ECG-gated dual-isotope acquisition protocol involving rest imaging with 201Tl and stress 99mTc-tetrofosmin (TF) SPECT was designed for the simultaneous assessment of rest/stress myocardial perfusion and rest ventricular systolic function. This study assessed the feasibility and diagnostic accuracy of this protocol. Forty-five patients underwent the dual-isotope SPECT protocol. Twenty minutes after resting injection of 111 MBq of 201Tl, 370 MBq of 99mTc-TF was administered at a peak exercise. The dual-isotope gated SPECT acquisition was performed 1 hour later. Then, the regional count increase rate (%WT) of 99mTc-TF from end-diastole end-systole was calculated using an automated method which was developed for quantification of regional wall thickening based on circumferential profile analysis in our laboratory. Myocardial perfusion and contractility analysis was carried out using 8 segments of left ventricle with comparison of coronary angiographical findings. The sensitivity and specificity for the detection of diseased coronary vessels (> = or 75% stenosis) were 76% and 94%, respectively. Infarcted regions showing reversible defect had significantly greater %WT as compared with those with fixed defects (63 seg; 12.7 +/- 6.1% vs. 36 seg; 8.9 +/- 7.2%, p < 0.01). In conclusion, this dual-isotope protocol has some advantages; i.e., shortening an examination time, having the exact registration of stress/rest perfusion, and simultaneous evaluation of resting regional wall thickening.  相似文献

A patient with seizures and a contrast-enhancing temporal lesion after radiation therapy for a chondrosarcoma of the nasal septum is described. To differentiate between radiation necrosis and recurrent tumor, thallium-201 (201Tl) SPECT was used. 201Tl SPECT revealed high local accumulation suggesting tumor growth; however, pathologic examination demonstrated focal necrosis with reactive changes but without tumor. The 201Tl SPECT findings in this patient demonstrate a possible diagnostic pitfall in differentiating recurrent tumor from radiation necrosis.  相似文献

We studied the relationship between 201Tl uptake and the efficacy of radioiodine therapy in thyroid carcinoma. METHODS: Forty-four patients with metastases of well-differentiated thyroid carcinoma received 201Tl scintigraphy within the 2 mo before their initial 131I therapy. Patients were classified into two groups according to the tumor-to-background (T/B) ratio on the late 201Tl scan: high 201Tl uptake (T/B > or = 2.1) and low 201Tl uptake (T/B < 2.1). The therapeutic outcome was judged by the percent reduction in the tumor diameter at 6 mo after the treatment. The treatment was defined as effective when the tumor showed more than 50% reduction in the tumor diameter. The patients in whom radioiodine was ineffective were followed up to determine if the tumor showed further growth. RESULTS: Of the 44 patients, 25 had high 201Tl uptake and 19 had low 201Tl uptake. The therapy was effective in 15 patients and was ineffective in 29. All the patients in whom radioiodine was effective had low 201Tl uptake. On the other hand, 25 of 29 patients in whom radioiodine was ineffective had high 201Tl uptake. Eight patients, in whom radioiodine was ineffective despite good 131I uptake, had high 201Tl uptake. There were no significant differences in the positive predictive value and the negative predictive value for effective treatment between 201Tl scintigraphy and therapeutic dose 131I scintigraphy. Among the 25 patients in whom radioiodine was ineffective and who had high 201Tl uptake, the tumor diameter increased in 7 (28%). However, none of the tumors with low 201Tl uptake increased in size during the follow-up period. CONCLUSION: Thallium-201 scintigraphy has a high predictive value for the efficacy of radioiodine therapy in metastatic thyroid carcinoma. Thus, it is helpful in determining the indication for radioiodine therapy and it seems to be an adjunct to tracer dose 131I scintigraphy.  相似文献

