This paper introduces readers to the concepts of implementation science, implementation theory, and implementation frameworks and models. A wide range of models has been published in the literature related to implementation. The paper will present an overview of the Consolidated Framework for Implementation Research (CFIR), which is a comprehensive typology that unifies and consolidates the array of constructs that influence implementation from the perspective of these models. The CFIR is then used to evaluate implementation models used in studies of substance use disorder (SUD) treatments. Implementation research is scarce, with few prospective studies of theory-driven implementation. We assert that future research in SUD needs to meet three overarching objectives to promote wider implementation of evidence-based practices: (a) differentiation of core versus adaptable components of evidence-based interventions need; (b) development of methods to design implementation strategies, effectively adapted to the broad context; and (c) design and testing of predictive models to assess likelihood of effective implementation and prospects for sustainability while taking into account salient contextual factors. Objective: To test the effectiveness of implementation intentions to decrease alcohol consumption and control for possible demand characteristics by employing an active control condition and contrasting experimenter-provided with self-generated implementation intentions. Design: Two hundred forty-eight participants were randomly allocated to 1 of 4 conditions: questionnaire-only; questionnaire plus planning instruction; questionnaire, planning instruction plus experimenter-provided implementation intention; or questionnaire, planning instruction plus self-generated implementation intention. Main Outcome Measure: Alcohol intake. Results: There were clinically and statistically significant decreases in alcohol consumption in the 2 experimental conditions, but not in the 2 control conditions, F(3, 237) = 3.34, p p2 = .04. There were no significant differences between experimenter-provided and self-generated implementation intentions (p = .62). Compliance moderated the effects of self-generated implementation intentions only, such that alcohol intake only significantly decreased in participants who complied with the instructions, F(1, 52) = 4.20, p p2 = .07. However, simply choosing an experimenter-provided implementation intention was just as effective as writing it out in full, implying that implementation intentions work even with minimal information processing. Conclusion: The findings further support use of implementation intentions to protect against health risk behaviors and are congruent with laboratory research showing that implementation intentions are a case of strategic automaticity. Why do some organizations succeed and others fail in implementing the innovations they adopt? To begin to answer this question, the authors studied the implementation of manufacturing resource, planning, an advanced computerized manufacturing technology, in 39 manufacturing plants (number of individual respondents?=?1,219). The results of the plant-level analyses suggest that financial resource availability and management support for technology implementation engender high-quality implementation policies and practices and a strong climate for implementation, which in turn foster implementation effectiveness—that is, consistent and skilled technology use. Implementation of new ideas and new practices are essential for an organization to remain competitive in a changing business environment. The absence of these new ideas will eventually lead to stagnation, but the challenge to organizations remains how to overcome the many barriers that appear during an implementation process and successfully convert these ideas into sustainable practices. Within the capital facility industry, this challenge is amplified based on long-held industry practices, corporate culture, and a necessary focus on minimizing risk. However, even the capital facility industry is being forced to recognize the need to adapt to changing business conditions through the implementation of best practices. Specifically, organizations require an implementation plan that provides enough detail that all parties impacted by the process understand the process (roadmap) that is going to be followed during implementation. This paper introduces the development of the staircase implementation model and how it addresses the need for an implementation roadmap that is applicable to all implementation initiatives. The paper introduces the model components, the proposed application of the model, and the initial testing of the model that led to specific implementation considerations. This paper has significant relevance to industry as it gives a practical introduction to the important focus of implementing new practices.  相似文献

The present study integrates institutional factors and employee-based collective processes as predictors of 2 key implementation outcomes: implementation effectiveness and innovation effectiveness (Klein, Conn, & Sorra, 2001). Specifically, the authors proposed that institutional factors shape employees' collective implementation efficacy and innovation acceptance. The authors further hypothesized that these employee-based collective processes mediate the effects of institutional factors on implementation outcomes. This integrative framework was examined in the context of 47 agencies and ministries of the Korean Government that were implementing a process innovation called E-Government. Three-wave longitudinal data were collected from 60 external experts and 1,732 government employees. The results reveal the importance of management support for collective implementation efficacy, which affected employees' collective acceptance of the innovation. As hypothesized, these collective employee dynamics mediated the effects of institutional enablers on successful implementation as well as the amount of long-term benefit that accrued to the agencies and ministries. For more than a decade, health care systems have attempted to implement evidence-based practices and guidelines. These efforts have demonstrated the difficulty in making practice changes in complex systems of care. Many health care systems, including the Department of Veterans Affairs (VA) and state community mental health systems, have made adoption of evidence-based treatments, especially psychotherapies, a priority. Psychologists, as behavioral change experts and clinical leaders, are positioned to aid local implementation efforts but may have limited knowledge of the “implementation science” literature. This article provides a brief introduction to the implementation literature and offers a guide for developing an implementation plan to adopt evidence-based psychotherapies in local health care settings illustrated by a hypothetical example. Many individuals intend to exercise, but fail to link this intention to behavior. The present study examined the impact of an implementation intention intervention (i.e., instructions to form specific if-then plans) on an exercise intention-behavior relationship among working adults who varied in reported occupational stress levels. Results indicated that implementation intentions backfired, such that participants who did not form an implementation intention exercised significantly more than participants who formed an implementation intention. Two common approaches have been used by governments for the implementation of public-private partnerships (PPPs): a finance-based approach that aims to use private financing to satisfy infrastructure needs, and a service-based approach that aims to optimize the time and cost efficiencies in service delivery. The implementation of PPPs, however, may suffer from legal, political, and cultural impediments. In the United States, the federal government enabled a number of acts to ease the impediments and promote PPPs for infrastructure development. Based on a detailed analysis of PPPs in the United Kingdom and British Columbia, Canada, this paper describes principles that would characterize the implementation of PPPs at the program level (e.g., whether the implementation is successful). The principles pertain to the: availability of a PPP legal framework and implementation units; perception of the private finance objectives, risk allocation consequences, and value-for-money objectives; maintenance of PPPs process transparency; standardization of procedures; and use of performance specifications. Guidelines for successful implementation are explained and discussed in the context of the United States PPPs experience and impediments.  相似文献

How does an individual practitioner, agency, or treatment system get from making a decision or being mandated to adopt an evidence-based practice (EBP) to successfully implementing and using the EBP? Research and theory on factors that help or hinder putting new treatments into practice focus on individual, organizational, and external levels. Although several factors have been identified that affect implementation, the level and specificity of evidence do not warrant the term evidence-based implementation strategies. Yet steps must be taken now to start using EBPs. This article includes a review of research and theory on implementation, a case example of statewide implementation, and recommendations for psychologists across multiple roles to assist in advancing the implementation of EBPs into clinical practice.
