INTRODUCTION: Thoracic trauma secondary to motor vehicle crashes (MVC) continues to be a major cause of morbidity and mortality. Specific vehicle features may increase the risk of severe thoracic injury when striking the occupant. We sought to determine which vehicle contact points were associated with an increased risk of severe thoracic injury in MVC to focus subsequent design modifications necessary to reduce thoracic injury. METHODS: The National Automotive Sampling System (NASS) databases from 1993 to 2001 and the Crash Injury Research and Engineering Network (CIREN) databases from 1996 to 2004 were analyzed separately using univariate and multivariate logistic regression stratified by restraint use and crash direction. The risk of driver thoracic injury, defined as an abbreviated injury scale (AIS) of score > or =3, was determined as it related to specific points of contact between the vehicle and the driver. RESULTS: The incidence of severe chest injury in NASS and CIREN were 5.5% and 33%, respectively. The steering wheel, door panel, armrest, and seat were identified as contact points associated with an increased risk of severe chest injury. The door panel and arm rest were consistently a frequent cause of severe injury in both the NASS and CIREN data. CONCLUSIONS: Several vehicle contact points, including the steering wheel, door panel, armrest and seat are associated with an increased risk of severe thoracic injury when striking the occupant. These elements need to be further investigated to determine which characteristics need to be manipulated in order to reduce thoracic trauma during a crash.  相似文献

Occupant injury in real world vehicle accidents can be significantly affected by a set of crash characteristics, of which impact direction and impact location (or damage location) in general scale interval (e.g., frontal impact is frequently defined as general damage to vehicle frontal end with impact angle range of 11–1 o’clock) have been identified to associate with injury outcome. The effects of crash configuration in more specific scale of interval on the injury characteristics have not been adequately investigated. This paper presents a statistical analysis to investigate the combined effects of specific impact directions and impact locations on the serious-to-fatal injuries of driver occupants involved in near-side collisions using crash data from National Automotive Sampling System-Crashworthiness Data System (NASS-CDS) for the calendar years of 1995–2005.  相似文献

Aim

To examine factors that contribute to the severity of work related crashes in New South Wales, Australia.

Methods

Workers’ Compensation data was linked to police crash records for the period 1998-2002. Multivariate analysis was carried out to assess the relationship between relevant risk factors and the severity of injury (permanent disability or death) in drivers who had received a claim for a work related crash.

Results

Age, gender, occupation, duty status, vehicle type, licence status, fatigue, speeding and location of the crash were independently associated with the severity of the crash. Drivers aged 65 years and older were nearly twice (OR: 1.824, 95% CI: 1.106-3.007) as likely to be permanently injured or die as a result of a work related crash compared to the younger age group (15-24 years old). The risk to older drivers was even higher in crashes occurring while on duty. Drivers involved in traffic crashes while commuting were more likely to be severely injured (OR: 1.28, CI: 1.15-1.42) than those on duty. Compared to car drivers, taxi drivers were more than twice (OR: 2.38, CI: 1.726-3.296) as likely to be severely injured.

Conclusions

The findings contribute to bridging the gap in knowledge in the area of work related crashes and highlight the higher risk of permanent disability and death in older drivers, taxi drivers and commuters.  相似文献

The Crash Injury Research Engineering Network (CIREN) database contains detailed medical and crash information on a large number of severely injured occupants in motor vehicle crashes. CIREN's major limitation for stand-alone analyses to explore injury risk factors is that control subjects without a given injury type must have another severe injury to be included in the database. This leads to bias toward the null in the estimation of risk associations. One method to cope with this limitation is to obtain information about occupants without a given injury type from the National Automotive Sampling System's Crashworthiness Data System (NASS-CDS), which is a probability sample of towaway crashes, containing similar crash information, but less medical detail. Combining CIREN and NASS-CDS in this manner takes advantage of the increased sample size when outcomes are available in both datasets; otherwise NASS-CDS can serve as a sample of controls to be combined with CIREN cases, possibly under a sensitivity analysis that includes and excludes NASS-CDS subjects whose status as a control is uncertain. Because CIREN is not a probability sample of crashes that meet its inclusion criteria, we develop a method to estimate the probability of selection for the CIREN cases using data from NASS-CDS. These estimated probabilities are then used to compute “pseudo-weights” for the CIREN cases. These pseudo-weights not only allow for reduced bias in the estimation of risk associations, they allow direct prevalence estimates to be made using medical outcome data available only in CIREN. We illustrate the use of these methods with both simulation studies and application to estimation of prevalence and predictors of AIS 3+ injury risk to head, thorax, and lower extremity regions, as well as prevalence and predictors of acetabular pelvic fractures. Results of these analyses demonstrate combining NASS and CIREN data can yield improvements in mean square error and nominal confidence interval coverage over analyses that use either the NASS-CDS or the CIREN sample alone.  相似文献