We prospectively investigated 200 patients with the clinical suspicion for head and neck tumors. The final diagnoses were 94 primary and 56 (37 confirmed, 19 excluded) recurrent squamous cell carcinomas (SCCs), 3 primary and 7 (4 confirmed, 3 excluded) recurrent adenoid cystic carcinomas (ACCs), 6 non-Hodgkin's lymphomas, 10 distant metastases, 6 other malignancies, 10 inflammatory and 8 other nonmalignant conditions. METHODS: Bone (600 MBq 99mTc-3,3-diphosphono-1,2-propane dicarboxylic acid tetrasodium salt) and hexakis-2-methoxyisobutyl isonitrile (MIBI) (600 MBq 99mTc-MIBI) SPECT were both performed under identical conditions (triple-head gamma camera; ultra-high-resolution, parallel-hole collimators; three-dimensional postfiltering) and judged independently and after superimposition. The results were compared to the results of biopsy, surgery and CT. RESULTS: The overall sensitivity/specificity of MIBI was 90%/78% for tumor detection and 90%/95% for the identification of malignant lymph node involvement (CT: 79%/66%, respectively 90%/79%). In the subgroup of recurrent SCC and ACC the sensitivity/specificity for tumor detection was 95%/71% for MIBI versus 78%/68% for CT. The isolated assessment of bone SPECT had a sensitivity/specificity of 100%/17% for osseous tumor spread. Image fusion of MIBI and bone SPECT differentiated between regio-local bone involvement and inflammatory changes and increased the specificity of bone SPECT to 100% in primary staging. Tumor size, stage, histology and pretreatment had no statistically significant effect on tracer uptake or diagnostic utility of scintigraphy. CONCLUSION: We propose the combined 99mTc-MIBI and bone ultra-high resolution SPECT as a highly useful imaging approach in the primary and secondary staging in patients with suspected malignancies in the head and neck region. The high specificity for malignancies in the head and neck region may be used in the differential diagnosis between head and neck malignancies and inflammatory disease in patients with the accidental finding of enlarged lymph nodes and no clinical signs of a primary tumor. Image fusion with bone scanning is mandatory for the topographical orientation and increases the specificity of bone scanning to differentiate between inflammatory or malignant causes of increased bone metabolism.  相似文献

SPECT using Tc-99m MIBI was performed in patients with brain tumors in order to determine Tc-99m MIBI uptake in brain tumors and to evaluate the efficacy of MIBI imaging in the assessment of treatment response. A MIBI uptake index was calculated on SPECT imaging. All tumors with a MIBI-index higher than 7.5 were malignant. In the tumors evaluated before and after radiochemotherapy, the MIBI-index immediately following therapy correlated with treatment response 2 months after therapy. Thus, semiquantitative assessment using Tc-99m MIBI SPECT imaging may be useful in the evaluation of treatment response.  相似文献

As several reinjection procedures have shown encouraging results in terms of imaging, we investigated whether the kinetics of thallium-201 would differ between the standard stress-redistribution-reinjection approach and the stress-immediate reinjection approach. In 53 consecutive patients with undiagnosed chest pain, 75 MBq (2 mCi) 201Tl was injected at maximal exercise. In 26 of these patients (group I), 37 MBq (1 mCi) 201Tl was reinjected immediately after completing the exercise images (the immediate reinjection procedure) and in 27 patients (group II), 37 MBq (1 mCi) 201Tl was reinjected after completing 3-h redistribution images (the standard reinjection procedure). Mean peak 201Tl blood activity after exercise was 17.7+/-12.5 kBq/ml (4.8+/-3.4 mCi/ml) for group I versus 16.4+/-9.2 kBq/ml (4.4+/-2.5 mCi/ml) for group II (NS). The relative increase in 201Tl blood activity after reinjection of half the initial dose [37 MBq (1 mCi)] exceeded 50% of the initial peak in both groups. The relative amount of 201Tl delivered to the myocardium was assessed by the area under the curve after both exercise and reinjection, and was 117%+/-72% for group I and 112%+/-73% for group II (NS). Blood clearance of 201Tl was at least biexponential. Mean early decay constants (lambda 1) after exercise and reinjection were 0.30+/-0.18 min-1 and 0.22+/-0.046 min-1 respectively for group I (T 1/2 2.3 min and 3.2 min respectively, NS), and 0.30+/-0.12 min-1 and 0.24+/-0.07 min-1 respectively for group II (T1/2 2.3 min and 2.9 min respectively, NS). For both procedures no significant differences were found between lambda 1 after exercise and lambda 1 after injection. The mean late clearance (lambda 2) from the blood was 0.032+/-0.056 min-1 and 0.012+/-0.012 min-1 respectively for group I (T1/2 21.6 min and 57.7 min respectively, NS), and 0.036+/-0.030 min-1 and 0.014+/-0.014 min-1 respectively for group II (T1/2 19.3 min and 49.5 min respectively, NS). Also, no significant differences were found between lambda 2 after exercise for both groups and between lambda 2 after reinjection for both groups. We conclude that reinjection of 37 MBq (1 mCi) 201Tl (half the initial dose) results in a relative increase in the initial peak and a relative increase in the amount of 201Tl delivered to the myocardium of more than 50% for both the standard and the immediate reinjection procedure. The clearance of 201Tl from the blood was not influenced by exercise or by the time of reinjection. Based on 201Tl kinetics as measured in the peripheral blood, there is no reason to postpone reinjection until 3-4 h following exercise.  相似文献

