PurposeTo measure the refractive index (RI) of commonly available soft contact lens (CL) materials, their packaging solutions and compare to the manufacturers’ nominal RI. The relationship between RI versus water content, and the effect of inaccurate RI when converting lens power measured in solution to in-air back vertex power were examined.MethodsThe RI of 18 single vision soft CL materials were measured using CLR 12–70 digital refractometer. Three lenses of each material were measured, in their packaging solution and then after soaking in standard phosphate buffered saline (PBS). The RIs of packaging solution were also measured. Accuracy requirements for correct wet to dry power conversion based on thick lens formula were projected.ResultsThe standard deviation between three samples was less than 0.005. The measured RI ranged from 1.3744 ± 0.001–1.4265 ± 0.0004 for PBS soaked and from 1.3739 ± 0.0003–1.4264 ± 0.0024 for packaging solution soaked materials. Comparing nominal with mean measured PBS and packaging solution RIs, 5 and 3 lens materials, respectively, fell outside ISO tolerance. The packaging solution RI of DailiesAquaComfortPlus had the largest difference of 0.0040, compared to RI of standard PBS. For converting lens power measured in PBS to in-air power, the difference between measured and nominal RI of 0.0104 would result in wrongly calculated in-air power 0.99 D for a -6.00 D lens.ConclusionThe CLR 12–70 is reliable and accurate refractometer for the measurement of soft CL materials. Accurate RI measurements are of relevance with increased use of wavefront sensors to measure lens power while they are immersed in solution. Even small errors in solution or material RI can lead to significant errors in converted in-air power. To obtain valid in-air lens power results, measurement conditions must match the material and solution RIs used for the conversion.  相似文献

PurposeThe purpose of this study was to evaluate the uptake and release of radiolabelled polyhexamethylene biguanide (PHMB) on reusable daily wear contact lenses (CLs) over 7 days.MethodsThree silicone hydrogel (SH) contact lens materials (lotrafilcon B, balafilcon A, senofilcon A) and two conventional hydrogel (CH) materials (etafilcon A, omafilcon A) were examined. In experiment 1 (1-day study), CLs were soaked in 2 mL of phosphate buffered solution (PBS) containing radiolabelled 14C PHMB (1 µg/mL) for 8 h. The release kinetics of 14C PHMB from the CLs was measured at t = 0.25, 0.5, 1, 2, 4, 8, and 24 h in fresh 2 mL PBS. In experiment 2 (7-day study), the CLs were soaked in the 14C PHMB (1 µg/mL) solution for 8 h followed by a 16-hour release in 2 mL PBS. The lens cycle was repeated daily for 7 days. After both experiments, the residual amount of PHMB remaining within the lenses was extracted to determine the total uptake of PHMB.ResultsIn experiment 1, the total uptake of PHMB for etafilcon A was significantly greater than senofilcon A (p = 0.01). There were no significant differences in total uptake of PHMB between other lens materials (p > 0.05). Etafilcon A released more PHMB compared to all other lens types over a 24-hr period (p < 0.001). In experiment 2, all CL materials continued to sorb more PHMB over time (p < 0.001). By day 7, the amount of PHMB sorbed by etafilcon A was significantly greater than senofilcon A (p = 0.02). After day 2, the CH materials released significantly more PHMB than the SH materials (p < 0.01).ConclusionThe CL materials continued to sorb PHMB with no signs of saturation after 7 days. All lens materials released a consistent amount of PHMB each day. Radioactive labelling provides a sensitive method of assessing the uptake and release of PHMB from CL materials.  相似文献

