BACKGROUND: At this time little information is available about the relationship between glaucomatous visual field defects and impaired blood flow in the optic nerve head. The purpose of this study was to examine blood flow of the juxtapapillary retina and the rim area of the optic nerve head in primary open-angle glaucoma with a borderline visual defect. METHODS: Juxtapapillary retinal and neuroretinal rim area blood flow was measured by scanning laser Doppler flowmetry (SLDF). The visual field was evaluated by static perimetry (Octopus-G1). The optic nerve head was assessed on 15 degrees color stereo photographs. We examined 116 eyes of 91 patients with POAG with controlled IOP and 66 eyes of 44 healthy individuals. The POAG group was divided into eyes with a mean defect lower than 2 dB (POAG group I) and in eyes with a mean defect equal to or greater than 2 dB (POAG group II). The mean age of POAG group I and POAG group II was 55 +/- 11 years and 57 +/- 10 years, respectively. The mean age of the control group was 45 +/- 15 years. The eyes of POAG group I had an average C/D ratio of 0.71 +/- 0.18 with an average mean defect of the visual field of 0.97 +/- 0.68 dB; the eyes of POAG group II had an average C/D ratio of 0.80 +/- 0.17 with an average mean defect of the visual field of 8.2 +/- 6.0 dB. The intraocular pressure on the day of measurement in POAG group I was 18.2 +/- 3.7 mmHg, in POAG group II 17.6 +/- 4.0 mmHg, and in the control group 15.1 +/- 2.5 mmHg. For statistical analysis, age-matched groups of 32 normal eyes of 32 subjects (mean age 52 +/- 10 years) were compared to 18 glaucomatous eyes of 18 patients (POAG group I, mean age 55 +/- 11 years) and 59 glaucomatous eyes of 59 patients (POAG group II, mean age 55 +/- 10 years). RESULTS: In the eyes of POAG group I and POAG group II, both juxtapapillary retinal blood flow and neuroretinal rim area blood flow were significantly decreased compared to an age-matched control group: neuroretinal rim area "flow" POAG group I -65%, POAG group II -66%; juxtapapillary retina "flow" POAG group I -52%, POAG group II -44%. All eyes of the POAG group I (MD < 2 dB) and 56 of 61 eyes of the POAG group II (MD > = 2 dB) showed a retinal perfusion lower than the 90% percentile of normal blood flow. We found no correlation between reduction of juxtapapillary or papillary blood flow and mean defect in POAG eyes. CONCLUSION: Glaucomatous eyes with no defects or borderline visual field defects as well as glaucomatous eyes in an advanced disease stage show significantly decreased optic nerve head and juxtapapillary retinal capillary blood flow.  相似文献

PURPOSE: When the optic disc has normal appearance with no abnormalities in routine automated perimetry, the subject is not considered to have glaucoma. The purpose of this study is to show how such patients may have localized retinal nerve fiber layer defects with corresponding functional abnormality. METHODS: The authors selected eight eyes of eight patients who had a localized retinal nerve fiber layer defect extending within a few degrees from fovea but in whom the optic disc appearance and Humphrey 30-2 visual fields were normal. Of the eight patients, three had positive family history of glaucoma, two had suspected retinal nerve fiber layer abnormality in routine eye examination, two had increased intraocular pressure (IOP), and one had advanced low-tension glaucoma in one eye with a normal fellow eye. The authors examined the central 10 degrees visual field with 1 degree resolution using Humphrey perimeter and the Ring and Centring programs of the high-pass resolution perimeter. RESULTS: A central field defect corresponding to retinal nerve fiber layer defect was found in six of eight patients: in both 10 degrees Humphrey field and Centring programs (2 eyes), in Humphrey only (2 eyes), and in Centring only (2 eyes). CONCLUSION: The results indicate that retinal nerve fiber layer photographs are helpful in diagnosing glaucoma because early glaucomatous abnormalities cannot be excluded without nerve fiber layer photography. Currently available routine perimetric examination programs do not always detect very early functional damage.  相似文献

