In pursuing an "acceptable" moral choice, the process of analysis is further directed by the ethical theories which are molded by time and tempered by the current context to which the action is undertaken. An OPTIMIN MODEL OF ETHICAL-DECISION MAKING PARADIGM is proposed, and health care policy on cultural diversity is adduced.

The results of combined surgical treatment of the occlusive forms of the deep vein thrombosis with the trophic ulcers in the lower extremities

The results of treatment of 156 patients with occlusion forms of the deep veins thrombosis of the lower extremities and trophic ulcers are adduced. The partial or complete thrombectomy the shunting, Linton and Cockett operations, plastic interventions, the pulmonary artery embolism prophylaxis were conducted, and the late terms--the vibro- and hydromassage.     

Functional results of surgical repair of chronic duodenal insufficiency associated with duodenal ulcer

The results of 312 patients examination in late follow-up period after conduction of the organ preserving intervention, gastric resection and reconstructive operation with chronic duodenal insufficiency correction, concurrent with duodenal ulcer, its acute complications, postvagotomy and postresection syndrome are adduced. An excellent and good result was noted in 70.2% of patients, fair--in 23.4%, bad--in 6.4%.     

Endoscopic diagnosis of chronic obstruction of the duodenum in association with peptic ulcer

The results of gut endoscopic examination in 460 patients with duodenal ulcer disease concurrent with its chronic obstruction (CI) are adduced. Direct and indirect endoscopic signs of duodenal CI are revealed in 399 (86.73%) of patients. Endoscopic criteria of the CI stage determination are described: duodenal stenosis, pyloric sphincter deformity and insufficiency, reflux-gastritis and the gastrostasis signs while satisfactory pyloric passability.     

Fictions and Facts About Evolutionary Approaches to Human Behavior: Comment on Lickliter and Honeycutt (2003).

Six misconceptions about evolutionary approaches to human behavior are exposed. Evidence is adduced to support the assertions that evolutionary approaches do not (a) adopt a reductionistic "gene-centered" level of analysis, (b) assume that natural selection is the only process that creates and designs ontogenetic processes and phenotypic outcomes, (c) assume that genes are the only agents responsible for the transgenerational inheritance of phenotypic traits and characteristics, (d) assume that genes are self-contained and impervious to extragenetic influences, (e) posit a strong form of genetic determinism, or (f) pay lip service to the role of the environment. Building straw men and knocking them down is an inherently destructive enterprise; integrating different approaches is a more constructive way of contributing to the growth of knowledge. Cardial vagotomy in the treatment of pyloroduodenal ulcer

The results of infracardial vagotomy conduction in 26 patients with pyloroduodenal zone ulcer are adduced. While vagotomy performance the gastroesophageal zone remains intact, gastric secretion is effectively reduced due to denervation of corpus and fundus containing parietal cells and antrum as a main gastrin source. No delay of gastric motor-evacuating function occur. The authors recommend to conduct the proposed vagotomy procedure in patients with obesity and in case of complications occurring at the time of procedure performance in cardioesophageal region.     

MSMSNET a "hit"

The results of treatment of 47 patients with an acute pancreatitis including 40 with necrotic pancreatitis are adduced. Of 34 (72.3%) patients operated there were 4 with pancreatic gland oedema. Laparoscopic intervention in 24-48 hours after the illness beginning was conducted in 21 (44.7%) patients, in concomitance with peritoneal dialysis--in 11, laparoscopic cholecystectomy--in 6. In 20 patients the abortive course of necrotic pancreatitis and the laparotomy conduction avoidance were achieved. Postoperative lethality was 8.8%.     

Morphohistological aspects of tumors and tumor-like changes in the thyroid gland

The results of the thyroid gland (TG) state investigation in 1027 patients, ageing from 14 to 75 years, operated on in 1986-1995 yrs., are adduced. In 60.5% of patients the diffuse-nodular goitre was diagnosed, in 9.5%--Hashimoto's thyroiditis, in 9.5%--TG cancer, in 2.8%--other pathology. To establish the interrelationship of pre-cancer changes and a TG cancer it is necessary to point out the concomitant morphologic change beside the main diagnosis, which determines the clinical tactics.     

