AOTA and managed care

The process of becoming a medical doctor in the Russian Federation is detailed in this paper. There has been a decline in the number of students entering the medical profession, as well as a marked decrease in the faculty members at the medical institutes since perestroika. This is secondary to a marked decrease in financial support as well as falling morale.     

Hospice and managed care

Carbon dioxide (CO2) lasers are capable of producing surface irregularities in human enamel which resemble those resulting from etching of enamel with orthophosphoric acid. This report presents the results of a laboratory study which examined the effect of selected tooth-related variables on shear bond strength between a current generation bonding agent (Scotchbond MultiPurpose) and acid-etched or laser-conditioned human enamel. There were no significant differences in shear bond strength between human maxillary central incisor teeth, first premolars, and third molars in either the acid etch or laser-conditioned groups. Polishing of enamel to give a flat surface increased the shear bond strength obtained with acid etching, but did not alter significantly bond strengths achieved with laser conditioning. There was a tendency for a higher bond strength with acid etching, but not with laser conditioning, in porcine molar enamel compared with human molar and bovine incisor enamel. In terms of the mode of operation of the laser, the repetitively pulsed mode resulted in a two-fold improvement in shear bond strength compared with the single pulse mode. These results indicate that shear bond strengths in the order of 10 MPa can be obtained reliably on human teeth using laser conditioning with pulsed modes in the absence of any other preparation of the natural enamel surface.     

Medicaid managed care and the marketplace

In 1994 and 1995, Tennessee, Hawaii, Rhode Island, and Oklahoma began massive expansions of Medicaid managed care, growing from three health plans covering a few enrollees to 27 plans covering the great majority a year later. Some firms aggressively pursued expansion, while others had very limited business objectives. Although established insurers often dominated the Medicaid markets, newly developed firms, some provider-sponsored, were also important. Despite the relatively low Medicaid capitation rates in the 1996-97 period, Medicaid plans in three states had an average 1% net profit margin.     

Understanding the managed care backlash

This paper examines the depth and breadth of the public backlash against managed care and the reasons for it. We conclude that the backlash is real and influenced by at least two principal factors: (1) A significant proportion of Americans report problems with managed care plans; and (2) the public perceives threatening and dramatic events in managed care that have been experienced by just a few. In addition, public concern is driven by fear that regardless of how well their plans perform today, care might not be available or paid for when they are very sick.     

The primary care physician and managed care

I have explored the major options open to PCPs. There are multiple variations on these themes. While the Connecticut market is fluid, one thing is clear, the solutions will vary from place to place. Health-care reorganization is a local phenomenon. Individual doctor groups can have a major influence on the direction of their local market. The future is uncertain, but whatever happens in Connecticut it will be evolutionary. PCPs must understand the realities of the current marketplace and plan a strategy to work with those realities to achieve their long-term goals. The choices we make today will have ramifications flowing far into future decades. PCPs and their patients will have to live with the consequences of these decisions. Please choose wisely.     

Getting at the management of care within managed care

The psychotherapist and the managed care setting.

This study represents a beginning effort to understand how alternative health care delivery systems affect psychotherapists and psychotherapy practice style. Professionals from various mental health disciplines employed in staff-model health maintenance organizations (N?=?43) served as interviewees. Therapists answered objective and semistructured questions about their work setting, "burn-out," graduate training, and the evolution of self and practice style. Although methodological limitations prohibit meaningful and valid generalizations, the results suggest that alternative health care settings may serve as catalysts by creating an atmosphere that shapes and influences the therapist to develop more eclectic, short-term, problem-focused psychotherapy practice habits. Legal liability and managed care.

In attempts to reduce the cost of mental health treatment, health insurers are turning to mechanisms for intensive oversight that fall under the rubric of managed care. These approaches have implications for the potential liability of clinicians and managed care entities. Clinicians may be confronted with legal duties to appeal adverse decisions, to disclose the impact of managed care on patients' treatment, and in some circumstances, to continue treatment after payment has been denied. Managed care entities are being held to duties to conduct review in a reasonable fashion, and may also have a duty to disclose the limitations managed care may place on patients' access to treatment, and to select appropriate providers of care.
