Amikacin was used in 77 treatment courses at a dosage of >/=7.5 mg/kg every 8 h, and patients were monitored for ototoxicity by following serial audiograms, serum creatinine, and amikacin blood levels. Patients were leukopenic (58), were infected by gentamicin-resistant organisms (11), or had cystic fibrosis (8). Three patients developed tinnitus, but none had vertigo or nystagmus. Of 55 courses with pre- and post-treatment audiogram, 13 (24%) were associated with development of high-frequency hearing loss, which was usually bilateral. No patient had conversational hearing loss, and audiograms reverted to normal in three patients. Onset of cochlear damage occurred in one patient after therapy was stopped. The group with high-tone hearing loss, in comparison to the group without audiographic changes, received a larger mean total dose (24 versus 9.6 g), were treated for a longer duration (19 versus 9 days), and more frequently had previous aminoglycosides. Fifty-seven percent of patients with a "peak" serum level exceeding 32 mug/ml and 55% of patients with "trough" levels exceeding 10 mug/ml developed cochlear damage. There was no difference between the groups in age, body weight, previous cochlear damage, renal disease before or during therapy, or average daily dose. Both monitoring of blood levels and limiting duration of therapy may prevent amikacin ototoxicity.  相似文献

Administered a 99-item questionnaire to 319 practicing psychotherapists (mean age 43.9 yrs) from 3 professional associations to assess their use of therapeutic techniques and skills as related to the variables of orientation, experience, sex, and personal psychotherapy. The predictive utility of reported interventions in classifying psychotherapists according to orientation was also examined. Principal component analyses yielded 13 scales. Reported therapy interventions varied as a function of the therapist's orientation and, to a lesser extent, as a function of experience, sex, and personal therapy. Eclectics had either the highest or 2nd-highest mean scores on each of the 13 scales, indicating a varied therapeutic arsenal. Across therapist variables, relationship-enhancing behaviors were reportedly most frequently used; flooding and frustration procedures least frequently used. On the basis of 11 scale scores, 90% of behavior therapists and 88% of psychodynamic therapists were correctly classified, but fewer than half of the eclectics were correctly classified. The validity of the use of the cochlear microphonic (CM) isopotential function as an index of auditory sensitivity was assessed by (a) examining the characteristics of CM recordings to determine if there were any sources of bias inherent in the technique that might influence its representation of sensitivity, (b) qualitatively comparing the shapes of CM and behavioral "audiograms" of 16 animal species obtained in previous studies, and (c) making a quantitative estimate and statistical evaluation of the correspondence between the 16 pairs of audiograms. Potential high frequency bias was found in the CM recording technique, which would be expected to reduce the correspondence between CM isopotential functions and behavioral audiograms. Examination of the 16 pairs of audiograms showed that on the average, the use of a linear rule to predict behavioral audiograms from CM isopotential functions could account for more than 40% of the variance in the behavioral data. The lack of a well-defined region of maximum sensitivity in the CM audiograms was the most consistent shape discrepancy between the 2 data sets. Moreover, the CM audiogram was only an indicator of relative, not absolute, sensitivity. Several possibilities for improving the correspondence between behavioral and CM audiograms are suggested. We have used a Bayesian neural network to distinguish between drugs and nondrugs. For this purpose, the CMC acts as a surrogate for drug-like molecules while the ACD is a surrogate for nondrug-like molecules. This task is performed by using two different set of 1D and 2D parameters. The 1D parameters contain information about the entire molecule like the molecular weight and the the 2D parameters contain information about specific functional groups within the molecule. Our best results predict correctly on over 90% of the compounds in the CMC while classifying about 10% of the molecules in the ACD as drug-like. Excellent generalization ability is shown by the models in that roughly 80% of the molecules in the MDDR are classified as drug-like. We propose to use the models to design combinatorial libraries. In a computer experiment on generating a drug-like library of size 100 from a set of 10 000 molecules we obtain at least a 3 or 4 order of magnitude improvement over random methods. The neighborhoods defined by our models are not similar to the ones generated by standard Tanimoto similarity calculations. Therefore, new and different information is being generated by our models, and so it can supplement standard diversity approaches to library design.  相似文献

