In this study a comparison is made between intraoral and extraoral radiographical techniques for quantitative measurements of bone density in the edentulous mandible. A phantom model, composed of a dry mandible surrounded by a soft tissue substitute was used. Radiographs were made of aluminium implants of known volumes. Intra- and extraorally placed reference wedges of aluminium or a combination of aluminium and plexiglas were used for the measurement of the aluminium volume of the above-mentioned implants and compared with the real volume. The IBAS image analysis system was used for image processing and measurements. A systematic deficit in the measured volume of 15 to 29% was found. This error was irrespective of the radiographical technique and indicates a constant underestimation of the aluminium volumes. The differential influence of the secondary radiation on the images of the jaw and the wedge is proposed to be a possible cause of this constant underestimation. The intraoral and extraoral techniques do not display significant statistical differences in the levels of validity or accuracy. Obtaining serial intraoral radiographs of adequate quality in the atrophic edentulous mandible can be problematic. It is concluded from this in vitro study that in these cases Oblique Lateral Cephalometric Radiographs may provide a valuable alternative for quantitative image analysis.  相似文献

Traditionally, the procedure of implant placement requires a surgical periosteal flap to be raised. In a percentage of implant cases, there is no need for flap surgery for implant placement, or for a follow-up surgical procedure for abutment connection. In this clinical investigation, 20 maxillary and mandibular implants were placed in seven adult male patients. The sites for implant placement were prepared according to an alternative surgical technique without raising a surgical flap. Patients were recalled periodically for 2 years to evaluate healing and clinical integration of implants. The results showed normal clinical healing at the first week of reexamination in all implant sites; periodontal probing of less than 2 mm circumferentially around all healing caps at 3 months and later at subsequent recall periods; no radiolucency observed in the peri-implant zone; no sign of clinical mobility during recall examination; and no persistent or irreversible sign or symptoms of pain, infection, or necrosis. This alternative surgical technique can provide several advantages over the traditional 2-step procedure.  相似文献

PURPOSE: To compare success rates for dental implants placed from 1985 through 1988 and from 1989 through 1991, and to investigate the factors associated with success or failure. PATIENTS AND METHODS: All hydroxylapatite-coated cylindrical implants placed from 1985 through 1991 were followed yearly. Lifetable survival analyses compared implant success for a "developmental period" from 1985 through 1988 (4 to 8 years follow-up) and a "recent period" from 1989 through 1991 (1 to 4 years follow-up). Reasons for success or failure, time from implant placement to removal related to failure reason, outcome after implant removal, and a morbidity analysis are included. RESULTS: The 7 to 8-year cumulative success rate for all implants placed in the developmental period (maxilla and mandible combined) was 86.5%; it was 84.2% for all maxillary implants and 87.5% for all mandibular implants. The cumulative success rate for all implants placed in the recent period was 97.5%; it was 97.5% for all maxillary implants and 97.6% for all mandibular implants. The difference between the two periods was statistically significant only for the anterior maxilla. Regression analysis on the interval success rates indicates that interval failure did not follow a linear relationship with time. The most common reasons associated with failure were lack of keratinized gingiva, poor oral hygiene, mechanical overload, and malposition. CONCLUSION: Comparison with previously reported cumulative success rates indicated learning curve experiences comparable with other implant systems. Improvements in hardware, surgical and prosthetic techniques, and patient selection have led to an improvement in success rates with the recent period implants.  相似文献

The purpose of this prospective study was to investigate the clinical outcome and marginal bone resorption of three different endosseous implants placed in the anterior mandibles of 15 elderly patients. Eleven women and 4 men (ranging from 65 to 80 years, mean 71 years) had three different endosseous implants placed in the anterior mandible; one titanium plasma-sprayed cylinder implant (4-mm diameter), one titanium cylinder implant with hydroxyapatite coating (4-mm diameter), and one standard threaded titanium implant (3.75-mm diameter). Three months later, at the second-stage surgical procedure, ball abutments were connected and an overdenture was placed. At 12, 24, and 36 months, marginal bone resorption and Periotest values were recorded. None of the implants was lost in this period. An analysis of variance with repeated measurement was performed annually to test the existence of significant differences between the implants. When differences appeared, paired t tests were used to identify which differences were significant. Bonferroni multipliers were used to adjust for multiple testing. When marginal bone resorption was concerned, threaded titanium and hydroxyapatite-coated implants had significantly better scores than titanium plasma-sprayed cylinder implants. Periotest values for hydroxyapatite-coated implants were significantly better than test values for the other implants after 2 years. After 3 years significance was obtained between hydroxyapatite and screw-shaped implants only (P < .05). It was concluded that titanium plasma-sprayed cylinder implants have a less favorable prognosis than the other implants used in this study.  相似文献

