Periodontal disease is a common infection-induced inflammatory disease among individuals suffering from diabetes mellitus. The purpose of this study was to assess the effects of treatment of periodontal disease on the level of metabolic control of diabetes. A total of 113 Native Americans (81 females and 32 males) suffering from periodontal disease and non-insulin dependent diabetes mellitus (NIDDM) were randomized into 5 treatment groups. Periodontal treatment included ultrasonic scaling and curettage combined with one of the following antimicrobial regimens: 1) topical water and systemic doxycycline, 100 mg for 2 weeks; 2) topical 0.12% chlorhexidine (CHX) and systemic doxycycline, 100 mg for 2 weeks; 3) topical povidone-iodine and systemic doxycycline, 100 mg for 2 weeks; 4) topical 0.12% CHX and placebo; and 5) topical water and placebo (control group). Assessments were performed prior to and at 3 and 6 months after treatment and included probing depth (PD), clinical attachment level (CAL), detection of Porphyromonas gingivalis in subgingival plaque and determination of serum glucose and glycated hemoglobin (HbA1c). After treatment all study groups showed clinical and microbial improvement. The doxycycline-treated groups showed the greatest reduction in probing depth and subgingival Porphyromonas gingivalis compared to the control group. In addition, all 3 groups receiving systemic doxycycline showed, at 3 months, significant reductions (P < or = 0.04) in mean HbA1c reaching nearly 10% from the pretreatment value. Effective treatment of periodontal infection and reduction of periodontal inflammation is associated with a reduction in level of glycated hemoglobin. Control of periodontal infections should thus be an important part of the overall management of diabetes mellitus patients.  相似文献

The monitoring of metabolic balance in diabetes mellitus involves the assay of cumulative markers of protein glycation. Glycated hemoglobin, particularly the major component HbA1c, and fructosamine, which reflects glycated plasma protein levels, are the most commonly used parameters. Nevertheless, their utilization is still under discussion with respect to methodologies used, as well as to their respective interest in clinical diabetology. This review shows current opinion concerning the analytical and physiopathological use of these biological indicators.  相似文献

The authors tested associations between glycated hemoglobin (HbA1c; an index of glucose homeostasis and an indicator of cardiovascular disease risk in nondiabetic populations) and socioeconomic status (defined by grade of employment) and psychosocial factors in 234 British civil servants. HbA1c concentration was inversely related to grade of employment. Higher HbA1c was associated with greater waist-hip ratio, lower control at work, lower internal locus of control, lower active coping, and lower social support. Control at work, internal and chance locus of control, and active coping were also related to socioeconomic status. The association of social support but not other psychosocial factors with HbA1c, was independent of socioeconomic status. HbA1c may complement measures of the metabolic syndrome and insulin resistance in studies of psychosocial factors in cardiovascular disease risk. Hemoglobin A1c (HbA1c) is a stable minor Hb variant formed in vivo by posttranslational modification by glucose, originally identified by using cation exchange chromatography, and containing primarily glycated N-terminal beta-chains. However, the structure(s) of the quantified species has not been elucidated, and the available methods lack a reference standard. We used electrospray ionization mass spectrometry to determine the extent of glycation of samples separated by boronate affinity and/or cation exchange chromatography. Analyses of clinical samples were consistent with the curvilinear relationship of patient glucose and HbA1c. As glycation increased, the ratio of beta-chain to alpha-chain glycation increased, and the number of glycation sites on the beta-chain increased, although these were relatively minor components. We found several glycated species that cochromatographed with HbA1c on cation exchange, including species with both glycated alpha- and beta-chains, nonglycated alpha- and glycated beta-chains, and multiply glycated beta-chains. The combined use of affinity and cation exchange chromatography with structural confirmation by electrospray ionization mass spectrometry was found to be useful in producing samples of sufficient purity for the standardization of glycohemoglobin clinical assays.  相似文献