Examined behavioral and cochlear functions in mice inbred for audiogenic seizure susceptibility and resistance. The cochlear action potential (AP) thresholds of the susceptible RB/1bg inbred mice were abnormally high, and the resistant inbred RB/3bg mice had normal AP audiograms. The F? hybrid showed heterosis for its cochlear function. Only the RB/1bg was susceptible to audiogenic seizures on the 1st acoustic exposure. The simple model introduced in Part I [J. Acoust. Soc. Am. 102, 413-429 (1997)] is used to simulate the response of the cochlea to three stimulus tones. The focus is on "emission suppression tuning curves" constructed using a third tone to suppress the cubic distortion tone emission (CDT, 2f1-f2) generated by two primary tones at frequencies f1 and f2 (intensities L1 and L2). A criterion decrease (here, 5 dB) of the CDT emission amplitude defines the 2f1-f2 emission suppression tuning curve. Applying traditional tuning curve measures to emission suppression tuning curves appears ineffective in determining the underlying cochlear amplifier characteristics. However, it is shown that there are three characteristics of emission suppression tuning curves which are particularly useful: (1) the "f2 threshold" which is the level of the third tone, L3, required for the criterion CDT amplitude decrease, under the condition that the third tone frequency, f3, is approximately equal to f2; (2) the "shoulder threshold" similarly defined for f3 < f2; and (3) the "tuning width," w40. The tuning width is defined to be the distance (in octaves) from the frequency f2 to the upper f3 frequency for which there is a criterion CDT decrease, in this case using the L3 level which is 40 dB above the f2 threshold. Model calculations appropriate to gerbils show that these measures are most accurately related to the underlying cochlear amplifier characteristics for parameters where the primary stimulus amplitudes satisfy L1/L2 > 20 dB and for which L1 is 25 dB or more below the sharp "notch" seen in the two tone input-output function. In this parameter region, the cochlear amplifier characteristics are related to measured quantities by the relationships wr approximately equal to w40 and Ga approximately equal to TE + w40gp. Here, Ga is the gain (dB) of the cochlear amplifier, defined as the total increase in cochlear response over the passive response, wr is the distance (octaves) over which the active cochlear response rises to a maximum, and gp is the passive increase (dB/octave) of the traveling wave along the basilar membrane. The measured quantities are TE, the difference (dB) between the shoulder threshold and the f2 threshold, and the tuning width, w40 (octaves), defined above. Model predictions are confirmed by measurements in adult gerbils.  相似文献

Recent studies have shown that MRS can substantially improve the non-invasive categorization of human brain tumours. However, in order for MRS to be used routinely by clinicians, it will be necessary to develop reliable automated classification methods that can be fully validated. This paper is in two parts: the first part reviews the progress that has been made towards this goal, together with the problems that are involved in the design of automated methods to process and classify the spectra. The second part describes the development of a simple prototype system for classifying 1H single voxel spectra, obtained at an echo time (TE) of 135 ms, of the four most common types of brain tumour (meningioma (MM), astrocytic (AST), oligodendroglioma (OD) and metastasis (ME)) and cysts. This system was developed in two stages: firstly, an initial database of spectra was used to develop a prototype classifier, based on a linear discriminant analysis (LDA) of selected data points. Secondly, this classifier was tested on an independent test set of 15 newly acquired spectra, and the system was refined on the basis of these results. The system correctly classified all the non-astrocytic tumours. However, the results for the the astrocytic group were poorer (between 55 and 100%, depending on the binary comparison). Approximately 50% of high grade astrocytoma (glioblastoma) spectra in our data base showed very little lipid signal, which may account for the poorer results for this class. Consequently, for the refined system, the astrocytomas were subdivided into two subgroups for comparison against other tumour classes: those with high lipid content and those without.  相似文献

