The contribution of interventional cardiology procedures to the population dose in Europe is poorly known. The estimation of the population dose from these procedures requires knowledge of both the typical dose received by patients and an estimate of the number of procedures undertaken annually. Data on the number of cardiology procedures in various European countries are available on the internet for a number of countries. However, this data set is incomplete or out of date. This paper describes the statistical analysis undertaken to estimate the number of interventional cardiology procedures in a number of European countries for 2007. Estimates of the number of procedures are given. On average, the number of cardiac catheterisations per million population is 5346, percutaneous transluminal coronary angioplasty is 1599 and 1214 stent procedures. In addition, there are an estimated 973 pacemaker insertions per million population.  相似文献

Within the Information System on Occupational Exposure in Medicine, Industry and Research (ISEMIR), a new International Atomic Energy Agency initiative, a Working Group on interventional cardiology, aims to assess staff radiation protection (RP) levels and to propose an international database of occupational exposures. A survey of regulatory bodies (RBs) has provided information at the country level on RP practice in interventional cardiology (IC). Concerning requirements for wearing personal dosemeters, only 57 % of the RB specifies the number and position of dosemeters for staff monitoring. Less than 40 % of the RBs could provide occupational doses. Reported annual median effective dose values (often <0.5 mSv) were lower than expected considering validated data from facility-specific studies, indicating that compliance with continuous individual monitoring is often not achieved in IC. A true assessment of annual personnel doses in IC will never be realised unless a knowledge of monitoring compliance is incorporated into the analysis.  相似文献

Training in radiation protection is a basic aspect of the optimisation of medical exposures. Council Directive 97/43/EURATOM establishes the need for an adequate theoretical and practical training of the staff working in radiological practices, and competence in radiation, for which Member States shall ensure the establishment of appropriate curricula. Keeping in mind the different specialities and professional responsibilities, training curricula must be proposed and endorsed to achieve a common core of knowledge in radiation protection throughout Europe, for different groups of health workers. In interventional radiology, previous initiatives led to the definition of a syllabus of educational objectives and to its testing in a specific course. The present paper presents educational objectives for interventional radiology, developed in the framework of the DIMOND European concerted action.  相似文献

In interventional cardiology (IC) staff can be exposed to high dose levels due to the long fluoroscopy procedures performed. Staff dosimetry can yield information on the optimisation level of radiation protection, which is influenced by the equipment performance, auxiliary protection devices, training in radiation protection and procedure complexity. Staff exposure data assessed in haemodynamic laboratories of four hospitals in Spain, Greece and Italy participating in the DIMOND concerted action are analysed and compared with data in the literature.  相似文献

At present, interventional radiology (IVR) tends to involve long procedures (long radiation duration), and physicians are near to the source of scattered radiation. Hence, shielding is critical in protecting physicians from radiation. Protective aprons and additional lead-shielding devices, such as tableside lead drapes, are important means of protecting the physician from scattered radiation. The purpose of this study was to evaluate whether non-lead aprons are effective in protecting physicians from radiation during IVR procedures. In this study, the radiation protection effects of commercially available protective lead and non-lead aprons, when exposed to diagnostic X rays, are compared. The performance of these non-lead and lead aprons was similar for scattered X rays at tube voltages of 60-120 kV. Properly designed non-lead aprons are thus more suitable for physicians because they weigh approximately 20% less than the lead aprons, and are non-toxic.  相似文献

Patient and staff dose values in an interventional cardiology laboratory for different operational modes and several patient thicknesses (from 16 to 28 cm, simulated using polymethylmethacrylate) are presented. When increasing patient thicknesses and depending on fluoroscopy and cine modes, occupational doses can increase >30 times the baseline level. Scatter dose rates at the cardiologist's position with no radiation protective tools ranged from 1 to 14 mSv h(-1) for fluoroscopy, and from 10 to 47 mSv h(-1) during cine acquisition. Patient entrance surface air kerma rates increased by nearly 3 and staff dose rates by up to 2.6 when fluoroscopy was moved from the low to the high mode, for a typical 20 cm thickness. The respective increase factors were 6 and 4.2 when patient thickness rose from 16 to 28 cm, and by 10 and 8.3, when comparing cine acquisition with the low fluoroscopy mode. The knowledge of typical dose rates for each X-ray system in use in catheterisation laboratories is essential in order to optimise protection of patients and staff.  相似文献