PurposeTo evaluate and compare the distribution of refractive power within the optic zone of different soft contact lenses and to investigate the effect of lens decentration on the power profiles.MethodsThe Nimo TR1504 instrument was used to measure the optical power across different aperture diameters (from 1.5 mm to 5.5 mm in steps of 0.5 mm) of four daily disposable contact lenses: DAILIES TOTAL1, Proclear 1-Day, SofLens daily disposable and 1-DAY ACUVUE MOIST. Measurements were performed using a wet cell. Power data were evaluated when contact lenses were in its centered position and after inducing different amounts of lens decentration (from 0.2 mm to 1.0 mm in steps of 0.2 mm).ResultsAll contact lenses showed an increase – more negative – in lens power with distance from the lens center. The amount of change varied depending on the lens. It was about 10% of lens power for DAILIES TOTAL1 (−0.29 diopters (D)), SofLens daily disposable (−0.36 D), and Proclear 1-Day (−0.32 D) whereas 1-DAY ACUVUE MOIST showed a percentage variation of 3.3% (−0.10 D). After inducing a lens decentration up to 1 mm, the power curves were shifted in the negative direction. However, the change obtained in lens power compared with well-centered position was always lower than a quarter of diopter both for all the lenses and aperture diameters.ConclusionsOur results showed a variation of the refractive power from the lens center, becoming more negative toward the periphery, with a negligible effect of the decentration for all disposable contact lenses studied.  相似文献

A simple hand-held refractometer was used to measure the refractive index of 27 rigid gas permeable contact lens materials. As a general rule, lenses with refractive indices lower than 1.458 are made from fluorosilicone acrylates; lenses with refractive indices in the range of 1.458 to 1.469 are made from either fluorosilicone acrylates or silicone acrylates; and lenses with refractive indices greater than 1.469 are made from silicone acrylates. It is demonstrated how refractometry can be used by contact lens practitioners for the identification and verification of rigid contact lenses.  相似文献

PurposeThe aim of this study was to evaluate the types of contact lenses fitted by hospital optometrists within the Midlands region of the United Kingdom (UK).MethodA questionnaire was sent to all the lead optometrists of the Midlands Hospital Optometry Group (MHOG). This group includes optometry hospital eye departments within the Midlands region of the UK. The questionnaire requested information of their last ten contact lens fitting appointments. Details of the patient’s age, gender, lens type, wearing times, and presenting condition were retrospectively taken from the patients’ records using the appointment diary to identify the last ten patients fitted with contact lenses.ResultsDetails from a total of 109 contact lens fits were collected. This included 45 females and 64 males with a mean age of 39.4 ± 17.4 years. The mean wearing time was 6.3 ± 1.0 days per week and 10.7 ± 5.1 h per day. Sixty-one percent of the contact lenses fitted were for patients with keratoconus and over half of all the contact lenses fitted were corneal rigid gas permeable lenses.ConclusionThis study highlighted that the main reason for fitting contact lenses in hospital contact lens practice is primary corneal ectasia, and mainly keratoconus. Whilst most patients with keratoconus were fitted with corneal rigid gas permeable contact lenses, around 1 in 6 were fitted with soft contact lenses. This study addresses a gap in the literature about contact lenses fitted in UK hospitals and how they differ from community contact lens practice.  相似文献

