Results of the National Dental Examining Board of Canada written examination and implications for certification

In 1994, following a request from the ten Provincial Licensing Authorities, the National Dental Examining Board of Canada (NDEB) implemented significant changes to the certification process for dentists seeking a license to practice in Canada. Prior to 1994, graduates of accredited Canadian dental programs were certified without further examination while graduates of United States and other international programs (non-Canadian, non-U.S.) were required to complete successfully a written and three-phase clinical certification examination. Changes implemented in 1994 required graduates of accredited Canadian programs to take both a Written and Objective Structured Clinical Examination (OSCE) Examination. The analysis of the results of the Written Examination for all candidates over the 1994-1996 period supports the following conclusions. There was no meaningful difference in performance of graduates across the ten Canadian dental programs; there was a small difference between the performance of graduates of Canadian and U.S. programs; and Canadian and U.S. graduates performed significantly better than graduates of other international programs. This level of candidate performance and changes to the respective accreditation processes supported the formal agreement providing reciprocal recognition of dental accreditation in Canada and the United States. As of January 1, 1997, graduates of dental programs in Canada and the United States are required to take the same certification examination while international graduates are required to complete a different certification process. These changes to the certification process were ratified by all ten Provincial Licensing Authorities, therefore maintaining a system of national portability for dental licensure in Canada that does not require preclinical or clinical board examinations for graduates of accredited North American dental programs.

The transfer of patients from the Community Dental Service in Greater Glasgow Health Board area to the General Dental Service

Between January and June 1990, 688 patients of Greater Glasgow Health Board Community Dental Service were asked to attend a General Dental Practitioner in future. At the end of June 1991, only 255 patients, 37% of the sample, were found to be registered on the General Dental Service Capitation Scheme. Registration was associated with the 'Neighbourhood Type' which is a reflection of the socioeconomic characteristics of the locality. Those living in the most favourable group of Neighbourhood Types were more likely to be registered with a General Dental Practitioner after leaving the Community Dental Service.

The Maryland State Board of Dental Examiners and the chemically dependent dentist

Non-convulsive status epilepticus may present as confusion, behavioural disturbances and psychiatric conditions. We present the case of a 17-year-old man who had episodes of non-convulsive status epilepticus as his only manifestation of epilepsy which was mis-diagnosed as a psychiatric condition for over 10 years. He has had almost complete resolution of his symptoms with the introduction of carbamazepine. Non-convulsive status epilepticus is probably commoner than previously thought, and should be considered as a possible diagnosis in all patients presenting with prolonged episodes of altered consciousness even without other manifestations of epilepsy.

Modifications to the technic of cesarean section after Stark

BACKGROUND: As the operating births (caesarean section) increase, many surgical equipes have been compelled to revise operating techniques in order to reduce fetus extraction times and the whole expense of operation without renouncing, at the same time to beauty advantages. With Stark technique, that we have modified, we have obtained all these aims, improving at the same time patients' postoperative course as well succeeding to extract the fetus in about five minutes. In the '70 Cohen explained the utility of a transiliac incision allowing the access to abdominal cavity with rectus muscles unsticking in an area in which these muscles should present a less adhesiveness. METHODS: Since 1988 Stark has used Cohen's technique changing however uterus closing times, peritoneal membranes and abdominal walls. Our technique is different since we performed the incision according to Pfannestiel. RESULTS: The times are considerably reduced to 4.8 minutes for fetus extraction and in postoperative time the complications are drastically reduced too (infection, pain, hematoma, adhesions). CONCLUSIONS: Therefore we can surely say that this kind of technique can be used with success in all gynaecological surgery, in extrauterine pregnancies and adnexial tumefactions (not malignant). Personal opinion is that spinal anesthesia is the best analgesic technique.     

Modifications to the rate of wound contraction by allopurinol

The effect of allopurinol (50 mg/kg) on the rate of full thickness excisional wound contraction in the Hooded Lister rat was assessed by planimetric and histological examinations. Compared with control animals, those treated with allopurinol showed a significantly lower coefficient of wound area contraction for days 0-7 (p < 0.05) than those of control animals. Histologically, in the allopurinol treated wounds the granulation tissue was less cellular but appeared to contain more collagen. This inhibition by allopurinol of the contraction phase of wound healing associated with an increased quantity of granulation tissue suggests that mediation of the process may involve a complex interaction between the fibroblasts/myofibroblasts and free radicals.     

Proposal to establish a National Institute of Kidney and Urologic Diseases--a report of the National Kidney and Urologic Diseases Advisory Board

A trial to compare the effect of military clothing treated by high-pressure spray with permethrin or placebo on the incidence of malaria in Royal Thai Army troops was conducted in northeastern Thailand. Bioassays of treated clothing using laboratory-reared Anopheles dirus females showed permethrin remained in the treated fabric for up to 90 days. Both permethrin- and placebo-treated uniform shirts provided > 84% protection from biting An. dirus in laboratory bioassays for the duration of the study. In laboratory tests, knockdown of An. dirus exposed to permethrin-treated cloth fell to < 20% after 3 hand washes, despite the presence of 28.7-59.9% of the original dose of permethrin. The use of permethrin-treated uniforms without adjunct application of topical repellents did not reduce malaria in Thai troops in an operational setting where incidence during 6 months was as high as 412 cases/1,000 in spite of chemoprophylaxis and use of untreated bednets.     

National Board of Examiners in Psychology.

Proposes the development of a National Board of Examiners in Psychology (NBEP) to combat the problem of an oversupply of PhD psychologists in the job market. The NBEP would license psychologists and gather information on job opportunities. This information would be used to limit the number of candidates who passed the examination. Examining the Business Impact of Owner Commitment to Sustainability

The purpose of this study was to establish a research mechanism to investigate the impact of corporate commitment to sustainability on capital project planning and capital project performance. The research hypothesized that increasing commitment to sustainability leads to increasing sustainable project planning, and to better cost and schedule performances in large projects. Two sustainability indices, CSCI and SCPPI, were used to determine the level of corporate sustainability commitment and its impact on sustainability practices in capital project planning, and the impacts of corporate sustainability commitment and sustainable practices in project planning on cost and schedule performances, design changes, and safety compliances. Limited data collected from 17 Fortune 100 owner organizations suggested that a relationship between the two indices existed and that there are strong grounds supporting further development of such sustainability indices. Such indices can help multinational corporations better allocate resources and strike a balance between sustainable practices and financial bottom line.
