grade II, in which vecuronium (elevated tryptase) and atracurium were incriminated; 4 anaphylactic shocks, in which three neuromuscular blocking drugs (suxamethonium, vecuronium and pancuronium), and one antibiotic (cloxacilline) were incriminated. The use of sample kits allowed an early diagnosis approach, confirmed by skin tests. Diagnosis should be thought closely between anaesthetists and immunologists for investigations.     

Postoperative hemorrhage due to avulsion of perihepatic adhesions after pneumoperitoneum

Serious laparoscopic injury is rare, occurring in less than 1/1000 procedures. To our knowledge, no liver injuries have been reported to date, but avulsion of hepatic adhesions may be a source of postlaparoscopic bleeding. A woman with chronic pelvic pain unresponsive to medical management underwent an uncomplicated diagnostic laparoscopy with extensive pelvic adhesiolysis. Adhesions of Fitz-Hugh-Curtis syndrome were seen over the right lobe of the liver. At the completion of the procedure no bleeding was noted, and the patient was discharged to home in stable condition. She returned 12 hours later with a significant hemoperitoneum necessitating laparotomy. It is important to consider this as a source of bleeding after laparoscopy in a patient with perihepatic adhesions.

以含有低熔点FeS₂的硫酸渣和烟煤为原料,对硫酸渣含碳球团进行还原熔分,考察了碱度、焙烧温度、焙烧时间和C/O摩尔比对铁回收率的影响,确定了最佳工艺参数.在碱度R=1.0、焙烧温度1450℃、焙烧时间20 min以及配煤量C/O=1.2的条件下还原熔分硫酸渣,可以获得93.1%铁回收率的珠铁.对尾渣的X射线衍射分析可知,硫酸渣中铁氧化物及含铁硫化物被还原,铁回收率较高,且熔分时间短造成大量铁以金属态滞留于渣中;还原熔分硫酸渣所得珠铁因其较高硫含量,可用于冶炼硫系易切削钢.     

Incidence of neoplastic polyps in the ileal pouch of patients with familial adenomatous polyposis after restorative proctocolectomy

PURPOSE: Although adenomatous polyps and even adenocarcinomas have been found in the terminal ileum of patients with familial adenomatous polyposis, the prevalence of neoplastic changes in the pouches of patients who have undergone restorative proctocolectomy is unknown. The objective of this study was to determine the frequency of pelvic pouch neoplasia in familial adenomatous polyposis patients after restorative proctocolectomy. METHODS: Patients in a polyposis registry who had undergone restorative proctocolectomy were recruited. Demographic, surgical, pathologic, and endoscopic data were obtained from patient records. Video pouchoscopy was done after two enemas and representative biopsies were taken. RESULTS: Of 102 eligible patients, 26 (17 males and 9 females) participated. Median age at ileal pouch-anal anastomosis was 31 (range, 12-58) years. Median follow-up period was 66 (11-156) months. Adenomas were found in the pouch of 11 (42 percent) patients, in the terminal ileum above the pouch in 1 patient, and in the anal canal of 4 patients. Among patients with pouch polyps, three patients had one lesion, three patients had two lesions, and five patients had more than ten lesions. The incidence of polyps increased steadily with time from restorative proctocolectomy. There was no relation between the incidence of pouch polyposis and the severity of colonic or duodenal disease. CONCLUSIONS: Proctocolectomy and ileal pouch-anal anastomosis is associated with a significant risk of pouch neoplasia in familial adenomatous polyposis patients. The severity of pouch adenomas was not related either to the severity of colonic or duodenal disease. The pelvic pouches of all patients with familial adenomatous polyposis who have undergone restorative proctocolectomy should be examined periodically.     

Risks to the offspring of women treated with hydantoin anticonvulsants, with emphasis on the fetal hydantoin syndrome

The fetal hydantoin syndrome is a variable pattern of altered growth and performance which includes unusual facies, distal phalangeal hyoplasia, and other defects occurring in some infants exposed in utero to hydantoins. A prospective study of 35 infants exposed prenatally to this class of anticonvulsants showed that 11% had sufficient features to be classified as having the fetal hydantoin syndrome. An additional 31% displayed some features compatible with the prenatal effects of hydantoins. A case-control study of 104 infants whose mothers received hydantoins during pregnancy supports these conclusions. Reduction of intellectual ability in infants with the fetal hydantoin syndrome is the area of greatest concern. Women being treated with hydantoin anticonvulsants should be told of the nature and magnitude of risks to the developing fetus before considering a pregnancy.     

Phylogenetic analysis of the Potyviridae with emphasis on legume-infecting potyviruses

The 3'-terminal nucleotide sequences of thirteen authenticated strains of bean common mosaic virus (BCMV) and one strain of bean common mosaic necrosis virus (BCMNV) were obtained. The regions sequenced included the coat protein coding sequence and 3'-end non-coding region. These data, combined with sequence information from other legume-infecting potyviruses and the Potyviridae were used for phylogenetic analysis. Evidence is provided for delineation of BCMNV as distinct from BCMV and the inclusion of azuki mosaic, dendrobium mosaic, blackeye cowpea mosaic, and peanut stripe viruses as strains of BCMV. This relationship defines the members of the BCMV and BCMNV subgroups. These data also provide a basis upon which to define virus strains, in combination with biological data. Other aspects and implications of legume-infecting potyvirus phylogenetics are discussed.