In order to evaluate the association between the watershed zone and glaucomatous optic damage, we performed indocyanine green fluorescence angiography with a scanning laser ophthalmoscope in 54 eyes of 27 patients with normal tension glaucoma. The visual field indices were measured with a Humphrey Field Analyzer. We identified 8 eyes (14.8%) of 7 patients with a watershed zone not including the optic nerve head (type I), 32 eyes (59.3%) of 20 patients with the zone partially including the optic nerve head (type II), and 14 eyes (26.0%) of 10 patients with the zone including the optic nerve head (type III). Of the total of 27 patients, 10 patients (37.0%) had different types in each eye. In these patients, the mean deviation (MD) of visual field indices was worse in the eye with the watershed zone which included a larger part of the optic disc than in the contralateral eye (p < 0.05). Conversely, the eye with worse MD than the contralateral eye had a watershed zone which included a larger part of the optic disc than the other eye (p < 0.05). The location of watershed zone appeared to influence the progression of the visual field defect.  相似文献

BACKGROUND: The assessment of the cup of the optic disc depends, among other criteria, on the disc area. A small cup in a small optic disc can indicate an advanced glaucomatous lesion, while on the other hand a large cup in a large optic disc can be normal. Therefore, a cumulative normalised rim/disc area ratio curve could help to distinguish between glaucomatous and normal optic cups. The aim of our study was to calculate normalised rim/disc area ratio curve. METHODS: Heidelberg Retina Tomograph examinations of the optic nerve head of 100 randomly selected eyes of 100 normal subjects were evaluated. We calculated the disc area-adjusted normalised rim/disc area ratio in sectors of 10 degrees. The 95th, 90th and 50th percentiles of each of the 36 sectors were displayed in descending order. RESULTS: In relation to the normal percentile curves, it is possible to display an individual normalised rim/disc area ratio curve. We obtained such curves for a normal optic disc, optic nerve heads with moderate and advanced lesions and a small optic disc with glaucomatous damage. CONCLUSION: We present a new display mode for the results of Heidelberg Retina Tomograph optic nerve head examination, which may be helpful for easy and reliable assessment of the local, diffuse and combined components of glaucomatous optic nerve head damage depending on optic disc size.  相似文献

BACKGROUND: Significance of topometric follow-up examinations of the optic nerve head in glaucomatous eyes depends on the reproducibility of the calculated parameters. Since the definition of the standard reference plane in software version 1.11 of the Heidelberg Retina Tomograph has been changed, intrapapillary parameters depend directly on the position of the contourline in the sector between -10 degrees to -4 degrees, and therefore on the observer variability to determine the disc border. We evaluated intra- and interobserver variability and present a simple approach to increase reproducibility. METHOD: The disc border of 4 glaucomatous eyes, 3 ocular hypertensive eyes and 3 eyes of healthy subjects were traced by two observers, 5 times using the free draw mode and 5 times by the addition of contourline circles. RESULTS: We found a median variability of the mean disc radius in sector -10 degrees to -4 degrees of 51 microns, which defines the position of the standard reference plane, resulting in a median variability of the position of the standard reference plane of 33 microns which caused a variability of 81 microns2 of the cup area. Addition of contourline circles smoothing the final contourline along the border of the optic disc resulted in a decrease of the coefficient of variation of the standard reference plane of 3.76% (6.76% vs. 3.0%), of the cup area of 2.34% (3.87% vs. 1.53%) and of the rim volume of 3.41% (9.75% vs. 6.34%). CONCLUSION: The calculation of the cup area using software version 1.11 of the Heidelberg Retina Tomograph depends on observer variability. The addition of contourline circles to define the final contourline along the disc border increases reproducibility. However, in follow-up of topometric examinations of the optic nerve head the software supported transfer mode should be used. Comparing topometric data of an individual optic disc in follow-up suppose the same definition of the contourline. Therefore, topometric data evaluated using software version 1.10 or earlier needs to be recalculated.  相似文献

The diagnosis of the open angle glaucoma in myopic eyes remains one of the major problems, as the changes of the optic head in myopia may be similar to glaucomatous changes. The aim of the study was to analyse biomorphometric parameters of the optic nerve head in myopic eyes with glaucoma with scanning laser ophthalmoscope from Laser Diagnostic Technologies Inc. There was 29 children examined and among them 12 children with glaucoma and myopia as a study group (I) and 17 children with myopia as a control group (II). RESULTS: TCA means values; group I: 1.994 +/- 0.158, group II:2.028 +/- 0.200 CD means values; group I; 0.315 +/- 0.072, group II: 0.247 +/- 0.057 Rim means values; group I: 1.366 +/- 0.164, group II: 1.511 +/- 0.136 Vol. A mean values; group I: +0.205 +/- 0.03, group II: +0.83 +/- 0.03 Vol. B mean values; group I: -0.159 +/- 1.608, group II: -0.117 +/- 0.08. Average values of the biomorphometric parameters of the optic nerve head were worse in group I than in group II, but there was not statistical significance between the differences. Scanning laser ophthalmoscopy may be used as very important additional examination in juvenile glaucoma cases with myopia.  相似文献