Interventional cardiology procedures are increasing because they offer many advantages to patients compared with other techniques: therefore the Italian National Institution for Insurance against Accidents at Work decided to start a survey for monitoring the state-of-the-art regarding the professionals involved in those procedures. The survey covered six cardiology and medical physics Italian departments. Each centre was asked to record 10 examinations for five types of procedures: coronary angiography (CA), electrophysiology studies (ES), pacemaker implantation (PI), percutaneous transluminal coronary angioplasty (PTCA) and radiofrequency catheter ablation (RA). For each examination all the centres were requested to fill in a questionnaire containing information regarding the operator performing the examination, the patient and the procedure. A total of 290 examinations were recorded: 103 CA, 14 ES, 68 PI, 79 PTCA and 26 RA. As occupational doses are strongly related to patient doses, both patients and operators radiation dose data are reported. Ratios of maximum to minimum mean patient doses across the hospitals surveyed were 2.0, 3.9, 7.0, 1.8 and 1.4 for CA, ES, PI, PTCA and RA, respectively. The calculated rounded mean dose-area product values across all participating hospitals were comparable with other values reported in the literature. In general, specific radiation protection tools were used by all operators performing different procedures in all hospitals. A major issue in this survey was the absence of information about correlation between staff and patient doses in a single procedure: future studies could be more aimed to prospective goals where occupational exposures per procedure are monitored specifically.  相似文献

The International Commission on Radiological Protection (ICRP) has published two reports giving recommendations dealing with the avoidance of deterministic injuries in interventional radiology and the management of patient dose in digital radiology in 2001 and 2004, respectively. Another document, on radiation protection for cardiologists performing fluoroscopically guided procedures, will be produced during 2005. This paper highlights some of the topics of the published reports, their relevance to European legislation on medical exposures and the importance of radiation protection research in underpinning the ICRP task groups' work in to producing these documents. It is also anticipated that the results, obtained in the cardiology work package of the European research project, will be used in the new document on radiation protection for cardiologists.  相似文献

The purpose of this study was to measure staff radiation doses in representative institutions of five European countries performing coronary angiographies (CA) and percutaneous transluminal coronary angioplasties (PTCA), as well as to investigate any correlation with patient doses. Patient doses were recorded in terms of dose-area product (DAP). Staff doses were measured by thermoluminescence dosemeters placed on the operator's left shoulder and left foot. Radiation protection measures were also recorded. Third quartile shoulder and foot doses were 90 and 66 microSv in CA, and 157 and 173 microSv in PTCA, respectively. Shoulder dose was poorly correlated with DAP in CA (R2 = 0.29) and only moderately correlated in PTCA (R2 = 0.69). DAP and foot dose correlation was poor both in CA (R2 = 0.16) and in PTCA (R2 = 0.02). Protective measures taken by staff varied between countries, which may explain the poor correlation of patient and staff radiation doses.  相似文献

The purpose of this investigation was to measure the patient and staff dose during routine interventional cardiology procedures for an image intensifier-based and a flat detector system using a water phantom. The Integris BH3000 image intensifier-based (Philips) and the Axiom Artis flat detector-based (Siemens) angiography units were used in this study. The accuracy of tubes potential and irradiation timers and also internal dosimeters were verified and confirmed. A water phantom with a thickness of 18 cm was used for patient and staff dose measurements. For the Philips system, phantom entrance dose rates were 2.77 and 38.97 microGym(2) s(-1) during fluoroscopy and cineangiography. The respective dose rates for the Siemens were 1.98 and 13.46 microGym(2) s(-1). Phantom entrance dose rate was 28.5 and 65% higher for the Philips system during fluoroscopy and cineangiography, respectively. Comparing the scattered dose rates at the operator location showed that the flat detector-based Siemens system delivers five times lower dose to the operator in comparison with the image intensifier-based Philips unit. The results suggest that the decrease in received dose of the patient and staff is achievable using the flat detector system. In addition, application of lead curtain and glass is recommended to lower the cardiologist dose especially for the image intensifier-based Philips system.  相似文献

Interventional cardiology (IC) is a rapidly growing field of medical specialisation. Such procedures are complex and may subject patients and operators to higher levels of risk than those encountered in general radiology. Acceptance testing and quality assurance (QA) of radiological equipment, including IC equipment, is a requirement of the EU Medical Exposures Directive (MED) (97/43/EURATOM). In addition, the MED identifies interventional radiology as an area of special concern. This study presents the results of a survey of 17 IC systems (including several flat panel detector systems) in Irish hospitals. The results of the survey indicate large differences in patient doses between manufacturers for equivalent levels of measured image quality. In addition, all systems were found to have failed one or more acceptance tests, with 60% of systems demonstrating significant problems at acceptance testing. The results of the survey demonstrate the importance of acceptance testing and QA in IC. The results also provide baseline data, which may be used in the development of future QA guidelines.  相似文献

