Although child sexual abuse has been a concern for many researchers, therapists, and advocates for the past 3 decades, several fundamental issues regarding child sexual abuse remain unresolved. In particular, the term child sexual abuse has never been unequivocally defined. The lack of a commonly accepted definition of child sexual abuse continues to inhibit research, treatment, and advocacy efforts. Early researchers used broad and inclusive definitions of child sexual abuse—definitions that often continue to be used today. The consequences of these definitions are discussed, and strategies for developing other definitions of child sexual abuse are discussed. The purpose of this special section is to further research, clinical practice, and teaching in the area of child sexual abuse. The purpose of this article is to further graduate-level education and training in child sexual abuse. Two graduate-level courses that focus on child sexual abuse are considered as well. The following points are made: (a) Training in child sexual abuse should be incorporated into professional training programs; (b) research and theory courses on child sexual abuse are indicated; (c) the topic of child sexual abuse should be mainstreamed into existing practica; (d) the multidisciplinary nature of the field necessitates consideration of resources and references beyond those specific to psychology as well as professional engagement with individuals from many disciplines; and (e) training in child sexual abuse elicits strong emotional responses in trainees and others. Systematic investigations of the consequences of sexual abuse and clinical literature on interventions with survivors have frequently neglected the impact of sexual abuse on parenting. Grounded in both object-relations and trauma theory, this article presents a case illustration to depict the unconscious repetition of one 30-yr-old mother's childhood abusive relationship with her own child. We explore how this survivor's parenting conflicts became increasingly salient during specific developmental stages of the child, as themes of sexuality and aggression triggered unresolved conflicts, distorted cognitions, and trauma reactions. The reenactment impairs parenting in critical ways, hence increasing the risk for sexual abuse in the next generation. Strategies for interventions to modify the mother's poor parenting responses are described using a therapeutic approach that integrates concepts from psychodynamic and cognitive-behavioral theories. Research on sexual abuse frequently fails to address the influence of the family as a risk factor for the onset of all kinds of sexual abuse and as a mediator of its long-term effects. Attachment theory provides a useful conceptual framework for understanding the familial antecedents and long-term consequences of sexual abuse. Themes associated with insecure parent–child attachment (rejection, role reversal/parentification, and fear/unresolved trauma) are frequently found in the dynamics of families characterized by sexual abuse, and specific categories of sequelae are related to probable attachment experiences. The Canadian legal system has made a number of significant reforms in the last few decades regarding the rights of child witnesses and, in particular, the rights of those who have disclosed sexual abuse. This paper provides an overview of the law in Canada as it pertains to child sexual abuse victims and witnesses, and reflects on the role and responsibilities of psychologists who work with child witnesses. It reviews the effects of protection reforms and preparation programs on children who must testify, examines some of the current major issues in the research literature in the area of interviewing and assessing sexually abused children, and considers the role of the psychologists as expert witness in court. In addition, it discusses potential ethical dilemmas for psychologists who work with child witnesses, and proposes recommendations for research and clinical practice. The authors examined how mock jurors respond to the testimony of hearsay witnesses in trials of child sexual abuse. In Experiment 1, participants watched a highly realistic videotape of a sexual abuse trial. In one condition, the child victim/witness testified on her own behalf, in the hearsay condition, the child's mother testified in lieu of her daughter. Conviction rates were significantly higher in the child condition versus the hearsay condition. In Experiment 2, participants read a trial summary of a sexual abuse case where either the child testified on her own behalf or the child did not testify and hearsay testimony was given by either the child's mother, doctor, teacher, or neighbor. Conviction rates were significantly lower in the child condition versus each of the hearsay conditions but not when the neighbor testified as the hearsay witness. The impact of hearsay testimony may depend on the perceived credibility or prestige of the hearsay witness. Several theoretical reasons are given to resolve the discrepancies in these and other studies that examine the impact of hearsay testimony in trials of child sexual abuse. We tested special and general explanations of male adolescent sexual offending by conducting a meta-analysis of 59 independent studies comparing male adolescent sex offenders (n = 3,855) with male adolescent non-sex offenders (n = 13,393) on theoretically derived variables reflecting general delinquency risk factors (antisocial tendencies), childhood abuse, exposure to violence, family problems, interpersonal problems, sexuality, psychopathology, and cognitive abilities. The results did not support the notion that adolescent sexual offending can be parsimoniously explained as a simple manifestation of general antisocial tendencies. Adolescent sex offenders had much less extensive criminal histories, fewer antisocial peers, and fewer substance use problems compared with non-sex offenders. Special explanations suggesting a role for sexual abuse history, exposure to sexual violence, other abuse or neglect, social isolation, early exposure to sex or pornography, atypical sexual interests, anxiety, and low self-esteem received