This study represents the first systematic analysis of the major treatment approaches used for selective mutism with the goal of drawing conclusions about selecting evidence-based procedures for practice. Based on nonparametric statistical tests of effect sizes, the major findings include the following: (a) treatment of selective mutism is more effective than no treatment, (b) behaviorally oriented treatment approaches are more effective than no treatment, and (c) no differential effectiveness was found between two common models of behavior therapy. In addition, researchers have given minimal attention to the impact of selective mutism on educational performance. In this study, which reviewed existing research, information regarding academic achievement was evaluated qualitatively. Recommendations are made for improving the methodological quality of future research on selective mutism within the context of research on evidence-based interventions. Implications for practice in school psychology are presented. (PsycINFO Database Record (c) 2011 APA, all rights reserved)  相似文献

This article reviews the current state of evidence for the psychopharmacological treatment of children diagnosed with selective mutism within the context of its link to social anxiety disorder. An increased focus on potential medication treatment for this disorder has resulted from significant monetary and resource limitations in typical practice, parental choice in utilizing treatment from medical personnel, and children who fail to respond to psychosocial interventions. A total of 21 publications within the English literature were located. From these, data for only 57 children between the ages of 4 to 17 years have been reported, with the majority of studies utilizing single-case design or case study methodology. Yet, pharmacological data appear promising for treatment resistant cases of selective mutism. The need for additional research via efficacy/effectiveness methodologies remains. Comments on the article by Miller and Campbell (see record 2010-17135-004), in which they provide an informative discussion of the importance of considering narcissistic personality trait research when attempting to understand narcissistic personality disorder. Their arguments might seem so straightforward and compelling that they are hardly worth presenting. However, it does seem that this considerable body of literature is at times neglected, if not ignored. (PsycINFO Database Record (c) 2010 APA, all rights reserved)  相似文献

The current study investigated how mechanisms of attention that have been well-characterized in the cognitive psychology literature (Lavie, Hirst, De Fockert, & Viding, 2004; Maylor & Lavie, 1998) may be differentially associated with psychopathic traits in nonincarcerated men. Previous research on cognition and psychopathy indicated that primary psychopathic traits were associated with overfocused attention and/or reduced processing of information peripheral to the focus of attention. Conversely, deficits in executive functioning, such as working memory and cognitive control, were implicated in secondary psychopathic traits. Results revealed a significant relationship between traits typically associated with primary psychopathy (e.g., low anxiety, social dominance, fearlessness, callousness) and reduced processing of task-irrelevant distractors, suggesting diminished basic attentional capacity among individuals high on these traits. In contrast, some characteristics linked to secondary psychopathy (e.g., social alienation, cynicism) showed a positive relationship with impaired working memory functioning, indicative of deficits in cognitive control, whereas other traits (i.e., self-centeredness, antagonism) did not. These results suggest that psychopathic traits are differentially related to selective impairments in attentional functioning, which may help explain the observed heterogeneity in psychopathic manifestations. The discriminant validity of the interpersonal-affective and social deviance traits of psychopathy has been well documented. However, few studies have explored whether these traits follow distinct or comparable developmental paths. The present study used the Multidimensional Personality Questionnaire (A. Tellegen, in press) to examine the development of the psychopathic traits of Fearless Dominance (i.e., interpersonal-affective) and Impulsive Antisociality (i.e., social deviance) from late adolescence to early adulthood in a longitudinal- epidemiological sample of male and female twins. Results from mean- and individual-level analyses revealed stability in Fearless Dominance from late adolescence to early adulthood, whereas Impulsive Antisociality declined over this developmental period. In addition, biometric findings indicated greater genetic contributions to stability in these traits and greater nonshared environmental contributions to their change over time. Collectively, these findings suggest distinct developmental trends for psychopathic traits from late adolescence to early adulthood. Although stability and pervasive inflexibility are general criteria for Diagnostic and Statistical Manual of Mental Disorders, 4th edition (American Psychiatric Association, 1994) personality disorders (PDs), borderline PD (BPD) is characterized by instability in several domains, including interpersonal behavior, affect, and identity. The authors hypothesized that such inconsistencies notable in BPD may relate to instability at the level of the basic personality traits that are associated with this disorder. Five types of personality trait stability across 4 assessments over 6 years were compared for BPD patients (N = 130 at first interval) and patients with other PDs (N = 302). Structural stability did not differ across groups. Differential stability tended to be lower for 5-factor model (FFM) traits in the BPD group, with the strongest and most consistent effects observed for Neuroticism and Conscientiousness. Growth curve models suggested that these 2 traits also showed greater mean-level change, with Neuroticism declining faster and Conscientiousness increasing faster, in the BPD group. The BPD group was further characterized by greater individual-level instability for Neuroticism and Conscientiousness in these models. Finally, the BPD group was less stable in terms of the ipsative configuration of FFM facet-level profiles than was the other PD group over time. Results point to the importance of personality trait instability in characterizing BPD. (PsycINFO Database Record (c) 2010 APA, all rights reserved)  相似文献