With the increase in number and complexity of interventional cardiology (IC) procedures, it is important to monitor skin dose in order to decrease skin injuries. This study investigated radiation doses for patients undergoing IC procedures, compare results with the literature and define a local dose-area product trigger level for operators to identify situations likely to exceed the threshold for transient skin erythema of 2 Gy. Dosimetric data were collected for 77 haemodynamic and 90 electrophysiological procedures. Mean maximum local skin doses (MSDs) were 0.28 Gy for coronary angiography, 1.03 Gy for percutaneous transluminal coronary angioplasty (PTCA), 0.03 Gy for pacemaker insertion, 0.17 Gy for radiofrequency ablation for nodal tachycardia, 0.10 Gy for WPW and 0.22 Gy for atrial flutter. Since MSD values for the other procedures were well below the deterministic effect limit, a trigger level of 140 Gy cm2 was derived for PTCA procedures alone.  相似文献

In this study, skin dosimetry of patients undergoing interventional cardiology procedures is presented. Three hospitals were included. Two methods were used for skin dosimetry--radiochromic dosimetry films and reconstruction of skin dose distribution based on examination protocol. Maximum skin doses (MSD) obtained from both methods were compared for 175 patients. For patients for whom the film MSD was >1 Gy, the reconstruction MSD differed from the film MSD in the range of ± 50 % for 83 % of patients. For remaining patients, the difference was higher and it was caused by longer fluoroscopy time. For 59 patients for whom the cumulative dose was known, the cumulative dose was compared with the film MSD. Skin dosimetry with radiochromic films is more accurate than the reconstruction method, but films do not include X-ray fields from lateral projections whilest reconstructions do.  相似文献

The interventional cardiology was recently implemented at the University Hospital of Osijek. Patients' absorbed doses during coronary angiography (CA) and the percutaneous transluminal coronary angioplasty (PTCA) procedures were measured and compared with published data and international standards. All patients undergoing CA or PTCA procedures during a 1-month period were included in the study. Patients' doses are expressed in terms of dose area product (DAP) per procedure. The patients' DAPs ranged from 2.6 to 210 Gy cm2 (average of 59 Gy cm2) during CAs, and from 61 to 220 Gy cm2 (average of 120 Gy cm2) during PTCAs. Patients' doses during CAs and PTCAs at the University Hospital of Osijek are in good agreement with the published ones. In complex cases, the radiochromic dosimetry films were used to show possible dose distributions across patient's skin. The film dosimetry showed a limitation of using only DAP values for the estimation of skin injuries risk.  相似文献

The purpose of this work was to develop an automatic management system to archive and analyse the major study parameters and patient doses for fluoroscopy guided procedures performed in cardiology and interventional radiology systems. The X-ray systems used for this trial have the capability to export at the end of the procedure and via e-mail the technical parameters of the study and the patient dose values. An application was developed to query and retrieve from a mail server, all study reports sent by the imaging modality and store them on a Microsoft SQL Server data base. The results from 3538 interventional study reports generated by 7 interventional systems were processed. In the case of some technical parameters and patient doses, alarms were added to receive malfunction alerts so as to immediately take appropriate corrective actions.  相似文献

In interventional cardiology (IC) the PTCA (percutaneous transluminal coronary angioplasty) procedure is the most frequent procedure with the highest dose to the patient. The procedure is usually performed by cardiologists having, in general, insufficient knowledge of radiation physics, radiation technology and radiation protection. The need for radiation protection is of paramount importance in this field of interventional radiology. Correlation between the complexity of PTCA procedure and irradiation parameters (fluoroscopy time, number of images and dose-area product--DAP) has been demonstrated. The presence of severe tortuosity and occlusion of > or = 3 months play a major role. Fluoroscopy time is better correlated with technical factors than DAP, which also includes the influence of patient size, collimation, operation modes, and X ray beam orientation. The number of acquired images is less correlated with the complexity of the PTCA procedure. A complexity index was conceived and procedures were divided into three groups, defined as: simple, medium and complex, which were significantly different in terms of patient dose.  相似文献

A national programme on patient and staff dose evaluation in interventional cardiology made in cooperation with the haemodynamic section of the Spanish Society of Cardiology has recently been launched. Its aim is to propose a set of national diagnostic reference levels (DRLs) for patients as recommended by the International Commission on Radiological Protection and to initiate several optimisation actions to improve radiological protection of both patients and staff. Six hospitals have joined the programme and accepted to submit their data to a central database. First to be acquired were the quality control data of the X-ray systems and radiation doses of patients and professionals. The results from 9 X-ray systems, 1467 procedures and staff doses from 43 professionals were gathered. Provisional DRLs resulted in 44 Gy cm(2) for coronary angiography and 78 Gy cm(2) for interventions. The X-ray systems varied up to a factor of 5 for dose rates in reference conditions. Staff doses showed that 50 % of interventional cardiologists do not use their personal dosemeters correctly.  相似文献