PurposeThis study was conducted to evaluate the effects of long-term use of rigid gas-permeable (RGP) contact lenses on corneal endothelium in keratoconus (KC) patients using non-contact specular microscopy. In addition, the correlation between wearing duration of RGP lenses (years & hours /d) and endothelial morphometric changes in KC patients was performed. This may provide more useful data for clinical application of RGP contact lens.Study designThis study was a prospective, observational, comparative, hospital based, nonrandomized, cross-sectional, and quantitative study.Subjects and methodsThe study included 40 eyes of non-wearer contact lens KC patients (group 1) and 38 KC eyes with a long history of Rose K2 RGP contact lens wearing (group 2). The corneal endothelial morphology such as endothelial cell density (ECD); coefficient of variation of cell area (CV); percentage of hexagonal cells (HEX); and central corneal thickness (CCT) were evaluated in all patients using SP3000P Specular Microscope.ResultsThe study included 40 eyes of non-wearer contact lens KC patients (20 mild KC eyes and 20 moderate KC eyes) and 38 KC eyes with a long history of RGP contact lens wearing (18 mild KC eyes and 20 moderate KC eyes). The mean duration of RGP lenses wearing was 5.67 ± 3.481 years & 12.11 ± 2.698 h/d in mild KC eyes and 7.15 ± 5.294 years &13.65 ± 2.889 h/d in moderate KC eyes. Wearing duration in hours in mild KC eyes showed a significant moderate negative correlation with ECD (r = ?0.529, p = 0.024) and a moderate positive correlation with CV (r = 0.565, p = 0.015). In addition, wearing duration in years in moderate KC eyes showed a significant moderate negative correlation with ECD (r = ?0.465, p = 0.039) and moderate positive correlation with CV (r = 0.627, p = 0.003). However, wearing duration in hours in moderate KC eyes showed a significant moderate negative correlation with HEX only (r = ?0.490, p = 0.028). This study reported significant corneal thinning in contact lens wearer KC eyes in contrast to non-wearer KC eyes.ConclusionThis study documented a significant correlation between a long-term use of Rose K2 RGP using the three-point touch and corneal endothelial morphometric changes in KC patients. In addition, the current study confirmed a significant corneal thinning in RGP contact lens wearer compared to non-contact lens wearer KC patients. Further studies are recommended to evaluate the association between different RGP lens materials with different oxygen permeability transmissibility; different lens design types; different fitting methods; the duration of contact lens wear and the endothelial morphometric changes in KC patients.  相似文献

Purpose. To determine the extent of orthokeratology (OK) contact lens fitting worldwide and to characterize associated demographics and fitting patterns.Methods. Survey forms were sent to contact lens fitters in 45 countries between January and March every year for 14 consecutive years (2004 – 2017, inclusive). Practitioners were asked to record data relating to the first 10 contact lens fits or refits performed after receiving the survey form.Results. Data were obtained for 295,044 contact lens fits, of which 2,702 were with OK lenses and 292,342 were with other lens types (non-OK). Overall, OK lenses represented 1.2% of all contact lens fits, with significant differences between nations (p < 0.0001), ranging from no fits recorded in Brazil, Egypt, Indonesia, Iran, Lithuania, Nepal and the United Arab Emirates, to 6.0% in The Netherlands. There has been a slight overall increase in OK lens fitting over the survey period (p < 0.0001). OK lenses were fitted to younger persons (OK, 25.0 ± 12.8 years vs. non-OK, 39.8 ± 14.9 years) (p < 0.0001). A higher proportion of males (55%) were fitted with OK lenses versus non-OK lenses (30%) (p < 0.0009). There was a skewed distribution towards OK lenses being fit with higher oxygen permeable materials (p < 0.0001) and on a planned replacement basis (p < 0.0001).Conclusions. OK contact lens prescribing is a niche activity, with this lens type typically being fitted in high oxygen permeable materials on a planned replacement basis to younger males. The slightly increasing rate of OK fitting, albeit at a very low level, may be attributed to practitioner interest in the reported myopia control properties of this lens type.  相似文献

PurposeThis study aims to report on the measured in vivo contribution of soft lenses of various powers to the optics of a piggyback system (PBS).MethodsThis prospective, non-dispensing clinical study was conducted on regular wearers of contact lenses who showed regular corneal profiles. Subjects were masked to the products used. The study involved the use of a spherical soft lens of three different powers in a PBS, used as a carrier for a rigid gas permeable lens. Baseline data were collected and soft lenses were then fitted on both eyes of each subject. Both lenses were assessed for position and movement. Over-refraction was obtained. Soft lens power contribution to the optics (SLPC) of a PBS system was estimated by computing initial ametropia, lacrymal lens, rigid lens powers and over-refraction. A set of data on one eye was kept, for each subject, for statistical analysis.ResultsThirty subjects (12 males, 18 females), aged 24.4 (± 4.5) years, were enrolled. The use of plus powered soft lenses enhanced initial RGP lens centration. Once optimal fit was achieved, all lenses showed normal movement. SLPC represented 21.3% of the initial soft lens power when using a −6.00 carrier, and 20.6% for a +6.00. A +0.50 did not contribute to any power induced in the system. These results are generally in accordance with theoretical model developed in the past.ConclusionOn average, except for the low-powered carrier, the use of a spherical soft lens provided 20.9% of its marked power. To achieve better results, the use of a plus-powered carrier is recommended.  相似文献