PURPOSE: To report bilateral congenital optic nerve head pits in monozygotic siblings. METHOD: Case reports. RESULTS: Pits were found in abnormally large optic disks in both eyes of two otherwise healthy female monozygotic siblings aged 15 years. Pit size increased and visual acuity decreased with increased optic disk area. In one eye, nonrhegmatogenous retinal detachment developed that eventually necessitated pars plana vitrectomy. The siblings' parents were unremarkable. CONCLUSIONS: Congenital optic nerve head pits can occur bilaterally in otherwise healthy monozygotic siblings with ophthalmologically unremarkable parents. Associated nonrhegmatogenous retinal detachment may be treated by pars plana vitrectomy. Pit size is positively correlated with disk area.  相似文献

AIM: The effect of breathing 100% oxygen on retinal and optic nerve head capillary blood flow in smokers and non-smokers was investigated using scanning laser Doppler flowmetry (SLDF) as a new non-invasive method to visualise and quantify ocular blood flow. METHOD: 10 eyes of 10 young healthy non-smoking volunteers (mean age 26 (SD 3) years) and nine eyes of nine young healthy smoking volunteers (mean age 26 (4) years) were investigated. All participants were asked not to smoke or consume caffeine containing drinks for at least 4 hours before the measurements. Blood flow measurements were performed before and after 100% oxygen was applied to the subjects through a mask over a period of 5 minutes (6 litres per minute). Juxtapapillary retinal and optic nerve head blood flow were determined in arbitrary units using SLDF representing a combination of laser Doppler flowmetry and a scanning laser system allowing visualisation and quantification of the retinal and optic nerve head blood flow. Blood flow was determined in an area of 100 microns x 100 microns. The level of carboxyhaemoglobin was determined in all subjects. A Wilcoxon matched pairs signed ranks test (non-parametric) was used for statistical evaluation. RESULTS: In the non-smoking group, retinal 'flow' was reduced by 33% (p = 0.005), optic nerve head 'flow' by 37% (p = 0.005). In the smoking group retinal flow was reduced by 10% (p = 0.01), optic nerve head flow by 13% (p < 0.008). The difference in reactivity to oxygen breathing between smokers and non-smokers was highly significant (p < 0.00001). Increased carboxyhaemoglobin levels were not found in either of the groups. A significant reduction of the mean arterial blood pressure of 6% (5%) (p < 0.02) was observed in the non-smoking group after administration of oxygen. CONCLUSION: These results indicate that hyperoxia leads to a decrease in capillary blood flow of the retina and optic nerve head secondary to vasoconstriction, and that smokers do not respond to oxygen breathing as non-smokers do. The findings might be based on factors such as long term effects of nicotine on the sympathetic and parasympathetic nervous system.  相似文献

In normal eyes, the retinal nerve fiber layer (RNFL) is usually best visible in the inferior temporal part of the fundus, followed by the superior temporal region, the nasal superior region and the nasal inferior region. This distribution correlates with the configuration of the neuroretinal rim, the diameter of the retinal arterioles, the location of the foveola, and the lamina cribrosa morphology. With increasing age, the RNFL visibility decreases diffusely without preferring special fundus regions and without the development of localized defects. With all optic nerve diseases, the visibility of the RNFL is decreased in addition to the age-related loss, in a diffuse and/or a localized manner. The localized defects are wedge-shaped and not spindle-like defects, running toward or touching the optic disk border. Typically occurring in about 20% of all glaucoma eyes, they can be found also in other ocular diseases, such as optic disk drusen, toxoplasmotic retinochoroidal scars, longstanding papilledema or optic neuritis due to multiple sclerosis. Since they are not present in normal eyes, they almost always signify an abnormality. RNFL evaluation is especially helpful for early glaucoma diagnosis and in glaucoma eyes with small optic disks. In advanced optic nerve atrophy, other examination techniques, such as perimetry, may be more helpful for following optic nerve damage. Considering its great importance in the assessment of optic nerve anomalies and diseases and taking into account the feasibility of its ophthalmoscopic evaluation using green light, the retinal nerve fiber layer should be examined during any routine ophthalmoscopy.  相似文献