There is a need to improve the protection to the thorax of occupants in frontal car crashes. Finite element human body models are a more detailed representation of humans than anthropomorphic test devices (ATDs). On the other hand, there is no clear consensus on the injury criteria and the thresholds to use with finite element human body models to predict rib fractures. The objective of this study was to establish a set of injury risk curves to predict rib fractures using a modified Total HUman Model for Safety (THUMS). Injury criteria at the global, structural and material levels were computed with a modified THUMS in matched Post Mortem Human Subjects (PMHSs) tests. Finally, the quality of each injury risk curve was determined. For the included PMHS tests and the modified THUMS, DcTHOR and shear stress were the criteria at the global and material levels that reached an acceptable quality. The injury risk curves at the structural level did not reach an acceptable quality.  相似文献

Fatal motor vehicle intersection crashes occurring in Norway in the years 2005–2007 were analyzed to identify causation patterns among their underlying contributing factors, and also to assess if the data collection and documentation procedures used by the Norwegian in-depth investigation teams produces the information necessary to do causation pattern analysis. 28 fatal accidents were analyzed. Causation charts of contributing factors were first coded for each driver in each crash using the Driving Reliability and Error Analysis Method (DREAM). Next, the charts were aggregated based on a combination of conflict types and whether the driver was going straight or turning. Analysis results indicate that drivers who were performing a turning maneuver in these crashes faced perception difficulties and unexpected behavior from the primary conflict vehicle, while at the same time trying to negotiate a demanding traffic situation. Drivers who were going straight on the other hand had less perception difficulties but largely expect any turning drivers to yield, which led to either slow reaction or no reaction at all. In terms of common contributing factors, those often pointed to in literature as contributing to fatal crashes, e.g. high speed, drugs and/or alcohol and inadequate driver training, contributed in 12 of 28 accidents. This confirms their prevalence, but also shows that most drivers end up in these situations due to combinations of less auspicious contributing factors. In terms of data collection and documentation, there was an asymmetry in terms of reported obstructions to view due to signposts and vegetation. These were frequently reported as contributing for turning drivers, but rarely reported as contributing for their counterparts in the same crashes. This probably reflects an involuntary focus of the analyst on identifying contributing factors for the driver held legally liable, while less attention is paid to the driver judged not at fault. Since who to blame often is irrelevant from a countermeasure development point of view, this underlying investigator approach needs to be addressed to avoid future bias in crash investigation reports.  相似文献

A new mathematical model was developed to estimate average injury and fatality rates in frontal car-to-car crashes for changes in vehicle fleet mass, impact speed distribution, and inherent vehicle protection. The estimates were calculated from injury/fatality risk data, delta-V distribution and collision probability of two vehicles, where delta-V depends on impact speed and mass of the colliding vehicles. The impact speed distribution was assumed to be unaffected by a change in fleet mass distribution.

