The authors have been developing a magnetically suspended centrifugal pump (MSCP). They have devised a pulsatile mode for the MSCP, which was generated by altering rotational speed. This article describes in vitro and in vivo studies with the pulsatile mode of the MSCP. Hemolysis tests were performed in two identical circuits to compare the nonpulsatile (NP) mode and the pulsatile (P) mode. In vivo studies were performed in sheep. First, biventricular assisted circulation was instituted in the left heart with the MSCP and in the right heart with the Biopump. The native heart was induced to ventricular fibrillation. Second, a left ventricular assisted circulation was instituted as the native heart was beating. An inflow cannula was inserted into the left atrium in one sheep and into the left ventricle in the other. The normalized indices of hemolysis of the NP and P groups were 0.0025 +/- 0.0018 g/100 L, and 0.0032 +/- 0.0024 g/100 L (N = 4, not significant). During ventricular fibrillation in the P mode, the pulse pressure was 14 mmHg (the rotational speed: 1,500 to 2,600 rpm). In a beating heart, at atrial withdrawal, the pulse pressure increased from 10 to 24 mmHg (2,100 +/- 500 rpm), while at ventricular withdrawal, it decreased from 17 to 40 mmHg (2,000 +/- 500 rpm) on P mode. The MSCP in pulsatile mode did not increase hemolysis. At ventricular withdrawal, it was easier to produce a pulsation than at atrial withdrawal. The pulsatile mode of the MSCP is applicable to a left ventricular assist system.  相似文献

To induce a pulsatile flow in a centrifugal pump, we developed a new device (pulsatile assist device for centrifugal pump: PADCP) using a new concept. This device consists of a flexible polyurethane tube with an air chamber which is connected to the arterial side of the centrifugal pump circuit directly. A mock circulation system was used for evaluation of this PADCP. Thirty to 40 mm Hg of pulse pressure was obtained under 3-6 L/min of flow rate. By increasing the driving pressure of the PADCP from 200 to 600 mm Hg in a mock system, 4-48 mm Hg of pulse pressure was gained accompanied by a decrease in pump flow and increased left atrial pressure. The decreased pump flow and increased left atrial pressure were recovered easily by increasing the flow rate of the centrifugal pump. Pressures at the proximal site of the PADCP were less than 500 mm Hg. The PADCP was useful to induce a pulsatile flow in a centrifugal pump.  相似文献

Hypoplastic pineal glands and decreased melatonin levels are found in sudden infant death syndrome. Excess oxygen radical production and inadequate radical quenching is suggested to cause oxidative stress in the brain of sudden infant death syndrome victims. Loss of intraventricular cerebrospinal fluid melatonin could play an important role in the disease process. Infants at risk might be protected by exogenous melatonin.  相似文献

A centrifugal pump with an impeller (Nikkiso Centrifugal Pump, Model HPM15; Nikkiso Co. Ltd.) was applied to cardiopulmonary bypass (CPB) in 14 patients who underwent elective coronary artery bypass grafting. Serum hemoglobin level, platelet count, and serum beta-thromboglobulin (beta TG) level were measured during CPB. The results were compared with those obtained in a comparative roller pump (RP) group (n = 10). There was no difference in the time on CPB between the NP (109 min) and RP (121 min) groups. The serum beta TG level (ng/ml) was lower in the NP group than in the RP group (obtained 90 min after the initiation of CPB). The plasma-free hemoglobin level also was lower in the NP group than in the RP group (obtained 90 min after the initiation of CPB, 120 min after the initiation of CPB, immediately after the termination of CPB, 3 h after termination of CPB; p < 0.01). There was no significant difference in platelet depletion. The HPM15 pump showed excellent hemodynamic performance with less blood trauma compared with the roller pump in its clinical application to open heart surgery.  相似文献

The perirhinal cortex (PRC) plays a major role in memory processes. This role may be influenced by activity in the adjacent entorhinal cortex (EC) and hippocampus (HPC), particularly during the processing of spatial information. In the current experiment we sought to determine whether the cholinergically mediated (type II) theta rhythm, which is a prominent electrophysiological feature of both HPC and EC activity, influenced neuronal firing in the PRC of urethane-anesthetized rats. When the spontaneous firing activity of single units recorded in PRC was related to theta recorded from the hippocampal fissure, it was determined that the firing of 50/163 (31%) PRC neurons exhibited a statistically significant phase relationship (mean phase angle = 188 degrees) to HPC theta. Thirty-three (66%) of these neurons tended to fire near the trough, and 17 near the peak, of this activity. These data indicate that a high proportion of PRC neurons participate in hippocampal-entorhinal theta activity. This activity may support information transmission and storage within and between these structures.  相似文献