BACKGROUND: Chronic papilledema may lead to irreversible damage of optic nerve fibers. To preserve visual functions, a decompression of the optic nerve is recommended by means of a fenestration of the optic nerve sheath. In this study long-term results after optic nerve sheath fenestration in patients with idiopathic pseudotumor cerebri are reported. PATIENTS AND METHODS: 14 patients were re-examined 15 to 145 months (mean 62 months) after a fenestration of the retrobulbar optic nerve sheath on 23 eyes by a transconjunctival approach. Surgery was done to treat progressive visual loss or in severe obscurations. The patient's symptoms, visual acuity, visual fields, and ophthalmoscopic findings of the optic nerve head and the central fundus were compared to the preoperative status. RESULTS: Re-examination revealed improvement or stabilisation of objective and subjective findings in 17 eyes, one of them was operated on the more involved contralateral side only. Six eyes showed a recurrence of the papilledema without a functional change for the worse after an interval of 7 to 121 months. Three eyes of two patients ended up with optic atrophy and extensive visual loss. Preoperatively, these eyes had shown cotton wool spots in the optic nerve head and a rapid deterioration of vision. CONCLUSIONS: Fenestration of the retrobulbar optic nerve sheath can prevent further visual loss in most patients with pseudotumor cerebri, unless the eye has already become nearly blind. Postoperatively, ophthalmological controls are necessary at regular intervals because relapses after successful surgery can occur after months or years.  相似文献

Retina and optic nerve of eyes enucleated due to an iris ring melanoma or a malignant melanoma of the ciliary body were studied to investigate the correlation between corpora amylacea count and glaucoma stages. Ten eyes from ten patients [age: 62.3 +/- 13.0 (mean +/- SD); range 43-84 years] were used for the present study. Eight eyes had shown elevated intraocular pressure readings preoperatively. The corpora amylacea count was correlated with histomorphometric changes of the intra- and parapapillary region. The corpora amylacea count of the retinal ganglion cells decreased significantly with increased histological ratio of cup to disc, decreased retinal nerve fiber layer thickness, and increased optic cup depth. There was no significant correlation between the corpora amylacea count of the bipolar cells and each histomorphometric datum. These results suggest that the corpora amylacea of the retinal ganglion cells decrease in number with advancing histological changes of secondary glaucoma.  相似文献

PURPOSE: The goal of these experiments was to identify the neurotransmitter in centrifugal axons of the macaque retina. METHODS: Macaca mulatta retinas and optic nerves were fixed overnight in carbodiimide and labeled with an antiserum to histamine with the use of an immunofluorescence technique. RESULTS: Several large histamine-immunoreactive axons ran from the optic nerve head to the peripheral retina, where they branched extensively and terminated in the inner plexiform layer, occasionally alongside retinal blood vessels. Other axons that emerged from the optic nerve head ran in the optic fiber layer to the central retina, circled the fovea, and then returned to the optic disc. These may be the source of histamine-immunoreactive axons that have been observed in central visual areas. No labeled cell bodies were present in the retina. Because perikarya in the posterior hypothalamus are the only known source of histamine in the primate central nervous system and because neurons there can be retrogradely labeled from the cut optic nerve, the histamine-immunoreactive axons must have originated there. CONCLUSIONS: Centrifugal axons in the macaque retina are part of the system of axons containing histamine that originate in the hypothalamus and project throughout the brain. Because the activity of these neurons is highest during the morning, histamine might play a role in preparing the retina to operate in daylight. The contacts of histamine-immunoreactive axons with blood vessels suggest that histamine may also play a role in regulating the retinal microvasculature.  相似文献