Attention-deficit/hyperactivity disorder (ADHD) in adulthood is conceptualized as originating in childhood. Despite considerable theoretical interest, little is known about how ADHD symptoms relate to normal personality traits in adults. In 6 studies, the Big Five personality dimensions were related to ADHD symptoms that adults both recalled from childhood and reported concurrently (total N=1,620). Substantial effects emerged that were replicated across samples. First, the ADHD symptom cluster of inattention-disorganization was substantially related to low Conscientiousness and, to a lesser extent, Neuroticism. Second, ADHD symptom clusters of hyperactivity-impulsivity and oppositional childhood and adult behaviors were associated with low Agreeableness. Results were replicated with self-reports and observer reports of personality in community and clinical samples. Findings support theoretical connections between personality traits and ADHD symptoms. This study was devised to test the validity of findings of previous research with regard to the personality correlates of performance on Witkin's (1954) measures of field dependence (FD). The results (with a sample of college Ss) suggest that the Embedded Figure Test (EFT) and the Rod-and-Frame Test (RFT) do not necessarily measure the same cognitive or personality factors. Previous findings were not, to a considerable extent, duplicated; some personality correlates were not found to be related to either EFT or RFT. FD people were characterized by a tendency to react with affective disruption and intellectual deficit to an external situation that lacks clear structure. From Psyc Abstracts 36:04:4HA27E. 200 college sophomores took the MMPI 3 times. Analysis showed the following results: (1) Variation of order or time of testing in the personal or social condition seemed to make no difference. (2) Intercorrelations between the 3 forms were low enough to show that the 3 tasks were different. (3) Variance on all 3 forms was lower than in the standardization of the MMPI. (4) The mean profiles were highly similar in all 3 groups. (5) The clinical scales were in general highest on self, as were the Si, A, and obvious scales. The L, K, and subtle scales were in general lowest on self. (6) Results based on analysis of item discrepancies rather than scale comparisons showed that the discrepancies tended to be very sizable. Previous research indicates that individuals with antisocial personality disorder (ASPD) evidence low distress tolerance, which signifies impaired ability to persist in goal-directed behavior during an aversive situation, and is associated with a variety of poor interpersonal and drug use outcomes. Based on theory and research indicating that psychopathic traits are associated with hypo-reactivity in emotional responding, a unique hypothesis emerges where psychopathic traits should have the opposite effect of ASPD and be related to high levels of distress tolerance. In a sample of 107 substance-dependent patients in an inner-city substance use residential treatment facility, this hypothesis was supported. ASPD was related to lower distress tolerance, while psychopathic traits were related to higher distress tolerance, with each contributing unique variance. Findings are discussed in relation to different presentations of distress tolerance as a function of psychopathic traits among those with an ASPD diagnosis. Objective: To estimate PTSD prevalence in parents of children with chronic illnesses or undergoing invasive procedures, and its association with higher risk of PTSD among parents. Methods: Sixteen studies reporting prevalence of PTSD in parents of children with chronic illnesses were identified through a systematic review in Pubmed, Web of Science, Pilots and PsycINFO databases. Main Outcome Measures: Pooled current PTSD prevalence was calculated for parents from these studies. Pooled PTSD prevalence ratios were obtained by comparing parents of children with chronic diseases with parents of healthy children. Meta-regression was used to identify variables that could account for the lack of homogeneity. Results: Pooled PTSD prevalence was 19.6% in mothers, 11.6% in fathers, and 22.8% in parents in general (p  相似文献

Questionnaire data from 2,033 participants in the National Anxiety Disorders Screening Day sample were used to assess the presence of panic and comorbid anxiety problems. These participants were selected from more than 15,000 attendees on the basis of never having received treatment for a psychiatric disorder and meeting screening criteria for panic disorder. With each comorbid anxiety problem (generalized anxiety disorder, posttraumatic stress disorder, social phobia, and obsessive-compulsive disorder), participants had a corresponding increase in interference in daily living as well as readiness to seek treatment. The addition of generalized anxiety or depression with panic symptoms resulted in marked increases in interference scores. Clinical treatment implications for panic disorder are discussed in terms of the effects of comorbid anxiety problems. (PsycINFO Database Record (c) 2010 APA, all rights reserved)  相似文献