PurposeTo investigate the efficacy of spherical aberration (SA) correction with aspheric contact lenses (aspheric lenses) based on lens power, and compare the results with those of spherical contact lenses (spherical lenses).MethodsOcular higher-order aberrations were measured with a wavefront sensor, in 11 myopic subjects wearing an aspheric lens (Medalist Fresh fit (PUREVISION 2 HD); Bausch + Lomb) or a spherical lens (ACUVUE Oasys; Johnson & Johnson). Six different lens powers (−7.00 diopters (D), −5.00 D, −3.00 D, −1.00 D, +1.00 D, +3.00 D) were used for all subjects. The amount of SA correction from the contact lens at each power was calculated as the difference between SA with the contact lens on-eye and SA of the eye alone.ResultsFor the spherical lenses, SA correction was close to 0.00 μm for the +1.00 D lens, became more positive as the labeled lens power increased and became more negative as the labeled lens power decreased. For the aspheric lenses, SA correction was consistent, from −0.15 to −0.05 μm, for all lens powers except for the −1.00 D lens. SA correction for the spherical and aspheric lenses was significantly different at −7.00 D (p = 0.040), −3.00 D (p = 0.015), −1.00 D (p < 0.001), +1.00 D (p = 0.006), and +3.00 D (p < 0.001) powers.ConclusionAn aspheric lens is capable of correcting SA at different lens powers, and has SA correction in the range of −0.15 to −0.05 μm over a 6 mm aperture.  相似文献

PURPOSE: The purpose of this work was to evaluate the ability of four silicone-hydrogel contact lenses (galyfilcon A, balafilcon A, lotrafilcon A and lotrafilcon B) to retain their equilibrium water content before and after wear, through measurements of refractive index and compare with that of a conventional disposable hydrogel contact lens (etafilcon A). METHODS: The refractive indices of 115 contact lenses were measured using an automated refractometer (CLR 12-70, Index Instruments, Cambridge, U.K.) before and after a schedule of daily wear by 58 patients for 30 days in the case of silicone-hydrogel lenses and 15 days for the conventional contact lenses. RESULTS: In the silicone-hydrogel contact lenses the changes on the refractive indices were not statistically significant, however after being worn the refractive index of the conventional etalfilcon A hydrogel contact lens increased significantly (p<0.001). CONCLUSION: The results presented here show that after being worn the silicone-hydrogel contact lens, show more capacity to retain or to reach their initial equilibrium water content than conventional hydrogel contact lenses. This suggests that the silicone-hydrogel contact lenses are less susceptible to spoilation over time maintaining its biocompatibility and contributing to the clinical success of lens performance.  相似文献

PurposeTo compare conventional method of contact lens fitting with software based contact lens fitting using Medmont corneal topographer in eyes with nebular and macular corneal scars.MethodsFifteen participants who were diagnosed with nebular and macular scars were fitted with rigid gas permeable lenses using conventional method of contact lens fitting. During the lens dispensing visit, participants underwent Medmont corneal topography. Agreement between the two methods of contact lens fitting was studied.ResultsParameters of contact lenses which were concluded using conventional method was compared to that of software based fitting. Mean difference in base curve of contact lens between the two methods was 0.094 mm ± 0.147 mm (95% CI: +0.383 to −0.194). Mean difference in diameter of contact lens between the two methods was 0.16 mm ± 0.172 mm (95% CI: +0.497 to −0.177). Contact lens parameters that were achieved using two different methods of fitting showed good correlation. Correlation coefficients, as comparison of two methods in base curve and diameter were 0.96 (P < 0.05) and 0.94 (P < 0.05), respectively.ConclusionSoftware based contact lens fitting would be useful for contact lens practitioners to predict initial base curve of contact lens in corneal scars.  相似文献