Pulmonary embolism after cardiac surgery is attributed to embolization from thrombus within the deep venous system. We report two cases of pulmonary embolism after coronary artery bypass surgery in which transesophageal echocardiography detected in situ right atrial thrombus. The right atrium should be screened for thrombus in patients who have pulmonary embolism after cardiac surgery.  相似文献

The successful use of centrifugal pumps as temporary cardiac assist devices strongly depends on their degree of blood trauma. The mechanical stress loading experienced by cellular components on their passage through the pump is a major cause of blood trauma. Prediction of the mechanical stresses will assist optimization of pump design to minimize hemolysis and platelet activation. As a theoretical approach to this task., the determination of the complete three-dimensional (3D) flow field including all regions of high shear stress is therefore required. A computational fluid dynamics (CFD) software package, TASCflow, was used to model flow within a commercially available pump, the Aries Medical Isoflow Pump. This pump was selected in order to demonstrate the ability of the CFD software to handle complex impeller geometries. A turbulence model was included, and the Newtonian as well as the Reynolds stress tensor calculated for each nodal point. A novel aspect was the assignment of scalar stress values to streaklines representing particle paths through the pump. Scalar stress values were obtained by formulating a theory that enables the comparison of a three-dimensional state of stress with a uniaxial stress as applied in all mechanical blood damage tests. Stress loading-time functions for fluid particles passing inlet, impeller, and outlet domains of the pump were obtained. These showed that particles undergo a complex, irregularly fluctuating stress loading. Future blood damage theories would have to consider an unsteady stress loading regime that realistically reflects the flow conditions occurring within the pump.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

There is increasing evidence that adenosine (ADO) and dopamine (DA) interact directly in the basal ganglia via actions at ADO A2a and DA D2 receptors, respectively. The purpose of this study was to determine 1) the extent to which these receptors modulate endogenous GABA release in discrete regions of the rat basal ganglia and 2) whether GABA release is modulated by a direct and opposing interaction between ADO A2a and DA D2 receptors. Tissue slices of striatum (STR) containing globus pallidus (GP; STR/GP) and micropunches of STR, GP, and substantia nigra pars reticulata (SNr) were studied. Radioligand binding demonstrated that ADO A1, ADO A2a, and DA D2 receptors were present in each of the tissue preparations with the exception of SNr, in which ADO A2a receptors were not detected. Stimulation of ADO A2a receptors with CGS 21680 (1-10 nM) increased electrically stimulated GABA release in STR/GP slices and GP micropunches. Consistent with the lack of A2a receptors in SNr, CGS 21680 had no effect on GABA release from this region. In contrast, stimulation of DA D2 receptors with N-0437 (1-100 nM) inhibited evoked GABA release from STR/GP slices and both GP and SNr micropunches. The D2-mediated inhibition of GABA release in GP was abolished in the presence of CGS 21680 (10 nM). These experiments demonstrate that stimulation of ADO A2a and DA D2 receptors has opposing effects on endogenous GABA release in STR and GP. These opposing actions may explain the antagonistic interactions between ADO and DA that have been observed in behavioral studies and support the hypothesis that the striatopallidal efferent system is an important anatomical substrate for the A2a/D2 receptor interaction.  相似文献

A magnetically suspended centrifugal pump (MSCP) has been developed for long-term ventricular support. Effective torque to blood of the MSCP is exactly proportional to a motor current because of no friction inside the MSCP. The authors have devised new driving modes using these characteristics and applied it to a chronic animal experiment. Three driving modes were compared: 1) a constant rotational speed (N), 2) a constant motor current (I), and 3) a controlled motor current (CI). In two of nine sheep, the MSCPs were operated by the N and in seven by the I mode. The motor current and the rotational speed were always monitored. The CI mode was studied by altering the resistance of the vessels. In the I mode, the rotational speed varied depending upon the pressure head, and the slope of the pressure-flow (P-Q) relationship was steeper than that of the N mode, so that the pump flow was stabilized. In the CI mode, in which the motor current increased to compensate for the decrease in pump flow as the rotational speed increased, the P-Q slope was effectively controlled when the resistance was changed. The MSCP was able to control the P-Q slope without monitor of the pump flow. Various driving modes could be selected according to changes in resistance.  相似文献