This study was designed to evaluate the feasibility of assessing myocardial viability using glucose loading followed by 201Tl SPECT. METHODS: First, the effect of insulin on the kinetics of 201Tl uptake was evaluated in isolated perfused rat hearts. Second, glucose-loading 201Tl myocardial SPECT was performed in 13 nondiabetic patients with histories of anterior myocardial infarction. Thirty minutes before acquiring rest 201Tl SPECT, 20 g of glucose were intravenously injected into the fasting subjects. Thallium perfusion defects were compared to those of conventional rest-redistribution SPECT images obtained within a 2-wk interval. SPECT images were divided into 21 segments, and a defect score in 17 segments was calculated as a sum of the semiquantitative defect scores (0 = normal; 1 = mildly decreased uptake; 2 = severely decreased uptake; 3 = absence of uptake). RESULTS: Thallium-201 uptake in isolated hearts showed a significant increase of 26% after insulin loading. Eleven (24%) of 45 segments showing perfusion defects on the conventional rest SPECT images demonstrated 201Tl uptake on glucose-loading SPECT imaging. Defect scores decreased significantly on the glucose-loading SPECT images (9.9 +/- 2.2 in early images; mean +/- s.e.) compared with the conventional rest-redistribution SPECT images (12.6 +/- 6.9 in delayed images, p < 0.05). CONCLUSION: Glucose-loading SPECT represents a superior method for assessing myocardial viability using 201Tl.  相似文献

AIM: It was the of this study to assess the value of the 99mTc-MIBI scintigraphy in SPECT technique in the preoperative lateral location of small parathyroid adenoma (PTA). METHODS: 25 consecutive patients (8 male, 17 female, mean age 63 +/- 13 years) with the established diagnosis of primary hyperparathyroidism and non-diagnostic ultrasonography were scanned preoperatively. After a thyroid examination to exclude radionuclide accumulating thyroid adenoma, planar and tomographic images were acquired 20 min. and 120 min. after i.v. injection of 740 MBq 99mTc-MIBI using a 3 head gamma camera (Picker Prism 3000). The first 10 patients underwent an additional 201Tl/99mTc subtraction scintigraphy in a 2 days protocol. RESULTS: All patients had small, solitary PTA (< 1 g). 201Tl/99mTc subtraction scintigraphy (n = 10) showed only a sensitivity of 50%. Using planar MIBI-scintigraphy lateral location of PTA was possible in 18 cases (sensitivity: 72%). There was an increase in sensitivity up to 96% using the SPECT technique and the 3D display (volume-rendered reprojection). CONCLUSION: In contrast to 201Tl/99mTc subtraction scintigraphy, which showed only a low sensitivity, a reliable lateral location of small PTA was obtained using the tomographic 99mTc-MIBI scintigraphy. This method offers e.g. the possibility for the surgeon to perform an unilateral parathyroidectomy.  相似文献