PurposeTo evaluate the success rate of highly gas permeable scleral contact lenses (SCL) for visual rehabilitation after penetrating keratoplasty (PK), over a period of up to 9 years.MethodsA total database of 31 consecutive patient fitted with SCL between January 2004 and December 2009 was retrospectively reviewed. Demographic data, etiology prior to lens fitting, visual outcomes, follow up time and complications were analyzed.ResultsAll eyes were fitted due to inadequate spectacle-corrected vision after successful penetrating keratoplasty or failure of other contact lens modalities. Out of 31 patients fitted, 28 (33 eyes) continue to wear SCL for periods between 0.5 and 8.8 years. The mean duration of follow-up after contact lens fitting was 5.2 ± 2.2 years. The mean age of corneal graft was 17.6 ± 11.4 years (range 4.3–42), and the mean interval between PK and initial contact lens fitting was 12.2 ± 10.7 years (range 0.7–36.0). The average steepest keratometry of our cohort was 55.0 ± 7.5 diopter (D) and the refractive astigmatism was 8.0 ± 4.4 D. The mean contact lens corrected visual acuity (BCVAcl) was 0.78 ± 0.25 (range 0.3–1.2). Twenty-three (82%) patients achieved a functional vision of 0.5 or more.During the studied period, ten (30.0%) eyes presented at least one graft rejection episode and two eyes (6%) had an episode of microbial keratitis. Corneal transplants of 20 years or more show a higher rate of refits due to ectasia recurrence.ConclusionsScleral lenses should be considered as lens of choice in eyes with complex corneal geometry, as besides visual rehabilitation, their use may delay or prevent further surgical involvement.  相似文献

PurposeThis study compared the biocompatibility and comfort of 4 lens care solutions currently marketed in France.MethodsThis was a randomized, interventional, double-masked, single-center crossover study assessing balafilcon A silicone hydrogel contact lenses, bilaterally, straight from the blister pack solution (control) and pre-soaked in the following lens care solutions: Regard® (containing sodium chlorite), ReNu® (containing a PHMB [polyhexamethylene biguanide] derivative), CyClean™ and MeniCare™ Soft (both containing PHMB). Subjects were randomized to the order of test solution use. For each of the 5 solutions tested, subjects attended a baseline/lens dispensing visit and an intervention visit 2 h later. At both visits, evaluation included slit-lamp examination, corrected-distance visual acuity, corneal staining, and subject-assessed photophobia, ocular comfort, and ocular redness.ResultsThirty subjects were enrolled and 28 were evaluable. Corneal staining severity was significantly worse than baseline after 2 h of wearing lenses soaked in CyClean, MeniCare, or ReNu (P  0.001). The MeniCare group alone demonstrated a significant improvement in ocular comfort after 2 h of lens wear (P = 0.02). No group demonstrated significant changes in ocular redness or photophobia. Corrected-distance visual acuity was similar between baseline and intervention visits for each test solution. No adverse events were reported during the study.ConclusionsSilicone hydrogel contact lenses presoaked in lens solutions containing PHMB or a PHMB derivative produced an increase in corneal staining after 2 h of lens wear. The higher levels of corneal staining in the 2 solutions did not correlate with increased discomfort within this 2-h timeframe.  相似文献