OBJECTIVE: This study aimed to develop a protocol to screen and monitor patients with diabetic macular thickening using optical coherence tomography (OCT), a technique for high-resolution cross-sectional imaging of the retina. DESIGN: A cross-sectional pilot study was conducted. PARTICIPANTS: A total of 182 eyes of 107 patients with diabetic retinopathy, 55 eyes from 31 patients with diabetes but no ophthalmoscopic evidence of retinopathy, and 73 eyes from 41 healthy volunteers were studied. INTERVENTION: Six optical coherence tomograms were obtained in a radial spoke pattern centered on the fovea. Retinal thickness was computed automatically from each tomogram at a total of 600 locations throughout the macula. Macular thickness was displayed geographically as a false-color topographic map and was reported numerically as averages in each of nine regions. MAIN OUTCOME MEASURES: Correlation of OCT with slit-lamp biomicroscopy, fluorescein angiography, and visual acuity was measured. RESULTS: Optical coherence tomography was able to quantify the development and resolution of both foveal and extrafoveal macular thickening. The mean +/- standard deviation foveal thickness was 174 +/- 18 microns in normal eyes, 179 +/- 17 microns in diabetic eyes without retinopathy, and 256 +/- 114 microns in eyes with nonproliferative diabetic retinopathy. Foveal thickness was highly correlated among left and right eyes of normal eyes (mean +/- standard deviation difference of 6 +/- 9 microns). Foveal thickness measured by OCT correlated with visual acuity (r2 = 0.79). A single diabetic eye with no slit-lamp evidence of retinopathy showed abnormal foveal thickening on OCT. CONCLUSIONS: Optical coherence tomography was a useful technique for quantifying macular thickness in patients with diabetic macular edema. The topographic mapping protocol provided geographic information on macular thickness that was intuitive and objective.  相似文献

STUDY DESIGN: This study analyzed the anatomic relation of the dorsal ramus of the cervical spinal nerve to the lateral mass. OBJECTIVES: To determine the location of the dorsal rami of the cervical spinal nerves from C3 to C7 in relation to the superior articular processes of the lateral masses. SUMMARY OF BACKGROUND DATA: The anatomic study of the cervical spinal nerve and its relations to adjacent bony structures have been addressed. No previous anatomic study with regard to the location of its dorsal ramus relative to the lateral mass has been reported. METHODS: Twelve specimens were obtained for study of the dorsal rami of the cervical spinal nerves. All soft tissues surrounding the cervical spinal nerves from C3 to C7 were dissected from the intervertebral foramens until the dorsal rami were clearly exposed. The facet joints in the corresponding levels were then opened by removal of the capsules. Three measurements, including the height of the dorsal ramus, the distance between the dorsal ramus and the tip of the superior articular facet, and the angle of the dorsal ramus relative to the superior articular surface, were taken for each ramus. RESULTS: The results showed that the mean height of the dorsal ramus for both sides decreased progressively from C3 (2.2 +/- 0.6 mm) to C7 (1.2 +/- 0.2 mm). The mean distance between the dorsal ramus and the tip of the superior facet showed an inconsistent change, with the maximum value seen at C5 (7.4 +/- 1.6 mm) and the minimum at C7 (5.5 +/- 2.9 mm). The mean angle of the dorsal ramus relative to the superior articular facet ranged from 23.3 degrees +/- 14.3 degrees to 29.8 degrees +/- 11.2 degrees. CONCLUSIONS: The dorsal ramus of the cervical spinal nerve is closer to the anterolateral corner of the base of the superior articular processes. Lateral mass screws directed to the anterolateral corner of the base of the superior articular process should be avoided.  相似文献

PURPOSE: The aim of this study was to measure morphometric parameters of the optic disc in Togolese glaucoma patients and suspects by the mean of the millimetric scale of the slit lamp and the Goldmann contact lens. MATERIAL AND METHOD: We selected 202 patients (393 eyes) with a mean age of 36.69 years +/- 15.33 (standard deviation); they were divided into 2 subgroups A (162 glaucomatous) and B (40 glaucoma suspects); direct reading of the slit lamp millimetric scale and the Goldmann contact lens was used. RESULTS: In the group A, the optic disc vertical diameter was 1.792 +/- 0.21 mm; the horizontal diameter was 1.701 +/- 0.198 mm. In the group B, vertical disc diameter was 1.700 +/- 0.262 mm; the horizontal one was 1.662 +/- 0.190 mm. The vertical cup disc diameter was 1.147 +/- 0.274 mm in the group A and 0.708 mm +/- 0.274 mm in the group B. The neuroretinal area was 1.360 +/- 0.524 mm2 in group A and 1.786 +/- 0.467 mm2 in group B. CONCLUSION: This study using millimetric scale of the slit lamp and the three mirrors Goldmann contact lens was easy, simple and useful clinically. It could be helpful in conducting quantitative studies in countries with low resources because this method is costless compared with others.  相似文献

