For nearly 30 years, psychologists have relied almost exclusively on child abuse and neglect reporting statutes to understand the legal duty to report child maltreatment. Although these statutes go far in delineating the contours of reporting requirements, mandated reporters may further their understandings of their duties through knowledge of judicial interpretation of these statutes. Accordingly, this article reviews the liability provisions found in the 50 states' child abuse and neglect reporting statutes and summarizes relevant court findings. Courts seem motivated by 2 key considerations, namely, that statutes should be interpreted broadly and that states' interests in such reporting is compelling. These opinions offer practical guidance as well as information relevant to the debate on how these reporting statutes should be reformed. The experience of being male or female is constructed and performed through daily interactions within the greater social context. Therapy contributes to the construction of gender whether consciously addressed or not. This article begins to make visible the potential influence of play therapy on gender by exploring how Child Parent Relationship Therapy (CPRT) changes this dimension. Gender is conceptualized as a relational process affected by socialization practices and the dominant discourse. Attending to gender in CPRT is consistent with the purposes for which the model was developed and involves heightened practioner awareness of this dimension in therapy and how it is influenced by the model. Children exhibiting aggressive problem behaviors typically reach a peak of aggressive acts at an early age, providing evidence that early intervention for aggression is needed. Child-centered play therapy (CCPT) is 1 intervention that offers the child an environment in which aggression can be expressed and empathically responded to by a play therapist. Forty-one elementary school age children were assigned to a CCPT condition or a wait-list control group condition. Children who participated in 14 sessions of CCPT showed a moderate decrease in aggressive behaviors over children in the control group, according to effect sizes as reported by parents (N = 32). Teachers reported that both groups significantly improved over time (N = 41). Post hoc analysis revealed that children assigned to CCPT decreased aggressive behaviors statistically significantly and children assigned to control group demonstrated no statistically significant difference. Although the U.S. child-centered approaches of Garry Landreth and Louise Guerney have the same principles and practice skills as nondirective play therapy as practiced by therapists trained at the University of York, there are a few differences in their approach. Therapists’ practice of “congruence” is actively encouraged for York-trained therapists but not for Landreth and Guerney-trained therapists. The theoretical and practice rationales for expressing congruence that underlie the York approach are examined here, as well as potential pitfalls. Examples of therapists and parents verbally expressing their congruent feelings in therapy are given from both play therapy and filial therapy practice. Objective: This article examined links between childhood maltreatment and risky sexual behavior (early sexual contact, promiscuity, prostitution) and HIV in adulthood. Design: Using a prospective cohort design, physically and sexually abused and neglected children (ages 0-11) with documented cases during 1967-1971 were matched with nonmaltreated children and followed into adulthood. Main Outcome Measures: Early sexual contact, promiscuity, and prostitution were assessed through in-person interviews and official records (prostitution) at approximate age 29 (N = 1196). HIV tests were conducted at approximate age 41 (N = 631). Results: Child maltreatment was associated with prostitution (OR = 2.47, 95% CI = 1.35-4.50) and early sexual contact (OR = 1.73, 95% CI = 1.24-2.40). Prevalence of HIV in the abuse/neglect group was twice that in controls (OR = 2.35, 95% CI = .64-8.62), although this difference did not reach conventional levels of statistical significance. SEM provided significant support for a model linking child abuse and neglect to prostitution through early sexual contact and a marginal link to HIV through prostitution. Conclusion: These findings provide prospective evidence that maltreated children are more likely to report sexual contact before age 15, engage in prostitution by young adulthood, and test positive for HIV in middle adulthood. Mandated child abuse reporting laws have been in effect in the United States for more than 20 years, yet practitioners vary in their understanding and opinions of these laws. A sample of licensed psychologists were surveyed and reported being relatively well-informed. However, their performance on a knowledge measure suggests knowledge deficits and a tendency to overreport. Legal considerations encourage reporting, whereas beliefs that one can provide client treatment deter reporting. Opinions of the laws were generally favorable, with some concerns about child protection systems and the impact of reporting on the therapeutic alliance. Filial therapy involves training parents, in groups of 6 to 8, to conduct play sessions with their emotionally disturbed young children, using an orientation and methodology modeled after client-centered play therapy. After training, the parents conduct their play sessions at home while continuing their weekly group meetings. Parents' sessions with their therapist begin with discussion of the play sessions, but may extend to any other areas that are emotionally relevant. Preliminary experience with 2 groups suggests that this type of method is deserving of further exploration as a method of increasing leverage of professional resources, and as a tool for gaining further insight into children's fantasy and parent-child relationships. The primary purpose of this study was to conduct a randomized effectiveness trial of Multisystemic Therapy for Child Abuse and Neglect (MST-CAN) for physically abused youth (mean age = 13.88 years, 55.8% female, 68.6% Black) and their families. Eighty-six families being followed by Child Protective Services due to physical abuse were randomly assigned to MST-CAN or Enhanced Outpatient Treatment (EOT), with both interventions