PurposeTo evaluate the performance of two experimental contact lenses (CL) designed to induce relative peripheral myopic defocus in myopic eyes.MethodsTen right eyes of 10 subjects were fitted with three different CL: a soft experimental lens (ExpSCL), a rigid gas permeable experimental lens (ExpRGP) and a standard RGP lens made of the same material (StdRGP). Central and peripheral refraction was measured using a Grand Seiko open-field autorefractometer across the central 60° of the horizontal visual field. Ocular aberrations were measured with a Hartman-Shack aberrometer, and monocular contrast sensitivity function (CSF) was measured with a VCTS6500 without and with the three contact lenses.ResultsBoth experimental lenses were able to increase significantly the relative peripheral myopic defocus up to −0.50 D in the nasal field and −1.00 D in the temporal field (p < 0.05). The ExpRGP induced a significantly higher myopic defocus in the temporal field compared to the ExpSCL. ExpSCL induced significantly lower levels of Spherical-like HOA than ExpRGP for the 5 mm pupil size (p < 0.05). Both experimental lenses kept CSF within normal limits without any statistically significant change from baseline (p > 0.05).ConclusionsRGP lens design seems to be more effective to induce a significant myopic change in the relative peripheral refractive error. Both lenses preserve a good visual performance. The worsened optical quality observed in ExpRGP was due to an increased coma-like and spherical-like HOA. However, no impact on the visual quality as measured by CSF was observed.  相似文献

Cosmetically tinted soft contact lenses are an attractive option for contact lens wearers. Data that we have gathered from annual contact lens fitting surveys demonstrate that those wearing tinted lenses are more likely to be female (4.6% of all soft lenses fitted vs. 1.6% for males; p < 0.0001) and younger (27 ± 11 years vs. 33 ± 13 years for those wearing non-tinted lenses; p < 0.0001). Tinted lenses tend to be worn more on a part-time basis and are replaced less frequently than non-tinted lenses. The decline in fitting tinted lenses over the past 12 years may be due to (a) the current limited availability of tinted lenses in silicone hydrogel materials and daily disposable replacement frequencies, which together represent a significant majority (78%) of new soft lenses fits today, (b) growing concerns among lens wearers and practitioners relating to the risks of complications associated with the wearing of tinted lenses, and (c) reduced promotion of such lenses by the contact lens industry.  相似文献

BackgroundSince the introduction of Silicone hydrogel (SiHy) contact lenses 20 years ago, industry has continued to modify lens materials, designs, lens care products and manufacturing processes, striving to improve contact lens physiological performance, comfort, and convenience for wearers. The purpose of this study was to investigate whether the ocular health of habitual soft contact lens wearers today is better than it was in previous decades.MethodsBaseline ocular physiology data for 3624 participants from a The Brien Holden Vision Institute clinical trials database were retrospectively reviewed. Records were grouped into 3 time periods; A: >2 decades ago (1997–1999), B: one decade ago (2009–2014) and C: recent years (since 2015). Physiology data for both neophytes and habitual contact lens wearers included; bulbar, limbal and upper palpebral conjunctival redness, corneal and conjunctival staining and conjunctival indentation from contact lenses.ResultsCorneal staining levels are similar between neophytes and contact lens wearers at time points A and C but worse for contact lens wearers at time point B. Limbal redness was greater in contact lens wearers than in neophytes at time point A but at time points B and C they are not different to the non-contact lens wearing population. In recent years, most ocular physiological variables in habitual contact lens wearers are similar to neophytes.ConclusionsWhile there have been changes over the past two decades in ocular physiological responses to contact lens wear, it appears that ocular health with current day contact lens wear is similar to no lens wear in most respects.  相似文献