support. Explanations focusing on attitudes and beliefs about women or sexual offending, family communication problems or poor parent–child attachment, exposure to nonsexual violence, social incompetence, conventional sexual experience, and low intelligence were not supported. Ranked by effect size, the largest group difference was obtained for atypical sexual interests, followed by sexual abuse history, and, in turn, criminal history, antisocial associations, and substance abuse. We discuss the implications of the findings for theory development, as well as for the assessment, treatment, and prevention of adolescent sexual offending. A review of theories to predict the mental health effects of child sexual abuse suggests that existing models have not adequately defined stress and coping constructs and have not specified how those variables might interact with other environmental factors. This article outlines a transactional model that conceptualizes sexual abuse as a stressor consisting of a series of abuse events, abuse-related events, and disclosure-related events that each tend to increase risk for maladaptive outcomes. The model also proposes that cognitive appraisals and coping responses mediate the effects of these events, that developmental and environmental factors may moderate relationships between sexual abuse stressors and victim responses, and that victims' initial responses may effect subsequent levels of abuse-related stress. Empirical studies relevant to the major components of this model are reviewed, and the implications of these findings for future research are considered. Reviews the book, Child sexual abuse: Critical perspectives on prevention, intervention, and treatment edited by Christopher R. Bagley and Ray J. Thomlison (1991). This book is a compilation of a series of literature reviews originally commissioned by Health and Welfare Canada in 1987 and completed in 1988. The individual papers included in this edited version represent a cross section of Canadian academicians, clinicians, and case workers who are integrally involved in the policies and practices regarding child sexual abuse in Canada. The work provides a comprehensive perspective on prevention at the primary, secondary, and tertiary levels. The list of authors is impressive in terms of their expertise and experience. At a time when numerous books on child sexual abuse are appearing on the market, it is nice to see a book that has some unusual aspects. Aside from its distinctly Canadian perspective, the book addresses several important, yet frequently ignored, topics. The book provides an up-to-date review of several core issues: conceptualization of the problem, prevention strategies, impact of sexual abuse, investigative interviewing, treatment outcome studies, and treatment issues for child molesters. The more unique topics include a review of the strategies used to evaluate prevention programs, prevalence rates among a number of special populations, the role of medical practitioners in preventing and intervening in child sexual abuse, and sexual abuse and exploitation among disabled individuals. Overall, I can recommend this book for clinicians and researchers in Canada and elsewhere who are interested in child sexual abuse. This compilation of literature reviews highlights the leading role that Canadian social service agencies have taken in developing programs for sexually abused children and their families. In this study, the authors assessed 48 female survivors of child sexual abuse (CSA) and 71 female control participants using measures of adult sexual function, psychological function (i.e., depression and anxiety), and sexual self-schemas. The primary purpose of this study was to examine whether differences existed between women with and without a history of CSA in the way that they viewed themselves as a sexual person and, if so, whether such differences mediated the link between early unwanted sexual experiences and later adult sexuality. CSA survivors were found to view themselves as less romantic and passionate than women who were not abused. In particular, CSA survivors showed an inverse relationship between romantic/passionate sexual self-schemas and negative sexual affect during sexual arousal. The relationship between CSA and negative sexual affect was independent from symptoms of depression and anxiety, suggesting that the impact of CSA on sexual self-schemas may be independent from the impact that the abuse may have in other areas of the survivor's life. Several studies have indicated that high proportions of practicing psychologists fail to report cases of suspected child sexual abuse despite their knowledge of mandatory reporting laws. We surveyed 279 practicing psychologists from two states, using a set of controlled vignettes. Results indicate that clinicians' decisions to report suspected sexual child abuse are affected by the accused father's admission or denial of abuse and by the clinician's expectation of what effect reporting would have on continued therapy. The empirical basis for the child sexual abuse accommodation syndrome (CSAAS), a theoretical model that posits that sexually abused children frequently display secrecy, tentative disclosures, and retractions of abuse statements was reviewed. Two data sources were evaluated: retrospective studies of adults' reports of having been abused as children and concurrent or chart-review studies of children undergoing evaluation or treatment for sexual abuse. The evidence indicates that the majority of abused children do not reveal abuse during childhood. However, the evidence fails to support the notion that denials, tentative disclosures, and recantations characterize the disclosure patterns of children with validated histories of sexual abuse. Family courts frequently rely on the expertise of mental health professionals to assess allegations of sexual abuse within the context of child custody evaluations. Such evaluations are complex and require knowledge of techniques used in sexual abuse and sexual offender evaluations, as well as knowledge of child custody practices. Preliminary findings from a national survey of 84 psychologists indicated that respondents