Full oral rehabilitation with a high degree of success is now possible with osseointegrated implants. Osseointegration is a direct connection between living bone and the titanium implant at the level of the light microscope. Osseointegrated implants are currently used to replace single teeth, support fixed bridges and stabilize full dentures. These implants can also be placed extraorally for attachment of facial prosthesis. The surgical technique used to place implants intraorally into jaws or facial skeleton is performed in two stages using a local anesthetic and/or conscious sedation. During stage I surgery, holes are placed into the jaw using a series of gradually larger diameter burs until the desired diameter and depth of the bony preparation is achieved. The implant is then placed. The implant must remain undisturbed for 4 months for osseointegration to take place. Stage II surgery is then required to remove the mucosa over the implant and place the transmucosal abutment. After 1-2 weeks of healing, the restorative dentist can take an impression and fabricate the prosthesis. On occasion, it is necessary to augment the height and width of the atrophic jaw with autogenous or allogeneic bone grafts prior to implant placement. Bone grafts are sometimes placed on the floor of the nose or the floor of the maxillary sinus. Guided tissue regeneration is a technique used to generate bone within bony defects adjacent to implants. With long-term rates of success (5 years) of 99% for implants placed in the mandible and 95% for those placed in the maxilla, reconstruction of the jaws and cranial facial skeleton with osseointegrated implants has become the treatment of choice.  相似文献

PURPOSE: This prospective study was designed to evaluate the long-term clinical and radiographic results of porous block hydroxyapatite (PBHA) used as a synthetic bone graft in orthognathic surgery and craniofacial augmentation. PATIENTS AND MATERIALS: A total of 245 consecutive patients were treated initially. Inclusion criteria for this study included a minimum clinical and radiographic follow-up of 5 years. In addition, all patients with known implant failures were included regardless of whether they met the study criteria. There were 111 patients that met the criteria for inclusion in this study. All patients had undergone orthognathic surgery with rigid fixation and had had inlay or onlay PBHA implants placed. Ninety-six percent of the implants were placed through an intraoral approach. Long-term postoperative radiographs were visually compared with immediate postoperative radiographs for implant position, stability, resorption, and other significant radiographic changes. The clinical examination evaluated for signs and symptoms of infection, wound dehiscence, implant exposure, implant displacement, changes in the overlying mucosa, and development of oronasal or oroantral fistulae. RESULTS: Four hundred seventy-one implants were placed: 403 in the maxilla, 44 in the mandible, and 24 in the periorbital region. There were 289 implants placed in direct communication with the maxillary sinus. The average follow-up time was 7.2 years (range, 5.0 to 10.3 years). Twenty-three implants (4.9%) were removed during the evaluation period. Lateral maxillary wall grafting had 95.7% success, with nine implants being lost in three patients. One chin implant was removed because of dissatisfaction with the aesthetics. Seven (14%) midpalatal implants used for maxillary expansion were lost, primarily because of exposure of the implant to the oral or nasal cavity at the time of surgery. When PBHA was used for alveolar cleft grafting, there was a 100% failure rate. CONCLUSION: The use of PBHA as a bone graft substitute in orthognathic surgery and for facial augmentation showed a high percentage of success and efficacy. However, adequate soft tissue coverage in the nasal floor and on the palate are paramount for success of midpalatal implants. PBHA should not be used for alveolar cleft grafting. Rigid fixation for inlay implants in the maxilla is important to provide stress shielding of the material and minimize micromovement during the initial healing phase.  相似文献