PurposeThe aim of the present study was to compare the objective and subjective visual performance of three different soft multifocal contact lenses.Methods10 subjects (habitual soft contact lens wearers) between the ages of 40 and 45 years participated in the study. Three different multifocal silicone hydrogel contact lenses (Acuvue Oasys, Air Optix and Biofinity) were fit within the same visit. All the lenses were fit according to the manufacturers’ recommendation using the respective fitting guide. Visual performance tests included low and high contrast distance and near visual acuity, contrast sensitivity, range of clear vision and through-focus curve. Objective visual performance tests included measurement of open field accommodative response at different defocus levels and optical aberrations at different viewing distances.ResultsAccommodative response was not significantly different between the three types of multifocal contact lenses at each of the accommodative stimulus levels (p > 0.05). Accommodative lag increased for higher stimulus levels for all 3 types of contact lenses. Ocular aberrations were not significantly different between these 3 contact lens designs at each of the different viewing distances (p > 0.05). In addition, optical aberrations did not significantly differ between different viewing distances for any of these lenses (p > 0.05). ANOVA revealed no significant difference in high and low contrast distance visual acuity as well as near visual acuity and contrast sensitivity function between the 3 multifocal contact lenses and spectacles (p > 0.05).ConclusionsThere was no statistically significant difference in accommodative response, optical aberrations or visual performance between the 3 multifocal contact lenses in early presbyopes.  相似文献

PurposeContact lenses offer a good option for patients with presbyopia, especially with improved optical designs available in modern multifocal contact lenses. Due to the ageing population there is good opportunity to increase contact lens penetration by managing these patients better. However, multifocal contact lenses achieve low penetration in the market.MethodsA questionnaire was administered to people aged above 40 years, to investigate their perceptions of contact lenses for presbyopia. Only people, with presbyopia, who were existing contact lens wearers or willing to try contact lenses were included. Participants were recruited from United Kingdom (UK), United States of America (USA), Netherlands, Germany, France, Spain and Italy.ResultsData from 1540 participants above the age of 40 years was collected, 57.9% were females and 42.1% males. Overall, 50.8% of the participants wore contact lenses, but contact lens wear was less common amongst older participants. Some data supported earlier studies, such as 6.1% wore gas permeable lenses. However, only 25% of the contact lens wearers used multifocal contact lenses. The reasons the participants wanted to wear contact lenses were similar to younger patient such as sports or cosmesis reasons. Reasons why participants had dropped out of contact lenses included discomfort and dry eye related issues. Poor visual performance with contact lenses was a reason to dropout of contact lenses for the older participants.ConclusionsThe study highlights some failings by eye care practitioners in the management of patients with presbyopia. It seems that patients of this age group are seeking suggestions and recommendations from their eye care practitioner including upgrading contact lenses and dual wear options. The day-to-day problems encountered by the contact lens wearers in this study seem to be, in the main, things that could be easily tackled by additional counselling and instruction from the eye care practitioners.  相似文献

PurposeTo investigate the ocular subclinical inflammatory response in two soft contact lens materials when used in combination with two different care systems (‘solutions’).MethodsFifteen established soft contact lens wearers wore each of two lenses: Acuvue® 2 (CH) and Acuvue® Oasys® (SiH) or no lenses in random sequence for one week. Two care systems were used: a peroxide system: Oxysept 1 Step® (P) for the right lens and Opti-Free® Replenish® (MPS) for the left lens. After one week of lens/no lens wear, in vivo confocal microscopy (IVCM) of corneal and conjunctival regions was carried out. Density of presumed dendritic cells (DC) and ‘white spots’ (WS) were quantified. Impression cytology of the bulbar and upper eyelid margin conjunctivae was carried out and samples were analysed for CD45+, CD3+ and CD11c+ cells.ResultsThe SiH + MPS combination showed a greater inflammatory response compared to SiH + P in seven out of 12 parameters investigated (p < 0.05). The remaining five were not statistically different between the two solutions. The converse was found for the CH lens, with four of the 12 parameters showing a greater response with the P solution compared to the MPS (p < 0.05).ConclusionThis study suggests that the SiH lens investigated demonstrated an upregulated response when used with a MPS compared to a P solution whereas the converse is true for the CH lens which showed a reduced response when used with a MPS compared to a P solution. This finding aids in the understanding of the biological underpinnings of manifest corneal inflammatory events during contact lens wear.  相似文献