Sex differences in intelligence: a rejoinder to Mackintosh

The author's theory that among adults men have a higher mean IQ than women has been critized by Mackintosh. His criticisms are examined and found wanting. New evidence is adduced that men obtain higher means than women on Raven's Progressive Matrices.     

Varieties of vascular anastomoses in the course of renal allotransplantation

Cytogenetic damage and mutagenesis in mammalian and human cells induced by ionizing radiation with varying LET

Base oneself on analysis of literature and proper data the conclusion was inferred about essential role of structural chromosomal (and gene) damages for radiation-induced mutagenesis of mammalian and human cells (for example HPRT loci). The evidences are adduced of their increase role in mutagenesis after influence of ionizing radiations with high LET. The consequences of HPRT-gene damages have been examined hypothetically.     

Cross-linking of the CAMPATH-1 antigen (CD52) mediates growth inhibition in human B- and T-lymphoma cell lines, and subsequent emergence of CD52-deficient cells

Emotional hyperthermia, circadian variations and the rise of body temperature related to exercise, have all been attributed to setpoint temperature shifts. The accepted theory holds that core temperature is regulated by corrective thermoregulatory responses opposing the core temperature deviations from the setpoint level. However, in fever and anapyrexia the thermoregulatory responses appear to be not corrective but helping, that is in the same direction as the core temperature deviation. A supplementary ad hoc hypothesis that setpoint level shifts explains why the thermoregulatory responses still could be considered "corrective" in spite of being in the same direction as the core temperature deviation. But supplementary ad hoc hypotheses immunize a theory to experimental challenges and therefore can no longer be considered a scientific theory. The present work shows that most of the arguments adduced to explain almost every biothermal phenomenon as being due to setpoint shifts cannot withstand a critical analysis.     

Diagnosis of cancer of the pancreatic body and tail

Results of 194 patients observation with cancer of corpus et cauda pancreatis are adduced. Clinical forms of the disease are distinguished. Rational algorithm for the examination of patients with probable cancer of corpus et cauda pancreatis diagnosis was elaborated grounded on the efficacy analysis of various methods of examination (ultrasonic investigation, computed tomography, endoscopic retrograde cholangiopancreatography, selective, angiography and so on). The algorithm application permitted to improve significantly the diagnosis of disease.     

Surgical interventions in acute pancreatitis

Results of treatment of 240 patients with an acute pancreatitis are adduced. Of them 42 (17,5%) were operated on. In 6 patients an acute edema of pancreas was revealed (one patient have died), in 36--destructive forms of an acute pancreatitis, 8 (22%) died. Results of treatment of patients, operated on in one week after disease beginning, were the best.     

Characteristics of a neural clock regulating perception and psychomotor performance in man

A biological clock regulating some psychomotor aspects of perception has been adduced from the results of the repetitive administration of a maximal speed tapping test of 15 sec per hand per min for a total of 20 min. 412 longitudinal timed tests from 8 healthy control subjects, 4 physically healthy and 6 brain damaged psychiatric patients formed the basis for the investigation of the properties of this clock. It was found that this perceptual clock is endogenous, self-sustaining and innate. Its free-running oscillations are independent of psychic state, emotions and affect. They are, however, dependent upon the subject's neural integrity and their phase may be influenced by an orientating reflex. Timing is precise but the preceding phase may influence a succeeding one. Despite the muscular activity of the task the clock is relatively independent of body temperature. It is also immune to most psychoactive drugs.     

Powder Metallurgy Overcomes Economic Difficulties

The state of the production of metallic and ceramic powders (including nano-dimensional powders) and of articles fabricated from these powders in North America is discussed. New technologies that produce an increase in the density and improvement in the mechanical properties of powder-based articles are considered.