The surgical repositioning of the inferior alveolar nerve facilitates the placement of implants in the posterior mandible. The entire height of the posterior mandible is made available and enables implants to be anchored in two cortical plates, improving the primary fixation and increasing the area of the bone-implant interface. The posterior mandible can thus be predictably treated with implants in those cases where posterior support is considered important for occlusal stability and masticatory efficiency. The rapid and total recovery of sensation to the lip following nerve repositioning renders the procedure more acceptable to both the patient and the clinician.  相似文献

Between 1990 and 1995, 214 implants were placed in 29 maxillae and mandibles of 22 patients following extraction of all residual teeth as a consequence of severe periodontal disease. All patients were discharged wearing immediate dentures. The implants were analyzed with regard to the number per arch, location, length, and diameter. The 5-year cumulative survival rate was 98.5%. The mean number of implants per arch was 7.5 for the maxilla and 7.2 for the mandible. The preferred implant locations were canines, central incisors, lateral incisors, and second premolars in the maxilla; and lateral incisors, first molars, and canines in the mandible. The mean implant length was 14.7 mm in the mandible and 14.5 mm in the maxilla. The mean implant diameter was 3.8 mm in the maxilla and 3.8 mm in the mandible. The results of the present study indicate that immediate implantation for fixed full-arch reconstruction can be considered a viable treatment alternative in patients with severe periodontal disease.  相似文献

We performed a retrospective review of Sutter silicone metacarpophalangeal (MP) joint arthroplasties in 34 patients (42 hands, 168 implants) with rheumatoid arthritis. Patients were evaluated at an average of 27 months (minimum follow-up period, 12 months). Twenty percent of the implants were shown to be definitely fractured on final follow-up examination, and 45% followed for more than 3 years were definitely fractured. At the final follow-up examination, the average ulnar drift in intact implants was 11 degrees and in the fractured implants, 23 degrees. However, there was no correlation between implant fracture and patient satisfaction. Eighty percent of patients said they would undergo the procedure again. Because of a significantly higher implant fracture incidence at a relatively shorter follow-up period than that of most studies of silicone MP implants of the Swanson design, we have abandoned the use of the Sutter implant.  相似文献

A preliminary report presenting the results of fibular strut grafting in the severely resorbed mandibular and maxillary region is presented. Thirteen patients were treated due to severe resorption of alveolar and basilar bone of 49 segments of the mandible and the maxilla. Two patients additionally had pathological fractures of the mandible. In 10 cases the strut graft was harvested by means of a new minimally invasive technique. After modelling the fibular bone it was fixed to the recipient site by miniscrews or implants. After a mean follow-up period of 20 months (max. 31, min. 11 months) a retrospective analysis of clinical and radiological findings was carried out. It showed that a mean augmentation of 16 mm was achieved. Compared to other studies the fibular strut graft was resorbed less, and due to the primary stability it could be used for the treatment of fractures of the mandible. No more than natural resorption was observed when the patients received their prostheses fixed to dental implants.  相似文献

Insulinoma in a patient with pre-existing diabetes is exceedingly rare. Only a small number of well-documented cases have been reported in the world during the last 40 years. We describe a case with non-insulin-dependent diabetes mellitus who after seven years of sulfonylurea treatment experienced recurrent episodes of hypoglycemia. Endogenous hyperinsulinism was found and radiographical examination and transhepatic venous sampling confirmed an insulin secreting pancreatic tumor. After surgical excision of the tumor, patient was relieved from hypoglycemic attacks but required to initiate insulin injection for the treatment of hyperglycemia.  相似文献

Clinical evaluations of a new porous-surfaced implant concept (Endopore) in a large population of fully and partially edentulous patients are reported, and a technique of spreading buccal and lingual plates with osteotomes to place these implants in proximity to the sinus of the posterior maxilla is described. Three-dimensional, interconnecting pores on this implant's bone interface surface give a great surface area for bone engagement. When the maxilla is prepared by this spreading procedure, these implants can be successfully placed in areas having limited available bone. Our success rates are 97.0% for implants stabilizing a mandibular overdenture and 94.8% for implants placed in partially edentulous patients. Many times, sinus lift or other augmentation procedures can be avoided in the maxilla and mandible, allowing for less patient morbidity and for an implant reconstruction that is more affordable for the patient.  相似文献