delivered by therapists employed at a community mental health center. Across five assessments extending 16 months post baseline, intent-to-treat analyses showed that MST-CAN was significantly more effective than EOT in reducing youth mental health symptoms, parent psychiatric distress, parenting behaviors associated with maltreatment, youth out-of-home placements, and changes in youth placement. Also, MST-CAN was significantly more effective at improving natural social support for parents. Effect sizes were in the medium to large range for most outcomes examined. Although fewer children in the MST-CAN condition experienced an incident of reabuse than did counterparts in the EOT condition, base rates were low and this difference was not statistically significant. The findings of this study demonstrate the potential for broad-based treatments of child physical abuse to be effectively transported and implemented in community treatment settings. This article describes an in-depth case study analysis of the process of working therapeutically with a family, who adopted a young male after several child maltreatment experiences, using filial family play therapy. Psychological and behavioral assessments conducted before and after the play therapy process illustrates a varying picture of results in the scores of both parent and child functioning. In-depth narratives collected from the adoptive parents a year later reflect the positive influence the play therapy process had on long-term family dynamics. Child-centered play therapy describes the therapeutic relationship between the child and play therapist as a facilitative environment that provides the child with an experience of congruence within their sense of self, promoting self-directed healing. Within this environment, the child is able to confront emotional pain imbedded in lived, relational experiences, processing and gaining mastery over it through repetitive symbolic play. However, very little research exists regarding repetitive symbolic play as a therapeutic process. This article aims to illustrate, through the use of a qualitative, interpretative case study and hermeneutic enquiry, how a 6-year-old boy's use of repetitive symbolic play assisted him toward healthier adjustment in his home and school environments. Hermeneutic analysis suggests that the child's use of repetitive symbolic play became a therapeutic process in and of itself that contributed toward self-directed change within his sense of self, which promoted healthier adjustment within his environment. Differences in the training of nondirective play therapists between University of York (U.K.) approach and the Guerney and Landreth approaches (U.S.) has been raised by Ryan and Courtney (2009). York-trained therapists are encouraged to initiate expression of their own feelings at selective times to assure that therapists are congruent in their relationship with children in nondirective play-therapy. Congruence, an important component of the necessary and sufficient conditions for therapeutic personality change (Rogers, 1957), is based on the importance of therapist genuineness to maintain a trustful and safe therapeutic relationship. This article compliments Ryan and Courtney (2009) for introducing the importance of congruence in play therapy. The assumptions of nondirective play therapy in the Guerney approach was influenced significantly by their development of filial therapy and to the training of parents and therapists, This article asserts that congruence, in the Guerney approach, is incorporated by therapists expressing their own feelings only when this is initiated overtly or covertly by the child. Responding to a child in this way is dependent on the empathic attunement to a child's emotional motivation by a skillful parent or therapist. Child sexual abuse (CSA) is a pervasive, traumatic event (A. H. Heflin & E. Deblinger, 2007) affecting hundreds of thousands of ethnically and socioeconomically diverse children and families across the United States (F. W. Putnam, 2003). E. Gil (2006) and J. S. Shelby and E. D. Felix (2006) have noted that integrative therapies--those that combine directive and nondirective strategies--possess the capacity to benefit a child traumatized by sexual assault. Jungian analytical play therapy (JAPT) is a creative, integrative therapy that may be beneficial when applied to children affected by CSA (J. Allan, 1988). Within the safety of a nonjudgmental, therapeutic relationship, children affected by CSA may become consciously aware of and subsequently resolve conflicting emotions associated with sexual assault in symbolic, less-threatening ways. Through participation in JAPT, the child's psyche may begin the therapeutic process of integrating inner and outer emotional polarities in an archetypal quest for self-healing after sexual trauma (E. This article presents an advanced filial therapy model. This model was developed and implemented by the author in an effort to provide parents with ongoing support and skill development after the completion of a standardized filial model called Child–Parent Relationship Therapy. The advanced filial therapy model is described, including an exploration of how it was developed and how it compares with basic filial therapy models. Examples from the author's clinical experience are provided to illustrate various components of this approach. Finally, implications for research and practice are considered. This study was designed to determine the effectiveness of an intensive child parent relationship therapy (CPRT)—filial therapy training (G. L. Landreth & S. Bratton, 2006)—as a method of intervention for Israeli parents in Israel, families who are experiencing prolonged stress and trauma since Israel's creation. Results of the multivariate analysis of covariance revealed that the Israeli parents in the experimental group (14 parents) significantly increased their level of empathic interactions with their children and their attitude of acceptance toward their children and significantly reduced their level of stress related to parenting as compared with parents in the nontreatment comparison group (13 parents). Objective: This study examines the relationship between childhood abuse and neglect and sexual risk behavior in middle adulthood and whether psychosocial factors (risky romantic relationships, affective