Play therapy is a well-established therapeutic approach for working with children and families in the West. This article describes the current situation of play therapy in Hong Kong—a cosmopolitan city where East meets West. The strengths, weaknesses, opportunities, and threats confronting the development of play therapy as an extension of core graduate-level mental health training are analyzed. Through this process, a number of strategic issues central to the growth and development of professionalism in play therapy are identified and discussed. A path for the future development of play therapy for mental health professionals in Hong Kong over the next decade is presented. Stoiber's vision and other critical issues for the evidence-based intervention (EBI) movement in school psychology. Trivalent lanthanide ions display fascinating optical properties. The discovery of the corresponding elements and their first industrial uses were intimately linked to their optical properties. This relationship has been kept alive until today when many high-technology applications of lanthanide-containing materials such as energy-saving lighting devices, displays, optical fibers and amplifiers, lasers, responsive luminescent stains for biomedical analyses and in cellulo sensing and imaging, heavily rely on the brilliant and pure-color emission of lanthanide ions. In this review we first outlined the basics of lanthanide luminescence with emphasis on f-f transitions, the sensitization mechanisms, and the assessment of the luminescence efficiency of lanthanide-containing emissive molecular edifices. Emphasis was then put on two fast developing aspects of lanthanide luminescence: materials for telecommunications and light emitting diodes, and biomedical imaging and sensing. Recent advances in NIR-emitting materials for plastic amplifiers and waveguides were described, together with the main solutions brought by researchers to minimize non-radiative deactivation of excited states. The demonstration in 1999 that erbium tris(8-hydroxyquinolinate) displayed a bright green emission suitable for organic light emitting diodes (OLEDs) was followed by realizing that in OLEDs, 25% of the excitation energy leads to singlet states and 75% to triplet states. Since lanthanide ions are good triplet quenchers, they now also play a key role in the development of these lighting devices. Luminescence analyses of biological molecules are among the most sensitive analytical techniques known. The long lifetime of the lanthanide excited states allows time-resolved spectroscopy to be used, suppressing the sample autofluorescence and reaching very low detection limits. Not only visible lanthanide sensors are now ubiquitously provided in medical diagnosis and in cell imaging, but the feasibility of using NIR emission of ions such as YbⅢ is now being tested because of deeper penetration in biological tissues.  相似文献

Chafetz, Ochs, Tate, and Niederehe (November 1982) have produced an interesting review of employment opportunities for geropsychologists. Although their report explores current employment status and the possibilities of an expanding market, they only briefly state the policy issues that act against the growth of psychology in the field of aging services. It is not surprising that they have found the largest interest for geropsychologists in facilities dedicated to research and assessment--facilities that do not depend directly on third-party reimbursement. Reviews research contributions that have been beneficial to disabled citizens including work on the helping relationship, contingencies of reinforcement, vocational rehabilitation, assessment scales, and family adjustment. Support by the federal government for future research will be guided by goal statements that address the refinement of assessment instruments, the psychological effects of childhood disability, the effects of aging on the physically disabled, the psychosocial impact of disabilities in minority groups, and improved service delivery strategies. The hospital financial records of 120 consecutive patients who underwent unilateral knee replacement surgery at one hospital during 1995 were reviewed to determine opportunities for control of hospital cost for total knee arthroplasty. The average hospital length of stay for these patients was 4.27 days (range, 3-10 days). The average hospital cost was $10,231. All 120 patients were classified under Diagnosis Related Group 209, principle procedure 81.54 primary total knee arthroplasty. Medicare paid for 70% of the patients. All payers were profitable except Medicaid and one managed care organization. When hospital cost for total knee arthroplasty was allocated to hospital service centers, 78% of the cost was attributed to the operating room, nursing units, recovery room, and pharmacy. When hospital cost for total knee arthroplasty was allocated to hospital days, 80% of the hospital cost occurred during the first 48 hours of hospitalization. Hospital reimbursement for total knee arthroplasty is primarily a prospective case price payment system. After initial cost containment efforts reduce the hospital length of stay for total knee arthroplasty to 4 to 6 days, additional control of hospital cost should focus on these areas of opportunity.  相似文献

Metallic and operating cost as well as specific energy consumption are the key to long term viability of steel plants. Usha Martin achieved the highest directly reduced iron (DRI) rotary kiln campaign life which is a good case study for Indian steel makers who consume large quantities of DRI produced through the rotary kiln route. The factors affecting rotary kiln life and how the kiln life was extended at M/s Usha Martin have been elaborated in this study. Energy optimizing furnace (EOF) has good potential for steelmaking in India and it is technically and commercially viable with indigenously available raw materials. The advantage of EOF in the Indian context especially for mini steel plants not having captive mines is highlighted.  相似文献

As assisted reproduction technology advances, more types of procedures are becoming available, bringing more success at solving many types of infertility. In vitro fertilization has become simpler and less invasive, with success rates as high as 30% per cycle. Intracytoplasmic sperm injection has solved many types of male infertility. This paper follows “Construction Engineering: Reinvigorating the Discipline.” That paper argued that a new flow-based operating system was poised to replace the activity-centered operating system of traditional project management and that a coherent organizational and contractual approach would evolve. Shifting to a new operating system based on the theory, principles, and practices of production system design will reduce waste, spur learning and innovation, and deliver more value to clients and other participants. After a reflection on a model for making sense, the paper explores the consequences of this shift from a number of perspectives and speculates on the impact on practice and research. OBJECTIVE: To study the effect of cisplatin on plasma concentrations and urinary excretion of carnitine in ten patients with different malignancies treated with chemotherapy. METHODS: Carnitine concentrations were determined using a radioenzymatic assay and other metabolites by routine methods of clinical chemistry. Renal clearances were calculated by dividing urinary excretions by the respective plasma concentrations. RESULTS: Before treatment, all patients had a normal plasma carnitine concentration. During treatment with cisplatin, the plasma total carnitine concentration increased by approximately 30% and normalized 7 days after stopping therapy. Urinary excretion of total carnitine increased by a factor of 10 during cisplatin administration and also normalized 7 days after cessation of chemotherapy. This increase was due to excretion of both free carnitine and acylcarnitine and averaged approximately 1 mmol carnitine per day. Similarly, urinary clearance of total carnitine was increased during therapy with cisplatin by a factor of approximately 8 and returned to normal 7 days after chemotherapy. In comparison, patients with similar malignancies treated with radiotherapy showed no significant increase in renal carnitine excretion. Similar to urinary excretion of carnitine, excretion of glucose and phosphate, two metabolites also reabsorbed by the proximal tubule of the nephron, was increased during therapy with cisplatin. There was a strong linear correlation between urinary excretion of free carnitine and acylcarnitines. CONCLUSIONS: Treatment with cisplatin is associated with a tenfold increase in renal carnitine excretion, most likely due to inhibition of carnitine reabsorption by the proximal tubule of the nephron. Well-nourished patients support this loss of carnitine even after repeated cycles of chemotherapy without developing hypocarnitinaemia. However, cachectic patients with decreased dietary carnitine uptake may develop carnitine deficiency when treated repeatedly with chemotherapies including cisplatin.  相似文献
