Methadone-related overdose deaths in South Australia, 1984-1994. How safe is methadone prescribing?

We report a patient in whom 11 years after the treatment of a breast tumour, bilateral ovarian metastases were disclosed. Surgical exploration confirmed unique ovarian metastases within a 'clean' abdomen and pelvis. The particularities of this case compared with the literature are the rather long interval of 11 years and the fact that no other metastases were present. The literature indeed reveals that most of the patient with ovarian metastases are premenopausal, have bilateral involvement; the ovarian metastases occur within a few years and are frequently accompanied by other metastases.     

Assessing students' plans for college.

Based on a contemporary cognitive-psychoanalytic theory (control-mastery theory; J. Weiss, H. Sampson, & Mount Zion Psychotherapy Research Group, 1986), this study proposed that college students have plans for college that consist of conscious and unconscious goals and obstructions to be overcome in meeting those goals. The construct of unconscious guilt was used to mediate the dual goals of autonomy and attachment. The idiographic plan-formulation method was adapted to derive plans for 12 sophomore women. Acceptable interjudge reliabilities and criterion validity were demonstrated, and several themes in the goals and obstructions were illustrated. Students worried more about mothers than about fathers, particularly when they perceived the mother as weak or needy. Midwifery education for safe motherhood

A series of new safe motherhood midwifery education modules was evaluated in nursing and midwifery education institutions, regional training centers, acute care hospitals, and community settings in Ethiopia, Fiji, Lesotho, Mozambique, and Nepal in 1995. The series was developed by the World Health Organization's Maternal Health and Safe Motherhood Program. A total of 36 teachers, 82 midwives or nurse-midwives, and 60 post-basic midwifery students were enrolled in a 2-week clinical skills course and an 8-day training in module use. In subsequent questionnaires and focus group discussions, participants indicated the modules were understandable, relevant, easy to use, and of high quality and the guidelines for assessing competence were adequate. Difficulties encountered included insufficient recommended time frames for some of the sessions, a limited availability of clinical cases for teaching the specific skills in the modules, difficulties obtaining data for a community profile, and a lack of resources to support application of skills learned. Participants indicated they would benefit from having copies of the technical material used in the modules for reference after the course. Overall, these findings indicate the modules have the potential to strengthen the education of midwives in developing countries and thereby to make motherhood safer. Weak health system infrastructures, including regulatory measures, represent the major obstacle to successful program application.

New drug lodoxamine (alomide) opens new vistas in the treatment of allergic diseases of the eyes highly prevalent both in adults and children. This drug prevents release of mast cell mediators and delays eosinophil migration to conjunctival and corneal tissue, thus exerting a spectrum of antiallergic effects. Clinical studies carried out in 170 patients demonstrated a high efficacy of alomide in the treatment of subacute and chronic pollenosis conjunctivitis, spring keratoconjunctivitis, multiple and toxic allergic keratitis, and other allergic conjunctivities. Alomide can be used as a preventive drug in patients with allergies under high-risk conditions and in patients wearing contact lenses. It is effective in combined therapy of keratitis and keratouveitis. Alomide eye drops are well tolerated.     

Nurse prescribing in diabetes

The adjustment of diabetes medication is vital for the prevention of complications. Insulin dose adjustment by diabetes specialist nurses is common practice but its legality is questionable. Two surveys were undertaken to identify common practice. The results were used to establish the basis for an effective multiprofessional protocol for nurse administration of diabetes medication.