We evaluated the effects of recombinant porcine somatotropin (pST) and the beta-adrenergic agonist salbutamol on plasam endocrine and metabolite profiles and muscle chemistry in three genotypes of growing barrows (n = 96, 139 d old). Treatments were in a 2 x 2 x 3 factorial arrangement, and main effects were pST (0 or 4 mg/d) and salbutamol (0 or 2.75 ppm); the three genotypes including purebred Meishan (M), 1/4 Duroc, 3/4 White composite (D x Wc), and 1/4 Meishan, 3/4 White composite (M x Wc). Individual pigs were injected daily with buffer or pST at 0700 and allowed ad libitum access to a corn-soybean meal diet (1.2% lysine) and water for 33 d. Plasma was obtained 4 h after injection and 3 h postprandially on d 0, 14, and 28 for determination of growth hormone (GH), insulin, IGF-I, glucose, urea N (PUN), NEFA, triglycerides, and cholesterol. Longissimus and semitendinosus samples were obtained, and protein, RNA, and DNA were quantified and loin chop shear force was measured. In general, plasma hormones and metabolites on d 14 and 28 were not affected by salbutamol in the absence of pST, although salbutamol tended to increase d 14 and 28 GH concentrations. Salbutamol lowered plasma IGF-I (d 14 and 28, P < .05), insulin (d 14, P < .01), and NEFA (d 28, P = .07) when pST was administered, although concentrations still exceeded those for control pigs. Salbutamol reduced (P < .05) IGF-I in M and M x Wc pigs, and GH was not changed in M pigs. Meishan pigs had a greater increase in glucose with pST than M x Wc or D x Wc pigs, although the effect was not consistent over time. Individual treatment with pST caused GH, insulin, IGF-I, glucose, NEFA, and triglycerides to be increased and PUN to be decreased on d 14 and 28. Cholesterol on d 14 and 28 was decreased by pST in M pigs, whereas no effects were found in the other genotypes. Muscle protein and RNA were increased by salbutamol and were consistently lowest for M pigs. Furthermore, pST did not affect muscle protein, but it increased RNA more in M pigs than in the others. Overall, pST and salbutamol seemed to act separately and by different mechanisms to alter muscle composition, but blood criteria generally representing fat metabolism (insulin, glucose, NEFA, triglycerides) were interactively affected. OBJECTIVE: To assess the validity of a short calcium food frequency questionnaire (FFQ) for use in young children. DESIGN: Calcium intake from an estimated 4 d diet record (4DDR) was compared with the calcium intake from a 35 item FFQ specifically designed to assess habitual calcium intake and previously validated for adult women. SUBJECTS: Forty-one girls and 26 boys aged 3-6 y recruited by advertisement for studies of nutrition and bone health. RESULTS: Mean (s.d.) calcium intakes were 798 mg (271) and 942 mg (419) for the 4DDR and FFQ respectively, (r = 0.52). Mean difference (s.d. of difference) in calcium intake between the two methods was 144 mg (355), showing that the FFQ may estimate calcium intakes 565 mg below to 854 mg above diet record values. 84% of subjects when classified by the 4DDR fell into the same or adjacent quartiles when classified by the FFQ. Only two subjects were classified in extreme quartiles for the two methods. The FFQ correctly identified 68% of children with recorded intakes less than 800 mg. CONCLUSIONS: The short calcium FFQ tended to overestimate actual calcium intakes in young children, and would not be appropriate for determining calcium intake of individuals. However, the FFQ demonstrated good ability to classify subjects into extremes of calcium intake. Moreover, the predictive value of the FFQ in identifying children with intakes below the current recommended intake of 800 mg was reasonably high (79%).  相似文献

