A focus on health promotion, disease prevention, and health education for all members of society has emerged from the federal guidelines of Healthy People 2000 and, more important, from the demonstrated needs of vulnerable populations. New and potent organisms and diseases have stressed the health care system. Nursing administrators face significant challenges in managing health care delivery and nursing within the multidisciplinary team in the transformed system. In all areas, education must be directed to meet the challenges and nursing must assume the leadership for ensuring the health care needs of individuals and communities, now and in the future.  相似文献

An emerging global reality is the presence of a culturally diverse workforce in health care environments. Not only are nurses delivering care to culturally diverse clients, the nurses themselves may come from different cultural and educational backgrounds. In order to provide culturally competent health care, such diversity must be molded into professional collaboration and respect. To date, health care to the citizens of Saudi Arabia has been provided largely by a foreign labor force, the vast majority of whom are non-Arabic speaking. This paper explores a number of strategies aimed at educating expatriate non-Saudi health professionals in the provision of culturally competent and culturally congruent health care. Leininger's transcultural health care theory is identified as the framework for achieving this goal. The theory has proved to be of great relevance in a hospital setting where some 40 different nationalities are represented among the work force. In such a setting, the potential for cultural conflict and stress is very real. Health professionals new to the Kingdom are introduced to the cultural dimensions of health care as it relates to patient care modes, as well as living and working together in a transcultural environment. Transcultural nursing is a major component of a professional nurse practice model which provides a visionary perspective for nursing care. Within the practice model, transcultural care principles are used to guide education, clinical practice and nursing research. Furthermore, Leininger's theory serves to grasp a comprehensive view of generic (folk) and professional health systems and to identify ethical issues confronted by nurses in the transcultural setting.  相似文献

Reviews the quality assurance requirements that are currently being applied to community mental health centers. The requirements were initially developed from an inpatient perspective. Application to an outpatient mental health area presents many problems, not the least of which is that no generally accepted standards exist for mental health care. Describes a model of quality management, designed by a task force of the Canadian Psychological Association, to meet revised standards of the Canadian Council on Health Facilities Accreditation for psychology services. The incorporation of aspects of Continuous Quality Improvement, Total Quality Management, Indicators, and Benchmarking are discussed in relation to their application to psychology services in health care facilities. The Quality Management Model is based on the assumptions that everyone wants to do their job well and improve in their job performance; that the improvement in the quality of services is everyone's responsibility; and that psychological service provision in a health care facility involves complex linkages between providers, suppliers, and customers. The model also assumes that the major problems affecting quality of service are usually those same linkages, or the customers themselves; and that identifying and monitoring these linkages is crucial to understanding how to improve quality. To further the goals of greater legitimacy and power, it is necessary for nursing to abandon many of the profession building strategies it has pursued in the past. We describe the traditional framework for profession building in nursing and some reasons it is no longer effective. In contrast, a contemporary framework for profession building in nursing is presented. Because nursing, like other complex adaptive systems, is strongly anchored by its past trajectory, it will require considerable energy and resources to make the transition.  相似文献

As the nation's health system moves away from earlier models to one grounded in population health and market-based systems of care, new challenges arise for public health professionals, primary care practitioners, health plan and institutional managers, and community leaders. Among the challenges are the need to develop creative concepts of organization and accountability and to assure that dynamic, system-oriented structures support the new kind of leadership that is required. Developing tomorrow's integrated community health systems will challenge the leadership skills and integrative abilities of public health professionals, primary care practitioners, and managers. Contraceptive compliance is a multifaceted issue that is influenced by many factors. These factors can directly affect the level of patient compliance, thereby affecting contraceptive method efficacy rates. A review of the literature reveals many studies about contraceptive compliance but a dearth of studies addressing how to change noncompliant behaviors. This article describes the contraceptive methods currently available and their efficacy rates. Patient characteristics and the components of compliance are described as they affect contraceptive efficacy and patient care. Suggestions are made for the use of alternative terminology to include adherence to or continuance of a contraceptive method. Health care providers should realize the impact they can have on a patient's education, decision-making process, and ultimate compliance with a contraceptive method. It is the patient, however, who ultimately makes the decision, either actively or passively, to comply or not and whether to have an unplanned pregnancy.  相似文献

Domestic violence is a highly prevalent event in the United States. Health providers frequently treat abused patients; however, many do not routinely assess for family violence or suspect it when the injuries seem fairly obvious. Because abused women feel stigmatized by the violence, they are hesitant to volunteer the abuse. In 1990, the Joint Commission on the Accreditation of Healthcare Organizations added requirements for emergency and ambulatory care services to develop and use protocols for the identification of violence among patients.