The results showed that safety in frontal crashes would improve 27–35% by a 10% increase in fatality risk parameters, which reflected substantial improvement in inherent vehicle protection. A 40% safety improvement was attained by a 10% impact speed reduction. Consequences of vehicle fleet mass were not as strong, but depended on the average mass ratio of the fleet. A reduction in mass range would be the most beneficial, while a uniform mass reduction of 20% would increase the fatality rate by 5.4%. The model estimates trends in traffic safety and may help to identify priorities in active and passive safety.  相似文献

In this study, we investigated and assessed the dependence of dummy head injury mitigation on the side curtain airbag and occupant distance under a side impact of a Dodge Neon. Full-scale finite element vehicle simulations of a Dodge Neon with a side curtain airbag were performed to simulate the side impact. Owing to the wide range of parameters, an optimal matrix of finite element calculations was generated using the design method of experiments (DOE); the DOE method was performed to independently screen the finite element results and yield the desired parametric influences as outputs. Also, analysis of variance (ANOVA) techniques were used to analyze the finite element results data. The results clearly show that the influence of moving deformable barrier (MDB) strike velocity was the strongest influence parameter on both cases for the head injury criteria (HIC36) and the peak head acceleration, followed by the initial airbag inlet temperature. Interestingly, the initial airbag inlet temperature was only a ~30% smaller influence than the MDB velocity; also, the trigger time was a ~54% smaller influence than the MDB velocity when considering the peak head accelerations. Considering the wide range in MDB velocities used in this study, results of the study present an opportunity for design optimization using the different parameters to help mitigate occupant injury. As such, the initial airbag inlet temperature, the trigger time, and the airbag pressure should be incorporated into vehicular design process when optimizing for the head injury criteria.  相似文献

The aim of this study was to find which drugs and drug combinations were most common in drivers who died, in particular, in single vehicle crashes where the responsibility for the crash would be referred to the driver killed. The study included all available blood samples from drivers, who died within 24 h of the accident, in the years 2001 and 2002 in the five Nordic countries (total population about 24 million inhabitants). The samples were analysed for more than 200 different drugs in addition to alcohol, using a similar analytical programme and cut-off limits in all countries. In three countries (Finland, Norway and Sweden) blood samples were available for more than 70% of the drivers, allowing representative prevalence data to be collected. 60% of the drivers in single vehicle crashes had alcohol and/or drug in their blood samples, compared with 30% of drivers killed in collisions with other vehicles. In single vehicle accidents, 66% of the drivers under 30 years of age had alcohol and/or drugs in their blood (alcohol only – 40%; drugs only – 12%; alcohol and drugs – 14%). The drugs found were mostly illicit drugs and psychoactive medicinal drugs with warning labels (in 57% and 58% respectively of the drivers under 30 with drugs present). Similar findings were obtained for drivers 30–49 years of age (63% with alcohol and/or drugs). In drivers aged 50 years and above, killed in single vehicle crashes (48% with alcohol and/or drugs) illicit drugs were found in only one case, and psychoactive medicinal drugs were detected less frequently than in younger age groups. In 75% of single vehicle crashes, the driver was under 50 years. Thus, the majority of accidents where the drivers must be considered responsible, occurred with drivers who had recently used alcohol, or drugs, alone or in combination. The drugs involved were often illicit and/or psychoactive drugs with warning labels. Therefore a large proportion of single vehicle accidents appear to be preventable, if more effective measures against driving after intake of alcohol and drugs can be implemented.  相似文献

In this paper, we obtain detailed data on road traffic crash (RTC) casualties, by severity, for each of the eight state and territory jurisdictions for Australia and use these to estimate and compare the economic impact of RTCs across these regions. We show that the annual cost of RTCs in Australia, in 2003, was approximately $17b, which is approximately 2.3% of the Gross Domestic Product (GDP). Importantly, though, there is remarkable intra-national variation in the incident rates of RTCs in Australia and costs range from approximately 0.62 to 3.63% of Gross State Product (GSP). The paper makes two fundamental contributions: (i) it provides a detailed breakdown of estimated RTC casualties, by state and territory regions in Australia, and (ii) it presents the first sub-national breakdown of RTC costs for Australia. We trust that these contributions will assist policy-makers to understand sub-national variations in the road toll better and will encourage further research on the causes of the marked differences between RTC outcomes across the states and territories of Australia.  相似文献