The response map scheme introduced by Evans and Nelson (1973) and modified by others, including Davis et al. (1996) for use with gerbils, has been used primarily for classifying units recorded in the cochlear nucleus of unanesthetized decerebrate preparations. Units lacking spontaneous activity (SpAc) have been classified as either type I/III or type II units based on the relative strength of their responses to broad-band noise compared to their responses to best-frequency (BF) tones. The relative noise index (rho), a ratio of these responses after SpAc is subtracted out, provides a convenient measure of this relative strength. In this paper, responses of 320 units recorded in the dorsal cochlear nucleus (DCN) of barbiturate-anesthetized gerbils to short-duration BF tones and broad-band noise were recorded. Since 87.5% of these units lacked SpAc, their response maps resembled those of type II and type I/III units. Units were characterized by rho and the normalized slope (m) of a best line fit to the BF rate versus level plot starting from the sound level corresponding to the first inflection point of the rate curve (typically its maximum value or the start of its sloping saturation). The distributions of rho and m values do not form distinct clusters as they do for units in the decerebrate preparation. Thus, the criteria developed for classifying DCN units in the decerebrate preparation do not appear appropriate for units in the barbiturate-anesthetized preparation. Deposits of horseradish peroxidase were used to locate 52 units. Most of the low SpAc units, 56% with poor noise responses (5/9) and nearly 70% with strong noise responses (25/36), and nearly all of the high SpAc units (6/7), were located either within or below the fusiform cell layer.  相似文献

Determined behavioral audiograms for 3 horses and 2 cows. Horses' hearing ranged from 55 Hz to 33.3 kHz, with a region of best sensitivity from 1 to 16 kHz. Cattle hearing ranged from 23 Hz to 35 kHz, with a well-defined point of best sensitivity at 8 kHz. Of the 2 species, cattle proved to have more acute hearing, with a lowest threshold of –21 db (re 20 μN/m–2) compared with the horses' lowest threshold of 7 db. Comparative analysis of the hearing abilities of these 2 species with those of other mammals provides further support for the relation between interaural distance and high-frequency hearing and between high- and low-frequency hearing. The study aimed at the development of a clinically applicable methodology that could: (1) discriminate transient evoked otoacoustic emission (TEOAE) recordings from normal hearing or hearing impaired individuals; (2) classify the nature of the hearing loss as conductive or as cochlear, and (3) define clear-cut TEOAE clinical criteria. A classification algorithm based on a multivariate discriminant analysis of fast Fourier transform data from recordings evoked by click stimuli of 50 +/- 2, 62 +/- 2, 68 +/- 2 and 80 +/- 2 dB SPL was used to discriminate 302 normal subjects from 383 subjects suffering from mild to moderate hearing losses. The best discriminant model (QDF80) produced a sensitivity of 93.8% and a specificity of 79.4%. When extra correlation criteria were serially applied to the classification outcome, the specificity was increased to 85.3%, but the sensitivity was marginally decreased to 91.7%. The classification of the correctly identified hearing-impaired cases yielded 93.8% identification of conductive and 75.1% identification of cochlear cases. A sensitivity analysis of the misclassified hearing-impaired cases suggested that the TEOAE spectra are well correlated with the 2-kHz but poorly correlated with the 4-kHz octave frequency.  相似文献

The results of a study in which visual evoked responses (VERs) and a modified Pulfrich method were compared showed that both methods are very effective for the diagnosis of multiple sclerosis. With VERs, 97% of the multiple sclerosis cases were diagnosed correctly, while the corresponding value for the Pulfrich method was 93%. In contrast to VERs, the Pulfrich method allows only measurement of latency differences between the two visual pathways. This method involves measuring the speed required to cause a shift in the apparent depth location of a large, moving, striped pattern observed with a neutral density filter over one eye. A pathological transmission time was inferred when the patients observed a shift in the depth of the moving pattern either without any filter at all or with a filter whose attentuation was no more than 0.2 log units. A further criterion for pathology was a difference of more than 10% between the two eyes in the retinal speed required for a depth displacement using a 1.5 log unit filter. This test requires about 15 minutes, and can be carried out by a technical assistant.  相似文献