Early in the development of implant technology it became apparent that conventional dental imaging techniques were limited for evaluating patients for implant surgery. During the treatment planning phase, the recipient bed is routinely assessed by visual examination and palpation, as well as by periapical and panoramic radiology. These two imaging modalities provide a two-dimensional image of mesial-distal and occlusal-apical dimensions of the edentulous regions where implants might be placed. When adequate occlusal-apical bone height is available for endosteal implants, the buccal-lingual width and angulation of the available bone are the most important criteria for implant selection and success. However, neither buccal-lingual width nor angulation can be visualized on most traditional radiographs. Although clinical examination and traditional radiographs may be adequate for patients with wide residual ridges that exhibit sufficient bone crestal to the mandibular nerve and maxillary sinus, these methods do not allow for precise measurement of the buccolingual dimension of the bone or assessment of the location of unanticipated undercuts. For these concerns, it is necessary to view the recipient site in a plane perpendicular to a curved plane through the arch of the maxilla or mandible in the region of the proposed implants. Implant dentists soon recognized that, for optimum placement of implants, cross-sectional views of the maxilla and mandible were the ideal means of providing necessary pre-operative information. Today, the two most often employed and most applicable radiographic studies for implant treatment planning are the panoramic radiograph and tomography. Although distortion can be a major problem with panoramic radiographs, when performed properly they can provide valuable information, and are both readily accessible and cost efficient. To help localize potential implant sites and assist in obtaining accurate measurements, it is recommended that surgical stents be used with panoramic radiographs. In simple cases, where a limited number of implants are to be placed, panoramic radiography and/or tomography may be used to obtain a view of the arch of the jaw in the area of interest. For complex, cases, where multiple implants are required, the CT scan imaging procedure is recommended. Because of its ability to reconstruct a fully three dimensional model of the maxilla and mandible, CT provides a highly sophisticated format for precisely defining the jaw structure and locating critical anatomic structures. The use of CT scans in conjunction with software that renders immediate "treatment plans" using the most real and accurate information provides the most effective radiographic modality currently available for the evaluation of patients for oral implants. To follow patients after implant surgery, DSR can be helpful by addressing the limitations of other radiographic modalities in detecting postoperative changes. By eliminating unchanged information, DSR allows the clinician's eye to focus on actual changes that have occurred between the recordings of two images.  相似文献

PURPOSE: This retrospective study investigated the survival of dental implants placed in the maxilla after composite grafting of the sinus and an average of 55 months of loading. PATIENTS AND METHODS: Maxillary sinuses of 88 patients were grafted with autogenous cancellous bone combined with dense hydroxyapatite particles. After an average healing period of 3.4 months, hydroxyapatite-coated titanium endosseous implants were placed. A total of 388 implants were placed in grafted sinus floors, and 82 were placed in onlay grafted nonsinus position in the canine region. The implants were loaded with overdentures and fixed bridges 4 months (mean) after implantation, with a follow-up for a mean of 55 months. RESULTS: The cumulative implant survival was calculated according to the Kaplan-Meier method. Implant survival from the time of loading was 89% in full reconstructed cases and 90% in partially edentulous cases. The overall cumulative implant survival rate, including the loss in the surgical stage, was 82%. CONCLUSION: Implant loss in composite grafted maxillae after 70 months of follow-up was similar to loss in nongrafted maxillae.  相似文献