Mobility practices have changed dramatically in Romanian towns over the last 25 years, following the collapse of socialist mobility restrictions. Urban areas like Cluj-Napoca are facing both increasing immigration and car mobility, and therefore increasing levels of road traffic crashes.The analysis of traffic crashes is one of the most important elements for improving the road safety policy. This paper is divided in two parts. In the first one, the authors focus on identifying the traffic crash hotspots along the street network, while in the second part they discuss the social background of road traffic crash occurrence.The first step in analyzing traffic crashes is to determine crash hotspots. A four-year record (2010–2013) provided by the Traffic Department of the General Inspectorate of Romanian Police (GIRPTD) was used. As a method of hotspot determination, the Kernel Density Estimation tool was employed, in the frame of the spatial analysis along network (SANET). The outcome was the hotspot map of traffic crashes in Cluj-Napoca. The results have revealed 4 categories of street segments: not-dangerous, low-dangerous, medium-dangerous and high-dangerous. Based on this classification, at least 4 dangerous areas were identified, located at the city entrances-exits (in the West, North-West and East) and the city center (the most dangerous zone).The second part of the paper focuses on social groups involved in car crashes. The following are considered: age, gender and blood alcohol concentration of the person (driver or pedestrian) found guilty for every individual crash.  相似文献

Objective

Evaluating the relation between Iranian drivers’ knowledge, attitude, and practice (KAP) regarding traffic regulations, and their deterministic effect on road traffic crashes (RTCs).

Setting

Two cities of Tehran and Zahedan, Iran.

Methods

A cross-sectional study was designed. Using a simplified cluster sampling design, 2200 motor vehicle drivers including 1200 in Tehran and 1000 in Zahedan were selected. Sixty locations in Tehran and 50 in Zahedan were chosen. In each pre-identified location, 20 adult drivers were approached consecutively. A questionnaire developed by researchers was filled by each participant. The questionnaire had four sections including items assessing the demographics, knowledge, attitude and practice of drivers toward traffic regulations. Logistic regression analysis was used to evaluate the relationship between the RTCs and KAP variables.

Results

The study sample consisted of 619 (28.1%) occupational and 1580 (71.8%) private drivers. Among them, 86.4% were male. The median age was 33.6 ± 10.83. Drivers in Tehran and Zahedan had no significant differences between their mean scores of KAP items of the questionnaire. Higher knowledge, safer attitude, and safer practice were associated with a decreased number of RTC. After adjusting for possible confounders, increase of one standard deviation in attitude and practice scores (but not knowledge) resulted in 26.4% and 18.5% decrease in RTC, respectively. Finally, considering knowledge, attitude and practice of drivers in one model to assess their mutual effect, it was shown that only attitude is significantly associated with a decrease of RTC (OR = 0.76, P = 0.007).

Conclusion

Increase in attitude and practice accompanied with decreased number of RTCs in Iranian drivers. Specifically, drivers’ attitude had the crucial effect. It is not knowledge and standard traffic education; rather it is how such education is registered as an attitude that translates what is being learned into actions. Without safer attitude, even safer self-reported practice will not result in lower RTCs.  相似文献

Objectives

Previous research has found that older driver fatal crash involvement rates per licensed driver declined substantially in the United States during 1997–2006 and declined much faster than the rate for middle-age drivers. The current study examined whether the larger-than-expected decline for older drivers extended to nonfatal crashes and whether the decline in fatal crash risk reflects lower likelihood of crashing or an improvement in survivability of the crashes that occur.

Methods

Trends in the rates of passenger vehicle crash involvements per 100,000 licensed drivers for drivers 70 and older (older drivers) were compared with trends for drivers ages 35–54 (middle-age drivers). Fatal crash information was obtained from the Fatality Analysis Reporting System for years 1997–2008, and nonfatal crash information was obtained from 13 states with good reporting information for years 1997–2005. Analysis of covariance models compared trends in annual crash rates for older drivers relative to rates for middle-age drivers. Differences in crash survivability were measured in terms of the odds of fatality given a crash each year, and the historical trends for older versus middle-age drivers were compared.