Examined the correspondence between the critical period, during which acoustic trauma will profoundly alter subsequent auditory behavior, and the broader sensitive period, during which acoustic trauma is most damaging to cochlear functions in the young ear, using 133 C57BL/6 and 183 CBA mice (aged 12–54 days) as Ss. Ss were exposed to 2 min of 124-db octave band noise (8–26 kHz). A noninvasive electrocochleographic technique was used to assess cochlear microphonic (CM) and action potential (AP) thresholds in exposed Ss and nonexposed littermate controls. Noise had no effect on 12-day-old CBA Ss but produced a maximal threshold elevation at 30–36 days in 54-day-old Ss. Susceptibility to audiogenic seizures in exposed CBA Ss was greatest at the peak of the sensitive period for cochlear damage. 12-day-old C57BL/6 Ss were also unaffected by noise exposure; 36-day-old C57BL/6 Ss had maximal AP (23 db) and CM (17 db) threshold; elevations and 54-day-old C57BL/6 Ss had an 18-db elevation of the AP; and their CM was no longer affected. It is concluded that both genotypes have a sensitive period for the effects of noise trauma on CM and AP thresholds: CBA has a sensitive period for acoustic priming for audiogenic seizures, and C57BL/6 has a critical period for acoustic priming. A compound cochlear action potential can be recorded in man from the promontorium, after perforation of the tympanic membrane. This same cochlear action potential (along with the responses of the brain stem auditory nuclei) can also be recorded by means of earlobe-vertex electrodes. Recordings were made in patients with chronic perforations of the tympanic membrane from the promontorium and simultaneously from the earlobe (-vertex) in order to see if there is a difference in response threshold. As click intensity was decreased, the responses disappeared in both recordings at about the same stimulus intensity (threshold). At near-threshold click intensities, the response seen in the promontorium recording was the compound cochlear action potential while in the earlobe vertex recording, the response often seen was seen was that from the inferior colliculus. Thus, in most cases, there is no real advantage to be gained by perforating the tympanic membrane in order to record from the promontorium since an identical response threshold can be obtained using surface electrodes.  相似文献

The predictive accuracy of classifying children at risk for disruptive disorders through latent class analyses (LCA) was evaluated. Child Behavior Checklist/4-18 (T. M. Achenbach, 1991) items, reflecting symptoms of Diagnostic and Statistical Manual of Mental Disorders (4th ed., American Psychiatric Association, 1994) defined disruptive disorders, alone and in combination with other child and familial risk factors, were used to predict children's risk for disruptive disorder. Predictive accuracy reached a positive predictive value of 69%. Children incorrectly classified (false positive and false negative) were compared with correctly classified children (true positive and true negative) on risk factors in the family context to test whether the screening procedure could be improved through a multiple-gating procedure. The differences in familial context factors between these children were limited, and no clear indications were found on how to use familial context factors to improve the screening procedure after the initial classification by LCA. This study was designed to test the 1992 International Standards for Neurological and Functional Classification of Spinal Cord Injury. One hundred and six professionals in the field of spinal cord injury attending an instructional course at the 1994 ASIA Meeting participated in the test. Participants completed a pretest and posttest in which they classified two patients who had a spinal cord injury (one with complete tetraplegia and one with incomplete paraplegia) by sensory and motor levels, zone of partial preservation (ZPP), ASIA Impairment Scale and completeness of injury. Between tests, three members of the ASIA Standards Executive Committee gave presentations on the neurological assessment, scoring, scaling and classification of spinal cord injury and a video of the actual examinations of the two cases was viewed. Percent 'correct' (as defined by the ASIA Standards Committee) was calculated for sensory and motor levels, ZPP, ASIA Impairment and completeness. Overall, the analyses showed that participants had very little difficulty in correctly classifying the patient with complete tetraplegia. Pretests scores ranged from 72% (left motor level) to 96% (complete injury), posttest scores from 73% (left motor level) to 100% correct (complete injury). For the patient with incomplete paraplegia (Case 2), scores were considerably lower. Pretest scores ranged from 16% (right motor level) to 95% correct (incomplete injury); posttest scores from 21% (right motor level) to 97% correct (incomplete injury). The results showed that further revisions of the 1992 Standards and more training is needed to ensure accurate classification of spinal cord injury.  相似文献

