Ethics in sports medicine--health and/or performance?

Ethics in the field of sports medicine embraces a number of issues of general human interest. While it is important to determine the salubrious potential of exercise and sports, it is open to question whether it is reasonable to invest massive research resources in attempts to induce high-performance elite athletes to perform yet better, or to remedy sports injuries to enable athletes to continue exposing themselves to the risk of further injuries. When such questions arise as the extent to which physicians should attempt to modify unhealthy lifestyle factors, for example the use of doping, or whether it is right that athletes should take precedence in waiting lists for health care, it is important to be aware of the singular practical potential of sports medicine.

Profession building in the new health care system

To further the goals of greater legitimacy and power, it is necessary for nursing to abandon many of the profession building strategies it has pursued in the past. We describe the traditional framework for profession building in nursing and some reasons it is no longer effective. In contrast, a contemporary framework for profession building in nursing is presented. Because nursing, like other complex adaptive systems, is strongly anchored by its past trajectory, it will require considerable energy and resources to make the transition.     

Classification of the effectiveness of combination treatments

According to FDA regulations, a combination drug is not efficacious unless each component contributes to the claimed effects. For a univariate endpoint, this implies that the combination at specific doses must be superior to each of its components at the same doses. More demanding is the property of synergy, in which the effect of the combination must be superior to the effect expected based on those of its components. If it is equal to those effects, it is additive, and if it is inferior, it is antagonistic. We give regions in the combination dose plane where these concepts are well defined. If the effect of the combination is greater than the greatest effect achievable by any of its components it is therapeutically synergistic. A combination can be antagonistic, yet its components can still contribute to the claimed effects. If it is additive, synergistic or therapeutically synergistic, its components must contribute to the claimed effects. We relate these concepts and provide designs and sequential procedures for determining whether a combination is therapeutically synergistic, synergistic, additive, antagonistic and contributing or antagonistic and non-contributing.     

浓密机事故分析及保护装置的设计

贵溪冶炼厂选矿车间一台浓密机投入使用不久发生了传动蜗轮箱崩裂、主轴扭曲的事故,浓密机是选厂用于脱水的主要设备,一旦出了故障,停产处理时间长,费用高。经分析造成此次故障的主要原因是浓密机设计机械强度不足,无过载保护装置。为了避免类似事故的再次发生,设计安装了一套可靠的浓密机保护装置,并通过降低传动电机功率,提高铸件的制造质量防止浓密机过负荷时出现零件破坏。重新投入后浓密机耙子能根据负荷情况自动升降,使用近8个月浓密机运转正常。

Nursing diagnosis: language antithetical to nursing's ontology

The term nursing diagnosis is constructed in male, medical language. As such, it is proposed that it has no legitimacy in nursing discourse as it is antithetical to notions claimed foremost in nursing's ontology. It is also posited that the term 'nursing diagnosis', whilst reinforcing biomedicine, has little or no meaning to many nurses, is not understood by patients and has no justifiable place in health care discourse.

Factors affecting drug binding in plasma of elderly patients

Tranfusional malaria in countries located outside zones of endemia is difficult to evidence because it is rarely aparent. The clinic can be compared to that of malaria through inoculation by anophele, but it can be difficult to evidence because of its intricacy with the symptomatology of the affection which justified the transfusion. Usually, it takes the appearance of a febril gastric problem, but it may have other aspects: intestinal, hepato-biliar, neurologic or hematologic symptoms. As soon as the diagnosis is established, it is easy to confirm by a cytologic test on peripheric blood and/or on bone marrow and specially by a sero diagnosis using immuno fluorescence.