tend to adhere to the child custody guidelines of the American Psychological Association. However, few practitioners followed formal models, protocols, or guidelines when evaluating alleged victims or alleged perpetrators of sexual abuse in conjunction with child custody disputes. Implications for professional practice are discussed, along with a proposed comprehensive model for assessing sexual abuse allegations in child custody cases. This reply to the comment article of E. Loftus, M. Garry, and J . Feldman (1994) focuses primarily on responding to major points raised by these authors. E. Loftus et al. agreed with the conclusion that the best scientific evidence shows that people can and do forget child sexual abuse. The present reply elaborates on this. Apparently, forgetting can take place even when the abuse occurred when the child was older, had experienced dramatic events surrounding the abuse, or had suffered repeated molestation. It is argued here that research suggests that memory of child sexual abuse may be influenced by more than the "simple forgetting" that E. Loftus et al. suggested. The goal of the present study was to evaluate the relationship between child sexual abuse and men's dyadic adjustment, using a theoretical model integrating attachment, that is anxiety about abandonment and avoidance of proximity, and psychological distress. Participants were 316 men forming a probabilistic sample of French-Quebecer living in couple. Structural equation analyses showed that sexual abuse predicted dyadic adjustment through anxiety about abandonment and psychological distress. Moreover, there was no relationship between sexual abuse and avoidance of proximity, while this variable was associated to dyadic adjustment. This article describes sexual abuse as well as several other dimensions of victimization, such as institutional racism, that American ethnic minority children are likely to encounter. Some of these other forms of victimization parallel the dynamics of child sexual abuse and posttraumatic stress disorder. The Four Traumagenic Dynamics model, which examines the dynamics of the effects of child sexual abuse, is applied to the experiences of ethnic minority children. The authors agree with the main elements of M. Bruck and S. J. Ceci's (see record 1996-09956-001) amicus curiae brief for the case of State v. Michaels, but they believe that 4 specific aspects of child sexual abuse investigations deserve a more constructive emphasis. Research has tended to concentrate on the suggestibility of children and neglected other topics that may be of great utility to those who investigate suspected cases of child sexual abuse. More attention should have been paid in the brief to the complexities of assessment and the special characteristics of the Wee Care case. Too much emphasis has been placed on children and not nearly enough attention on remedial actions to minimize the problems associated with suggestion. Finally, research shows that it may be possible to develop reliable and valid techniques to assess child sexual abuse allegations. OBJECTIVE: As an initial effort to examine Chinese sexual abuse trauma in relation to the cultural construction of sexual victimization of women, this study applies Finkelhor and Browne's (1985) traumagenic dynamics model to examine post-abuse trauma among Chinese child and adolescent survivors in Taiwan. In analyzing sexual abuse trauma in the context of Chinese patriarchal familialism and cultural fetish for female chastity, this study attempts to explore cultural constructional process of sexual abuse in Chinese society. METHOD: This study conducted in-depth interviews with 19 survivors of, and three social workers on, sexual abuse in Taiwan. These interviews were guided by a semi-structured questionnaire on sexual abuse incidents and post-abuse responses/concerns, and had generated a total of 84 pages of written reports, which serve as the text-data for the analysis. RESULTS: This study identifies sexual stigmatization and senses of disempowerment and betrayal as the most pervasive trauma complex among Chinese sexual abuse survivors in Taiwan. This study found that, in addition to lowering self-esteem, sexual stigmatization results in polarized sexuality among adolescent survivors, as manifested in aversion to and preoccupation with sexuality. The abuse-induced disempowerment is manifested in the survivors' fear for personal safety and heightened sense of vulnerability to re-victimization. The sense of betrayal results in interpersonal difficulties as manifested in self-imposed isolation from others and/or suspicious attitudes toward others, which further impair the survivor's efficacy in meeting her abuse-induced clinging need. CONCLUSIONS: Overall, this study observes similar traumatic symptoms between Chinese survivors and their American counterparts as documented in the literature. Nevertheless, this study observes the sexual stigmatization represents the most prevailing post-abuse trauma among Chinese sexual abuse survivors, and reflects the patriarchal fetish for female chastity in the Chinese cultural construction of sexual victimization.  相似文献

Similar to the first edition, the second edition is divided into three main sections: Section I gives a broad but in-depth overview of the characteristics and dynamics of incest; Section II provides a breadth of information on the long-term effects, assessment, and diagnosis; and Section III details current evidence-informed best practice in working with survivors of prolonged child sexual abuse. The book presents a wide range of treatment modalities that represent a bevy of both empirically supported and supplemental techniques, from cognitive–behavioral to expressive therapies, such as art or poetry. The second edition of Healing the incest wound is an important read for any practitioner, novice or experienced, working with child sexual abuse survivors. Courtois both provides a solid basis for those beginning work with child sexual abuse survivors and adds to the knowledge of seasoned practitioners by bringing together the most current empirical evidence and clinical wisdom regarding child sexual abuse treatment.