Eighteen normal human eye-bank eyes (age: 18-81 years), five fetal eyes (16-24 weeks), 11 primary open-angle glaucoma (POAG) eyes (age: 76-89 years), and two Schnabel's cavernous optic atrophy eyes were examined using a biotinylated-hyaluronan binding protein to study the changes in the distribution of hyaluronic acid (HA) in the fetal, adult and glaucomatous optic nerve head. The vitreous body served as a positive control. Sections treated with Streptomyces hyaluronidase were used to confirm specificity. Monoclonal antibodies to myelin basic protein (MBP) and glial fibrillary acidic protein (GFAP) were used as additional controls. In fetal optic nerve, HA was localized in blood vessels, peripapillary sclera and the pial septae in the retrolaminar nerve. No staining was associated with axons. Staining for MBP was negative. In adults, HA was found surrounding the myelin sheaths in the retrolaminar nerve; staining decreased with age. In contrast, HA staining in myelinated peripheral nerves (e.g. ciliaries) remained unchanged with age. HA also was localized to the adventitia of arteries and veins throughout the posterior segment. Compared to age-matched normal eyes, HA staining was virtually absent around myelin sheaths of the retrolaminar nerve in POAG eyes. Similar changes were not found in other HA positive structures. In Schnabel's cavernous optic atrophy. HA was present in increased amount in the atrophic area, but virtually absent in the remaining retrolaminar nerve. HA staining was invariably positive in vitreous, and Streptomyces hyaluronidase treated sections were negative. In adults, staining of MBP was associated with the myelin sheath in the retrolaminar nerve. In contrast to HA, staining of MBP was unchanged with age and in POAG. In Schnabel's atrophy, MBP staining disappeared only in the atrophic area. HA in the retrolaminar optic nerve appears to be associate with the space-filling matrix between myelin sheaths. HA is not present in the axon bundles prior to myelination of the optic nerve. HA in the retrolaminar optic nerve appears to decrease with age and is further reduced in POAG; however, corresponding changes are not found in MBP or in peripheral nerves. Perhaps, decreased amounts of HA is related to a higher susceptibility to elevated intraocular pressure or to optic nerve atrophy. In Schnabel's cavernous optic atrophy, HA is present in increased amount only in the atrophic area while MBP is markedly decreased, suggesting in situ production of HA in areas of optic nerve atrophy.  相似文献

OBJECTIVE: To search a sensitive parameter for the early diagnosis of primary open angle glaucoma (POAG). METHOD: A system of computerized image analysis was used to acquire images of the optic nerve head of patients with POAG, suspect glaucoma (SG) and of normal persons. Each of these groups contained 31 eyes of 31 cases. The mean relative depths of disc rim and cup at different areas in the optic nerve head were measured. All these patients were followed up for four to six months, and the changes of the mean depths of every sector were compared between each pair of the three groups with statistic analysis. RESULTS: The mean depth of the neural rim and cup of each sector and that of total rim and total cup of POAG and SG patients were all deeper than that of normal controls. The mean depths of neural rim of the superior sector, inferior sector and the total rim area of POAG were deeper than that of SG patients. Compared to their first measurements, the changes of mean depths of neural rim of superior sector and total neural rim area of POAG patients were more prominent than that of normal controls and SG. We also compared the relative depth of the total rim area in the different areas in the optic nerve head with other two dimensional optic disc parameters such as cup/disc ratio, etc. CONCLUSION: Our study suggests that three-dimensional stereoscopic measurement of the surface of optic nerve head and follow-up be of paramount importance in the early diagnosis of POAG patients.  相似文献