We present a simultaneous gated SPECT (G-SPECT) dual-isotope technique using 201Tl for perfusion and 99mTc blood-pool labeling for function imaging. METHODS: Seventeen patients (13 with previous myocardial infarction, MI) and a control group of three normal volunteers were investigated. They received, 15 min after a 201Tl stress/redistribution protocol with reinjection, 900-950 MBq 99mTc-HSA for blood-pool labeling. Eight frames per R-R interval were recorded in the G-SPECT mode with three windows: window A with 20% centered at 71 keV for 201Tl, window B with 10% centered at 105 keV for Tc scatter contamination and window C centered at 140 keV with 20% for 99mTc. Nongated, crosstalk-corrected 201Tl SPECT perfusion images were reconstructed according to normalized projection-by-projection subtraction from data from windows A and B. G-SPECT data from window C were reconstructed with the same reconstruction limits to allow topographic correlations of left ventricular perfusion and wall motion abnormalities. Polar maps of perfusion and function were used to divide the myocardium into 20 segments. Perfusion was expressed as the percentage of thallium uptake and function corresponding to diastolic to systolic shortening normalized by end diastolic volume. RESULTS: Segmental comparison of uncontaminated-to-contaminated and corrected 201Tl patient images demonstrated an overall agreement score of 93%, with a kappa statistic of 0.76 +/- 0.06 when normal perfused segments were excluded. Segmental matching of perfusion against function at rest showed no correlation for the 10 patients with preserved ejection fraction of 59% +/- 8% nor for the control group. For the remaining seven patients with an ejection fraction of 34% +/- 10%, there was linear correlation between perfusion and function (r2 = 0.61). CONCLUSIONS: The feasibility of dual TI-Tc G-SPECT was examined at rest and suggests low perfusion hypokinesis that matches linear dependence for CAD patients with low ejection fraction.  相似文献

BACKGROUND: The utility of contrast MRI for assessing myocardial viability in stable coronary artery disease (CAD) with left ventricular dysfunction is uncertain. We therefore performed cine and contrast MRI in 24 stable patients with CAD and regional contractile abnormalities and compared MRI findings with rest-redistribution 201Tl imaging and dobutamine echocardiography. METHODS AND RESULTS: Delayed MRI contrast enhancement patterns were examined from 3 to 15 minutes after injection of 0.1 mmol/kg IV gadolinium diethylenetriamine pentaacetic acid (Gd-DTPA). Comparable MRI and 201Tl basal and midventricular short-axis images were subdivided into 6 segments. By means of ECG gating of tomographic (SPECT) 99mTc-sestamibi (MIBI) images, myocardial perfusion and wall thickening have been evaluated after a single tracer injection. To determine if left ventricular ejection fraction (LVEF) can also be measured from gated MIBI SPECT, 30 patients 1 wk to 6 mo after myocardial infarction (MI) received 22-30 mCi 99mTc-MIBI during treadmill exercise. Eight frame per cardiac cycle gated MIBI 180 degrees SPECT was performed 60 min thereafter. Using 6.4-mm thick mid-ventricular vertical and horizontal long-axis slices from R-wave triggered end-diastolic and end-systolic frames, two independent observers manually drew endocardial borders at a count level of 34% of the maximum. LVEF was computed by the Simpson's rule method, corrected for the average point spread function of the SPECT camera. Results were correlated with LVEF determined from planar gated 99mTc-blood-pool studies performed within 4 days. LVEFs calculated from gated MIBI SPECT ranged from 0.21 to 0.73 and correlated linearly with gated blood-pool values (correlation coefficients ranged from 0.79 to 0.88; interobserver variability r = 0.75; intraobserver reproducibility r = 0.75). We conclude that in patients with MI resting LVEF can be determined from gated MIBI SPECT, thereby considerably augmenting the technique's diagnostic and prognostic value.  相似文献

Seventy-six patients with thyroid nodules were studied. Initially, 74 MBq of thallium-201 was injected. The thyroid gland was imaged 15 min (early) and 3 h (delayed) after the injection. Thereafter, 185 MBq technetium-99m pertechnetate was injected. Immediately after the injection, a 1-min perfusion image was acquired, followed by an image at 20 min. Increased early and delayed 201Tl uptake compared with the contralateral thyroid tissue was adopted as the criterion for malignancy. Sensitivity, specificity and negative predictive values were found to be 85%, 64% and 78%, respectively, in operated patients, but these values were 86%, 87% and 95%, respectively, in the whole group, including patients followed with fine-needle aspiration biopsy. With the purpose of investigating the relationship between perfusion and early 201Tl uptake, both perfusion and early images were graded comparing nodular activity with contralateral thyroid activity. There was a poor correlation between perfusion and 201Tl uptake. The correlation was even worse in hyperactive nodules. It is concluded that early and delayed 201Tl imaging should not be used in the differential diagnosis of cold nodules and that early 201Tl uptake seems to be more closely related to factors other than perfusion.  相似文献