Administered the MMPI and the 16 PF to 21 homosexual and 72 heterosexual prison inmates. J. Panton's Hsx scale and the 16 PF I (tough- vs. tender-minded) scale did not differentiate between the sexual groups. The Mf scale, M. Manosevitz's abridged forms of the Mf scale, and the 16 PF C scale (affected by feelings vs. emotionally stable) did differentiate the groups. We have previously documented the safety of 1 unit (50 gram) of human polymerized hemoglobin (Poly SFH-P) in healthy volunteers. This report describes the first patient trial to assess the therapeutic benefit of Poly SFH-P in acute blood loss. Thirty-nine patients received 1 (n = 14), 2 (n = 2), 3 (n = 15), or 6 (n = 8) units of Poly SFH-P instead of red cells as part of their blood replacement after trauma and urgent surgery. There were no safety issues related to the infusion of Poly SFH-P. The plasma hemoglobin concentration ([Hb]) after the infusion of 6 units (300 gram) of Poly SFH-P was 4.8 +/- 0.8 g/dL (mean +/- SD). Although the red cell [Hb] fell to 2.9 +/- 1.2 g/dL, the total [Hb] was maintained at 7.5 +/- 1.2 g/dL. Poly SFH-P maintained total [Hb], despite the marked fall in red cell [Hb] due to blood loss. The utilization of O2 (extraction ratio) was 27 +/- 16% from the red cells and 37 +/- 13% from the Poly SFH-P. Twenty-three patients (59%) avoided allogeneic transfusions during the first 24 hours after blood loss. Poly SFH-P effectively loads and unloads O2 and maintains total hemoglobin in lieu of red cells after acute blood loss, thereby reducing allogeneic transfusions. The bactericidal activity of gaseous ozone was investigated using a commercial ozone generator. Five species of fish bacteria, Pseudomonas putida, Shewanella putrefaciens, Brochothrix thermosphacta, Enterobacter sp. and Lactobacillus plantarum, were inoculated on agar surfaces and exposed to different ozonation times in a gas chamber. Results showed ozone in relatively low concentrations (< 0.27 x 10(-3) g l-1) was an effective bactericide of vegetative cells of the five fish bacteria. The age of the cell culture was shown to influence the cell response following exposure. Survival rate was not linearly related to ozonation time, but exhibited biphasic death over an extended period. Similar bactericidal effects were observed on fish skin treated with ozone daily in the laboratory, with decreases of 1.0 log cfu cm-2 for the micro-organisms studied. Whole fish treated daily in the laboratory using a commercial ozone generator showed improved scores for sensory analyses compared with the controls. The results were statistically significant. Fish treated on board ships were also analysed for microbiological and sensory changes. Controls were obtained from a similar vessel without the ozone facility in the hold. Similar trends to those recorded in the laboratory for the microbiological and sensory results on ozonated fish were observed.  相似文献

Comments on the article by S. C. Hayes et al (see record 1988-11276-001) on the use of treatment utility as a criterion for evaluating assessment procedures. The present author argues that the treatment utility concept has several logical difficulties concerning which assessment practices have treatment utility and how they influence intervention outcomes. What kinds of change are associated with what kinds of practices, practitioners, patients, and problems in what contexts are at the center of these logical difficulties. It is well known that hemoglobin (Hb) possesses many oxidative enzyme activities, including a pseudo-peroxidase activity. It has also been shown by many investigators that various peroxidases in the presence of hydrogen peroxide and a halide ion exert a potent cytotoxic activity toward various mammalian cell types. It has further been observed by various investigators that the administration of relatively large amounts of purified Hb or a Hb derivative to a host animal during resuscitation experiments leads to a number of unrelated types of tissue damage and cell damage in the host. The first objective of this investigation was to determine if the observed tissue and cell damage may be due to a cytotoxic activity that Hb may exert in vivo analogous to that of the peroxidases. We also showed some time ago that peroxidases are able to activate peritoneal macrophages to the cytocidal state. Hence, we also addressed the question whether or not Hb is able to activate macrophages in a similar manner. Our results were negative with regard to both questions. Further investigations indicated that, unlike the peroxidases, ferryl-Hb is unable to oxidize iodide to iodine at a measurable rate, which appears to be the reason for the lack of cytotoxic activity.  相似文献

By plotting histograms and calculating the variation coefficient of the primary drive’s torque in the cells of a continuous rolling mill, the stability of the process may be assessed. The elastic torques and motor torques in cells 5–7 of the 1680 mill in strip rolling were measured simultaneously at OAO Zaporozhstal’, and the results were statistically analyzed. The motor’s electromagnetic torque reflects the frequency properties of the transient process in the mechanical system of the drive and the reaction to technological perturbations, such as variation in strip temperature. By superposition of the motor’s torque histograms and calculation of the torque’s variation coefficient, the stability of rolling in the cells of a continuous mill may be qualitatively and quantitatively estimated. Numerical estimates of the rolling stability are presented for strip batches with and without intermediate coiling devices.  相似文献