Patients (219) with prostatic adenocarcinoma were classified on the basis of whether or not their bone scans were positive for metastasis. Acid and alkaline phosphatase determinations and clinical evaluations for bone metastases were reviewed. Of those with proved metastases, 43% had no bone pain, 39% had normal acid phosphatase levels, 23% normal alkaline phosphatase levels, 19% normal levels of both enzymes, and 15% normal enzyme levels without bone pain. Twenty-four per cent of the patients with normal enzyme levels and clinically unsuspected bone metastases had bone scans which proved positive for metastasis; 62% of these had normal radiographs.  相似文献

The purpose of the present study was to evaluate evoked potential and single fibre responses to biphasic current pulses in animals with varying degrees of cochlear pathology, and to correlate any differences in the physiological response with status of the auditory nerve. Six cats, whose cochleae ranged from normal to a severe neural loss (< 5% spiral ganglion survival), were used. Morphology of the electrically evoked auditory brainstem response (EABR) was similar across all animals, although electrophonic responses were only observed from the normal animal. In animals with extensive neural pathology, EABR thresholds were elevated and response amplitudes throughout the dynamic range were moderately reduced. Analysis of single VIIIth nerve fibre responses were based on 207 neurons. Spontaneous discharge rates among fibres depended on hearing status, with the majority of fibres recorded from deafened animals exhibiting little or no spontaneous activity. Electrical stimulation produced a monotonic increase in discharge rate, and a systematic reduction in response latency and temporal jitter as a function of stimulus intensity for all fibres examined. Short-duration current pulses elicited a highly synchronous response (latency < 0.7 ms), with a less well synchronized response sometimes present (0.7-1.1 ms). There were, however, a number of significant differences between responses from normal and deafened cochleae. Electrophonic activity was only present in recordings from the normal animal, while mean threshold, dynamic range and latency of the direct electrical response varied with cochlear pathology. Differences in the ability of fibres to follow high stimulation rates were also observed; while neurons from the normal cochlea were capable of 100% entrainment at high rates (600-800 pulses per second (pps)), fibres recorded from deafened animals were often not capable of such entrainment at rates above 400 pps. Finally, a number of fibres in deafened animals showed evidence of 'bursting', in which responses rapidly alternated between high entrainment and periods of complete inactivity. This bursting pattern was presumably associated with degenerating auditory nerve fibres, since it was not recorded from the normal animal. The present study has shown that the pathological response of the cochlea following a sensorineural hearing loss can lead to a number of significant changes in the patterns of neural activity evoked via electrical stimulation. Knowledge of the extent of these changes have important implications for the clinical application of cochlear implants.  相似文献

Studies have found that individuals with bulimia nervosa can be classified into dietary and dietary-negative affect subtypes and that the latter exhibit greater eating pathology, psychiatric comorbidity, and functional impairment; a more protracted clinical course; and a worse treatment response. In this report, the authors describe 2 prospective studies that found that young women with threshold (n = 48) and subthreshold (n = 83) bulimic pathology can be classified into dietary and dietary-negative affect subtypes; that two subtyping approaches produced similar results (mean κ = .94); that the subtyping distinction showed 4-week test-retest reliability (κ = .61); and that the dietary-negative affect subtype showed greater eating pathology, emotional distress, functional impairment, treatment seeking, and lower likelihood of recovery over 6-month and 3-year follow-ups than the dietary subtype. The dieting-negative affect subtyping distinction evidenced greater test-retest reliability and concurrent and predictive validity than did the purging-nonpurging subtyping distinction. The additional evidence for the reliability and validity of this subtyping scheme, particularly the prognostic utility, suggests it is worth additional inquiry.