Thallium-201 (201Tl) late reinjection after stress-redistribution imaging improves the detection of viable myocardium. Recently, early reinjection of 201Tl immediately after stress imaging was proposed as a new method for distinguishing ischemic myocardium, hibernating myocardium, and myocardial scar. However, there are no data on the influence of the timing of reinjection on "fill-in." This study was designed to assess whether the reinjection time influences "fill-in" in chronic coronary artery disease. Thirty-three patients with chronic coronary artery disease were studied. All patients underwent exercise 201Tl tomography. Immediately after stress imaging, 37 MBq of thallium was reinjected earlier than usual and early reinjection delayed image (ERDI) was acquired 3 h later. With the same protocol, all patients also underwent a second study involving late reinjection of 201Tl within 1 week. An additional 37 MBq of thallium was reinjected 3 h after stress imaging, and late reinjection delayed image (LRDI) was obtained 10 min later. All images were analyzed qualitatively using a 4-point grading uptake score. Of the 72 hypoperfused segments on stress images, 66 segments showed fill-in and 6 showed persistent defects on ERDI, and of the same 72 segments 55 segments displayed fill-in and the remaining 17 showed persistent defects on LRDI (p<0.05). The delta uptake score (the uptake score of the delayed image minus that of stress image) in early reinjection was 1.60+/-0.80, which was significantly higher than that in late reinjection (1.24+/-0.94, p<0.01). A small dose of thallium reinjected immediately after stress imaging with delayed images obtained 3 h later is convenient and might provide another technique for determining myocardial viability.  相似文献

There is increasing interest in the literature regarding the use of SPECT (single photon emission computed tomography) with 201Tl in the evaluation of cerebral tumours. Thallium (201Tl) is one of the isotopes most commonly used in studies of the myocardium and in the diagnosis of various tumours. Normal brain takes up very little 201Tl, but in viable tumour tissue the 201Tl becomes intracellular and is rapidly cleared from the blood stream. Such cerebral tumour uptake is related to changes in the permeability of the blood-brain barrier, regional blood flow and transport by means of the Na(+)-K+ adenosine triphosphate pump. Once the SPECT images have been obtained following administration of the radioisotope, one can calculate the indices of early and of late uptake together with the retention index. The usefulness of SPECT in the preoperative detection of malignant expansive pathology, choice of tumour region during stereotaxic biopsy, control of tumour reproduction and their differentiation from post-radiotherapy necrotic tissue, and evaluation of chemotherapy in treatment of cerebral tumours are analyzed. Finally the application of SPECT in metastatic conditions and the differential diagnosis of expansive lesions is considered.  相似文献

We performed 201Tl single photon emission CT (SPECT) studies on 62 patients with brain tumors prior to any therapy for the tumor. Ten lesions of open-scar and 17 patients without brain tumor were also studied. The SPECT imaging were initiated 5 min (early image) and 4 hours (delayed image) after injection with 74 MBq of 201Tl-chloride. For a semi-quantitative analysis, we used the ratio of delayed count density to the early count density of the tumor (Td/Te), the ratio of tumor to normal brain tissue (Te/B(e)) on the early SPECT and the index of T/B ((Td/Te)/(Bd/B(e))). Delay to early ratio of 34 normal brain tissue (Bd/B(e)) was 1.22 +/- 0.15 and tended to decrease with the age. The Td/Te ratio of meningioma and region of operation scar was about 0.75 which was less than that of normal brain tissue. In 31 patients with astrocytic tumor, Td/Te ratio was clearly higher in poorly differentiated tumor than in well differentiated one. The index of Td/Te ratio was superior to Te/B(e) or T/B ratio for differentiating brain tumor and grading astrocytic tumor.  相似文献