The aim of this study was to evaluate the clinical function and long-term prognosis of overdentures retained by a small number of implants in the maxilla and mandible using one of two different attachment systems. Included in the study were all patients referred to specialty clinics in J?nk?ping and Link?ping, Sweden, during the treatment period who needed an overdenture and could be provided with a minimum number of two bilaterally-placed implants. Excluded were patients with bone-grafted jaws, irradiated cancer patients, heavy bruxers, and patients who had lost a fixed prosthesis because of implant losses. The patients were randomly assigned to receive one retentive system, either a round 2-mm-diameter bar with clips or ball attachments (Nobel Biocare). Eighteen overdentures were placed in maxillae and 32 in mandibles, supported by a total of 115 Br?nemark implants. Of the implants placed, 86.1% were continuously osseointegrated. The cumulative implant survival rates after 7 years of loading were 75.4% in the maxillae and 100% in the mandibles. There was no difference in implant survival rate between the attachment systems. Patients with implant losses were characterized by severely resorbed maxillary ridges and inferior bone quality, together with unfavorable loading circumstances such as short implants combined with long leverages. Complications and prosthetic adjustments were mostly resolved early and easily.  相似文献

A patient with a severely atrophic right posterior mandible had three endosseous implants placed in conjunction with transposition of the inferior alveolar nerve. Three weeks following implant placement surgery, the patient experienced a spontaneous fracture of the mandible involving the two anterior implants. The two implants were removed, and the fracture was treated with open reduction and fixation with titanium mesh. The fracture healed, and the posterior implant integrated. This report suggests that the buccolingual and superior-inferior position of the mandibular canal can increase the possibility of mandibular fracture by increasing the size of the buccal cortical plate that is removed to expose the nerve during surgery.  相似文献

The treatment of young people with implants requires advanced planning and coordination of many different specialities within dentistry. Timing and sequence of therapy will often decide the success or failure of treatment. Congenital absence of many teeth (oligodontia) associated with or without syndromes poses not only functional but also psychosocial problems for young people. A case is presented in which orthodontist, restorative dentist, and periodontist evaluated and performed the necessary therapy for a young person who at the age of 9 was diagnosed with oligodontia/l. Nine implants were placed: 13 years, 8 months in the mandible and 15 years, 6 months in the maxilla. All implants were restored as single teeth. The patient was followed until age 20.  相似文献

A survey was undertaken to analyze osseointegrated implants placed in irradiated maxillae and mandibles. It was found that nine centers in Japan and two centers in the United States had placed 228 implants in 44 patients. Of 228 implants, 59 were placed in the maxilla, 169 in the mandible. Three of 169 implants placed in irradiated mandibles were removed; 17 of 59 were removed from maxillae. One hundred nineteen implants were placed in the mandible without adjunctive hyperbaric oxygen treatment, of which only 2 were removed. From these results, it is suggested that osseointegrated implants can be placed in the irradiated mandibles of selected patients without hyperbaric oxygen treatment.  相似文献

A severely resorbed maxilla was reconstructed utilizing a combined surgical technique that included the placement of a horseshoe-shaped bicortical calvarium onlay graft, fixed by six endosseous implants. Two more implants were placed in the pterygoid process to support and stabilize the prosthesis. The dentition was rehabilitated with a fixed implant-supported hybrid prosthesis.  相似文献

Twelve patients with segmental mandibular defects were reconstructed with fibula osteoseptocutaneous flaps and simultaneous placement of osseointegrated implants. Decision to perform this procedure was based on the facts that all patients had benign diseases, did not require postoperative radiotherapy, were in good general and oral conditions, and were psychologically motivated. A total of 34 fixtures was inserted in the first stage. Eight patients underwent second stage surgery, which consisted of connection of the implant abutments to the fixtures and the use of palatal mucosal grafts around the implants. Final dental prostheses were fixed 1 month later in seven patients, at this time. All flaps survived after surgery, and no implant failure was observed after a mean follow-up period of 25 months. Only one fixture was not used during the subsequent stage and was left as a sleeper. Fixed dental prostheses were used in five patients and removable overlay prostheses in the other two. Chewing function was recovered between 4 and 6 weeks after the start using the definitive dental prosthesis. In contrast to previous results, we conclude that excellent results can be achieved when this combined procedure is used in carefully selected patients. In addition, it is confirmed that the fibula osteoseptocutaneous flap is a versatile, reliable composite tissue that facilitates primary placement of osseointegrated dental implants during mandible reconstruction, thus allowing full oral rehabilitation in a shorter period of time.  相似文献