Results

Fatal crash involvement rates declined for older and middle-age drivers during 1997–2008 (1997–2005 for the 13 state subsample), but the decline for drivers 70 and older far exceeded the decline for drivers ages 35–54 (37 versus 23 percent, nationally; 22 versus 1 percent, 13 states). Nonfatal injury crash involvement rates showed similarly larger-than-expected declines for older drivers in the 13 state subsample, but the differences were smaller and not statistically significant (27 percent reduction for older drivers versus 16 percent for middle-age drivers). Property-damage-only crash involvement rates declined for older drivers (10 percent) but increased for middle-age drivers (1 percent). In 1997, older drivers were 3.5 times more likely than middle-age drivers to die in police-reported crashes (6.2 versus 1.8 deaths per 1000 crashes), but this difference was reduced during the 9-year study period to 2.9 times, as the rate of older drivers dying in a crash declined (5.5 deaths per 1000 crashes in 2005) and the death risk remained relatively stable for middle-age drivers.

Conclusions

Contrary to expectations based on increased licensure and travel by older drivers, their fatal crash risk has declined during the past decade and has declined at a faster rate than for middle-age drivers. The decreased risk for older drivers appears to extend not only to nonfatal injury crashes but also to property-damage-only crashes, at least as reported to police in the 13 states included in the nonfatal injury analysis. Although insurance collision data suggest that overall crash risk of older drivers may not be changing relative to middle-age drivers, the current analysis indicates that the reduced fatality risk of older drivers reflects both less likelihood of being involved in a police-reported crash and greater likelihood that they will survive when they do crash.  相似文献

Abstract

Although many studies in developed countries on the practice of EIA have been conducted, there is a lack of similar attention in Hong Kong. This gap in knowledge is addressed in this study which summarises and records the current practice and lessons learned from priority projects. In the case of Hong Kong, this lack of understanding is important, since a significant number of infrastructure projects with applications pending for environmental approvals were delayed, awaiting the court ruling of the judicial review. In this research, the EIA system and practices in Hong Kong were comprehensively reviewed by a comparison study between Hong Kong, Mainland China, Taiwan and Singapore. Then field work of interviews with professionals and focus group meetings with Green Groups were conducted to seek opinions of interviewees and members about the mechanism of EIA. Recommendations on aspects of EIA systems are provided at the end of the paper.  相似文献

Bicycle-related head injury: a study of 86 cases   总被引:1,自引:0,他引:1  
Within the framework of a bicycle helmet research program, we have set up a database of bicycle accident victims, containing both accident and clinical data. The database consists of a consecutive series of 86 victims of bicycle accidents who underwent a neurosurgical intervention in our hospital between 1990 and 2000. Data were obtained from police files, medical records, computed tomography head scans and a patient questionnaire. In only three victims, the wearing of a helmet was documented. In this study, the head injuries are analysed and the relation between the different types of head injuries and outcome is assessed. Forty-four accidents were collisions with a motor vehicle and 42 accidents were falls. Most impacts occurred at the side (57%) or at the front (27%) of the head. The most frequent injuries were skull fractures (86%) and cerebral contusions (73%). Age was negatively correlated with outcome (P = 0.0002 ) and positively correlated with the number (P = 0.00002) and volume (P = 0.00005) of contusions and the presence of subdural haematomas (P = 0.000001). The injuries with the strongest negative effect on outcome were: subarachnoid haemorrhage (P = 0.000001), multiple (P = 0.000005) or large ( P 0.0007) contusions, subdural haematoma (P = 0.001) and brain swelling (P = 0.002). A significant coexistence of these four injuries was found. We hypothesise that in many patients the contusions may have been the primary injuries of this complex and should therefore be considered as a main injury determining outcome in this study. We believe that such findings may support a rational approach to optimising pedal cyclist head protection.  相似文献