The purpose of this study is to determine whether left ventricular dysfunction following coronary artery spasm by 123I-BMIPP myocardial imaging. To reveal the clinical efficacy of 123I-BMIPP SPECT, 20 patients with vasospastic angina were studied using resting, 3-hour delayed image with 123I-BMIPP and exercise, 3-hour delayed image with 201Tl SPECT. 123I-BMIPP uptake was decreased compared to 201Tl (discordant) in 12 patients (60%) and in 49/100 myocardial segments (49%). The extent and severity score in resting image with 123I-BMIPP were significantly larger than that in delayed image with 201Tl (p < 0.01). In 123I-BMIPP SPECT, the severity score in the latest ischemia were significantly larger than that in others. The incidence of a complete agreement of decreased 123I-BMIPP uptake and coronary artery spasm was significantly higher (75%) than that in 201Tl (28%, p < 0.01). Furthermore, compared to 201Tl uptake, decreased 123I-BMIPP uptake much more corresponded to reduced wall motion in 9 of patients with mismatching. The severity of regional wall motion abnormality was significantly correlated with severity score of 123I-BMIPP. Late redistribution in delayed image with 123I-BMIPP was seen in 6 patients. The regional washout rate and the severity of regional wall motion abnormality in 6 patients was significantly lower than that in others (p < 0.05). Thus, metabolic abnormality assessed by 123I-BMIPP is well associated with left ventricular asynergy and spastic region in patients with vasospastic angina. In conclusion, 123I-BMIPP SPECT may sensitively delineate the impaired myocardium following coronary artery spasm, and it is very useful in diagnosing and estimating the severity of vasospastic angina.  相似文献

123I-BMIPP [15-(p-iodophenyl)-3-(R,S)-methylpentadecanoic acid] was developed for metabolic imaging with SPECT. A multicenter collaborative study was conducted on a large patient series to determine whether 123I-BMIPP and 201Tl myocardial SPECT are of use in predicting the prognosis and ventricular function after acute myocardial infarction (AMI). Patients with uncomplicated first AMI underwent resting 123I-BMIPP and 201Tl myocardial SPECT in the subacute phase after the onset of AMI. Of these, 167 patients who had been followed up for an average of 22 months were retrospectively reviewed to predict serious cardiac events and recurrent ischemia. In addition, the association between changes in radionuclide parameters and recurrent ischemia was investigated in Subgroup A (58 patients) who had repeated SPECT in the chronic phase. Furthermore, prediction of the ejection fraction (EF) was investigated in Subgroup B (94 patients) and Subgroup C (76 patients) in whom left ventriculography was performed at the time of discharge and 90 days or more after the onset, respectively. The prognosis was generally favorable, with 4 cases of cardiac death (2%), 3 of heart failure (2%), 4 of nonfatal reMI (2%), and 25 of recurrent ischemia (15%). The results of Cox multivariate regression analysis revealed a high probability of serious cardiac events in patients who were elderly (p = 0.04), who had 90% or more residual stenosis of the infarct-related artery (p = 0.09), and who had a high BMIPP defect score (p = 0.17). There was a high probability of recurrent ischemia in elderly patients (p = 0.10) who had multi-vessel disease (p = 0.03), but no association was found with radionuclide parameters in the subacute phase. In Subgroup A, however, the probability of recurrent ischemia tended to be high in patients with a large mismatch scorebetween 123I-BMIPP and 201Tl in the subacute to chronic phase. An important observation was that the extent of BMIPP defect was more strongly correlated with EF at the time of discharge and 90 days or more after the onset than the extent of Tl defect (r = -0.60 vs. r = -0.47, and r = -0.53 vs. r = -0.43, respectively). In addition, multiple regression analysis showed that parameters related to the BMIPP defect were also better predictive factors of EF both at the time of discharge and 90 days or more after the onset. In conclusion, resting 123I-BMIPP and 201Tl myocardial SPECT performed in the subacute phase of AMI were shown to be useful in predicting prognosis and ventricular function for patient management.  相似文献