Questions C. L. Olson's (see record 1976-25211-001) claim that the Pillai-Bartlett statistic (V) is superior to the Wilks λ (W) and the Hotelling-Lawley trace (T) for general use in multivariate ANOVA because of much greater robustness against unequal covariance matrices. It is shown by a sampling of studies from the literature that the example Olson used to demonstrate superiority had extreme subgroup variance differences, which occur very infrequently. For subgroup variance differences much more likely to occur, it is shown that the actual Type I error rates for V, T, and W are very similar. There are different opinions on the duration of post-operative shoulder immobilisation following arthroscopic repair of labrum tears. Between 1993 and 1994 at our department arthroscopic repairs of the glenoid labrum as described by Habermeyer (15, 16) have been performed in 38 patients. In 20 of these patients postoperative treatment included immobilisation of the operated shoulder in a Gilchrist-bandage for 3 weeks, while in the remaining 18 patients this postoperative immobilisation period was decreased to only 1 or 2 weeks. Physiotherapy started the day after the operation, using a device for passive-motion exercises. The mean follow-up time of our examinations was 15.4 month. The mean score by Rowe rose significantly from 20.0 preoperatively to 82.6 at the follow-up examination. Results were excellent or good in 89.4% of all cases, new shoulder dislocations did not occur in any case. Patients having their shoulders immobilised for 3 weeks reached significantly higher Rowe-scores at our follow-up than those patients, whose shoulders were immobilised for only 1 or 2 weeks. Therefore, postoperative management following arthroscopic repair of labrum tears should include immobilisation of the operate shoulder for 3 weeks and an early start of passive-motion exercises.  相似文献

A study was conducted to evaluate coronal microleakage of Super EBA and Ketac-Endo when used as sealers with single-cone gutta-percha (GP) root canal obturation. The root canals of 24 extracted human teeth were instrumented with flared preparations to a minimum #40 master apical file size. Ten teeth were obturated with a single GP cone and Super EBA as the sealer and ten teeth were obturated with a single GP cone and Ketac-Endo as the sealer. Four teeth were used as controls. Salivary bacterial microleakage studies were conducted to determine whether these sealers could prevent coronal microleakage through the root canal in the absence of a coronal tooth restoration. There was no bacterial penetration through the apical foramen for either sealer tested during the 60-day test period.  相似文献

Users of multivariate analysis of variance must choose which of several available test statistics to employ as a generalization of the usual univariate F statistic. A review of statistical literature concerning the power and robustness of the 4 most promising tests leads to the recommendation of K. C. Pillai's (1955) and M. S. Bartlett's (1939) trace statistic for general use. A survey of recent experimental reports revealed that psychologists have been using a 2nd best statistic and that they have frequently failed to specify their statistic to let readers judge its appropriateness. To facilitate increased use of the Pillai-Bartlett statistic, information is given concerning computation, the availability of significance tables, and a convenient F approximation. Since 1987, 33 patients have undergone surgery at Kobe University Hospital for aneurysm of the descending aorta using left heart bypass with a heparin-coated centrifugal pump and heparin-coated tubes. Sixteen patients had true aneurysms of the descending thoracic aorta, 7 had thoracoabdominal aneurysms, and 10 had aortic dissection (DeBakey's Type III). Heat exchangers and oxygenators were not included in the bypass circuit in any of the cases. Perfusion time was from 42 to 205 min (average 90 min). Left heart bypass was established with 1 mg/kg of systemic heparinization in 5 cases, 0.5 mg/kg in 5 cases, and 0 mg/kg in 23 cases. There were no complications such as perioperative embolism, acidosis, or hypothermia. During aortic cross-clamping, the arterial pressure of the lower extremity was maintained above 70 mm Hg, but there was no relationship between the distal perfusion pressure and bypass flow. The urine output during left heart bypass was related to the distal perfusion flow by centrifugal pump. Of 23 patients who underwent bypass with less than 40 ml/kg/min of distal perfusion flow, 7 showed transient renal dysfunction postoperatively, and 1 developed postoperative renal failure. The other patients who were bypassed with over 40 ml/kg/min of pump flow stayed in the normal range of renal function. Postoperative paresis occurred in 2 patients, who were also perfused with less than 40 ml/kg/min of bypass flow. It could be concluded that left heart bypass by centrifugal pump is safe and acceptable as a circulatory support in the surgical treatment of aneurysm of the descending aorta.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