A comprehensive analysis is performed to evaluate the effect of weight, height and body mass index (BMI) of occupants on side impact injuries at different body regions. The accident dataset for this study is based on the National Automotive Sampling System-Crashworthiness Data System (NASS-CDS) for accident year 2000–08. The mean BMI values for driver and front passenger are estimated from all types of crashes using NASS database, which clearly indicates that mean BMI has been increasing over the years in the USA. To study the effect of BMI in side impact injuries, BMI was split into three groups namely (1) thin (BMI < 21), (2) normal (BMI 24–27), (3) obese (BMI > 30). For more clear identification of the effect of BMI in side impact injuries, a minimum gap of three BMI is set in between each adjacent BMI groups. Car model years from MY1995–1999 to MY2000–2008 are chosen in order to identify the degree of influence of older and newer generation of cars in side impact injuries. Impact locations particularly side-front (F), side-center (P) and side-distributed (Y) are chosen for this analysis. Direction of force (DOF) considered for both near side and far side occupants are 8 o’clock, 9 o’clock, 10 o’clock and 2 o’clock, 3 o’clock and 4 o’clock respectively. Age <60 years is also one of the constraints imposed on data selection to minimize the effect of bone strength on the occurrence of occupant injuries. AIS2+ and AIS3+ injury risk in all body regions have been plotted for the selected three BMI groups of occupant, delta-V 0–60 kmph, two sets (old and new) of car model years. The analysis is carried with three approaches: (a) injury risk percentage based on simple graphical method with respect to a single variable, (b) injury distribution method where the injuries are marked on the respective anatomical locations and (c) logistic regression, a statistical method, considers all the related variables together. Lower extremity injury risk appears to be high for thin BMI group. It is found that BMI does not have much influence on head injuries but it is influenced more by the height of the occupant. Results of logistic analysis suggest that BMI, height and weight may have significant contribution towards side impact injuries across different body regions.  相似文献

This paper presents analyses of data from the Highway Safety Information System (HSIS) for the State of Illinois. Our analyses focuses on whether various changes in road network infrastructure and geometric design can be associated with changes in road fatalities and reported accidents. We also evaluate models that control for demographic changes. County-level time-series data is used and fixed effect negative binomial models are estimated. Results cannot confirm the hypothesis that changes in road infrastructure and geometric design have been beneficial for safety. Increases in the number of lanes appears to be associated with both increased traffic-related accidents and fatalities. Increased lane widths appears to be associated with increased fatalities. Increases in outside shoulder width appear to be associated with a decrease in accidents. Inclusion of demographic results does not significantly change these results but does capture much of the residual time trend in the models. Potentially mis-leading results are found when the time trend is not included. In this case a negative association between vertical curvature and both accidents and fatalities. No statistical association with changes in safety is found for median widths, inside shoulder widths, and horizontal and vertical curvature.  相似文献

对增加了表面防护层的国产碳纤维/增韧环氧树脂CCF300/5228A层板的低速冲击及冲击后压缩性能进行了试验研究。通过落锤式低速冲击试验,得到了各组层板的冲击接触力历程、凹坑深度和内部分层面积等特征,而冲击后压缩试验结果可用来对各组层板的损伤容限性能进行评估。结果表明,同裸板相比,加了表面防护层的层板其分层起始载荷变化不大,但形成同样的1.0 mm凹坑所需的冲击能量增大了24%~46%。而对于内部分层,在一定的冲击能量范围下,加表面防护层的层板的C扫分层面积比裸板减小了20%~50%,而在同样凹坑深度的情况下,层板在加了表面防护层之后分层面积变化不大。冲击后压缩性能与内部分层情况具有较大的关联性,同样冲击能量下分层面积较小的各组带表面防护层板,其冲击后压缩强度和破坏应变相对于裸板的提高在15%~50%之间,而在凹坑深度相同的情况下,二者的冲击后压缩强度和破坏应变相差不大。  相似文献

Oil exploration presently takes place offshore NW of Greenland. The recent entry of the oil industry in this sparsely populated area carries the potential for radical and unpredictable societal change. To ensure local adaptation, Public participation (PP) is implemented as a legal requirement in environmental impact assessment of offshore oil exploration. However, NGOs and associations, industry and individuals in Greenland express general frustration of how PP is conducted. This paper presents an analysis of stakeholders' PP perceptions and their implications in Greenland. It is found that differences in PP purpose perceptions among stakeholders cause disagreement on what is considered good performance. Furthermore, the stakeholders disagree on the desired level of engagement. While NGOs emphasise a need for PP to influence decision-making, the public seems to accept a role as passive recipients of information about decisions already made. This leads to a discussion about the need for a more specific PP guideline based on dialogue among stakeholders, with emphasis on the cultural barriers related to power structures and communication.  相似文献