Thallium-201 single-photon emission computerized tomography (SPECT) was used to clarify the relationship between 201Tl uptake and the response in chemotherapy to platinum compounds in 21 patients with small-cell lung cancer. 201Tl-SPECT scans were obtained twice: at 15 min (early scan) and 120 min (delayed scan) after an intravenous injection of 111 MBq (3 mCi) of thallium-201 chloride. We obtained the uptake ratio from each scan and calculated the retention index:uptake ratio = region of interest uptake/contralateral normal lung uptake; retention index = (delayed ratio - early ratio)/early ratio. After 201Tl scintigraphy, 12 patients received chemotherapy consisting of platinum compounds and nine were treated with chemoradiation. Among patients receiving only chemotherapy, the retention index correlated with the responses to chemotherapy. In an in vitro study, ouabain, an inhibitor of the Na,K-ATPase pump, reduced sensitivity to cisplatin and inhibited intracellular thallium uptake in the small-cell lung cancer cell line. These studies suggest that 201Tl-SPECT is a useful indicator of response to chemotherapy with platinum compounds in small-cell lung cancer, and that Na,K-ATPase is commonly involved in transporting both thallium and platinum compounds into cancer cells.  相似文献

Various radiopharmaceuticals for breast cancer detection have been used for scintimammography and PET. However, few comparative studies have described the uptake of radiopharmaceuticals as a method of detecting breast cancer. The aim of this study was to assess the radiopharmaceuticals for breast cancer imaging in experimental mice implanted with breast cancer cells. METHODS: Six radiopharmaceuticals were studied: three for PET [18F-fluorodeoxyglucose (FDG), L-18F-alpha-methyltyrosine (FMT) and 11C-methionine (C-Met)] and three for scintimammography [99mTc-tetrofosmin (TF), 99mTc-sestamibi (MIBI) and 201Tl-chloride (Tl)]. Biodistributions of six different tracers in mice implanted with MCF-7 breast cancer cells were studied 1 and 3 hr after injection. RESULTS: Tumor uptake 1 hr after injection was FMT = C-Met > FDG = TF > MIBI = Tl. Thallium-201-chloride showed the highest tumor-to-blood ratio (T/B) among all radiopharmaceuticals because of its fast clearance from circulation. The T/B of the six radionuclides used in this study ranged from 1.26 for C-Met to 12.83 for Tl. Tumor-to-muscle ratio (T/M) revealed FMT = C-Met > FDG > MIBI > TF = Tl. The T/M ranged from 0.20 for TF to 2.29 for FMT. Tumor-to-lung ratio (T/L) varied from 0.45 for TF to 2.41 for FMT. FMT revealed the highest T/L of all six radiopharmaceuticals. CONCLUSION: Among radiopharmaceuticals for PET, FMT seemed to be suitable in detecting MCF-7 tumor; whereas for scintimammography, MIBI, TF and Tl appeared to have almost the same detectability of MCF-7 tumor. The results of this study strongly suggest that FMT may have a potential in breast cancer imaging.  相似文献

Technetium-99m-tetrofosmin uptake was compared to that of 201Tl in the setting of low flow and systolic dysfunction. METHODS: In nine open-chested dogs, a severe left anterior descending (LAD) coronary artery stenosis resulted in a 54.3% mean flow reduction and decreased left ventricular thickening from 21% +/- 1% to -3 +/- 2%. After 30 min, 37 MBq (1 mCi) of 201Tl and microspheres were injected and initial and 2-hr redistribution images acquired. Two hours later, 370 MBq (10 mCi) of 99mTc-tetrofosmin and microspheres were injected and an image was obtained. LAD: left circumflex (LCX) count ratios for both tracers and flows were calculated by well counting postmortem, and 201Tl and 99mTc-tetrofosmin defect magnitudes were determined by quantitative image analysis. RESULTS: LAD:LCx flow ratios were similar during 201Tl and 99mTc-tetrofosmin injections (0.48 +/- 0.04 versus 0.49 +/- 0.05, p = n.s.). Final 201Tl activity (0.66 +/- 0.04) was significantly higher than 99mTc-tetrofosmin (0.55 +/- 0.05; p < 0.05). LAD/LCx 99mTc-tetrofosmin image defect count ratio was similar to 201Tl defect count ratio on the initial rest 201Tl scan (0.57 +/- 0.03 versus 0.56 +/- 0.02, p = ns), but significantly less than 201Tl defect count ratio at 2 hr (0.57 +/- 0.03 versus 0.65 +/- 0.02, p < 0.05). CONCLUSION: In a low-flow model with profound systolic dysfunction, myocardial 99mTc-tetrofosmin uptake ( > 50%) reflective of viability was observed in the asynergic zone perfused by the stenotic LAD.  相似文献