One of the central results of recognition memory is the list-length effect (E. K. Strong, 1912); recognition accuracy decreases as the number of events to be memorized increases. Global-matching memory models (MINERVA 2 and search of associative memory [SAM]) predict that recognition accuracy decreases because the underlying theoretical distributions representing unstudied and studied items increasingly overlap as the list length increases because each additional item adds variability. Consequently, the ratio of the variances of these distributions should approach 1 (equal variance). Two experiments were conducted to test this prediction. Exp 1 used a multilist design and found that the variance ratios were equal and not different from 1 for short and long lists. Possible modifications explored within the SAM framework (adding a recall process to recognition and a continuous memory assumption) did not improve the model's ability to handle the data. In Exp 2, Ss received only a single list. There was a small but significant increase in the variance ratio as list length increased, in line with model predictions. In the developing forebrain, mounting evidence suggests that neural stem cell proliferation and differentiation is regulated by growth factors. In vitro in the presence of serum, stem cell proliferation is predominantly mediated by fibroblast growth factor-2 (FGF-2) whereas neuronal differentiation can be triggered by FGF-1 in association with a specific heparan sulphate proteoglycan. On the other hand, astrocyte differentiation in vivo and in vitro appears to be dependent on signalling through the leukaemia inhibitory factor receptor (LIFR). The evidence suggests that in the absence of LIFR signalling, the stem cell population is present at approximately the same frequency and can generate neurons but is blocked from producing astrocytes that express glial fibrillary acidic protein (GFAP) or have trophic functions. The block can be overcome by other growth factors such as BMP-2/4 or interferon-gamma, providing further evidence that the inhibition to astrocyte development does not result from loss of a precursor population. Signalling through the LIFR, in addition to stimulating astrocyte differentiation, may also inhibit neuronal differentiation, which may explain why this receptor is expressed at the earliest stages of neurogenesis. Another signalling system which also exerts its influence on neurogenesis through active inhibition is Delta-Notch. We show in vitro that at high cell densities which impede neuronal production by FGF-1, lowering the levels of expression of the receptor Notch by antisense oligonucleotide results in a significant increase in neuronal production. Thus, stem cell differentiation appears to be dependent on the outcome of interactions between a number of signalling pathways, some which promote specific lineages and some which inhibit.  相似文献

Premature infants have higher circulating concentrations of growth hormone (GH) than term infants. Previous investigations of these differences have used sampling frequencies of every 30 min with subsequent application of pulse detection algorithms, such as the CLUSTER program, to assess serum GH pulse parameters. To determine differences in GH secretory rates or GH t1/2 values between premature and term infants, we have sampled 11 neonates at 15-min intervals. We performed deconvolution analysis of the resultant plasma GH values to estimate GH secretory and clearance parameters. Five premature infants (gestational age range 24-34 wk) and six term infants (gestational age range 38-42 wk) were sampled every 15 min for 6 h. All subjects had indwelling arterial catheters. GH was measured (in duplicate) by RIA using 10 microL of plasma. Premature infants had higher secretory burst amplitudes (2.2 +/- 0.13 micrograms/L/min versus 1.4 +/- 0.27 micrograms/L/min, p = 0.02), higher production rates (product of the total number of bursts and the mean mass of GH secreted per burst, 811 +/- 173 micrograms/L/6 h versus 283 +/- 77 micrograms/L/6 h, p = 0.03), and a higher mass of GH per secretory burst (106 +/- 25 micrograms/L versus 38 +/- 11 micrograms/L, p = 0.049) than term infants. The tested hypothesis was that ultrasound-induced hemolysis in blood supplemented with a microbubble contrast agent varies with ultrasound intensity and pulse duration. Human erythrocytes in autologous plasma containing 3.6% v:v Albunex microspheres were exposed to 1.07-MHz ultrasound pulses of 5 to 1000 mus at SPTP intensities of 0 to 1100 W/cm2. The dependence of hemolysis on the mechanical index (MI) value of the exposures was also examined. Ultrasound-induced hemolysis: (1) was evident at all pulse/intensity combinations; (2) increased generally with increasing pulse duration at constant intensity; and (3) increased with increasing MI at constant pulse duration. For pulses of 10 to 30 mus, ultrasound-induced hemolysis remained low (< or = 2%) at MI values < approximately 2 and increased sharply with further increase in MI; for 5-mus pulses, this abrupt increase in hemolysis was associated with a larger MI (approximately 3).  相似文献