symptoms, drug and alcohol use, and delinquent and criminal behavior) mediate this relationship. Method: Children with documented cases of physical abuse, sexual abuse, and neglect (ages 0–11) processed during 1967–1971 were matched with nonmaltreated children and followed into middle adulthood (approximate age 41). Mediators were assessed in young adulthood (approximate age 29) through in-person interviews between 1989 and 1995 and official arrest records through 1994 (N = 1,196). Past year HIV-risk sexual behavior was assessed via self-reports during 2003–2004 (N = 800). Logistic regression was used to examine differences in sexual risk behavior between the abuse and neglect and control groups, and latent variable structural equation modeling was used to test mediator models. Results: Child abuse and neglect was associated with increased likelihood of risky sexual behavior in middle adulthood, odds ratio = 2.84, 95% CI [1.74, 4.64], p ≤ .001, and this relationship was mediated by risky romantic relationships in young adulthood. Conclusions: Results of this study draw attention to the potential long-term consequences of child abuse and neglect for physical health, in particular sexual risk, and point to romantic relationships as an important focus of intervention and prevention efforts. The researchers surveyed 295 members of the Association for Play Therapy on their attitudes related to working with families when treating children. The results indicated the majority of play therapists held attitudes conducive to involving families in their approaches with children. However, mixed findings were found in specific areas related to the implementation of play therapy with families, suggesting barriers may exist. These mixed findings included a decreased percentage of play therapists that felt like play therapy was effective in family therapy when compared with the high level who felt that play and family therapy approaches could be integrated. Moreover, the respondents were divided on issues such as parents' willingness to be involved in therapy with their children and if parents were actually resistant to being included in sessions with their children. These mixed findings suggest that a wide range of attitudes and experiences about parental involvement exist among play therapists in the field. The authors raise key questions for the play therapy field to consider in more depth and suggest improvements that may be needed in play therapy education to increase the efficacy of play therapists' skills in working with families. This article presents case studies exploring two boys' uses of limit testing as a therapeutic tool in child-centered play therapy (CCPT). Both boys were referred for behavior that was abnormally disruptive for their age and setting, which was kindergarten in an elementary school that serves a very high poverty community and is accustomed to handling misbehavior. Data evidencing progress is provided as a reference point, while analysis is focused on conceptualization and mechanisms of change related to each child's use of limit testing in CCPT. The authors suggest that each boy used limit testing in CCPT to try out the therapeutic relationship as a stand-in for other relationships, to rethink relational expectations inside and outside of therapy, revisit unmet needs and meanings of early experiences, and change self-concept in ways that affected positive behavioral change. The rationale for utilizing filial therapy as a treatment intervention with child witnesses of domestic violence while residing with their mothers in a shelter facility is explored and the effectiveness of an intensive 12-session filial therapy parent training group, conducted within 2-3 weeks, is described (n=11; aged 4-10 yrs). Results of analysis of covariance revealed that child witnesses in the experimental group significantly reduced behavior problems prevalent in child witnesses and significantly increased their self-concept as compared to child witnesses in the non-treatment comparison group. In addition, t-test results showed that mothers who facilitated treatment of the experimental group scored significantly higher after training on both their attitudes of acceptance and their empathic behavior. Comparative analysis revealed that intensive filial therapy as facilitated by the children's mothers was as effective in reducing behavior problems as was intensive individual play therapy and intensive sibling group play therapy as facilitated by professionally trained therapists. Reviews the book, Child sexual abuse: Critical perspectives on prevention, intervention, and treatment edited by Christopher R. Bagley and Ray J. Thomlison (1991). This book is a compilation of a series of literature reviews originally commissioned by Health and Welfare Canada in 1987 and completed in 1988. The individual papers included in this edited version represent a cross section of Canadian academicians, clinicians, and case workers who are integrally involved in the policies and practices regarding child sexual abuse in Canada. The work provides a comprehensive perspective on prevention at the primary, secondary, and tertiary levels. The list of authors is impressive in terms of their expertise and experience. At a time when numerous books on child sexual abuse are appearing on the market, it is nice to see a book that has some unusual aspects. Aside from its distinctly Canadian perspective, the book addresses several important, yet frequently ignored, topics. The book provides an up-to-date review of several core issues: conceptualization of the problem, prevention strategies, impact of sexual abuse, investigative interviewing, treatment outcome studies, and treatment issues for child molesters. The more unique topics include a review of the strategies used to evaluate prevention programs, prevalence rates among a number of special populations, the role of medical practitioners in preventing and intervening in child sexual abuse, and sexual abuse and exploitation among disabled individuals. Overall, I can recommend this book for clinicians and researchers in Canada and elsewhere who are interested in child sexual abuse. This compilation of literature reviews highlights the leading role that Canadian social service agencies have taken in developing programs for sexually abused children and their families.