Objective: Decisions about the composition of personality assessment in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM–V) will be heavily influenced by the clinical utility of candidate constructs. In this study, we addressed 1 aspect of clinical utility by testing the incremental validity of 5-factor model (FFM) personality traits and borderline personality disorder (BPD) symptoms for predicting prospective patient functioning. Method: FFM personality traits and BPD features were correlated with one another and predicted 2-, 4-, 6-, 8-, and 10-year psychosocial functioning scores for 362 patients with personality disorders. Results: Traits and symptom domains related significantly and pervasively to one another and to prospective functioning. FFM extraversion and agreeableness tended to be most incrementally predictive of psychosocial functioning across all intervals; cognitive and impulse action features of BPD features incremented FFM traits in some models. Conclusions: These data suggest that BPD symptoms and personality traits are important long-term indicators of clinical functioning that both overlap with and increment one another in clinical predictions. Results support the integration of personality traits and disorders in DSM–V. (PsycINFO Database Record (c) 2010 APA, all rights reserved)  相似文献

The present study used meta-analytic techniques (number of samples = 92) to determine the patterns of mean-level change in personality traits across the life course. Results showed that people increase in measures of social dominance (a facet of extraversion), conscientiousness, and emotional stability, especially in young adulthood (age 20 to 40). In contrast, people increase on measures of social vitality (a 2nd facet of extraversion) and openness in adolescence but then decrease in both of these domains in old age. Agreeableness changed only in old age. Of the 6 trait categories, 4 demonstrated significant change in middle and old age. Gender and attrition had minimal effects on change, whereas longer studies and studies based on younger cohorts showed greater change. This case study of process and outcome is based upon data obtained during a 5-year psychoanalysis of an adult male with avoidant personality disorder (AVPD). To date, no known systematic case studies, effectiveness studies, or randomized control trials exist for psychoanalysis in the treatment of AVPD. In this study, self-reported symptoms and observer-rated personality disorder (PD), global functioning, object relations, and psychological health were gathered at intake, after each year of treatment, and at 1-year follow-up. Process data was gathered to determine the extent to which the treatment adhered to prototypes of psychodynamic, cognitive-behavioral, and interpersonal therapy. Results indicated that the patient achieved clinically significant reductions in PD, symptom severity, and relational pathology. Gains were maintained at 1-year follow-up. The treatment significantly adhered to psychodynamic principles throughout, with some use of cognitive-behavioral and interpersonal principles in the third year of treatment. These findings warrant further investigation of psychoanalysis for AVPD and demonstrate the usefulness of assessing multiple domains of patient functioning and treatment process. Personality may directly facilitate or constrain coping, but relations of personality to coping have been inconsistent across studies, suggesting a need for greater attention to methods and samples. This meta-analysis tested moderators of relations between Big Five personality traits and coping using 2,653 effect sizes drawn from 165 samples and 33,094 participants. Personality was weakly related to broad coping (e.g., Engagement or Disengagement), but all 5 traits predicted specific strategies. Extraversion and Conscientiousness predicted more problem-solving and cognitive restructuring, Neuroticism less. Neuroticism predicted problematic strategies like wishful thinking, withdrawal, and emotion-focused coping but, like Extraversion, also predicted support seeking. Personality more strongly predicted coping in young samples, stressed samples, and samples reporting dispositional rather than situation-specific coping. Daily versus retrospective coping reports and self-selected versus researcher-selected stressors also moderated relations between personality and coping. Cross-cultural differences were present, and ethnically diverse samples showed more protective effects of personality. Richer understanding of the role of personality in the coping process requires assessment of personality facets and specific coping strategies, use of laboratory and daily report studies, and multivariate analyses. In this longitudinal study, we followed a large sample of first-time parents (both partners) across the first 2 years of the transition to parenthood. Guided by attachment theory (Bowlby, 1969), we tested several predictions about how attachment anxiety and avoidance are related to the incidence, maintenance, increase, and decline of depressive symptoms in both sexes across the first 2 years of the transition. We found that (a) the association between attachment anxiety and depressive symptoms was moderated by factors related to the marital and/or romantic relationship; (b) the association between avoidance and depressive symptoms was moderated by factors related to family responsibilities; (c) styles of caregiving provided by romantic partners affected depressive symptoms differently among anxious and avoidant persons; and (d) in certain predictable situations, depressive symptoms persisted at higher levels or increased to higher levels in anxious or avoidant persons across the 2-year transition period. Important implications of these results are discussed. (PsycINFO Database Record (c) 2011 APA, all rights reserved)  相似文献

The authors examined associations between 5-factor personality traits and retirement in a diverse community sample. Longitudinal analyses (n = 367) compared personality trajectories of participants who remained employed and participants who retired. Personality at baseline did not predict future retirement, but compared to participants who remained employed, retirees increased in Agreeableness and decreased in Activity, a facet of Extraversion. In cross-sectional analyses among retirees (n = 144), those low in Neuroticism and high in Extraversion reported higher retirement satisfaction, and those high in Extraversion reported higher postretirement activity levels. Findings suggest that the trait perspective contributes to the understanding of the retirement process. (PsycINFO Database Record (c) 2010 APA, all rights reserved)  相似文献