BACKGROUND: This study investigated whether regional variations in the course of the optic nerve fibers through the lamina cribrosa may be one of the reasons why the local susceptibility for glaucomatous optic nerve fiber loss differs among the various regions of the optic disc. METHODS: The study included 34 human eyes enucleated because of a malignant melanoma of the peripheral choroid without involvement of the anterior chamber angle or the optic nerve. Anterior-posterior sections through the pupil and the optic disc were histomorphometrically evaluated. In the central region and the peripheral part of the optic disc, we measured the thickness of the lamina cribrosa and the length of the lamina cribrosa "channels" through which the nerve fibers pass. RESULTS: In the peripheral parts of the disc, compared with its central region, the lamina cribrosa was significantly thicker (P < 0.0001, Wilcoxon test), the lamina cribrosa "channels" with the nerve fibers passing through were significantly longer (P < 0.0001), and the ratio of length of the fiber "channels" to the thickness of the lamina cibrosa was significantly higher (P = 0.0001). CONCLUSION: The lamina cribrosa is thicker and the course of the optic nerve fibers through the lamina cribrosa is more curvilinear in the disc pheriphery than in the disc center. These variations in the anatomy of the lamina cribrosa may be one of several factors influencing the regional susceptibility for glaucomatous optic nerve fiber loss within the optic nerve head.  相似文献

PURPOSE: To evaluate the possibilities of magnetic resonance imaging (MRI) for quantification of pharmacologically induced changes in optic nerve microcirculation. METHOD: T2-weighted MRI sequences were used to image the eye, optic nerve, and frontal cortex in rats. Two sets of control images before and one set during Gd (DTPA) infusion were recorded. Blood flow values for two regions of the optic nerve (an anterior part, including the optic nerve head, and a more posterior part) and the frontal cortex were calculated by image analysis from the change in signal intensity, as already reported for cerebral blood flow. For each rat, a control experiment before drug administration and a second experiment 30 minutes after subcutaneous injection of either placebo (n = 7), timolol (n = 7), or SDZ GLC-756, a dopamine D-1 antagonist and D-2 agonist (n = 7), were carried out in a double-blind fashion. RESULTS: Mean basal blood flow values were found between 29.4 and 45.6 ml/100 g per minute in the anterior part of the optic nerve, 38.3 and 42.9 ml/100 g per minute in the posterior part of the optic nerve, and 68.0 and 75.0 ml/100 g per minute in the frontal cortex. Placebo and timolol did not cause significant changes. SDZ GLC-756 significantly increased blood flow by 238% +/- 65% in the anterior part and by 87% +/- 40% in the posterior part of the optic nerve. CONCLUSIONS: These results suggest that MRI provides quantification of optic nerve blood flow and that dopaminergic substances increase optic nerve blood flow.  相似文献

PURPOSE: Since the glaucomatous loss of nerve fibers changes the appearance of the optic disc, we evaluated the morphology of the surface of the optic disc in normal and glaucomatous eyes by using a computerized system to provide the reciprocal position of a large number of points placed on its surface in order to study the clinical significance of differences in the 'smoothness' of optic disc surface. METHODS: The morphology of the optic disc surface was evaluated by means of simultaneous stereoscopic videographic pictures (IMAGEnet X Rev-3.51b-Topcon Europe, The Netherlands): the reciprocal distribution of a large number of points located on the surface of one eye of 100 subjects randomly chosen (45 normal and 55 glaucoma patients) was studied. In order to define the level of 'smoothness' of the optic disc surface, the differences of the relative position of each surface point were studied by measuring the standard deviation (SD) from the average heights of the points (n. ranging from 623 to 1916 depending on the size of the disc area) that identify the optic disc surface. RESULTS: The coefficient of variation of the reciprocal location of the points, placed on the optic disc surface at the different measurements performed by a single operator was 10.4%. The differences in Optic Disc Surface Smoothness (ODSS) between glaucoma and normal group were statistically significant (p < 0.0001 using Mann-Whitney U test). No correlation was detectable between age and standard deviation. The best threshold value, calculated using ROC methodology, able to separate the two groups was: normal group: SD < or = 17.79 (-1 x 10(-2) mm); glaucoma group: SD > -17.79 (-1 x 10(-2) mm). Such threshold value had a sensitivity of 82.1%, a specificity of 92.2% and a diagnostic precision (DP) of 86.5% in dividing the glaucoma group from the normal group. CONCLUSION: ODSS is a global index of optic disc conditions based on quantitative measurements of the morphology of the optic disc surface. As such it does not provide information about the location and the characteristics of optic disc damage. Nevertheless, ODSS measurement is able to separate normal from glaucomatous optic disc with a rather interesting sensitivity, specificity and diagnostic precision (DP).
